  • In this experiment, the effects of cage density (CD) and probiotic supplementation (PS) on laying performance, metabolic profile, and egg quality in peak-producing hens were evaluated. After blocking according to the cage location, Lohman layers (n = 180, 46 wks of age) were allocated randomly to two levels of CD (540 vs. 360 $cm^2$/hen) and three levels of PS (0, 0.15, and 0.30%). Probiotic contained Enterococcus faecium culture (10${\times}$10$^9$ cfu/g). Egg production (EP) and feed consumption (FC) were measured daily; egg weight (EW) was measured bi-weekly; BW was measured before and after the experiment; and blood samples were obtained at the end of the experiment. The data were analyzed using two-way ANOVA. Increasing CD decreased FC (125.0 vs. 120.8 g/d, p<0.0001) and FCR (1.93 vs. 1.87, p<0.0001) and did not alter EP, EW, and BW. Increasing level of PS linearly decreased FC (p<0.02) and FCR (p<0.006). Averages were 123.9, 123.2, and 121.6 g/d for FC and 1.91, 1.92, and 1.86 for FCR in hens supplemented with 0, 0.15, and 0.30% probiotic, respectively. Hens placed in high-density cages had greater serum corticosterone concentration than hens placed in normal-density cages (12.8 vs. 11.3 $\mu$g/dL, p<0.04); CD did not affect concentrations of other metabolites. Increasing level of PS linearly increased serum glucose, albumin, and creatine concentrations and quadratically increased total protein, globulin, Ca, and P concentrations. Average concentrations (mg/dL) were 260, 297, and 305 for glucose; 6.28, 8.09, and 7.58 for total protein; 1.98, 2.48, and 2.38 for albumin; 4.30, 5.62, and 5.19 for globulin; 0.40, 0.52, and 0.54 for creatine; 16.0, 16.5, and 16.3 for Ca; and 6.27, 8.14, and 7.17 for P in hens supplemented with 0, 0.15, and 0.30% probiotic, respectively. There was no effect of CD on egg quality. Increasing level of PS linearly improved yolk color (YC) and quadratically increased albumen index (AI) and Haugh unit (HU). The mean values were 9.67, 9.75, and 10.58 for YC; 8.94, 6.93, and 8.72% for AI; and 85.6, 74.9, and 82.9 for HU for hens supplemented with 0, 0.15, and 0.30% probiotic, respectively. There was also CD by PS effect on FC, EP, and serum glucose, total protein, albumin, globulin, creatine, Ca and P concentrations. In conclusion, increased CD partially depressed laying performance and caused stress. Probiotic supplementation improved laying performance and metabolic profile. It also partially alleviated the adverse effects of stress resulting from increased caging density.

  • Osteoporosis is defined as a progressive systemic skeletal disorder characterized by low bone mineral density, microarchitectual deteriorations of bone and susceptibility to fracture. numerous methods have been used for quantitative assessment of the skeleton in osteoporosis. QCT has been shown to measure changes in trabecular mineral content in the spine with great sensitivity and precision. To provide the normal reference values and changes of lumbar spinal bone mineral density in korean adult spinal bone mineral density was evaluated in 451 women (229 premenopausal and 222 postmenopausal women) and 206 men, aged 20 to 74 years old in Wonkwang hospital from 2000 to 2004, which was carried out by using QCT. women with oophorectomy, vertebral compression fracture, any history of endocrine disease and use of drugs that alter bone metabolism were excluded. According to the WHO definition, a patient is osteoporotic based on a bone mineral density(BMD) measurement that is 2.5 standard deviations (SDs) below typical peak bone mass of young healthy white women. This measurement of standard deviation from peak mass is called the T score. BMD values of normal women in their 20-24 years, 25-29 years, 30-34 years, 35-39 years, 40-44 years, 45-49 years, 50-54 years, 55-59 years, 60-64 years, 65-69 years, over 70 years were 168.95㎎/㏄ K₂PHO₄, 155.41㎎/㏄ K₂PHO₄, 166.87㎎/㏄ K₂PHO₄, 160.67㎎/㏄ K₂PHO₄, 154.06㎎/㏄ K₂PHO₄, 132.04㎎/㏄ K₂PHO₄, 114.05㎎/㏄ K₂PHO₄, 91.78㎎/㏄ K₂PHO₄, 78.61 ㎎/㏄ K₂PHO₄, 61.35㎎/㏄ K₂PHO₄, 50.53㎎/㏄ K₂PHO₄ Mean bone density of normal women was 115.77K₂PHO₄ K₂PHO₄. BMD values of normal men in their 20-24 years, 25-29 years, 30-34 years, 35-39 years, 40-44 years, 45-49 years, 50-54 years, 55-59 years, 60-64 years, 65-69 years, over 70 years were 171.46㎎/㏄ K₂PHO₄, 162.19㎎/㏄ K₂PHO₄, 155.62㎎/㏄ K₂PHO₄, 147.28㎎/㏄ K₂PHO₄, 137.56㎎/㏄ K₂PHO₄, 137.56㎎/㏄ K₂PHO₄, 101.25㎎/㏄ K₂PHO₄, 109.00㎎/㏄ K₂PHO₄, 103.32㎎/㏄ K₂PHO₄, 91.53㎎/㏄ K₂PHO₄, 88.35㎎/㏄ K₂PHO₄ Mean density of normal men was 115.77㎎/㏄ K₂PHO₄. Peak bone density of women and men was in the age group of 20-24 years and 168.95㎎/㏄ K₂PHO₄, 171.46㎎/㏄ K₂PHO₄, respectively. Bone loss was increased with aging and was accelerated in postmenopausal women than that of premenopausal women. The total loss of BMD for women and men was 70.09% and 48.47%, respectively. Postmenopausal women(mean BMD : 85.83㎎/㏄ K₂PHO₄) had significantly lower BMD than premenopausal women(meand BMD : 144.80㎎/㏄ K₂PHO₄)(p<0.001). The annual loss of BMD of women and men was 2.702㎎/㏄ K₂PHO₄ and 1.795㎎/㏄ K₂PHO₄, respectively. This study provided the BMD reference data for normal korean adult. further studies on BMD in healthy adult and comparison with published data are needed.

  • This experiment was conducted to determine the effects of cage density (CD) and humate supplementation (HS) on laying performance, metabolic profile, and egg quality during the peak production period in hens. Lohman layers (n = 180, 46 wks of age) were blocked according to the location of cages and then allocated randomly to two levels of CD (4 or 6 hens per cage or 540 vs. 360 $cm^2$/hen) and three levels of HS (0, 0.15, and 0.30%). Egg production (EP) and feed consumption (FC) were measured daily; egg weight was measured bi-weekly; and BW was measured before and after the experiment. Blood and additional egg samples were obtained at the end of the experiment for determination of metabolic profile and egg quality. The data were analyzed using two-way ANOVA as repeated measures. Except for FC, CD did not affect laying performance parameters. Hens placed in high-density cages had lower FC than hens placed in normal-density cages. Increasing HS level linearly increased FC, EP, and feed conversion ratio (FCR). There was a CD by HS interaction effect on FC and EP. Hens placed in high-density cages had greater serum glucose, total protein, albumin, globulin, Ca, and P concentrations and tended to have greater serum corticosterone concentration than hens placed in normaldensity cages. Increasing HS level linearly increased serum glucose, total protein, albumin, globulin, creatine, and Ca concentrations and linearly decreased serum triglyceride and very low-density lipoprotein concentrations. There was a CD by HS interaction effect on serum glucose and albumin concentrations. There were no alterations in egg quality parameters in response to increasing CD. Albumen index and Haugh unit decreased linearly and other egg quality parameters did not change as HS level increased. In conclusion, increased caging density adversely affected metabolic profile, despite insignificantly deteriorating laying performance. Moreover, benefits from humate supplementation seem to be more noteworthy for hens housed in stressing conditions than for hens housed in standard conditions.

  • To evaluate the effect of air pollution on respiratory health in children, We conducted a longitudinal study in which children were asked to record their daily levels of Peak Expiratory Flow Rate(PEFR) using potable peak flow meter(mini-Wright) for 4 weeks. The relationship between daily PEFR and ambient air particle levels was analyzed using a mixed linear regression models including gender, age in year, weight, the presence of respiratory symptoms, and relative humidity as an extraneous variable. The daily mean concentrations of $PM_{10}$ and $PM_{2.5}$ over the study period were $64.9{\mu}g/m^3$ and $46.1{\mu}g/m^3$, respectively. The range of daily measured PEFR in this study was $182{\sim}481\;l/min$. Daily mean PEFR was regressed with the 24-hour average $PM_{10}(or\;PM_{2.5})$ levels, weather information such as air temperature and relative humidity, and individual characteristics including sex, weight, and respiratory symptoms. The analysis showed that the increase of air particle concentrations was negatively associated with the variability in PEFR. We estimated that the IQR increment of $PM_{10}$ or $PM_{2.5}$ were associated with 1.5 l/min (95% Confidence intervals -3.1, 0.1) and 0.8 l/min(95% CI -1.8, 0.1) decline in PEFR. Even though this study showed negative findings on the relationship between respiratory function and air particles, it was worth noting that the findings must be interpreted cautiously because exposure measurement based on monitoring of ambient air likely resulted in misclassification of true exposure levels and this was the first Korean study that $PM_{2.5}$ measurement was applied as an index of air quality.

  • This study aimed to determine the effects of the blockage of visual feedback on joint dynamics of the lower extremity. Fifteen healthy male subjects(age: $24.1{\pm}2.3\;yr$, height: $178.7{\pm}5.2\;cm$, weight: $73.6{\pm}6.6\;kg$) participated in this study. Each subject performed single-legged landing from a 45 cm-platform with the eyes open or closed. During the landing performance, three-dimensional kinematics of the lower extremity and ground reaction force(GRF) were recorded using a 8 infrared camera motion analysis system (Vicon MX-F20, Oxford Metric Ltd, Oxford, UK) with a force platform(ORG-6, AMTI, Watertown, MA). The results showed that at 50 ms prior to foot contact and at the time of foot contact, ankle plantar-flexion angle was smaller(p<.05) but the knee joint valgus and the hip flexion angles were greater with the eyes closed as compared to with the eyes open(p<.05). An increase in anterior GRF was observed during single-legged landing with the eyes closed as compared to with the eyes open(p<.05). Time to peak GRF in the medial, vertical and posterior directions occurred significantly earlier when the eyes were closed as compared to when the eyes were open(p<.05). Landing with the eyes closed resulted in a higher peak vertical loading rate(p<.05). In addition, the shock-absorbing power decreased at the ankle joint(p<.05) but increased at the hip joints when landing with the eyes closed(p<.05). When the eyes were closed, landing could be characterized by a less plantarflexed ankle joint and more flexed hip joint, with a faster time to peak GRF. These results imply that subjects are able to adapt the control of landing to different feedback conditions. Therefore, we suggest that training programs be introduced to reduce these injury risk factors.

  • Background: While inhalation therapy efficacy hinges on attaining proper peak inspiratory flow rate (PIFR), the prevalence of inappropriate PIFR among patients with chronic obstructive pulmonary disease (COPD) remains unstudied in Korea. This study aimed to assess the prevalence of inappropriate PIFR, its correlation with COPD assessment test (CAT) scores, and factors associated with suboptimal PIFR. Methods: We enrolled 108 patients with COPD who had been using the same inhaler for at least 1 year without exacerbations. PIFR was measured using an inspiratory flow meter (In-Check DIAL G16). Demographic, clinical, pulmonary function, and CAT score data were collected. Inappropriate was defined as PIFR <60 L/min for dry power inhaler (DPI) users, and >90 L/min for aerosol device users. Results: The cohort comprised 87 (80.6%) men, mean age 71.0±8.5 years, with mean post-bronchodilator forced expiratory volume in 1 second of 69.1%±1.8% predicted. Twenty-nine (26.9%) used aerosol devices only, 76 (70.4%) used DPIs only, and three (2.8%) used both. Inappropriate PIFRs were found in 17.2% of aerosol device users, and 42.1% of DPI users. CAT scores were significantly higher in the inappropriate PIFR group than in the appropriate PIFR group (11.2±7.7 vs. 7.5±4.9, p=0.003). In DPI users, female, shorter height, lower body weight and maximal voluntary ventilation (MVV) were associated with inappropriate PIFR. Conclusion: The prevalence of inappropriate PIFR among patients with COPD is 17.2% for aerosol device users, and 42.1% for DPI users. Suboptimal PIFR correlates with female gender, shorter stature, lower weight and MVV in DPI users.

  • Introduction: Northeastern Iran is known as a high risk area of upper gastrointestinal cancers. Recent reports have suggested a declining trend for these cancers as well as an increase in the incidence of other malignancies including breast cancer. Our present aim was to describe the epidemiological pattern of breast cancer in this region during 2004-2009. Methods: All new cancer cases from public and private diagnostic and therapeutic centers of Golestan province were registered. A structured questionnaire was prepared and used based on the standerds of the International Association of Cancer Registries. The international classification of diseases for oncology was considered for coding. Age standardized incidence rates (ASR) of breast cancer were calculated. Results: A total of 11,038 new cancer cases were registered during 2004-2009, of which, 1,101 (10%) were females with breast cancer. The median age of the breast cancer patients was 46 years. The ASR for breast cancer was 28 per 100,000 person-years. We found an unusual rapid increase in breast cancer rate at the age of 25 years. The ASR of breast cancer was significantly lower in females from Turkmen ethnicity and those from rural areas(P value <0.01). Conclusion: Our study showed high rate of breast cancer in Golestan province of Iran. We found an unusual peak of breast cancer in young women. So, the age of starting screening programs may need to be revised in this area. The rate of breast cancer was significantly lower in women from Turkmen ethnicity. Further studies are warranted to clarify the role of important determinants, especially regarding the ethnic disparity, on breast cancer in this region.

  • Purpose: Although trauma is the most common cause of death under age 18, Korean national pediatric trauma data has lack of clinical data. This study is to prepare manpower resources, equipment, and make a correct policy decision on pediatric trauma victims Methods: The study enrolled 528 patients under age 16 with traumatic injury visited Wonju Severance Christian Hostpital Trauma Center, from February 12, 2015 to December 31, 2016. We analyzed the distribution of gender, age, place and time of the accident, injury mechanism, injury severity, and injured organ by medical record. Results: The major injury mechanisms were blunt injury in 485 (91.90%), penetrating injury in 27 (5.10%), burn in 13 (2.50%), near drowning in 2 (0.40%), and foreign body ingestion in 1 (0.20%). Ninety-seven (18.4%) patients were injured at home and 67 (12.7%) patients were injured at school. The overall mortality rate was 1.13% (n=6). 5 mortalities were related to automobile accident and one was fall down. Mean Injury Severity Score (ISS) was 4 (2, 8). No statistical significance was observed in the mean ISS between each age group. The peak time of accident occurrence was between 16 and 17 o'clock. The mean ISS was higher in blunt injury group than penetrating injury with statistical significance ($6.50{\pm}7.60$ vs. $3.00{\pm}8.10$; p<0.05). The most common injury site was upper extremity. Mean ISS was highest in thorax injury. However, mean ISS of thorax injury was higher with statistical significance only compared with face, neck and upper extremity injury. Conclusions: We reported our pediatric trauma patients data of our hospital level I trauma center, which is the only one level I trauma center of Gangwon Province. These data is useful to prevent and prepare for pediatric trauma.
