The Fitness and Organizational performance Analysis of Hospital Specialization Strategy Types (병원전문화 전략유형의 적합성과 조직성과 분석)

  • This research classified hospital specialization strategy types through cluster analysis, analyzed fitness of hospital specialization strategy types for external environment or organizational structure, and examined relation between hospital specialization strategy types and organizational performance. This research surveyed 1,437 hospitals which have more than 30 patient's bed and practice national health service in Korea. Specifically, this research divided into two part : external fit - analysis of relation between external environment and specialization strategy, internal fit-analysis of relation between organizational structure factors and specialization strategy. also, as the organizational performance for achieving specialization strategic purpose, not only the productivity, efficiency, profit but also the medical quality was considered. In case of external fit, many hospitals chose integration type if there are a lot of competitive hospitals and regional population. Particularly, if there are many competitive hospitals, concentration type is chosen. In contrast, if there are many doctors in the region, differentiation type is chosen. In case of internal fit, according to organization type and patient's bed number, hospitals chose different types. If it is a general hospital and has a few bed number, generalization or concentration type is chosen. Tertiary hospital or the hospital with many patient's bed chose differentiation type. According to the number of specialists, if there are a few specialists, generalization or concentration type is chosen. If there are many specialists, differentiation type has high fitness for the hospital. In relation to strategy types and organizational performance, differentiation type has best result. Differentiation type has a good result in 7 items out of 11.

Factors Affecting Patient Moving for Medical Service Using Multi-level Analysis (환자이동에 영향을 미치는 개인 및 병원요인 분석)

  • The purpose of this study is to find out factors affecting patient moving to receive medical service. This study is analyzed by multi-level model with patient and hospital level by using SAS 9.3. Total number of patients is 600,000 persons for inpatients and 550,000 patients for outpatients. The degree of the factors, which is combined with personnel factor and hospital factor, can be analyzed by Intra-Class Correlation (ICC). The percentage of group(hospital) level variance of the total variance for out-bound moving case are 30.6% at inpatients, and 28.3% at outpatients. And the percentage of hospital level variance of the total variance for moving distance, are 26.7%, 32,5% respectively. Conclusionally, although the main factor of moving is patient level, hospital is also very important factor to make decision to go out-bound. It contributed to about 1/3 for hospital choice. And, when the one make decision, he will consider the hospital type, number of bed, and training institute in hospital level. Through this study to find out hospital factors affecting patient moving for medical service, it must be continued to find out which factors have more influence to choice the hospital among disease type after this.

Design of Rectangular-Type Four-Finger Rehabilitation Robot for Stroke Patient (뇌졸중환자를 위한 직교형 4개 손가락 재활로봇 기구설계)

  • This paper describes the design of a rectangular-type four-finger rehabilitation robot for flexibility rehabilitation of stroke patients' fingers and other patient's paralyzed fingers. The four-finger rehabilitation robot is composed of a body and each finger rehabilitation robot instrument. The four-finger rehabilitation robot could exercise four fingers (forefinger, middle finger ring finger and little finger) of patient for their rehabilitation. The four-finger rehabilitation robot instruments move according to the trace which spread out the patient's fingers and then turn them inward for the fingers' flexibility, while at the same time performing the force control with the reference forces for fingers' safety, simultaneously. A control characteristic test of the developed rectangular-type four-finger rehabilitation robot was carried out, and the results confirmed that the robot could be used for the flexibility rehabilitation exercise for the fingers of normal person and patients.

  • We successfully performed bleeding control using roll-gauze packing in a patient with Type IV Ehlers-Danlos syndrome and this patient was suffering from spontaneous hemothorax. Thoracotomy for controlling ongoing bleeding in a patient with Type IV Ehlers-Danlos syndrome should be performed as a last resort after due consideration.

A Case Report of Taeeumin Patient with Diabetes Treated with Yeoldahansotang-Gamibang (태음인 당뇨 환자의 열다한소탕가미방 치험 1례)

  • Objectives The aim of this study is to report the improvement in Taeeumin's dryness-heat symptom of a 54-year-old man diagnosed with type-II diabetes mellitus after treated by herbal medicine. Methods The Taeeumin patient took Yuldahansotang-gamibang for 9 months to manage Taeeumin's dryness-heat symptom. We examined the changes of Body Mass Index(BMI), Fating Blood Sugar(FBS), Hemoglobin A1c(HbA1c), and ketouria. Results Type-II diabetes mellitus symptoms in the patient with Taeeumin's dryness-heat symptom were thirst, fatigue, and increased body weight. All numerical levels for blood sugar, HbA1c, and ketouria were reduced after taking Yuldahansotang-gamibang. Conclusions The treatment of the patient with Taeeumin's dryness-heat symptom and type-II diabetes mellitus with Yuldahansotang-gamibang was efficient, and further research is needed to prove the effect of Yuldahansotang-gamibang for treating such patients.

A Case Study of Soyang Patient with Post COVID-19 Syndrome Treated with Hyeongbangjihwang-tanggamibang (형방지황탕가미방으로 호전된 코로나-19 후유증 소양인 환자 치험 1례)

  • Objectives This study was aimed to report a significant improvement of patient with post COVID-19 syndrome, diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine. Methods The patient was diagnosed as Soyang type and had treatment with Sasang Constitutional Medicine three times everyday from June 13th to July 16th. Clinical improvement was evaluated with the degree of dizziness, migraine, palpitation and nausea caused by COVID-19. Results The symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were improved after treated with Hyeongbangjihwang-tanggamibang. Conclusions After the treatment with Hyeongbangjihwang-tanggamibang, the symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were disappeared. This suggests that Hyeongbangjihwang-tanggamibang is effective in recovering post COVID-19 syndrome for patient who diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine.

A Case Study of Overactive Bladder Patient Treated with Soeumin Sibimigwanjung-tang (소음인 십이미관중탕으로 호전된 과민성 방광 환자 치험 1례)

  • This study was aimed to report an significant improvement of urinary disturbance in overactive bladder patient using Soeumin's Sibimigwanjung-tang based on Sasang constitutional medicine. Methods A female overactive bladder patient with urinary disturbance including urgency, frequency, nocturia, tenesmus, weak stream was enrolled in this study. The patient was diagnosed as Soeum type greater-yin pattern and medicated with Sibimigwanjung-tang three times a day. Clinical improvement was evaluated with urination diary, International Prostate Symptom Score, Overactive Bladder Symptom Score, Numeric Rating Scale, and percentage of improvement compared to the day of hospitalization. Results Significant improvement was seen in urinary disturbance after 26 days of admission treatment. Also, symptoms of dyspepsia, coldness of hands and feet, and facial edema were improved. Conclusions This clinical case suggests that Sibimigwanjung-tang can be valuable option in treating urinary disturbance in overactive bladder within Soeum type greater-yin pattern.

  • Hereditary tyrosinemia type I (fiunarylacetoacetate hydrolase deficiency) is an autosomal recessive inborn error of tyrosine metabolism that results in liver failure in infancy or chronic liver disease with cirrhosis, frequently complicated by hepatocellular carcinoma in childhood or early adolescence. Early detection of this condition is very important to early intervention for better prognosis of patients. Neonatal screening test using tandem mass spectrometry (MS-MS) is performed, and this method facilitates detection of the inborn error of tyrosine. For early treatment of tyrosinemia type I, phenylalanine and tyrosine restricted diet and NTBC (2-nitro-4-trifluoromethylbenzoyl-1,3-cyclohexanedione) for inhibition of succinylacetone production are recommended. We studied a 10-month-old Korean boy with tyrosinemia type I whose condition was not discovered earlier through conventional neonatal screening testing available in Korea. The patient presented hyperbilirubinemia, liver failure, bleeding tendency, colicky pain and skin melanin pigmentation in neonatal period. MS-MS made it possible to detect tyrosinemia type I and allowed immediate treatment of the patient. This was the first successful NTBC trial on tyrosinemia type I patient in Korea.

Posterior Left Ventricular Wall Rupture After Mitral Valve Replacement (승모판 치환술후 발생한 좌심실 후벽 파열)

  • Rupture of the posterior left ventricular wall following mitral valve replacement is a rare but fatal complication. Over a 10 year period from August 25 1980 to November 27 1990, we have experienced 6 such patients among 884 cases of mitral valve replacement with 4 deaths and 2 survivors. One patient had a type I rupture and another a type II rapture with the remaining four patients having suffered type III ruptures. All of the ruptures were dis covered intraoperatively enabling prompt reinstitution of the cardiopulmonary bypass and subsequent cardioplegic arrest prior to repair. Overzealous removal of calcified valve leaflets seemed to be responsible for the single type I rupture, and untethering of the so called ventricular loop appeared to be the main mechanism responsible for the type III ruptures. The single type II rupture that had occurred seemed to have been caused by inadvertent laceration of the papillary muscle with resultant rupture of the posterior LV wall at the base of the papillary muscle. Among the type III ruptures, 2 patients required intraaortic balloon pump[IABP] support only for mechanical assistance and 1 patient required both the IABP and the Biomedicus LV assist device for successfull weaning following repair of the LV rupture Another patient with a type II rupture also required the circulatory assistance of both the IABP and the bio-medicus LV assist device for weaning from the bypass. Attention to meticulous technical considerations such as avoiding over aggressive removal of heavily calcified valvular tissue, preservation of as much mural leaflet tissue and chordal stuctures as possible seemed helpful in preventing this catastrophic complication from occurring. Fusion and fibrous stricture of the chordal structures appeared particularly conducive to the type II ruptures as a result of the increased susceptibility to papillary injury during operation.

Complication rates and patient satisfaction with removable dentures

  • PURPOSE. The purpose of this clinical study was to evaluate the frequency and type of prosthetic complications in relation to type and properties of removable dentures and to investigate the influence of these complications and several data about the existing dentures on patient satisfaction. MATERIALS AND METHODS. Ninety nine patients (44 males and 55 females) wearing removable dentures have been included in the study. The complications of the patients were recorded; patient satisfaction was determined with a Visual Analog Scale (VAS) and the relationship of complications and patient satisfaction with several data about the dentures such as denture age, type of denture, centric relation and vertical dimension was investigated. Kruskal Wallis, Mann Whitney U and Chi square tests were used for statistical analyses. The results were evaluated statistically at a significance level of $P$ <.05. RESULTS. Need for addition of artificial teeth for dentures with correct centric relations was found to be significantly lower than dentures with wrong centric relations ($P$ <.01). Loss of retention, ulcerations and high vertical dimension affected the VAS chewing ability scores negatively and ulcerations affected the VAS phonation scores negatively ($P$ <.05). CONCLUSION. Considering the results of this study, it can be concluded that loss of retention, ulcerations and high vertical dimension caused patient dissatisfaction. Additionally, dentures with wrong centric relations caused need for addition of artificial teeth.
