조직안전문화가 방사선사의 환자 안전에 대한 의식 및 안전 행동에 미치는 영향분석 (Analysis of the Impact of Organizational Safety Culture on Radiology Technologists Awareness of Patient Safety and Safety Behavior)

  • 본 연구는 조직 안전 문화가 방사선사의 환자 안전에 대한 인식도와 안전 행동에 미치는 영향을 살펴보고자 진행된 연구이다. 연구는 설문조사 결과를 바탕으로 진행하였으며 설문조사는 일반적 특성, 환자 안전을 위한 노력, 환자 안전 문화의 3개 요인으로 나누어 진행하였다. 연구 결과 환자 안전에 대한 인식의 차이를 나타내는 요인은 일반적 특성에서 연령, 업무경력, 업무에 대한 만족도가 영향 요인으로 나타났고 성별이나 학력, 근무 형태는 환자 안전에 대한 인식도의 차이에 영향을 주는 요소로 포함되지 않았다. 환자 안전을 위한 노력으로는 병원의 지원 및 교육, 환자 안전 활동 및 오류에 대한 대응 방식 모두 영향 요인으로 나타났으며 특히 오류에 대한 대응 방식이 다른 항목에 비하여 더 높은 연관성을 나타내었다. 환자 안전 문화는 팀워크와 개방적인 의사소통 두 가지 항목에 대해 분석하였으며 연구 결과 두 항목 모두 방사선사의 환자 안전에 대한 인식도와 높은 관련성을 나타내었다.

Debriefing in pediatrics

  • Debriefing is a conversational session that revolves around the sharing and examining of information after a specific event has taken place. Debriefing may follow a simulated or actual experience and provides a forum for the learners to reflect on the experience and learn from their mistakes. Originating from the military and aviation industry, it is used on a daily basis to reflect and improve the performance in other high-risk industries. Expert debriefers may facilitate the reflection by asking open-ended questions to probe into the framework of the learners and apply lessons learned to future situations. Debriefing has been proven to improve clinical outcomes such as the return of spontaneous circulation after cardiac arrest and the teaching of teamwork and communication in pediatrics. Incorporating debriefing into clinical practice would facilitate the cultural change necessary to talk more openly about team performance and learn from near misses, errors, and successes that will improve not only clinical outcome but also patient safety.

수술실의료진의 환자안전문화 인식수준 및 관련요인 (Perceived Level and Associated Factors of Patient Safety Culture among Health Care Providers in an Operating Room)

  • Purpose: The purposes of this study were to compare the level of perception and to identify factors associated with perception on patient safety culture among health care providers working in an operating room(OR). Methods: A cross-sectional survey design was used. Data were collected conveniently from 154 RNs and 116 physicians working in a tertiary hospital in Seoul. Patient safety culture was measured using "The Hospital Survey on Patient Safety Culture" developed by the Agency for Healthcare Research and Quality (AHRQ). Descriptive statistics, t-test, ANOVA, and Spearman rank correlation coefficients were used for statistical analysis with the SPSS version 17.0. Results: The perception level of nurses and physicians on patient safety culture was "moderate" (3.14). Compared to physicians, nurses showed a significantly lower perception on the items of "teamwork within units" (t=-6.904, p<.001) and "overall perception of patient safety" (t=-4.327, p<.001), but had a higher perception about "frequency of events reported" (t=2.769, p=.006). The physicians' professional positions, years of working experience, age, and working hour per week were identified as factors associated with patient safety culture. Conclusion: Level of perception on patient safety culture may vary among health care providers working in the OR. The study finding suggests that a tailored education and training strategies should be considered to develop an effective safety culture for healthcare professionals working in OR.

간호대학생의 다제내성균 감염관리역량에 영향을 미치는 요인 (Factors Affecting Competency of Infection Control of MultiDrug-Resistant Organisms among Nursing students)

  • Purpose: This study aimed to identify the factors influencing competency for Multi-Drug Resistant Organisms (MDRO) infection control in nursing students with clinical practice experience. Methods: This cross-sectional descriptive study was conducted from May 2020 to August 2020 by including 175 nursing students in two nursing schools located in Daegu. The data were collected using self-report questionnaires. The data were analyzed using the SPSS/WIN 25.0 program with descriptive statistics, independent t-test, ANOVA, correlation, and multiple regression. Results: The competency of MDRO infection control was 3.41.±0.38. The attitude to patient safety management and - knowledge of MDRO infection control were 3.81±0.47 and 16.98±3.02, respectively. Factors influencing the competency of MDRO infection control were perceived benefit(β=.38, p<.001), knowledge toward MDRO infection control(β=.21, p=.001), attitude in patient safety management(β=.17, p=.028), and perceived barrier(β=-.15, p=.029) with an explanatory power of 34.2%. Conclusion: A systemic education program regarding MDRO infection control is needed for nursing students to provide knowledge related to MDRO infection control and help establish positive beliefs toward MDRO infection control and attitudes in patient safety management.

환자안전과 질 향상을 위한 다른 나라의 개선 전략과 전담인력 (Strategies and Experts in Other Countries for Patient Safety and Quality Improvement)

  • This study was done to investigate the independent organizations established for patient safety, related policies, and the duties of experts in other countries. Australia established an organization called the Commission in 2006, the United Kingdom established the National Patients Safety Agency in 2001, and the United States assigned its work to the Agency for Healthcare Research and Quality in 2005. This was done by law in all three countries. The experts for patient safety were mainly called the "patent safety and quality coordinator", and although there was no qualification system for carrying out patient safety work, all three countries had licenses in the health care field or required more than 4-5 years of practical experience. The main duties were planning on patient safety and quality of healthcare service, data collection and analysis, and education, etc. and for this, competencies such as communication, leadership, and teamwork were required.

의사의 진료설명 이해 용이성에 영향을 미치는 환자의 특성 (Patient's Characteristics Influencing Patient's Comprehensibleness of Doctor's Explanation)

  • Purpose:The purpose of this study was to identify the patient's characteristics influencing patient's comprehensibleness of doctor's explanation. Methods: This study was conducted as a secondary data analysis using the raw data of The 6th Korea National Health and Nutrition Examination Survey (KNHANES VI) conducted in 2015. The survey sampled 22,948 adults, and 4,469 of them were included in this analysis. Multiple regression analysis was performed to determine the patient's characteristics influencing patient's comprehensibleness of doctor's explanation. Results: The patient's comprehensibleness of doctor's explanation were more higher as age (t=5.65, p<.001), female (t=4.40, p<.001), subjective good health status (t=3.48, p=.001) were higher. On the other hand, the patient's comprehensibleness of doctor's explanation were more higher education level (t=-6.80, p<.001), not-recent outpatient experience (t=-6.04, p<.001), pain/discomfort (t=-2.64, p=.008), anxiety/depression (t=-2.58, p=.010) were lower. Conclusion:It is necessary to be provided the patients who are higher education level, pain/discomfort and anxiety/depression with applying the doctor's detailed explanation and intervention programs.

Frequency, Expected Effects, Obstacles, and Facilitators of Disclosure of Patient Safety Incidents: A Systematic Review

  • Objectives: We performed a systematic review to assess and aggregate the available evidence on the frequency, expected effects, obstacles, and facilitators of disclosure of patient safety incidents (DPSI). Methods: We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines for this systematic review and searched PubMed, Scopus, and the Cochrane Library for English articles published between 1990 and 2014. Two authors independently conducted the title screening and abstract review. Ninety-nine articles were selected for full-text reviews. One author extracted the data and another verified them. Results: There was considerable variation in the reported frequency of DPSI among medical professionals. The main expected effects of DPSI were decreased intention of the general public to file medical lawsuits and punish medical professionals, increased credibility of medical professionals, increased intention of patients to revisit and recommend physicians or hospitals, higher ratings of quality of care, and alleviation of feelings of guilt among medical professionals. The obstacles to DPSI were fear of medical lawsuits and punishment, fear of a damaged professional reputation among colleagues and patients, diminished patient trust, the complexity of the situation, and the absence of a patient safety culture. However, the factors facilitating DPSI included the creation of a safe environment for reporting patient safety incidents, as well as guidelines and education for DPSI. Conclusions: The reported frequency of the experience of the general public with DPSI was somewhat lower than the reported frequency of DPSI among medical professionals. Although we identified various expected effects of DPSI, more empirical evidence from real cases is required.

정신병원 간호사의 낙상에 대한 지식, 태도, 환자안전문화인식이 낙상예방활동에 미치는 영향 (The Effect of Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities in Mental Hospital Nurses)

  • 본 연구는 정신병원 간호사를 대상으로 낙상에 대한 지식, 태도 및 환자안전문화인식이 낙상예방활동에 미치는 영향을 파악하기 위한 목적으로 시도되었다. 연구방법은 부산광역시와 경상남도 지역의 7개 기관의 정신병원에 근무하는 간호사 153명을 대상으로 한 서술적 조사연구로, 2018년 3월 5일부터 2018년 3월 30일까지 자료를 수집하였다. 연구통계방법은 IBM SPSS/win 24.0 프로그램을 t-test, ANOVA 그리고 다중회귀분석을 이용하였다. 그 결과, 연구 대상자의 면허나 자격에 따라 낙상태도에 차이가 있었고, 교육수준이 높을수록 환자안전문화인식정도가 높았으며, 근무경력이 많을수록 낙상예방활동이 높았다. 그리고 환자안전문화인식이 높을수록 낙상간호에 대한 태도가 높았으며, 낙상예방활동이 높을수록 낙상태도와 낙상안전문화인식이 높은 것으로 나타났다. 연구대상자의 낙상에 대한 지식, 태도, 환자안전문화인식이 낙상예방활동에 미치는 영향을 다중회귀분석 한 결과, 낙상예방활동에 대한 설명력은 12.5%로 나타났다. 그러므로 정신병원 간호사의 낙상예방활동을 증진시키기 위해서는 계속적인 교육을 통해 지식을 확장하며 낙상태도를 변화시킬 수 있는 교육프로그램 개발 및 제공이 요구된다. 또한 병원조직 차원에서 안전에 대한 전반적인 인적, 제도적 환경의 변화 등 다방면의 지원이 요구된다.

간호대학생의 입원아동 안전간호활동 인식 영향 요인 (Factors Affecting Nursing Students' Perception on Pediatric Patient Safety Culture and Nursing Activity)

  • Purpose: The purpose of this study was to identify the factors that affect nursing students' perception of pediatric safety nursing activities for children during hospitalization. Methods: The study sample included 304 nursing students who had participated in pediatric nursing practice. Data were analyzed using SAS 9.3 program. Results: Regression analysis showed that the model's explanatory power was 37%. Safety policy and procedure, safety priority, disaster experience, and knowing a place of refuge were factors affecting the perception of safety nursing activities for children during hospitalization. Conclusion: Findings show that safety policy and procedures and safety priority are major factors that affect the perception of safety nursing activities and indicate that effective education programs on safety policy and procedure and safety priority are necessary to improve the perception of safety nursing activities.

일개 의과대학 장기추적통합임상실습 시범과정 운영경험과 고찰 (Pilot Study Results of a Longitudinal Integrated Clerkship in a Single Institution)

  • The longitudinal integrated clerkship is an innovative approach in medical education that emphasizes continuity as a key principle to provide student-centered experiences and learning. Seoul National University College of Medicine decided to adapt longitudinal integrated clerkships to the new curriculum beginning in 2018, and therefore conducted pilot studies in 2016 and 2017. This study aimed to analyze the program evaluation results of the two pilot programs and discuss issues related to the successful implementation of longitudinal integrated clerkships in Korea. We conducted a focus group interview with nine students who participated in the 2016 pilot program and 13 students who participated in the 2017 pilot program. We also conducted a focus group interview with 11 faculty members who either participated in the pilot program or will participate in the main program. From the 2016 experience, we found that it is not appropriate to assign each patient to a single student and let the students contact their patients personally for feasibility and safety reasons. In the 2017 pilot program, we assigned each patient to a group of students, which made it more feasible for the students to follow-up with their patients. The students were satisfied with their new experience of longitudinal patient follow-up and regular meetings in the pilot program. Faculty emphasized the importance of establishing the course objectives and holding an orientation for the students and the faculty. Further study is planned to evaluate the early outcomes of the main longitudinal integrated clerkship.
