Early Results of Maze III Operation Without Cryoablation (냉동절제 없이 시행한 Maze III 술식의 조기 결과)

  • Background: Atrial fibrillation is one of the most prevalent of all arrhythmias and in up to 79% of the patients with mitral valve disease. This study examined whether the atrial fibrillation that occur in patients with mitral valve operation could be eliminated by a concommitant maze operation without cryoablation. Material and Method: From May 1997 to April 1998, 14 patients with atrial fibrillation associated with mitral valve disease underwent Maze III operation without cryoablation. Preoperatively there were 6 men and 8 women with an average age of 46.2${\pm}$10.7 years. Eleven patients had mitral stenosis, and three had mitral insufficiency. The associated heart diseases were aortic valve disease in 4, tricuspid valve regurgitation in 1 and ASD in 2. Using transthoracic echocardiography, the mean left atrial diameters was 54.7${\pm}$5.3 mm and thrombi were found in the left atrium of 2 patients. Postoperatively the ratio between the peak speed of the early filling wave and that of the atrial contraction wave (A/E ratio) was determined from transmitral flow measurement. Operations were mitral valve replacement in 13 including 4 aortic valve replacements, 1 DeVega annuloplasty and 2 ASD closures. Maze III operation was performed in 1 patient. Result: Five patients (38%) had recurred atrial fibrillation, which was reversed with flecainide or amiodarone at the average time of postoperative 38.8${\pm}$23.5 days. Postoperative complications were postoperative transient junctional rhythm in 6, transient atrial fibrillation in 5, reoperation for bleeding in 3, postpericardiotomy syndrome(1), unilateral vocal cord palsy(1), postoperative psychosis(1), and myocardial infarction(1). Postoperatively A/E ratio was 0.43${\pm}$0.22 and A wave found in 9(64%) patients. 3 to 14 months postoperatively (average follow- up, 8.1 months), all of patients had normal sinus rhythm and 9(64%) patients had left atrial contraction and 11(79%) patients were not on a regimen of antiarrhythmic medication. Conclusion: We conclude that Maze III operation without cryoablation is an effective surgical treatment in atrial fibrillation associated with the mitral valve disease.

Study for Correlation between Objective and Subjective Voice Parameters in Patients with Dysphonia (발성장애 환자에서 주관적 음성검사와 객관적 음성검사의 연관성 연구)

  • Background and Objectives Voice evaluation is classified into subjective tests such as auditory perception and self-measurement, and objective tests such as acoustic and aerodynamic analysis. When evaluating dysphonia, subjective and objective test results do not always match. The purpose of this study was to analyze the relationship between subjective and objective evaluation in patients with dysphonia and to identify meaningful parameters by disease. Materials and Method The total of 322 patients who visited voice clinic from May 2017 to May 2018 were included in this study. Laryngeal lesions were identified using stroboscopy. Pearson correlation test was performed to analyse correlation between subjective tests including GRBAS scale and voice handicap index, and objective tests including jitter, shimmer, noise to harmonic ratio (NHR), cepstral peak prominence (CPP), maximal phonation time (MPT), mean flow rate, and subglottic pressure. Results In vocal nodule and sulcus vocalis, among GRBAS system, grade and breathiness showed good correlation with CPP, and roughness showed good correlation with jitter or shimmer. In unilateral vocal cord paralysis (UVCP), grade and breathiness showed a very good correlation with CPP, and also good correlation with jitter, shimmer, NHR, and MPT. Also asthenia showed good correlation with CPP and MPT. Vocal polyp has a limited association with other diseases. Conclusion In patients with dysphonia, grade and breathiness showed good correlation with CPP, jitter, and shimmer, and reflect the state of voice change well especially in UVCP, CPP, and MPT.

Design Hourly Factor Estimation with Vehicle Detection System (차량검지기자료를 이용한 고속도로 설계시간계수 산정 연구)

  • Design Hourly Volume (DHV) is the hourly volume used for designing a section of road. DHV is also used to estimate the expected number of vehicles to pass or traverse the relevant section of road in a future target year. The Design Hour Factor (DHF) is defined as the ratio of DHV to Average Annual Daily Traffic (AADT). In addition to high precision of predicted traffic volume, in order to design a roadway to be the proper scale, applying appropriate DHFs considering traffic flow characteristics and type of area which surrounds the relevant roadway is important. This study categorizes sections of expressway (Suh Hae An Expressway) according to their area type and estimates DHFs utilizing traffic data obtained from a vehicle detection system (VDS). This study shows that DHFs calculated using VDS data are different from those using traffic data acquired from a coverage survey. While AADTs from both data show similar values, peak hour volumes from both data show significant differences especially for recreational areas. DHFs from the coverage survey are quite different from the values provided by the Korean design guide or previous research results and DHFs for urban areas are higher than recreational areas. However, DHFs from VDS shows similar values to previous research results. The result of this study suggests that using VDS for estimating DHFs is more reliable than using a coverage survey.

Changes in Properties of Silk Monofilament Caused by Drawing and Hydrolysis (견 Monofilament의 연신과 가수분해에 의한 특성변화)

  • The middle silk gland, that is a liquid silk thread gland consisting of silk protein, was taken out and a silk monofilament was made by drawing rapidly to approximately 3 times. In order to deteriorate the inter molecular hydrogen bonding force and to stretch in, the drawn silk filament was swoolen in boiling water. The results obtained are as follows ; The silk gland sample that just dried silk gland was occupied in crystalline region of silk-I type and random amorphous region. According to the examination of X-ray diffraction and thermal analysis, silk-II type crystal begins to appear partially in monofilament sample and spread to almost complet silk-II type crystal in 65.2% drawn sample. And, orientation of silk fibroin mlecule increased suddenly in early stage with a rise of drawing ratiofrom birefringence and density, and it was found that orientation of fibroin molecule was completed. As drawing ratio increases relation with time of hydrolysis, birefringence appeared almost fixed a tendency. Crystallization collapse by hydrolysis was not found in X-ray diffraction and thermal analysis. But, amorphous region began to flow by treated hydrolysis, that orientation of crystallization part was disturbed was supposed.

Spatial Extension of Runoff Data in the Applications of a Lumped Concept Model (집중형 수문모형을 활용한 홍수유출자료 공간적 확장성 분석)

  • Runoff data availability is a substantial factor for precise flood control such as flood frequency or flood forecasting. However, runoff depths and/or peak discharges for small watersheds are rarely measured which are necessary components for hydrological analysis. To compensate for this discrepancy, a lumped concept such as a Storage Function Method (SFM) was applied for the partitioned Choongju Dam Watershed in Korea. This area was divided into 22 small watersheds for measuring the capability of spatial extension of runoff data. The chosen total number of flood events for searching parameters of SFM was 21 from 1991 to 2009. The parameters for 22 small watersheds consist of physical property based (storage coefficient: k, storage exponent: p, lag time: $T_l$) and flood event based parameters (primary runoff ratio: $f_1$, saturated rainfall: $R_{sa}$). Saturated rainfall and base flow from event based parameters were explored with respect to inflow at Choongju Dam while other parameters for each small watershed were fixed. When inflow of Choongju Dam was optimized, Youngchoon and Panwoon stations obtained average of Nash-Sutcliffe Efficiency (NSE) were 0.67 and 0.52, respectively, which are in the satisfaction condition (NSE > 0.5) for model evaluation. This result is showing the possibility of spatial data extension using a lumped concept model.

Estimating design floods for ungauged basins in the geum-river basin through regional flood frequency analysis using L-moments method (L-모멘트법을 이용한 지역홍수빈도분석을 통한 금강유역 미계측 유역의 설계홍수량 산정)

  • The study performed a regional flood frequency analysis and proposed a regression equation to estimate design floods corresponding to return periods for ungauged basins in Geum-river basin. Five preliminary tests were employed to investigate hydrological independence and homogeneity of streamflow data, i.e. the lag-one autocorrelation test, time homogeneity test, Grubbs-Beck outlier test, discordancy measure test ($D_i$), and regional homogeneity measure (H). The test results showed that streamflow data were time-independent, discordant and homogeneous within the basin. Using five probability distributions (generalized extreme value (GEV), three-parameter log-normal (LN-III), Pearson type 3 (P-III), generalized logistic (GLO), generalized Pareto (GPA)), comparative regional flood frequency analyses were carried out for the region. Based on the L-moment ratio diagram, average weighted distance (AWD) and goodness-of-fit statistics ($Z^{DIST}$), the GLO distribution was selected as the best fit model for Geum-river basin. Using the GLO, a regression equation was developed for estimating regional design floods, and validated by comparing the estimated and observed streamflows at the Ganggyeong station.

A Study on Motion Estimator Design Using DCT DC Value (DCT 직류 값을 이용한 움직임 추정기 설계에 관한 연구)

  • The compression method is necessarily used to send the high quality moving picture that contains a number of data in image processing. In the field of moving picture compression method, the motion estimation algorithm is used to reduce the temporal redundancy. Block matching algorithm to be usually used is distinguished partial search algorithm with full search algorithm. Full search algorithm be used in this paper is the method to compare the reference block with entire block in the search window. It is very efficient and has simple data flow and control circuit. But the bigger the search window, the larger hardware size, because large computational operation is needed. In this paper, we design the full search block matching motion estimator. Using the DCT DC values, we decide luminance. And we apply 3 bit compare-selector using bit plane to I(Intra coded) picture, not using 8 bit luminance signals. Also it is suggested that use the same selective bit for the P(Predicted coded) and B(Bidirectional coded) picture. We compare based full search method with PSNR(Peak Signal to Noise Ratio) for C language modeling. Its condition is the reference block 8$\times$8, the search window 24$\times$24 and 352$\times$288 gray scale standard video images. The result has small difference that we cannot see. And we design the suggested motion estimator that hardware size is proved to reduce 38.3% for structure I and 30.7% for structure II. The memory is proved to reduce 31.3% for structure I and II.

Development of Ceramide NP Analysis Method in Cosmetic Formulations Using Liquid Chromatography (액체크로마토그래피를 이용한 화장품 제형 내 세라마이드엔피 분석법 확립)

  • In this study, a quantitative analysis method was developed using high-performance liquid chromatography (HPLC) to analyze the content of ceramide NP in lotion, cream, and cleanser formulations in cosmetics. The analysis was performed using a C18 column, and the mobile phase was set at a ratio of 70 : 30 for acetonitrile and methanol, the flow rate was set to 0.8 mL/min, and the column temperature was set to 20 ℃. The method was verified by analyzing specificity, linearity, limit of detection, limit of quantitation, accuracy, and precision in accordance with the ICH guidelines. As a result of validating the method, the linearity of the calibration curve was excellent (R2 = 0.99984). The accuracy of the lotion, cream, and cleanser formulations was confirmed with a recovery rate ranging from 95.11% to 100.48%. The precision analysis showed a low relative standard deviation (RSD) of less than 0.26%. The limit of detection was 0.902 ㎍/mL, and the limit of quantitation was 2.733 ㎍/mL. Through this quantitative analysis method of ceramide NP applied in cosmetics, it is expected to assist in the quality control of products by enabling measurement even when it is difficult to separate the main peak due to the influence of interfering substances.

Effects of Breathing Exercise Interventions on Lung Function in COVID-19 Survivors (호흡운동 중재방법이 COVID-19 생존자의 폐기능에 미치는 영향)

  • Purpose : People who have suffered from COVID-19 suffer from decreased pulmonary function and various side effects. This study aims to present three respiratory exercise intervention methods to improve pulmonary function in COVID-19 survivors. Therefore, the purpose of this study will investigate the effects of breathing exercise interventions (aerobic exercise, diaphragm breathe exercise, and inspiratory muscle training on resistance) on pulmonary function in COVID-19 survivors. Methods : The subjects who participated in this study were 35 male and female college students confirmed with COVID-19. All subjects were randomly assigned to A, D, and I groups according to breathing exercise intervention method. Groups A, D, and I each performed aerobic exercise, diaphragm breathing exercise, and inspiratory muscle training on resistance, 3 times a week for 6 weeks. Pulmonary function was measured using a spirometer, and FVC (forced vital capacity), FEV1 (forced expiratory volume in one second), FEV1/FVC % (forced expiratory volume in one second / forced vital capacity ratio), and PEF (peak expiratory flow) were measured at 0, 3, and 6 weeks. Data analysis was compared by repeated measures analysis of variance, and post hoc tests for time were compared and analyzed using paired t-tests. Results : In the results of this study, FVC values showed statistically significant improvement in all groups. FEV1 values also showed statistically significant improvement in all groups. And the FEV1/FVC % value also showed statistically significant improvement in all groups. And the PEF values also showed statistically significant improvement in all groups. Conclusion : The results of this study reported that aerobic exercise, diaphragm breathing exercise, and resistance inspiratory muscle training were all effective in improving pulmonary function in COVID-19 survivors. Therefore, application of the three breathing exercise intervention methods presented in this study will help improve pulmonary function in COVID-19 survivors.

Effect of Various Mixing Ratio of Non-glutinous and Glutinous Rice on Physical and Rheological Properties of Extrudate (멥쌀과 찹쌀의 혼합비율별 압출성형물의 물리적 성질 및 유동특성)

  • Effect of different mixing ratio of non-glutinous and glutinous rice on physical and rheological properties of extrudate prepared in a single screw extruder were examined. The extrusion conditions in term of screw speeds, moisture content and die temperature were 258 rpm, 18% and $120^{\circ}C$, respectively. The resisdence time distribution of the most of materials were within 30 second and small portion of them went up to 80 second. The expansion ratio was the highest value (2.93) for 70% of glutinous rice in the mixture, while the lowest value for 100% of non-glutinous rice. Breaking strength was in the range between 1,051g and 1,117g for $10{\sim}20%$ of glutinous rice in the mixture, while the lowest value (737g) for 80%r of glutinous rice. As the amount of glutinous rice increased, L and a values were increased and b value was decreased. The uncooked cold paste viscosity had 400 B.U. for 100% non-glutinous rice , while no peak for the 100% glutinous rice. As the amount of glutinous rice increased up to 100%, the water absorption index (WAI) was decreased, while water solubility index (WSI) was increased. The rheological properties of extrudate were accounted by the law of Oswald. The flow behavior index of extrudate was less than 1.0, which showed pseudoplastic behavior. Yield stress was the highest value for 20% of glutinous rice in the mixture and the lowest value for $80{\sim}100%$ of glutinous; rice. Number of air cell was between 128 and 159 for $80{\sim}100%$ of glutinous rice in the mixture, while $81{\sim}84%$ for $0{\sim}20%$ of glutinous rice. The degree of shapefact was increased more when the mixtures of glutinous and non-glutinous rice was used than when glutinous or non-glutinous rice was only used.
