Gracilis Muscle Free Flap Transplantation Followed Up Over 10 Years (10년 이상 추시된 박 근 유리 판 이식술)

  • Purpose: Gracilis muscle free flap transplantation is chosen in the medium sized soft tissue defect and bone exposure from trauma and chronic osteomyelitis in the lower extremity. We set a study to search for gracilis free flaps to know whether symptoms were cured or recurred in patients that have passed over 10 years from flap transplantation. Materials and Methods: From August 1995 through September 2010, we have performed 28 cases of gracilis muscle free flap in the lower extremities. Ever since no case visited to demand any discomfort, breakdown or recurrence in the flap site on outpatient basis. We made a telephone call to patients or relatives documented in the medical record and only 2 cases visited outpatient department and 9 cases postponed the visit who satisfied with the final result but 17 cases had wrong telephone numbers. Causes, area of lower extremity, recipient vessels in the lower extremity, condition of the donor thigh and overall satisfaction of the flap transplantation in activities of daily living were investigated and written down in the medical record. Results: 11 cases were reviewed after average postoperative 13.7 years. Gracilis muscle flaps were not break down at the recipient site in all cases. The wound of donor thigh wound healed good and overall activities of daily living was satisfied in all cases. Conclusion: Gracilis muscle flaps which had performed and followed up average 13.7 years revealed confident in the medium sized soft tissue defect and bone exposure from trauma and chronic osteomyelitis in the lower extremity.

Development and Long Term Evaluation of a Critical Pathway for the Management of Microvascular Decompression

  • Purpose: In order to provide a systematic and standardized treatment course for MVD patients, a critical pathway (CP) program was developed and the results of its long term application were analyzed. Methods: This was a methodological study. The CP was established and applied to 75 (step I) and 1,216 (step II). Another group of 56 with similar features was used as a control group. Results: The application of CP turned out to be useful in many regards: the rate of hearing loss was reduced from 1.8% to 0% (step I) and 0.5% (step II), and low cranial nerve palsy was reduced from 1.8% to 1.3% and 0.7%, respectively. The length of hospitalization decreased by 2.56 days (25.2%) for step I and 3.05 days (30.0%) for step II. Days of ICU stay were reduced by 7.9% and 1.8%. The total cost per patient was reduced by 14.8% (step I). The cost per day was increased by 13.7% and 52.4%. An increase in the patient satisfaction index was noted, as shown in the ICU information guide (p=.002). Conclusion: The development and application of CP was found to improve the quality of medical treatment and the efficacy of hospital management in MVD patients. Well organized and efficient system and multidisciplinary teamwork are the key component of the successful application of CP.

The Effect of Mild Cognitive Impairment Management Program on Traditional Korean Medicine in a Public Health Center (일개 보건소 한의약 경도인지장애 관리프로그램의 효과분석)

  • Objectives: The aim of this study was to evaluate the effect of a mild cognitive impairment (MCI) management program on traditional Korean medicine in a public health center. Methods: In order to examine the effect of the MCI management program, we investigated MCI patients who participated in the program. MCI patients were treated with Jodeung-san (JDS) or Dangguijagyag-san (DGJYS) for 6 weeks. Mini Mental State Examination-Dementia Screening (MMSE-DS), Geriatric Quality of Life-Dementia (GQOL-D), Geriatric Depression Scale in Korea (GDS-K), and biochemical examination of blood were conducted at intervals of 6 weeks. A satisfaction survey was collected after this program. Results: MMSE-DS mean score increased significantly, by 4.09 (p=0.000) in the JDS group and by 4.15 (p=0.000) in the DGJYS group. GQOL-D mean score also increased significantly, by 5.90 (p=0.000) in the JDS group and by 5.07 (p=0.000) in the DGJYS group. GDS-K mean score decreased significantly, by 10.63 (p=0.000) in the JDS group and by 9.69 (p=0.000) in the DGJYS group. MMSE-DS, GDS-K and GQOL-D scores were not significantly different between the JDS and DGJYS groups. All of the participants were satisfied with the program, and 90% of the participants expressed a high intention to participate in the program again. Conclusions: Both Jodeung-san and Dangguijagyag-san treatment showed improvement on cognitive function, depression and quality of life in this program. Additional studies with more subjects will be required to prove credibility and validity more clearly.

A survey of leisure participation and leisure exploration of community dwelling stroke patients (지역사회 거주 뇌졸중 환자의 여가 참여 및 여가 탐색 조사)

  • The purpose of this study was to provide basic information on leisure participation and leisure exploration of community-dwelling stroke patients through a multi-dimensional leisure participation evaluation tool. The subjects in this study were 35 people living with stroke in the community. As a result of study, the most involved leisure activities were in order of watching TV, communicating, and exercise alone. Most of the satisfaction level of leisure participation was 7-10 points, but exercise alone was 5.2 points due to the purpose of health management. Leisure exploration was the most frequent exercise alone and sports, and interest was 8-10 points. The most important obstacles for leisure participation were physical problems depending on the nature of the stroke disease. Through the results of this study, it is necessary to activate a stroke patient's discharge plan and activate a community stroke patient's leisure intervention program so that they can participate in leisure activities of their choice through voluntary leisure exploration.

The Comparative Study of Effectiveness of Cotreatment with $Samgieum-gagam$, Acupuncture and Sweet Bee Venom Acupuncture on Post-stroke Shoulder Pain (삼기음가감방과 봉약침, 체침의 병행치료가 뇌졸중 후 견관절 통증에 미치는 효과 비교연구)

  • Objectives : The purpose of this study is to evaluate the effect of cotreatment with $Samgieum-gagam$, acupuncture and sweet bee venom acupuncture on post-stroke shoulder pain. Methods : The subjects in this study were 43 patients with post-stroke hemiplegic shoulder pain. These patients were randomly divided into four groups : the group treated with acupuncture, the group cotreated with acupuncture and sweet bee venom acupuncture, the group cotreated with acupuncture and $Samgieum-gagam$, the group cotreated with acupuncture, sweet bee venom acupuncture and Samgieumgagam, They were treated for 4 weeks, and the effectiveness was assessed by visual analogue scale(VAS), Fugl-Meyer motor assessment(FMMA), painless passive ROM of shoulder external rotation(PROM), Modified Ashworth sacle(MAS) and Satisfaction. Results : All groups showed significant change in VAS, FMMA and PROM. The group cotreated with acupuncture, sweet bee venom acupuncture and $Samgieum-gagam$ showed more effectiveness in VAS and FMMA than the group treated with acupuncture. There was no significant difference in MAS among groups. Conclusions : This study suggests that cotreatment with $Samgieum-gagam$, acupuncture and sweet bee venom acupuncture is more effective than acupuncture treatment only on post-stroke shoulder pain. Further study based on many other combination methods, larger population, and long term follow-up is needed to confirm this suggestion.

The Clinical Effects of Acupuncture and Acupotomy Therapy for HIVD (요추 추간판 탈출증 환자의 침도요법의 효과에 대한 임상적 고찰)

  • Objectives : The study on Acupotomy therapy has recently become a popular academic field as proven to be an great alternative to the limitation of Western medical treatment. However, there has been little study examining acupotomy therapy on treating HIVD. Thus, this study aims to investigate the sense of improvement and satisfaction from the HIVD patients treated with acupotomy therapy. Methods : A total of 63 subjects that consists of HIVD patients were participated in this study. The experimental group(33subjects) was treated with acupotomy therapy and acupuncture, and the control group (30subjects) was treated with acupuncture. All the subjects were asked to answer VAS(visual analogue scale) and ODI(Oswestry disability index) before and after the treatment. Results : The results of the VAS and ODI of comparison analysis between two groups, show that the improvement of HIVD in acupotomy and acupuncture treatment group is more effective than that in acupuncture treatment group

The Effect of CF Herbal Acupuncture by Oswestry Disability Index (오스웨스트리 장애지수를 이용한 홍화약침(CF)의 효과)

  • Objective: The aim of this study was to assess the effect of CF herbal acupuncture for low back pain by Oswestry Disability Index Methods: Ten patients with back pain were selected and CF herbal acupuncture was administered one time per 5 days after admission. The degree of improvement was evaluated by Oswestry Disability Index and visual analogue scale(VAS). Oswestry Disability Index consisted of eleven items and they were scored as 5 or 6 points per one item. Results: All of the 10 patients, after CF herbal acupuncture, showed decreased score by Oswestry Disability Index and VAS. It means that the patient's satisfaction increased after treatment. Conclusion: These results suggest that the CF herbal acupuncture was effective for low back pain

Surgical Treatments and Clinical Outcomes for Idiopathic Osteoarthritis of the Tarsometatarsal Joints (족근 중족 관절의 특발성 골관절염에 대한 수술적 치료 및 임상적 결과에 대한 분석)

  • Purpose: The purpose of the study was to identify the subtypes of idiopathic osteoarthritis of the tarsometatarsal joints based on accompanying hindfoot, midfoot, or foot deformities and their corresponding surgical options and also to evaluate the overall clinical results. Materials and Methods: The study included 59 patients (67 feet) with idiopathic tarsometatarsal joint osteoarthritis. Tarsometatarsal fusion was performed for tarsometatarsal joint and accompanied secondary change was divided into subtypes and various bony reconstruction was carried out. The patients were evaluated with the AOFAS midfoot score and FFI. The average patient age was 60.2 years with 40.6 months follow-up. Fifty-four feet (80.6%) had been treated with realignment fusion. Twenty-six feet had first and second tarsometatarsal joint fusion, and 20 feet had first tarsometatarsal fusion only. Six subtypes were identified based on associated foot deformities: 1) in-situ without deformities (18%), 2) pes planovalgus (45%), 3) rockerbottom (15%), 4) cavus foot (1%), 5) hallux valgus (12%), and 6) hallux valgus with pes planovalgus or rockerbottom (9%). Plantar-medial closing-wedge resection was used in 10 feet to correct rockerbottom. For pes planovalgus, a medial sliding calcaneal osteotomy was done. Lateral column lengthening with medial sliding calcaneal osteotomy was done for severe pes planovalgus, and triple arthrodesis was done for rigid pes planovalgus. Hallux valgus was corrected with the Lapidus procedure (85.7%). Results: AOFAS midfoot scores improved from preoperative 34.1 points to postoperative 83.9 points (p<0.05). The Foot Function Index postoperatively also showed significant improvement (p<0.05), with a high satisfaction rate (86.6%). There were 29 complications, most commonly sesamoid pain. Conclusion: Idiopathic tarsometatarsal OA feet can be classified into six categories. Pes planovalgus feet should be treated with medial sliding calcaneal osteotomy, lateral column lengthening, or triple arthrodesis in addition to tarsometatarsal joint realignment fusion. Rockerbottom and hallux valgus deformities should also be addressed.

The Results of Triple Osteotomy in Adult Hallux Valgus Patients with Highly Increased Distal Metatarsal Articular Angle (고도의 원위 중족골 관절면 각을 동반한 성인 무지 외반증 환자에서의 삼중 절골술의 결과)

  • Purpose: To evaluate the clinical and radiographical results of triple osteotomy as a treatment for adult hallux valgus with highly increased distal metatarsal articular ankle (DMAA). Materials and Methods: From October 2003 to April 2005, we retrospectively reviewed 7 hallux valgus patients (3 cases: moderate, 4 cases: severe) treated with triple osteotomy and followed-up for more than 1 year after operation. The mean follow up was 15.1 months. The hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA) and the length of 1 : 2 metatarsal bone were measured. Proximal chevron osteotomy and distal biplanar chevron osteotomy were done in 1st metatarsal bone. Akin osteotomy was added to the base of the proximal phalanx. The clinical result was assessed using the AOFAS Hallux score, tenderness on the medial eminence, ROM of 1st metatarsophalangeal joint, calluses and patient satisfaction. Results: The mean HVA and IMA was improved from $37.5^{\circ}$ and $13.4^{\circ}$ to $10.5^{\circ}$ and $6.2^{\circ}$ respectively. The mean DMAA was corrected from $34.2^{\circ}$ to $11.2^{\circ}$ and mean shortening of 1st metatarsal was 2.4 mm (0.9-5.8 mm). The mean AOFAS hallux score was improved from 66.4 to 92.5 and VAS score (pain on the medial eminence) from 4.3 points to 0.4 points. Metatarsalgia disappeared in all cases and there was no complications such as necrosis of the metatarsal head. Conclusion: Triple osteotomy for adult hallux valgus with a highly increased DMAA is effective and should be considered as a part of the treatment armamentarium.

Treatment of Moderate Hallux Valgus with Proximal Chevron Metatarsal Osteotomy and Distal Soft Tissue Procedure (근위 중족골 갈매기형 절골술과 원위 연부조직 교정술을 이용한 중등도 무지 외반증의 치료)

  • Purpose: The authors intended to analyze the operative results of moderate hallux valgus with proximal chevron metatarsal osteotomy and distal soft tissue procedure. Materials and Methods: Seventy feet of fifty-seven patients were followed for more than 1 year after the proximal chevron metatarsal osteotomy. The mean age was 47.2 years, and the mean follow up period was 2 years and 3 months. Clinically preoperative and postoperative AOFAS MP-IP scale and satisfaction after the surgery were analyzed. Radiologically hallux valgus angle, hallux valgus interphalangeal angle, the intermetatarsal angle and sesamoid position before and after the operation were analyzed. Results: Additional Akin osteotomy was performed 48 out of 70 feet. Clinically AOFAS MP-IP scale was increased from 60.4 points preoperatively to 89.8 points postoperatively. Ninety-four percents of the patients were satisfied with the results. Radiologically hallux valgus angle was decreased from $34.8^{\circ}$ preoperatively to $12.8^{\circ}$ postoperatively. The intermetatarsal angle was decreased from $15.7^{\circ}$ preoperatively to $8.0^{\circ}$ postoperatively. Hallux valgus interphalangeal angle was increased from $7.4^{\circ}$ preoperatively to $9.8^{\circ}$ postoperatively. There were 3 recurrences, 1 hallux varus and 3 minor wound infections. There were no nonunion or malunion of the 1st metatarsal. Conclusion: Proximal chevron metatarsal osteotomy with distal soft tissue procedure and additional Akin osteotomy appears to be safe and satisfactory procedure.
