The Effect of Task-oriented Arm Movements and Muscle Enhancement Program Using Elastic Bands on Upper Limb Muscle Strength and Activities of Daily Living of Mitochondrial Myopathy Patient -Single subject design- (Mitochondrial Myopathy 환자에서 과제지향적 상지운동과 탄성밴드를 이용한 기능적 근력증진 프로그램이 상지근력과 일상생활활동에 미치는 영향 -단일사례연구-)

  • Purpose : The purpose of this study was to the effect of task-oriented arm movements and muscle enhancement program using elastic bands on limb muscle strength and activities of daily living of mitochondrial myopathy patient. Method : Single-subject experimental research design was applied to. AB Design was adopted. The study period was approximately four weeks. A baseline period of the three sessions of the experiment, the treatment period B, 3 sessions were conducted. Baseline period to observe the patient's daily life bardel index was measured as an independent feature, MMT as a limb muscle strength was assessed by measuring early. During the period of treatment with serabaendeu limb strength training 30 minutes after the break five minutes after the treatment using MMT limb muscle strength were evaluated. Task-oriented exercise program, and who exercise a week as a treatment was carried out in 30 minutes. Result : All of the scores for each sessional period of treatment when compared to base line and upper limb muscle strengthening exercises on the subjects that did not change significantly. Conclusion : If the muscles and nervous system involvement in patients with symptoms such as muscle weakness and paralysis of upper extremity functional use is difficult.

A Case Study on User Evaluation of Patients' Room for User-Oriented Design - Focused on H General Hospital in Busan - (사용자 중심 디자인을 위한 다인병실에 대한 사용자 평가연구 - 부산 H 종합병원 사례를 중심으로 -)

  • This study was carried out to focus on the user-oriented design of patients' room in general hospital. The purpose of the study was to examine the users' evaluation on their patients' rooms in H General Hospital and to suggest how to improve the patients' room design. The users were inpatients, care givers, and nurses, 300 subjects of each 100 users in H hospital. They evaluated 26 items related to the patients' room design using by 5-points Likert scale. Also, they answered three elements which bothered them, what needed improvement, and what they feel ill at ease. The results were as followings; 1) The users evaluated positively the location and size of window. This implies that the current one is relatively ideal and deserve to be maintained. 2) The users responded negatively to the thermostat system and the space for care-givers. 3) The lack of space for care-givers and visitors, common use of refrigerator and bathroom, and the lack of space bothered the users. 4) Most users mentioned the lack of space in patients' room. This implies that it needs to consider the change of the person number per patient's room, from 5 persons to 4 ones. 5) The lacks of sleeping space, dining space, and bathroom space bothered the care-givers. This implies that the space for care-givers such as sleeping, dinging, and bathing should be considered in patients' room.

  • In this paper, we present the design and implementation of the interbed network communication protocol, which links patient monitors, central stations, DB servers, and clinical workstations together in a patient monitoring system. We describe the requirements to be met thor real-time patient monitoring, propose 2 services Patient Locator Service(PL:7) and Remote Patient Monitoring Service( RPMS). PLS provides the information about how many patients are currently being monitored and where they are located, while RPMS allows the doctors to monitor their patients'vital sign in real-time. The messages for the services, their formats and exchange scheme are also presented with a whole picture of how they are implemented. We adopted the object-oriented programming paradigm in all the analysis and design processes. In the experiment performed in a real clinical setting, the services turned out to meet all the requirements needed for real-time patient monitoring.

User Evaluation for User-Oriented Children's Hospital Design - Focused on the Mothers of Child Inpatients and the Nurses - (사용자 중심의 어린이전문병원 계획을 위한 사용자 평가 - 어린이 환자의 보호자와 간호사를 중심으로 -)

  • This study aims to design user-oriented children's hospital by examining the user needs, especially the mothers of child inpatients and the nurses. The subjects participated in a questionnaire survey including the user's characteristics, awareness of patient's room, the preference on ward size, demands, satisfaction and hospital environment assessment. The survey was conducted of the mothers of child inpatients and the nurses in A children's hospital, and the data were analyzed by the SPSS WIN 18.0 Version software. A total of 115 copies of the questionnaire were finally analyzed. The results and conclusions are as follows. 1) The mothers' demand on family-centered space and private spaces were higher than the nurses. 2) The mothers preferred 1-bed-room to 4-bed-room due to safety and privacy. 3) The items of hospital environment assessment was categorized into four factors; 'Aesthetics', 'Accessibility', 'Comfort', and 'Lighting'. The most positive factor was 'Aesthetics', whereas 'Comfort' was the negative factor. 4) The mother's satisfaction was lower than the nurse's. The most influential factors on the satisfaction was 'Accessibility'. To improve the satisfaction of the mothers of child inpatients and the nurses, safety, privacy, accessibility should be considered.

The Effects of Task-Oriented Training for Left Trunk Flexion Pattern Using Real-Time Ultrasound Imaging -A Single-Subject Experimental Study- (편측무시 환자에게 실시간 초음파 영상을 이용한 왼쪽 몸통 굽힘 패턴을 적용한 과제지향적 훈련의 효과 -단일 사례 연구-)

  • Purpose: This study investigates the effects of task-oriented training for a left trunk flexion pattern using real-time ultrasound imaging in a stroke patient with unilateral neglect symptoms. Methods: This study used the ABA experimental design, which is a single-subject research method among individual case research methods. For the ABA experimental design, changes in the degree of unilateral neglect, balance ability, and the thickness of the lateral abdominal muscle were visually analyzed during the baseline process, in the intervention period, and after the intervention. The experiments were performed 24 times in total for 8 times in each of the 3 periods. The unilateral neglect was measured using the Albert test, balance ability was measured using the Berg balance test, and the thickness of the lateral abdominal muscle was measured using ultrasound imaging. The subject was a 50-year-old male patient with unilateral neglect caused by right cerebral hemorrhage. He performed task-oriented training for a voluntary left trunk flexion pattern using real-time ultrasound imaging during the intervention period. Results: The result of comparing the data collected during the intervention period with the data point average of the baseline process showed that balance ability improved and the tendency line was above the baseline. The tendency line of unilateral neglect was below the baseline and showed a decreasing tendency. The thickness of the lateral abdominal muscle showed an increasing trend and the tendency line was above the baseline. Conclusion: The results of this study suggest that the task-oriented training for left trunk flexion pattern using real-time ultrasound imaging has a beneficial effect on balance ability, the degree of unilateral neglect, and the strength of the lateral abdominal muscle in unilateral neglect patients.

Study on Clinical Patient-oriented Education System for Medical Organizations (의료기관에서의 환자 중심 교육 시스템 연구)

  • Recent healthcare studies evaluate the effective impacts of a patient educational information intervention in hospitals on adherence to communicating knowledge, life quality, and therapeutic response in patients treated with highly active therapy in medical organizations. Therefore, the aim of this study is to design and implement the effective and systematic frameworks by providing clinical patient-oriented education system(CPES), which enhances health promotion and medical cures. CPES initiatives accelerate positive financial returns with network-driven educational system. To validate the CPES, the author analyzes the efficiency and effectiveness using statistical survey works. With this outcome, it is compared with previous system. One of the findings is that the system can improve compliance with treatment right dietetics and give patients self-manage their conditions in S hospital. Patients' acknowledge of their condition and its treatment process can be improved through the educational systems.

  • In this paper, we present an object-oriented stereotactic radiosurgery planning system, which accepts medical images such as CT and angiography, transforms the coordinates to a reference frame coordinate, calculates dose distributions, and finally displays isodose curves over the images. The user finds an adequate one for radiosurgeries after performing computer simulations on different treatment parameter sets. The object-oriented design concept was fully applied to the system composed of seven manager objects of different classes: a patient information manager, a user-interface manager, a coordinate transformation manager, a blackboard manager, a dose calculation manager, an isodose curve display manager, and a report manager. All the user interactions are carried out through the use of mouse buttons. The performance of the system was verified by four physicians and two medical physicists, and now is being used in two clinical sites.

Effects of Task-Oriented programme on Balance and Upper Extremity in stroke (과제 지향적 프로그램이 뇌졸중환자의 균형과 상지기능에 미치는 효과: 단일사례연구)

  • Objective : The purpose of this study is to assess effects of task oriented programme on balance and upper extremity function in stroke patient. Methods : The subject is right hemiplegia with stroke and participated for 6 weeks. The design of study ABA designed of single subject research. The baseline phase consisted of 5 sessions. The intervention phase consisted of 15 sessions and performed task oriented training. The Maintenance phase consisted of 5 sessions. An assessment of upper extremity function was made using a Manual Function Test(MFT) and balance were assessed by the Time Up and Go test(TUG), Nintendo Wii Balance Board Test. Results : Dynamic balance improved from 51.53 to 42.62. At baseline, difference between left and right of the static balance was an average of 13.8, After treatment, the static balance difference was 6.29 and It was closer to zero than baseline, so static balance has been stable feature. Also, MFT point increased from 6 to 7.6. Conclusion : Task oriented programme can useful in balance and upper extremity function in stroke patient. Further studies need to generalize these findings.

Education that allows South Korean Colleges of Dentistry to teach Emergency Care

  • As the medical environment and dental services change, the importance of educating dentists in responses to systemic emergencies is increasing. The current student-oriented education paradigm is moving towards training students in the abilities required to address the daily crises they will face, while also providing them with the ability to deliver knowledge. Before addressing a patient's situation, emergency physicians begin by diagnosing symptoms. As they must decide on the tests and treatments that are immediately required and must solve problems through interdisciplinary treatment, emergency physicians require additional skills and communication abilities besides clinical knowledge. Since dentistry colleges provide education that emphasizes the skills dentists require to treat oral diseases, they do not have sufficient time to teach emergency care. Additionally, because their professors lack expertise in pedagogy, dental students also have insufficient motivation to study the pathophysiology of systemic diseases. This review proposes a direction of teaching that can help dental students recognize problems and situations in emergency cases and that can help them develop their capability to immediately make a decision and resolve the problem. To do this, the author surveyed the educational philosophy and knowledge provided in the instructional design of clinical professors who give lectures on emergency care, and also examined the teaching methods of the learner-oriented education paradigm.

The effects of EMG-triggered functional electrical stimulation on upper extremity function in stroke patients

  • Objective: The aim of this review is to explore the latest intervention trends and effects of EMG-triggered functional electrical stimulation on the upper extremity functions in stroke patients. Design: Systematic review on clinical trials. Methods: A systematic literature search was performed to identify clinical trials evaluating the effects of EMG-triggered functional electrical stimulation (EMG-FES) and task-oriented EMG-triggered FES on the hand functions in stroke patients. Literature review was conducted with the following key words: hand function, functional electrical stimulation, task-oriented, stroke. Results: Ten clinical trials were included; 8 of them were randomized controlled trial, 1 was block-randomized, and 1 was a pre-post comparison study. A positive effect of electrical stimulation was reported in the patient groups that were treated with functional electrical stimulation combined with specific tasks, and volitional muscle contraction-triggered stimulation that was synchronized with tasks. Motor capabilities of the hand and arm were improved after the rehabilitation. Conclusions: EMG-triggered electrical stimulation may be more effective than non-triggered electrical stimulation in facilitating the hand functions in stroke patients in terms of muscle strength and voluntary muscle contraction of the paretic hand and arm. Triggered electrical stimulation can be even more effective when it is combined with specific tasks.
