"Study for the orthotics & shoes of diabetic among Korean adult" (한국 성인에서의 당뇨병성 족부 질환의 신발 및 족부 질환 연구)

  • Purpose: To provide baseline data for prevention and education of diabetic foot in Korea. Materials and Methods: 216 diabetic patients are included in this survey. The average age is 58.64 year old with average 9.92 years of diabetes mellitus periods. Among them, 41% used insulin injection, 42% oral medication, 14% both and 3% diet were used for their blood sugar control. The diabetic foot survey was done. Results: The survey of patients symptom and disease of their feet results: 49% had sensory decrease in their feet. Associated disease in foot were callus(15%), heel pain(10%), bunion(4%), toe deformity(2%), Tinea pedis were 9%, tingle sensation were 17%, and swelling on feet were 2%, The survey of patients privious history of foot problems are: 8% had previous operation history on their feet and 17% had history of infection on their feet, 23% had bullae history, The survey of patients prevention and education are: Those who have had more than one diabetic foot education were 45.7%, and among them 24% had routine regular foot check up. Only 5% wear special diabetic shoe and 3% had custom orthotics. Conclusion: Interests to diabetic foot care is increasing in diabetic patients in Korea. However still we need more education, and efforts for prevention. Special education and diabetic shoe is still low, The survey data could be good baseline for diabetic prevention and education.

A Survey Examining Satisfaction with Korean Medical Treatment in Pediatric Patients Recovering from Traffic-Accident Injuries (소아 교통사고 환자의 한방치료 만족도 조사)

  • Objectives This study aims at using information about treatment satisfaction obtained from surveying pediatric traffic-accident patients to guide future improvement in the quality of traditional Korean medical services. Methods The survey included 95 pediatric patients treated for traffic-accident injuries at a traditional Korean medical institution. The questionnaire was developed from a similar satisfaction survey for adult traffic-accident patients and approved by a traditional Korean pediatrician. The year-long survey ran from April 2020 to April 2021. The collected data were analysed using t-tests, one-way analysis of variance, and cross-tabulation. Results Of the pediatric traffic-accident patients surveyed, the highest percentage of children were aged between 1 and 7 years (70.5%). Just over half the patients (55.8%) complained of sleeping difficulties, and 54.7% exhibited symptoms of anxiety and fear. Almost all the patients surveyed (97.9%) reported an improvement in their symptoms after undergoing traditional Korean medical treatment. The degree of satisfaction with the treatment correlated with the alleviation of symptoms. Approximately 83.2% of respondents reported 'difficult-to-administer treatment for children' and rated their satisfaction degree comparatively lower than those patients with no reported treatment difficulties. The most struggling treatment was herbal medicine (42.1%), highlighting the need for improvement in the adaptability of herbal medicine to the treatment of pediatric patients. Conclusions Treatment acceptance by pediatric traffic-accident patients affects reported treatment satisfaction. It is important to improve treatment experience and adaptability to pediatric patients to improve the perceived quality of traditional Korean medical services.

Survey of Psychiatric Convalescent Facilities in The Busan area (부산지구 정신요양원 실태조사)

  • THE author conducted a field survey form Feb.1, 1973 to March 31, 1973 in the Busan area, standing six "institutions" where mentally ill patients are kept. These six institutions are registered at Busan Government, but are not regular hospitals. There suits of these investigation are as follows: \circled1 There are six institutions. which are registered at Busan City Government. All six are operated by layman who have little psychiatric knowledge and little human is tic motivation. \circled2 These institutions are allegedly to promote the welfare of the mentally sick. However they give little help for the patients. And they do not even have a resident medical person. The staff of those places will not accept professional medical help, and try to keep all information secret. \circled3 The finances are largely composed of patients′fees and partially from city′s help. The buildings are of high quality compared with the poor therapeutic situation. There also exist factors which make the patients worse, i,e. very small room space, compelling the patients to sit in a uteral position. This situation makes them worse and more autistic. \circled4 At the time of this survey, those lucrative overcrowded six institutions had 1.000 patients. At the same time the legitimate hospitals in Busan area had about 200 patients, \circled5 In my opinion, the City Government must take a more positive policy for the mentally ill patients, instead of the passive attitude which has held till now. And most of all, these patients must be medically and humanistically helped, instead of being left in a forgotten, depressed snake pit.

Estimation of Adverse Events Scale relating Herbal Medicine in Korea (우리나라의 한약 부작용 규모 추정)

  • Objectives : The purpose of study was estimation of adverse events [AEs] scale relating herbal medicine in Korea using Korean Medicine Utilization and Herbal Medicine Consumption Survey (National statistics No. 117087). Methods : Using microdata of Korean Medicine Utilization and Herbal Medicine Consumption Survey 2017, the number of inpatients and outpatients who experienced AEs was calculated. The microdata included AEs of all treatment methods that have been performed by visiting Korean medical institutions for one year, so set up the data into three models; model A (in case all treatments were only herbal medicine for one year), model B (in case herbal medicines were a part of all treatment methods in 1 year), model C (in case herbal medicines were a part of treatment methods at least one time in 1 year). The proportion of patients who experienced AEs during the last 1 year was calculated and then, the number of AEs relating herbal medicine was estimated. Results : A total of 1,010 outpatients and 904 inpatients were included in Korean Medicine Utilization and Herbal Medicine Consumption Survey 2017. The number of patients who had experienced AEs in the past 1 year was 0 in the model A, 9 in the model B (5 for outpatients, 4 for inpatients), and 19 in the model C (10 for outpatients, 9 for inpatients). By consideration for the complex sample survey, estimating the number of AEs relating herbal medicine, the model A was 0, the model B was 36,457 patients (0 to 75,526 patients), and the model C was 84,830 patients (26,314 to 143,347 patients). Conclusion : From the results of this study, it was possible to estimate the scale of AEs relating herbal medicines in Korea, suggesting that it is necessary to understand the actual condition of AEs and establish its management system.

Clinicopathological Features and Surgical Treatment of Gastric Cancer in South Korea: The Results of 2009 Nationwide Survey on Surgically Treated Gastric Cancer Patients

  • Purpose: In an effort to examine the clinicopathological characteristics of GC and the status of its surgical treatment, the Korean Gastric Cancer Association (KGCA) conducted a nationwide survey targeting surgically-treated gastric cancer patients in 2009. Materials and Methods: A standardized electrical case report was sent to every member institution of the KGCA via E-mail with detailed instructions regarding the survey data. Completed data forms were retrieved from each institution and analyzed by the KGCA information committee. Results: Data on 14,658 patients was collected from 59 institutions. The mean patient age was $59.2{\pm}11.9$ years with a male to female ratio of 2.05 : 1. Lower third cancer (56.0%) was the most common among all gastric cancers. The histological type revealed poorly differentiated adenocarcinoma (34.1%) to be the most common, and the Lauren classification revealed the intestinal type (50.0%) to be the most prevalent. Curative surgery was performed in 92.4% of patients with laparoscopic surgery in 25.8% of patients. A Billroth I reconstruction was performed most frequently after a distal gastrectomy (63.4% of distal gastrectomy). T1 cancers accounted for 57.6% of all cases, and 62.6% of patients showed no lymph node metastasis. Compared to previous reports, it was found that patients are becoming older, laparoscopic surgery is being performed increasingly, and the proportion of T1 cancer is increasing with time. Conclusions: This survey presented the clinicopathological characteristics and current status of the surgical treatment of gastric cancer in Korea. This survey is expected aid research studies as well as planning and evaluation programs targeting cancer control.

Associations of Socioeconomic Status With Depression and Quality of Life in Patients With Hypertension: An Analysis of Data From the 2019 Community Health Survey in Korea

  • Objectives: We aimed to identify the factors related to depression and quality of life in patients with hypertension by using multilevel regression analysis. Methods: In 2019, 229 043 participants in the Korean Community Health Survey were selected as the study group. Individual factors were identified using data from the 2019 Community Health Survey. Regional factors were identified using data from the National Statistical Office of Korea. Multilevel regression analysis was conducted to find individual and local factors affecting depression and quality of life in patients with hypertension and to determine any associated interactions. Results: As individual factors in patients with hypertension, women, those with lower education-levels, recipients of basic livelihood benefits, and those with poor dietary conditions showed stronger associations with depression and quality of life. As regional factors and individual-level variables in patients with hypertension, lower gross regional personal income, fewer doctors at medical institutions, and lower rates of participation in volunteer activities presented stronger associations with depression and quality of life. In addition, the associations of depression with gross regional personal income, the number of doctors at medical institutions, and dietary conditions were significantly stronger in patients with hypertension than in patients without hypertension. The associations of gender and employment status with quality of life were also significantly greater. Conclusions: Policy interventions are needed to adjust health behaviors, prevent depression, and improve quality of life for patients with hypertension, especially for those with the risk factors identified in this study.

Approaching Knowledge, Attitudes, and Practices Model for Elderly with Dementia Who are Suspected to Have Hearing Impairment in Korea

  • Because the psychophysical symptoms of hearing loss and dementia in the elderly are very similar, untrained healthcare professionals in dementia facilities can easily overlook a severity of hearing loss in their patients. The present study identifies their knowledge, attitudes, and practices (KAP) on hearing loss using a survey whether they may help hearing problem of the patients with dementia. A total of 29 health-care professionals responded to the KAP survey. Also, 2 family members participated. The results showed that most of the nurses and caregivers in elderly medical welfare facilities who worked with dementia patients did not have knowledge of their hearing loss. Even the facility managers did not know how to conduct hearing tests for their patients although they did recognize that some of their patients had a hearing loss. Eventually, actual practice was not possible at this moment by the professionals. However, our respondents did have a positive attitude toward screening for hearing loss and help their patients with dementia wear hearing aids if a clinical guideline was provided. We suggest to develop clear and precise clinical guidelines of the hearing screening test for the dementia patients due to the interrelationship between dementia and hearing loss. When these guidelines apply to elderly residents in a medical welfare facility, early diagnosis and treatment of their sensory loss will help alleviate their dementia as well.

A survey about experience of combined medications of Korean herbal drugs and Western drugs with outpatients in oriental and western hospitals (한.양방 병원 외래환자의 한.양약 동시복용 경험 실태조사)

  • The purpose of this survey was to find out the actual condition and the problems of combined medications of patients who have general diseases, respiratory diseases. The objects of this survey were 1,769 outpatients who visited 8 Oriental hospitals and Seoul national university hospital respiratory clinics. SPSS program was used to analyze the survey data. The mean age of patients was lower in oriental hospital than in western hospital. The monthly family incomes were higher in oriental hospital patients than in western hospital patients. The condition of patients was better in oriental hospital than western hospital and self-assessed health status was more positive in oriental hospital patients than in western hospital patients. The percentage of having experienced of treated in oriental and western hospital at the same time was 21% in western hospital patients and 45% in oriental hospital patients. In patients with respiratory diseases, the percentage of having taken combined medications was 40.2% in oriental hospital and 13.7% in western hospital. In patients with non-respiratory diseases, the percentage of having taken combined medications was 27.4% in oriental hospital and 8.3% in western hospital. And in the patients with two different diseases, the percentage of having taken combined medications was 27.9% in oriental hospital and 6.3% in western hospital. The percentage of having experienced synergistic effects was 33.0% in western hospital and 56.3% in oriental hospital. The percentage of side effects was 12.6% in western hospital and 12.2% in oriental hospital. This survey was meaningful in that it reported the actual condition of combined medications of patients who have general diseases, respiratory diseases. For the deep study on synergistic and side effects of combined medications, it is necessary to collect many cases of synergistic and side effects by doctors in western and oriental hospitals.

Study on Status Survey in Female Infertility Patients Admitted to Korean Medicine Hospital (한방병원에 내원한 구사(求嗣) 환자의 실태 분석 및 현황 조사)

  • Purpose: This study was to investigate status survey and cost of infertility patients in ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital. Methods: From January 2012 to June 2012, 171 new patients were admitted to ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital for treatment of infertility. We reviewed their medical records retrospectively and had telephone consultations to find out whether they were pregnant or not. Results: The mean age of outpatients was $32.72{\pm}4.08$ years and mean BMI of outpatients was $20.26{\pm}2.68$. 55.0% of patients who did not have childbirth or miscarriage. 82.5% of patients had normal menstrual period. 93.5% of patients visited OB/GYN, 33.3% were treated with ovulation induction, 18.7% underwent intrauterine insemination, and 18.1% underwent in vitro fertilization and embryo transfer. 99.7% were treated with acupuncture and moxibustion, 100% had taken Herbal Medicine. The mean treatment duration of outpatients was $8.84{\pm}8.17$ weeks, and the mean medical expense was $761,994{\pm}586,502$ won. It was found that 32.4% of patients were pregnant after treatment. Conclusions: We investigated status survey and cost of infertility patients. Further study about Korean medical treatments on infertility is required.
