  • Both immune reaction and hypersensitivity reaction are occurred by the same mechanism, the antigen and antibody reaction. The favorable result of this reaction towards a host is called clinically an immune reaction and the opposite results is called an hypersensitivity reaction. Type IV hypersensitivity reaction is a delayed type which is related to the cellular immune reaction and a contact hypersensitivity is included in this type. Various dental materials such as metal (mercury, nickel, chrome, cobalt), resin and eugenol are etiologic substances. Patch test kit is composed of test substance with a controlled concentration which respond only to a susceptible patient and an aluminum chamber, and etiologic substances for hypersensitivity can be easily and comfortably found just by applying the kit to the patient's skin. In this case report, the patch test was performed to a patients with oral lichen planus and the allergen, restorative material was found. After removal of the matching restoration from the patient's mouth, the symptom was improved.

  • Myotonic dystrophy type 1 (DM1) is an autosomal dominant multisystem disorder and one of the most common muscular dystrophies affecting adults. Charcot-Marie-Tooth (CMT) disease, a common hereditary neuropathy, is characterized by atrophy of the distal limbs and peripheral nerve abnormalities. The authors report a rare case involving a 24-year-old female who was diagnosed simultaneously with both DM1 and CMT1A based on the results of a nerve conduction study (NCS). The patient, who had previously been diagnosed with DM1, was admitted for lower extremity pain. Her electrodiagnostic examination continued to reveal severe sensorimotor demyelinating polyneuropathy, and a genetic study was performed to confirm whether she had other hereditary neuropathies, except DM1, that suggested CMT1A, the most common phenotype of CMT. Severe abnormalities in an NCS in a DM1 patient may suggest the incidental coexistence of hereditary neuropathies, and further evaluations, such as genetic studies, should be performed for proper diagnosis.

Circadian Rhythms Characteristics of Nurses Providing Direct Patient Care: An Observational Study

  • Background: In today's modern world, longer working hours, shift work, and working at night have become major causes of the disruption of our natural circadian rhythms. This study aimed to investigate the effects of the type of shift work (rotating vs. fixed day), duty period (on-duty vs. off-duty), and working period within each shift (nighttime vs. daytime) on the circadian rhythm characteristics of nurses who provide direct patient care. Methods: This cross-sectional study used a purposive sampling method. Cosinor analysis was applied to analyze the actigraphy data of nurses providing direct patient care for seven consecutive days. The linear mixed effects model was then used to determine any variances between shift type, duty period, and working period within each shift for the nurses. Results: The mesor value did not differ according to nurses' shift type, duty period, and working period within each shift. The amplitude was statistically higher in on-duty nurses and in daytime working hours. The acrophase was significantly delayed in nighttime working hours. As well as nurses in rotating shift had experience. Conclusion: Our findings revealed that the peak activity of nurses occurs significantly later at night while working and nurses working during nighttime hours may have a weaker or less distinct circadian rhythm. Thus, this study suggests that limits be placed on the number of rotating nighttime shifts for nurses.

  • 1. Objective We report a series of 3 cases Euphorbiae Kansui. Radix(Gam-sui) which is based on Sasang constitutional Medicine for Tension-Type Headache in Soyangin. 2. Methods We treated a 3 Soyangin patients that had Tension-Type Headache. We prescribed Euphorbiae Kansui. Radix(Gam-sui) for their physical symptoms. The improvement of their Tension-Type Headache was evaluated 3. Results After the Euphorbiae Kansui. Radix(Gam-sui) was given, the patient's Headache and physical symptoms were improved. 4. Conclusions This cases studies showed an efficient result of using Euphorbiae Kansui. Radix(Gam-sui) in the Tension-Type Headache of Soyangin.

  • From June 1th to December 31th 2001, the study was carried out 285 person who came to the Daejon oriental hospital for the examination of CVA. The result was as follows : 1. Hyperlipidemia patient is 41%, 50's is the highest. 2. In the case of Hyperlipidemia patient, Taeyum type is 49%, Soyang type is 36% and Soyum type is 15%. but in the Control group Taeyum type is 36%, Soyang type is 44% and Soyum type is 20%. 3. In the case of Abnormal EKG, Hyperlipidemia group is 34%, Control group is 32%. 4. In the case of Liver disease, Hyperlipidemia group is 17%, Control group is 9%. 5. In the case of Diabet mellitus, Hyperlipidemia group is 30%, Control group is 7%. 6. In the case of Hypertension, Hyperlipidemia group is 50%, Control group is 42%.

Surgical Correction of TO in Adults - 42 Cases Report - (성인 활로씨 사징증의 외과적 교정)

  • This report describes our 10-year experience with intracardiac repair in 42 patients older than 16 year with tetralogy of Fallot. The mean age was 22.0$\pm$5.18 years[range 16~41]. The preoperative clinical manifestations were cyanosis & clubbing[93%], frequent URI [55%], anoxic spell[40%], pulmonary tuberculosis[21%], tuberculous empyema[7%], chronic renal failure[7%], congestive heart failure[7%] and subacute bacterial endocarditis [2%], etc. The previous shunt procedure for palliation had been performed in 7 patients. The type of VSD were typical perimembranous type[67%], total canal defect[28%] and combined type[5%]. The right ventricular outflow tract obstruction were combined type[69%], infundibular type[21%] and valvular type[10%]. Transannular patch was used in 50% of patients. Associated cardiovascular anomalies were ASD[33%], PFO[31%], Rt. aortic arch[10%], Lt. SVC[10%], single Rt. pulmonary artery, single Lt. coronary artery, ASI, proximal stenosis of Rt. pulmonary artery and anomalous systemic venous return, etc. Hospital mortality was 7.1%[3 cases]in overall. The causes of hospital deaths were revealed low cardiac output & acute renal failure[2 cases], postoperative bleeding[1 case]. There were 2 late deaths 3 & 68 months after surgery. Residual intracardiac shunt was detected in 2 patients. one patient was successfully reoperated and another patient had Qp /Qs ratio less than 1.5.

The Administration Patterns of Sedatives for Sedation of Mechanically Ventilated Patients and Patient Assessment by Nurses (기계환기기 치료 환자들의 진정상태유지를 위한 투약양상 및 사정내용에 대한 일 조사연구)

  • Purpose: The purpose of this descriptive study was to investigate the administration patterns of sedatives and paralytic agents for mechanically ventilated patients. Method: The participants were 18 adult patients who received mechanical ventilation therapy for more than 1 day in the MICU. Collected data were type of injected medications, administration interval, injected amount, administration purpose and patient assessment by nurses. Result: Mean age of the patients was 61 years old. Seventy-two percent were male. Only 2 drugs (midazolam and norcuron) were administered to most of subjects. The type of medication administered was less heterogeneous than in previous studies. Most of the patients received sedatives and paralytics intermittently. Six different methods were used by nurses to administer sedatives and paralytics. The mean injection frequency was 9.3 times/day for sedatives and 10.8 times/day for paralytic agents. The most common purpose for administration of the drugs was full sedation of the patient (57%). Conclusion: Since a few patients received sedatives and paralytic agents higher than usual dose and a few Patients received those drugs lower than usual dose, nurses need to use proper protocols and guidelines for sedation to avoid oversedation and undersedation.

  • Purpose : In order to estimate clinical effects of Oriental Medicine Treatment with acupotomy therapy of herniated lumbar intervertebral Disc patient needed operation. Methods : From 17th April, 2008 to 17th April, 2008, 1 female patient diagnosed as herniated lumbar intervertebral Disc at L4/5(right postero-lateral extrusion type)was treated with general oriental medicine therapy(acupuncture, moxibustion, cupping, physical therapy, herbal medication) and acupotomy. Results : The patient's chief complaints - Low back pain, both buttock pain, Right. leg paresthesia were remarkably improved. Conclusions : This study demonstrates that oriental medical treatment with acuputomy therapy has notable effect in improving functional disability and symptoms of lumbar radiculopathy. as though we had not wide experience in this treatment, more research is needed.

Ex vivo Boosted Immune Cell Therapy for Canine Hepatic Disease

  • A 12-year-old male American Cocker Spaniel was diagnosed with a type of chronic hepatits (CH) called cholangioheaptits. Routine supportive medication was administered to the patient, and ex vivo boosted immune cell (EBI-C) therapy was used for the treatment. A histopathologic examination of the liver 19 months later revealed that the cholangiohepatitis had progressed to cholangiocarcinoma. The medication and immune cell therapy was maintained. Two months after the new diagnosis, the patient's state worsened, and the dog died 635 days after the first visit. EBI-C therapy is a type of immunotherapy, where immune cells are isolated from the patient's peripheral blood mononuclear cells, expanded ex vivo, and then infused into the patient intravenously every two weeks. EBI-Cs (mean: 2.78 × 108 cells) were obtained 38 times and infused every two weeks. Most EBI-C were T-lymphocytes (99.24% of total EBI cells). T-lymphocytes produce large interferon (IFN)-γ, and IFN-γ inhibits liver fibrosis in dogs with CH. Moreover, in bile duct cancer, an increase in T-lymphocytes correlates with decreasing tumor invasion and metastasis. Thus, we propose that EBI-C therapy is applicable as a new supportive therapy for canine liver disease if other treatments like drug medication, surgery, or radiation are unavailable.

  • Purpose : Fluoroscopy equipment, depending on the type of changes that occur in the patient's position ESD and study the patient's scatter ray of ESD Practitioners considered a comparative analysis was to evaluate the correct dose. Materials and Methods : HITACHI four overtube type TU-8000 Flat Detector and Under tube C-Arm Philips' Multi Diagnost Eleva with Flat Detector type were measured by. Each devices is a measure of the patient's esd randophantom position in tabel unfors Xi multi funtion then fixed to the abdomen fluoroscopy and 10 seconds, spot was measured three times, practitioners of the incident surface dose by considering the patient's scatter ray of the table for each device in the average human stomach 21cm thickness acrylic phantom ($25cm{\times}25cm$) Place the practitioner position after position randophantom unfors Xi multi funtion in the thyroid and stomach 1 minute by a fixed one-time fluoroscopy and measured. Results : 10 seconds and the patient perspective of the c-arm ESD 1.2 times smaller on the AP and oblique measurements were measured in the 6-13 times smaller. spot positions to changes in the measured three times on the AP of the abdomen, ESD is 18 times smaller c-arm measurements and the oblique measurement was 19-30 times smaller. And 1 minute at practitioners fluoroscopy esd in the thyroid 2.12 times the c-arm, chest 1.75 times less the dose was measured. On the AP, depending on the device, but the lack of dose difference oblique positions of the two devices depending on changes in the area due to changes in both the AP than on the dose increased, the difference in dose between the two devices, the maximum difference was approximately 27 times.
