Investigation into Occupational Disease of Merchant Crew (상선승무원들의 질병실태 조사)

  • The purpose of this study was to find out morbidity rate and pattern of disease and affect of variables related disease and medical management of seafares'. The subjects this study were 1049 seafares' were took education in Korea Institute of Maritime and Fisheries Technology. This questionnaire was focused on finding the basic data for prevention of disease and promotion health for the seafares'. The collected data were analyzed by using descriptive statistics, Chi-square, cross tab, linear regression by SPSS 10.1 package. The result of this study are as follow. 1)The morbidity rate within recent 12 months was 69.0%. 2)there were significant differences qf occurred disease in age(p<0.05), income(p<0.01), career of ship on board(p<0.01), rank(p<.01), perceived health status(p<0.01), worry of health(p<0.01), fatigue symptoms(0.01), satisfy of job(p<0.05), rest time(p<0.05) 3) Considering disease unable to work more than 4 hour, the number of those who had oral disease 7.3%> tinea 6.6%> gastric ulcer 6.4, and musculoskeletal disease group were 20.9%, which revealed the highest rate oral disease 13.6%> skin disease> 12.4%, digestive disease> 12.1%. 4) As refer to medical management, The pain above 31days 35.7%, hospitalization and treatment below 7days were each 50.2%, 42.8%, medical service were doctor's office 27.9, which revealed the highest rate.

A New Similarity Measure for Categorical Attribute-Based Clustering (범주형 속성 기반 군집화를 위한 새로운 유사 측도)

  • The problem of finding clusters is widely used in numerous applications, such as pattern recognition, image analysis, market analysis. The important factors that decide cluster quality are the similarity measure and the number of attributes. Similarity measures should be defined with respect to the data types. Existing similarity measures are well applicable to numerical attribute values. However, those measures do not work well when the data is described by categorical attributes, that is, when no inherent similarity measure between values. In high dimensional spaces, conventional clustering algorithms tend to break down because of sparsity of data points. To overcome this difficulty, a subspace clustering approach has been proposed. It is based on the observation that different clusters may exist in different subspaces. In this paper, we propose a new similarity measure for clustering of high dimensional categorical data. The measure is defined based on the fact that a good clustering is one where each cluster should have certain information that can distinguish it with other clusters. We also try to capture on the attribute dependencies. This study is meaningful because there has been no method to use both of them. Experimental results on real datasets show clusters obtained by our proposed similarity measure are good enough with respect to clustering accuracy.

The Relationship between Isoniazid Resistance and 463 CodonMutation of katG Gne in Mycobacterium Tuberculosis (결핵균 katG 유전자내 463 Codon 돌연변이와 Isoniazid내성 관계)

  • Background: The 463 codon mutation of katG gene has been reported as an useful marker for the detection of isoniazid(INH) resistant strains of M. tuberculosis. This study aimed to elucidate relationship between 463 mutation in katG gene and INH resistance in M. tuberculosis. Method: DNA was extracted from 28 INH susceptible strains(MIC$\geq\;0.2{\mu}g/ml$ on the L$\ddot{o}$wenstein Jensen media) and used for amplification of 189bp fragment containing 463 codon by PCR. Amplified fragments were digested by restriction enzyme Msp I, analyzed by single strand conformation polymorphism(SSCP) in the MDE gel and sequenced to prove mutation. Result: Only 7(25%) out of 28 were digestible by restriction enzyme Msp I. The SSCP pattern of 21 strains were distinctly different from that of M. tuberculosis H37Rv. Msp I undigestible PCR fragment was substituted at 463 codon from Arg(CGG) to Leu(CTG). Conclusion: This finding clearly indicate no relationship between 463 codon mutation of the katG gene and INH resistance.

A Case of Miliary Tuberculosis Mimicking ARDS due to Bilateral Severe Pneumonia (양측성 중증 폐렴으로 인한 ARDS로 오진된 속립성 결핵)

  • Miliary tuberculosis is the most serious form of tuberculous disease, but is rarely complicated with acute respiratory distress syndrome (ARDS). When a patient with miliary tuberculosis initially presents with ARDS, the mortality is much higher. Therefore, the early detection of miliary tuberculosis as the underlying cause of ARDS is very important for the prognosis and survival of the patient. The diagnosis of miliary tuberculosis may be easy if the patient presents typical clinical manifestations associated with the characteristic pattern of miliary nodules on chest radiology. However, the diagnosis of miliary tuberculosis when complicated with ARDS can be difficult due to the nonspecific radiologic patterns, such as diffuse bilateral consolidation and ground glass opacity, without miliary nodular infiltration. However, these nonspecific patterns are known as less likely findings of miliary tuberculosis. We experienced a pregnant woman with miliary tuberculosis, mimicking ARDS due to bilateral severe pneumonia. She was admitted, via the emergency room, with sudden onset of fever, chill, cough and dyspnea. The initial chest PA and HRCT showed diffuse bilateral consolidation and ground glass opacity, without miliary nodular infiltration. All bacteriological studies, including blood and sputum cultures, tuberculosis-PCR and serologic study for infectious disease were negative. However, the definite diagnosis of unusual miliary tuberculosis as the underlying cause of ARDS was confirmed from the radiological finding and transbronchial fiberoptic lung biopsy. We report this case, with a review of the literature.

A Determination of the Maximum Potential Runoff of Small Rural Basins (소하천(小河川) 유역(流域)의 잠재유출량(潛在流出量) 결정(決定))

  • An effort of preliminary type has been made to develope a practical method for the waterway area determination of a drainage outlet in rural or agricultural areas. The Seoul meteorological station was selected as tile index station, and the maximum rainfalls-duration-frequency (R-D-F) relation of short-time intense rainfalls was first established. A frequency analysis of the daily rainfalls for the 75 stations selected throughout the country resulted the 50-year daily rainfall for each station. The rainfall factor, which is defined here as the ration of 50-year daily rainfalls of individual station and the index station, was determined for the 8 climatological regions divided in this study. Following the US SCS method the runoff number of a watershed was given based on the soil type, land-use pattern, and the surface treatment. With this runoff number and the R-D-F relationship the runoff factors for the index station were computed and hence a nomogram could be drawn which makes it possible to determine the runoff factor for a given rainfall number and a rainfall of specific duration and frequency. With this done, the potential runoff of a watershed for a given rainfall duration could be calculated, based on the unit hydrograph theory, by multiplying the rainfall factor, the runoff factor, and the drainage area of the watershed under consideration. Then, the maximum runoff potential was determined by varying the rainfall duration and finding out the duration which results the peak discharge of a gived return period.

Usefulness of Preoperative Breast MRI in Breast Cancer Diagnosed After Excisional Biopsy (유방 절제 생검 후 유방암이 진단된 환자에서 수술 전 자기공명영상의 유용성)

  • Purpose : The aim of this study was to evaluate the usefulness of MRI performed after excisional biopsy to diagnose residual cancer and additional lesions. Materials and Methods : A total of 16 patients who had breast cancer diagnosed by excisional biopsy underwent preoperative breast MRI between March 2005 and August 2007 were included. MRI findings were considered positive for residual cancer if focal, thick, or irregular rim enhancement or adjacent enhancing lesion was identified around the postoperative biopsy cavity. And additional lesions separated from biopsy cavity including multifocal, multicentric, or contralateral lesion were evaluated. We evaluated the diagnostic accuracy of MRI, comparing MRI with histopathologic finding, and the impact of MRI on surgical planning. Results : The sensitivity and specificity of MRI for detecting residual disease considering rim enhancement were 85.7%(6/7) and 55.6%(5/9). Additional lesions including multifocal, multicentric, or contralateral lesion were found in 6 patients. In 7 patients, results of MRI findings changed surgical treatment planning. Conclusion : The pattern of rim enhancement on MRI after excisional biopsy is not differential point to evaluate remnant lesion. But MRI has an important role to help the detection of multifocal or multicentric, or contralateral breast malignancies, resulting in beneficial change in surgical treatment planning.

Frequency, Clinical Pattern and Outcome of Thrombosis in Cancer Patients in Saudi Arabia

  • Objectives: Thrombotic risk is increased in patients with cancer and there are important implications for those who suffer a venous thromboembolism (VTE). We undertook this study to determine the frequency, clinical patterns, and outcome of VTE in Saudi patients with cancer. Methods: Cancer (solid tumors and lymphoma) patients who developed VTE from January 2004 to January 2009 were studied retrospectively. Demographics and clinical characteristics related to thrombosis and cancer were evaluated. Results: A total of 701 patients with cancer were seen during the study period. VTE was diagnosed in 47 (6.7%) patients (median age 52, range 18-80 years). Lower limb DVT was the most common type, seen in 47% patients, followed by PE in 19%, and 19% patients had both DVT & PE. Thrombosis was symptomatic in 72% patients while it was an incidental finding on routine workup in 28%. Cancer and VTE were diagnosed at the same time in 38% of patients, and 47% patients developed VTE during the course of disease after the cancer diagnosis. The majority of VTE post cancer diagnoses occurred during the first year (median 4 months, range 1-14). Additional risk factors for VTE were present in 22 (47%) patients and 14 (30%) of these patients were receiving chemotherapy at the time of thrombosis. Only 5 (10.6%) patients were receiving thrombo-prophylaxis at the time of VTE diagnosis. Most common types of tumors associated with thrombosis were breast cancer, non-Hodgkin's lymphoma and lung cancer. The majority of the affected patients (79%) had advanced stage of cancer. After a median follow-up of 13 (range 0.5-60) months, 38 (81%) patients had died. There was no difference in the mortality of patients with symptomatic or asymptomatic thrombosis (82% vs 78.6%). Conclusions: Thrombotic complications can develop in a significant number of patients with cancer, and almost half of the patients have additional risk factors for VTE. Thrombosis is usually associated with advanced disease and can be asymptomatic in more than a quarter of cases. Thromboprophylaxis in cancer patients is under-utilized. Community based studies are needed to accurately define the extent of this problem and to develop effective prophylactic strategies.

Design of Smart Platform based on Image Recognition for Lifelog (라이프로그용 영상인식 기반의 스마트 플랫폼 설계)

  • In this paper, we designed a LBS-based smart platform for Lifelog service that can utilize the other's lifelog information. The conventional Lifelog service means that the system records the daily activities of the smart device user so the user can retrieve the early-recorded information later. The proposed Lifelog service platform uses the GPS/UFID location information and the various information extracted from the image as the lifelog data. Further, the proposed Lifelog platform using DB can provide the user with the Lifelog data recorded by the other service user. The system usually provide the other's Lifelog data within the 500m distance from the user and the range of distance can be adjustable. The proposed smart platform based on image recognition for Lifelog can acquire the image from the smart device directly and perform the various image recognition processing to produce the useful image attributes. And it can store the location information, image data, image attributes and the relevant web informations on the database that can be retrieved by the other use's request. The attributes stored and managed in the image information database consist of the followings: Object ID, the image type, the capture time and the image GPS coordinates. The image type attribute has the following values: the mountain, the sea, the street, the front of building, the inside of building and the portrait. The captured image can be classified into the above image type by the pattern matching image processing techniques and the user's direct selection as well. In case of the portrait-attribute, we can choose the multiple sub-attribute values from the shirt, pant, dress and accessory sub-attributes. Managing the Lifelog data in the database, the system can provide the user with the useful additional services like a path finding to the location of the other service user's Lifelog data and information.

Application-specific Traffic Generator (응용 프로그램의 특성 반영이 가능한 트래픽 생성기)

  • Integrating massive components and low-power policies have been actively investigated for system-on-chip designs. But in recent years, finding the optimal interconnection structure among heterogeneous components has emerged as a critical system design issue. Therefore, various simulation tools to model interconnection designs are being developed and performance evaluation of simulation is reflected in the real design. But most of the simulation environments employ traffic generation based on the mathematical probability functions, and such traffic generation cannot fully cover for various situations that may be occurred in the real system. Therefore, the demand for traffic pattern generation based on real applications is increasing. However, there have been few simulators that adopt application-specific traffic generators. This paper proposes a novel traffic generation method in simulating various interconnection structures for multi-processor system-on-chip design. The proposed traffic generation method can generate traffic patterns that can reflect the actual characteristics of the application and evaluate the performance of an interconnection structure under more realistic circumstance than traffic patterns using mathematical probability functions. By comparing the differences between the proposed method and the one based on mathematical probability functions, this paper shows advantages of the proposed traffic generation method.

MR Findings of Papillary Neoplasms of the Breast (유두 종양의 자기공명 영상소견)

  • Purpose : To review MR imaging finding of papillary lesion identified as additional suspicious lesion on MR image in women with biopsy-proven breast cancer and to evaluate upgrading rates after subsequent surgical histopathological diagnosis. Materials and Methods: Among 1729 preoperative MR image of women with biopsy proven breast cancer, US-guided CNB-proven 22 papillary lesions from 21 patients, which showed additional suspicious contrast enhancement other than index cancer on MR image, were subjected to the study. Some of these lesions underwent surgery, thus the comparisons between the histopathologic results were able to be compared to the results of US-guided CNB. Also retrospective analysis was done for MR findings of these lesions by BI-RADS MRI lexicon. Results: On MR imaging, 8 mass lesions, 7 non-mass lesions, 7 focus lesions were detected. All of the focus lesion (100%, 7/7) was diagnosed as benign lesion and showed plateau and washout pattern in dynamic MR image. After excisional biopsy, one of 9 benign papilloma (11.1%), 3 of 3 papillary neoplasm with atypia component (100%), 3 of 5 papillary neoplasm (60%) were upgraded to malignancy such as ductal carcinoma in situ (DCIS), invasive ductal carcinoma (IDC), invasive lobular carcinoma (ILC). Conclusion: The MR images of papillary lesions diagnosed by US-guided CNB exhibit no significant differences between malignancy and benign lesion. Also 41.2% of the lesion (7/17) was upgraded after subsequent surgery. Thus all of the papillary lesions require excisional biopsy for definite diagnosis and the MR imaging, it's just not enough by itself.
