Validation of Fall Risk Assessment Scales among Hospitalized Patients in South Korea using Retrospective Data Analysis (후향적 자료분석을 통한 낙상위험 사정도구의 타당도 비교: 종합병원 입원 환자를 중심으로)

  • Purpose: The purpose of the study was to validate fall risk assessment scales among hospitalized adult patients in South Korea using the electronic medical records by comparing sensitivity, specificity, positive predictive values, and negative predictive values of Morse Fall Scale (MFS), Bobath Memorial Hospital Fall Risk Assessment Scale (BMFRAS), and Johns Hopkins Hospital Fall Risk Assessment tool (JHFRAT). Methods: A total of 120 patients who experienced fall episodes during their hospitalization from June 2010 to December 2013 was categorized into the fall group. Another 120 patients, who didn't experience fall episodes with age, sex, clinical departments, and the type of wards matched with the fall group, were categorized to the comparison group. Data were analyzed for the comparisons of sensitivity, specificity, positive and negative predictive values, and the area under the curve of the three tools. Results: MFS at a cut-off score of 48 had .806 for ROC curves, 76.7% for sensitivity, 77.5% for specificity, 77.3% for positive predictive value, and 76.9% for negative predictive value, which were the highest values among the three fall assessment scales. Conclusion: The MFS with the highest score and the highest discrimination was evaluated to be suitable and reasonable for predicting falls of inpatients in med-surg units of university hospitals.

A Study on Personality Traits of the Patients with TMJ Dysfunction through the MMPI (MMPI를 중심으로 한 악관절 기능장애 환자의 인성에 관한 연구)

  • Personality characteristics of TMJ dysfunction patients was studied by means of MMPI test. 100 TMJ dysfunction patients and 100 healthy controls were subjected at the Department of Oral Diagnosis and Oral Medicine, Pusan National University Hospital, during the period from March 1983 to February 1985. All the patients were divided into 2 groups namely, acute group and chronic group by duration of path and discomfort problems for 6 months or longer. The obtained results were as follows : 1. Mean value of T-scores on each clinical scale was within normal range in both patient and control group. 2. There was significant difference in scales of Hs, D. Hy, Pt, Sc and IR between patient and control group. 3. As compared with control group by sexual subclass, scales of Hs, Hy and Pt of both sex, scales of Sc and M-DI of male and scales of D of female in patient group were significantly higher than those in control group 4. The longer was duration of TMJ problems, i.e., in increasing order of control, acute and chronic group, the higher was mean value of T-scores on scales of Hs, D, Hy, Pt, Sc, IR and M-DI. 5. Mean values of T-scores on scales of Hs, D and Hy were the highest in all the clinical scales of total class and subclasses(i. e. male group, female group, acute group and chronic group) with TMJ dysfunction and showed 1-:i-2 (Hs, -Hy, -D. ) profile pattern, conversion "V.ot;V.

A case report of monitoring PSA level changes in two prostate cancer patients treated with Mountain Ginseng Pharmacopuncture and Sweet Bee Venom along with western anticancer therapy (양방 항암치료와 병행하여 산삼약침과 봉약침 치료를 받은 전립선암 환자들의 PSA 수치 변화 관찰 보고)

  • Objectives: The purpose of this report is to find out how Mountain Ginseng Pharmacopuncture(MGP) and Sweet Bee Venom(SBV) treatments are effective on prostate cancer patients by monitoring Prostate specific antigen(PSA) values. Methods: We treated two prostate cancer patients with MGP and SBV from October 2008 to April 2011. One patient had localized prostate cancer, the other was in the terminal stage of prostate cancer with lung and bone metastasis and both had been receiving western anticancer therapy. We had monitored the changes of PSA value. Results: In case 1, MGP and SBV treatments seemed to be helpful in preventing the recurrence of localized prostate cancer. In case 2, PSA value was decreased by MGP treatment. Conclusions: It is conceivable that MGP and SBV are effective treatments for patients with prostate cancer.

Comparison of Nursing Activity Costs of Chronic Otitis Media Surgery Patients among Time, RBRVS, and CP (상대가치, 소요시간, 표준임상경로지에 의한 만성중이염수술 간호원가 비교분석)

  • Purpose: The purpose of this study was to compare the cost using different methods of costing nursing activities for patients with chronic otitis media having surgery. Method: Data were collected from 30 patients who had mastoidectomy and tympanoplasty. To compare the cost, the researchers used three different costing methods; consumed time, resource based relative value scale (RBRVS), and critical pathway (CP). Results: Twenty-six nursing activities for surgical patients with chronic otitis media were found. Total cost was 83,843.7 won using RBRVS. The costliest activity was recording at 9,734.4 won, followed by confirmation of doctors' orders at 9,302.4 won, and injection with infusion pump at 9,072.0 won. There was a difference in nursing activities performed according to the length of hospital stay, and the cost was highest on the surgery day at 13,417.8 won. Comparatively, the total cost was 72,014.4 won using CP. Conclusions: Nursing activities are performed in various forms according to the disease and patient's condition, and different nursing activities are executed according to the length of hospital stay. In order to measure the load of nursing activities and distribute it appropriately, it is necessary to analyze the cost of nursing activities by the process of nursing services performed.

Prognostic Value of Preoperative Serum CA 242 in Esophageal Squamous Cell Carcinoma Cases

  • Purpose: Carbohydrate antigen (CA) 242 is inversely related to prognosis in many cancers. However, few data regarding CA 242 in esophageal cancer (EC) are available. The aim of this study was to determine the prognostic value of CA 242 and propose an optimum cut-off point in predicting survival difference in patients with esophageal squamous cell carcinoma (ESCC). Methods: A retrospective analysis was conducted of 192 cases. A receiver operating characteristic (ROC) curve for survival prediction was plotted to verify the optimum cuf-off point. Univariate and multivariate analyses were performed to evaluate prognostic parameters for survival. Results: The positive rate for CA 242 was 7.3% (14/192). The ROC curve for survival prediction gave an optimum cut-off of 2.15 (U/ml). Patients with CA 242 ${\leq}$ 2.15 U/ml had significantly better 5-year survival than patients with CA 242 >2.15 U/ml (45.4% versus 22.6%; P=0.003). Multivariate analysis showed that differentiation (P=0.033), CA 242 (P=0.017), T grade (P=0.004) and N staging (P<0.001) were independent prognostic factors. Conclusions: Preoperative CA 242 is a predictive factor for long-term survival in ESCC, especially in nodal-negative patients. We conclude that 2.15 U/ml may be the optimum cuf-off point for CA 242 in predicting survival in ESCC.

Hemiplegic gait : comparison of kinematic variables related to Bait speed (편마비 보행 :속도에 따른 관절 운동학적 변수 비교)

  • In hemiplegic gait, walking speed is an important factor to evaluate treatment effect. The purpose of this study was to describe and compare kinematic variables during differant speed hemiplegic gaits. Six hemiplegic patients(47-69 years old) after stroke and age - matched six volunteers in good health(51-61 years old) were studied. The patients were sorted into two groups, depending on their self - speed of walking : fast speed group(3 patients, $0.74\pm0.14m/s$) and slow speed group(3 patients, $0.29\pm0.09m/s)$. The results were following. 1. In the hip joint, the fast group had lower mean value than normal but had similar pattern to normal. The slow group had continuous flexed pattern. 2. In the knee joint, the fast group had similar mean value and pattern to normal. The slow group had continuous flexed pattern. 3. In the ankle joint, the two group had dorsiflexed pattern. The fast group had similar pattern to normal. Thus, the fast group was similar gait pattern to normal.

Clinical Efficacy of External Preparation Containing Herbal Extracts m Atopic Dermatitis Patients (아토피 피부염 환자에서 한방 추출물이 포함된 외용제품에 대한 임상 효과)

  • Background : Atopic dermatitis is a chronic and recurrent allergy eczema, called as 'Tae-yul'. It is a characteristic dermatitis, diagnosed from baby to adult, that carries severe itching. Objectivd : The aim of this trial is to examine the effect and safety of Medibebe in patients with atopic dermatitis. Methods : We made a comparison between before and after the use this product. Volunteers who satisfied the requirements were enrolled in the study. Degrees of severity of atopic dermatitis were measured by SCORAD index. Results : After 4 weeks' use of Medibebe, SCORAD index showed a decrease of $20.0{\pm}10.2$(95% C.I: 17.1-22.9) compared with when it was not applied, which is a statistically meaningful change(p-value<0.0001). Subjective pruritus and Sleeping loss reported after both 2 and 4 weeks' use of Medibebe decreased in a statistically meaningful way(p-value<0.0001) compared with before the application. but, 2 cases out of all 50 subjects for the clinical trial were excluded on the way. Conclusion : As a result of applying the 4 items of Medibebe(cream, lotion, soap, and body cleanser) to patients with atopic dermatitis and watching the progress, it is concluded and considered that these cleansing and moisturizing products can be used by atopic patients safely and effectively with almost no side-effect.

Expression and Prognostic Value of Matrix Metalloproteinase-7 in Colorectal Cancer

  • The purpose of this study was to evaluate expression and prognostic value of matrix metalloproteinase-7 (MMP-7) in colorectal cancer (CRC) patients. CRC tissues and corresponding distal normal mucosa tissues of 118 CRC patients were assessed by immunohistochemistry. Correlations between MMP-7 expression, patients' clinic pathological features, and overall survival rate were analyzed. We found that positive expression of MMP-7 in CRC tissues was significantly higher than that in distal normal mucosa (61.0% vs. 39.8%, p =0.001). Poor histological differentiation, advanced clinical stage and lymph node metastasis were significantly correlated with MMP-7 expression in CRC. The overall survival rate was significantly higher in the MMP-7 negative group than the positive group (Log-rank test= 9.957, p= 0.002). MMP-7 appeared as a significant independent prognostic factor through multivariate survival analysis. Collectively, we found MMP-7 expression to be correlated with progression and metastasis of CRC and thus could be used as a predictive marker of prognosis in CRC patients.

Making inferior alveolar nerve block more comfortable via computer-controlled local anesthetic delivery: A prospective clinical study

  • Background: The fear of needle insertion and pain during anesthesia is a source of patient dissatisfaction in dentistry. Inferior alveolar nerve block (IANB) remains the most common type of block and is in itself painful. Computer-controlled local anesthetic delivery (CCLAD) has been proven to reduce the pain associated with injection of anesthetics in various blocks. However, the efficacy of CCLAD for IANB in adults remains unknown. Methods: Sixty-four adult patients requiring bilateral IANB were selected and divided into two groups: group A (50 patients receiving IANB via CCLAD) and group B (50 patients receiving IANB using a conventional cartridge syringe). Pain perception and patient comfort were assessed using the visual analog scale and the 5-point semantic scale, respectively. Results: The pain perception was compared between the two groups using the Mann-Whitney U-test, and the P value was 0.003. The patient comfort was also compared using the same test, and the P value was 0.484. Conclusion: A significant difference was observed in the pain perception of the patients during CCLAD. The patient comfort was grossly equal for both techniques.

Acute Stress Disorder Patients Treated with Yanshimtang-Gamibang: Three Case Reports (급성 스트레스 장애에 대한 양심탕가미방(養心湯加味方) 치험 3례)

  • Objectives: The purpose of this case report was to propose Yangshimtang-Gamibang on three patients diagnosed with acute stress disorder. Methods: Yangshimtang-Gamibang was treated on three patients diagnosed with acute stress disorder after experiencing a traffic accident. The effects of Yangshimtang-Gamibang were evaluated through BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), respectively, and quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there was significant decrease in the value of BAI score, a measurement of anxiety, as well as the value of NRS, NDI, ODI, a measure of pain. Also, the sleep quality of three patients improved. Conclusions: The results indicate that Yangshimtang-Gamibang may have an effect on people with acute stress disorder who recently experienced a traffic accident.
