Stress and Psychological Status between a Stroke and a Fracture Patient's Caregivers (뇌졸중환자와 골절환자 보호자의 스트레스 및 심리상태)

  • Purpose : The purpose of this study was to compare stress and psychological status between a stroke and a fracture patient's caregivers. Methods : Data was collected from September to December 2018 and participants of this study were 60 stroke and 61 fracture patient's caregivers. Stress was evaluated with the Global Assessment of Recent Stress scale-Korean Version and psychological status was measured with the Symptom check List-90-Revision. Results : The stroke patient's caregivers scored significantly higher than the fracture patient's caregivers in stress relevant to interpersonal, change in relationship, sickness or injury, financial, change/no change in routine (p<.05). The stroke patient's caregivers scored significantly higher in somatization, interpersonal sensitivity, depression, anxiety, paranoid ideation, and psychoticism than the fracture patient's caregivers (p<.05). Conclusion : It was found that the severity of mental health in the stroke patient's caregivers is necessary to develop a customized mental health improvement program for stroke patient's caregivers.

A Case Report of Post-stroke Depression Patient Using Bunsimgi-eum(Fenxingiyin) (분심기음으로 호전된 뇌경색 후 우울증환자 치험 1례)

  • Object : The purpose of this case report is to show an efficacy of Bunsimgi-eum(Fenxingiyin) for patient with Post-stroke Depression. Methods : This Patient was 64 year-old man who was diagnosed with Stroke by Brain MRI. And after the stroke, the patient showed some Post-stroke depression(PSD) behaviors, like mutism, waxy flexibility and have no willness of rehabilitation. He was treated by Herbal medicines, acupunctore and moxibustion therapy. 40 days after hospitalization, Bunsimgi-eum was prescribed to the patient. Results : After Bunsimgi-eum Herbal treatment, the symtoms of Post-stroke depression have improved remarkably. The grade of Rt. side hemiparesis and time of rehabilitation treat were also increased. Conclusions : This study suggests that Bunsimgi-eum could have a positive efficacy for Post-stroke depression. As a result, it makes the patient to take more rehabilitation treatment.

  • Objectives: The aim of this report is to report the effects of Banhahubak-tang on dysphagia in a stroke patient. Methods: A case of a 67-year-old female stroke patient with dysphagia is presented. The patient was treated with the herbal medicine Banhahubak-tang. Results : After administration of the herbal medicine Banhahubak-tang, dysphagia was improved, which reduced time per meal. Conclusion : Banhahubak-tang was an effective treatment for dysphasia in a stroke patient.

  • Object : The purpose of this study is to report the effect of acupuncture for Restless Leg Syndrome in ischemic stroke patient. Methods : Ischemic stroke patient was admitted who was suffering from a distressing urge to move the legs and triggered by rest or inactivity accompanied with insomnia. The patient was diagnosed with Restless Leg Syndrome. In the point of Differentiation of Syndromes(辨證), we diagnosed this patient as deficiency of yin(陰虛) and treated with acupuncture. Results : After acupuncture, the symtoms including unpleasant sensation or pain, urge to move the leg and insomnia had withdrawn. Conclusions : This case suggest the possiblity of acupuncture for Restless Leg Syndrome and more researches should be followed.

  • Objectives : Cerebrovascular disease in children is more common than once recognized and the etiology. In stroke patient, dysphagia is a common. Dysphagia often affects the rehabilitation of stroke patients by increasing the risk of aspiration pneumonia and malnutrition. This is the clinical report about 1 case of patient with dysphagia who had a stroke developed after operation. Methods : The herb medicine, moxibustion and acupuncture therapies were applied for treating this patient's chief symptom. Results : This patient's dysphagia was improved by oriental medical therapy. Conclusions : The result suggests that the herb medicine and acupuncture therapies is efficacious against dysphagia associated with stroke. We though that more studies about the Oriental medical treatment on dysphagia associated with stroke are needed.

  • Objectives : We studied the aspects of headache in the patients with stroke. We compared character of headache on stroke with functional one. Methods : The subject of this study was based on 63 patients with headache who were admitted to the oriental hospital after stroke. We analyzed patients into sex & age, month, severity of headache on stroke type & lesion, site & character of headache. associated symptoms & signs on headache state.Results : The age of headache with stroke is higher than general headache. There was significant differences between general headache and headache with stroke. The cerebral hemorrhage is severer than cerebral infaction in th pain of headache. The most frequently appeared site of headache is temporal area and associated symptoms is vertigo & nausea.Conclusions : The headache of stroke patient is different form functional headache. because of cerebral vascular accident influence on inducing headache. Therefore we should study different methods between the headache of stroke patient and the others in medical treatment.

Analysis of 363 Consecutive Patient with Acute Ischemic Stroke f개m the Hanbang Stroke Registry (뇌혈관 한의학 기반 연구사업 등록자료를 통한 363명의 급성기 뇌경색 환자의 기초 임상자료 분석)

  • Objectives : To gain better insights on the characteristics of stroke patients admitted to oriental medical hospitals, we analyzed the data of 363 consecutive patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Methods : Subjects' enrollment was in the oriental medical hospitals of 3 universities located in the metropolitan region from October 2005 to October 2006. We assessed the subjects' general characteristics, risk factors, and etiology of stroke. Each patient's TOAST classification type was confirmed by two independent specialists. Those were small vessel occlusion (SVO), large artery atherosclerosis (LAA), cardioembolism (CE), stroke with other determined etiology (SOE), and stroke with undetermined etiology (SUE).Results 'The distribution of the subjects' general characteristics and risk factors for stroke were similar with result storm previous reports. However, in the TOAST classification, SVO was the major type occupying 78.5% in the total subjects, which is the highest share compared with other research with similar methods. These results imply that patients with more severe symptoms rarely visit oriental medical hospitals. Conclusion : Assuming that this research will continue adding patient's data continuously, this work will help us to understand the features of stroke patients at oriental medical hospitals, and contribute to expansion of the Korean Hanbang Stroke Data Bank.

Safety and effectiveness of early cardiac rehabilitation in a stroke patient with heart failure and atrial fibrillation: a case report

  • Stroke patients have reduced aerobic capacity. Therefore, intensive structured exercise programs are needed. We report the case of a patient with stroke and cardiac disease who underwent early inpatient cardiac rehabilitation (CR). A 38-year-old male patient with atrial fibrillation, heart failure, and cerebral infarction underwent a symptom-limited exercise tolerance test (ETT) without any problems on day 45 after admission. He completed a 2-week inpatient program and an 8-week home-based CR program. Follow-up ETT showed increased exercise capacity. The present case might be the first to report a safely performed CR program in a patient with stroke and cardiac comorbidity in Korea. Systematic guidance is needed for post-stroke patients to receive safe and effective CR for the secondary prevention of stroke and cardiovascular risk.

  • Objectives : The purpose of this study was to understanding the constraint induced movement and Self-efficacy with arm training on upper motor function in Stroke Patients. Methods : Stroke, the leading cause of functional disability, causes a variety of impairments that compromise quality of life. Upper limb hemiparesis, a commonly seen impairment, is particularly problematic given its impact on activities of daily living. Because stroke was a disease to correspond to the first during domestic cause of death, and was accompanied by a lot of side aftereffects after a survival, stroke rehabilitation bought a patient and a family and a physical therapist, and it was main concern of. Results : Looks into upper extremity excrise of a subacute stroke patient estranged a acute convalescence later by a rehabilitation treatment in this consideration, and evaluates an effect to wind up constraint induced movement for an early treatment of stroke and Self efficacy, and help is one to an early rehabilitation of an stroke patient. Conclusions : Overuse sound tends after the stroke occurrence in the early stage in order to recompense for stroke, and at the time of a new aspect called learned nonuse syndrome by a movement of a paralysis part dusting off wealth with this step thing later. Constraint induced movement using self efficacy could be an effective for improving function of stroke.

  • Purpose : The purpose of this study was conducted to find out observation at gait analysis of the stroke patient with proprioceptive neuromuscular facilitation(PNF) concept. Methods : This is a literature study with books, seminar note and international PNF course book. Results : Stroke patient gait was poor initial contact by weakness of tibialis anterior or weakness of contralateral plantar flexor, poor loading response by loss of deep sensation, poor mid stance by loss of deep sensation, weakness of tibialis anterior and weakness of plantar flexors eccentric control, poor terminal stance, pre-swing, initial swing by loss of deep sensation and stiffness fo deep toe flexors. Conclusion : Stroke patient gait determine on loss of mobility, pain, fear, trunk muscle weakness, loss of coordination, loss of deep sensation, neglect and apraxia. Therefore observational gait analysis of the stroke patient focus on gait cycle and take out hypotheses from the gait cycle. These hypotheses have to define accept or not by parameters. Treatment plan made with the hypotheses.
