목적 : 우리나라의 주된 사망원인인 암은 최근 들어 진단기술의 발전과 치료방법의 개선으로 전점 만성화되고 있어 결과적으로 집에서 치료받고 있는 재가암환자의 수가 증가추세에 있다. 그러나 지속적이고 체계적인 암환자 관리체계의 미비로 많은 환자들이 적정한 보건의료서비스를 받지 못하고 방치되고 있는 실정이다. 본 연구는 재가암환자의 삶의 질을 높이고 가족의 부담을 줄여주기 위한 지역사회 재가암환자 관리체계를 개발하기 위한 전단계로서 재가암환자의 서비스 요구를 파악하고자 한다. 방법 : 본 연구는 재가 암환자와 환자를 주로 돌보고 있는 가족을 대상으로 그들이 겪고 있는 어려움과 보건의료 분야의 도움을 필요로 하는 요구를 직접 면접 조사를 실시하여 조사하였다(연구대상, 중앙암등록본부 등록 환자, 455명). 결과 : 1) 암환자의 가장 중요한 증상인 통증조절에 대한 만족도는 경증 환자에서는 25.5%, 중증환자의 경우에는 46.5%가 불만이라고 응답하여 만족스러운 통증조절이 안되고 있었다. 2) 재가 암환자 및 보호자에 대한 요구도 조사결과를 보면 가장 필요한 도움으로는 경제적인 도움, 통증 및 증상조절 의료에 대한 정보제공 및 상담이었다. 3) 기본간호에 대해서는 간호필요율에 대한 충족율이 대부분 $20{\sim}30%$ 대로 낮았다. 특수간호의 경우에는 요구도가 있는 재가암환자가 많지 않았으며(5% 이내), 기본간호에 비해 간호충족율이 높았으나, 여전히 50% 정도로 낮아, 충분한 서비스가 충족되고 있지 않았다. 결론 : 재가암환자들의 서비스 요구도를 분석한 결과 암환자들이 충분한 통증 및 증상조절이 이루어지지 않고 있으며, 기본 및 특수간호의 충족률이 매우 낮은 실정이다. 향후에 재가암환자의 삶의 질을 높이고 가족의 부담을 줄여주기 위한 재가암환자 관리체계를 개발이 필요하다.
최근 수술 관련 의료사고가 증가하고, 일부 의료사고가 보건범죄와 연루되었다는 사실이 언론에 보도되었다. 환자단체는 수술실 내 CCTV 설치 및 운영 의무화를 촉구하였고, 이에 대한 이해관계인들의 논의가 활발히 이루어지고 있어 관련 법령에 대한 검토가 필요한 상황이다. 본 연구에서는 수술실 CCTV에 대한 특성을 파악하고, 수술실 CCTV 설치 및 운영에 관련 법령에 대해 비판적으로 검토하고자 한다. 현재 의료기관 내에서 CCTV는 시설물 관리용 및 환자안전관리용을 주목적으로 사용되고, 수술실의 경우 의료기관이 선택적으로 CCTV를 설치 및 운영하고 있다. 헌법은 모든 개인의 사생활 및 통신의 비밀과 자유를 침해받지 않을 권리를 보장하고 있으나, 이는 공공복리를 위해 법률로써 제한할 수 있다고 규정하고 있다. 그러나, 수술실 CCTV 설치 및 운영과 관련하여 법률이 현재 존재하지 않기 때문에 이는 법률의 흠결에 해당할 수 있다. 현 법체계상으로는 수술실 CCTV 설치가 의무화될 경우 정보주체인 의료진의 권리에도 불구하고 보건의료서비스 공급자의 특성상 개인정보자기결정권을 침해받을 가능성이 크다. 또한, 영상정보처리기기운영자의 CCTV 조작 시 열람과 업무 중 알게 되는 비밀의 누설에 관한 제한규정이 미흡한 상태여서 영상정보의 안전성이 위협받을 수 있다. 나아가, 수술실 CCTV의 경우 영상정보 보관기간, 보관장소 등이 명확히 규정되지 않는다면 환자안전과 의료사고 예방이라는 본래 목적에 부합되지 못할 가능성이 크다. 수술실 CCTV 설치 및 운영에 대해서 현재 활발하게 논의가 이루어지고 있고, 관련 의안이 몇 차례 발의되고 있어 이에 대한 법적 검토의 필요성이 증가하고 있지만 이에 관한 선행연구가 거의 없다는 측면에서 본 연구의 의의가 있다. 향후 수술실 CCTV 설치 및 운영 관련 법령 제·개정 시 본 연구 결과를 활용할 수 있을 것으로 기대한다.
We have developed a prototype patient monitoring system including module-based bedside units, interbed network, and central stations. A bedside unit consists of a color monitor and a main CPU unit with peripherals including a module controller. It can also include up to 3 module cases and 21 different modules. In addition to the 3-channel recorder module, six different physiological parameters of ECG, respiration, invasive blood pressure, noninvasive blood pressure, body temperature, and arterial pulse oximetry with plethysmogaph are provided as parameter modules. Modules and a module controller communicate with up to 1Mbps data rate through an intrabed network based on RS-485 and HDLC protocol. Bedside units can display up to 12 channels of waveforms with any related numeric informations simultaneously. At the same time, it communicates with other bedside units and central stations through interbed network based on 10Mbps Ethernet and TCP/IP protocol. Software far bedside units and central stations fully utilizes gaphical user interface techniques and all functions are controlled by a rotate/push button on bedside unit and a mouse on central station. The entire system satisfies the requirements of AAMI and ANSI standards in terms of electrical safety and performances. In order to accommodate more advanced data management capabilities such as 24-hour full disclosure, we are developing a relational database server dedicated to the patient monitoring system. We are also developing a clinical workstation with which physicians can review and examine the data from patients through various kinds of computer networks far diagnosis and report generation. Portable bedside units with LCD display and wired or wireless data communication capability will be developed in the near future. New parameter modules including cardiac output, capnograph, and other gas analysis functions will be added.
본 논문에서는 의학적 센서 모듈과 무선 통신 기술이 결합된 멀티 에이전트 기반 원격 헬스케어 시스템을 제안한다. 제안된 헬스케어 시스템은 멀리 떨어진 곳에서도 환자 모니터링, 의사의 진단과 처방, 환자와 의사, 병원 관계자 사이의 정보 교환 등 폭 넓은 서비스를 지원한다. 또한 Body Area Network(BAN)과 병원의 의사와 관계자들과 연결되며, 이에 대한 유기적이고 병원 서버의 중앙 부하를 감소시키는 확장된 JADE기반 헬스케어 시스템을 디자인하고 개발했다. 에이전트들은 센서로부터 수집된 환자의 정보를 수집, 통합, 전달하고 모바일(PDA)에 표현하기도 한다. 제안된 시스템은 모바일(PDA)과 같은 모바일 디바이스 등을 통하여 섬과 같이 병원과 멀리 떨어진 지역에서도 긴급 상황을 판단하여 원격으로 처리할 수 있다는 장점을 얻게 된다. 또한 환자(노인)의 상태를 실시간으로 모니터링 함으로써 요구되는 시간과 비용을 많이 단축하게 되고, 의료 서비스의 지원에 대한 효율성을 높이게 된다.
The primary and secondary survey was designed to identify all of a patient's injuries and prioritize their management. However 15 to 22.3% of patient with missed injuries had clinically significant missed injuries. To reduce missed injury, special attention should be focused on patients with severe anatomical injury or obtunded. Victims of blunt trauma commonly had multiple system involvement. Some reports indicate that inexperience, breakdown of estalished protocol, clinical error, and restriction of imaging studies may be responsible for presence of missed injury. The best way of reducing clinical significant of missed injuries was repeated clinical assessment. Here we report a case of severe blunt hepatic injury patient and pericardial injury that was missed in primary and secondary survey. After damage control surgery of hepatic injury, she remained hemodynamically unstable. Further investigation found cardiac tamponade during intensive care. This was managed by pericardial window operation through previous abdominal incision and abdominal wound closure was performed.
중추신경계와 말초신경계의 손상으로 인한 다양한 기전에 의해서 신경병성 통증이 생길 수 있다. 특정한 질병과 관련된 기전에 의해서 생기는 경우는 거의 없고, 진단과는 상관없이 한 환자에서 여러 가지 기전이 동시에 관여하여 생긴다. 신경병성 통증은 신경학적 검사와 환자의 문진으로부터 쉽게 진단할 수 있으나 치료는 아직 만족할 만하지 못하다. 신경병성 통증의 치료가 어렵다 하더라도 의사가 치료시 문제점을 완전히 이해하고 있다면 적절한 치료에 도달될 수 있을 것이다. 적절한 약물의 선택은 환자마다 효과가 있는 약제, 용량, 혈중농도 등이 각기 다르기 때문에 치료의 시도와 실패의 반복을 통해서 얻어질 수 있다. 효과가 있다고 알려진 각 약물들의 적절한 치료연구는 신경병성 통증의 약물치료에 있어 핵심일 것이다. 삼환계 항우울제는 일차약물로 알려져 있고 이에 대한 효과가 만족스럽지 못할 경우에는 항경련제, 국소마취성 항부정맥제제, clonidine, 마약성 진통제, 국소도포제 순으로 사용해 볼 수 있다. Oral and maxillofacial infection is a common complication in patients undergoing chemotherapy. The treatment of oral diseases in such patients differs from that administered to healthy patients. This paper reports a case of acute osteomyelitis of odontogenic origin following a recent chemotherapy session. The patient's condition was life-threatening because of neutropenic fever and sepsis that developed during the inpatient supportive care. However, the patient showed prompt recovery within 40 days following the use of appropriate antibiotics and routine dressing, without the requirement for surgical treatment, except tooth extraction. As seen in this case, patients undergoing chemotherapy are more susceptible to rapid progression of infections in the oral and maxillofacial areas. Therefore, accurate diagnosis through prompt clinical and radiological examination, identification of the extent of infection, and assessment of the patient's immune system are crucial for favorable outcomes. It is also necessary to eliminate the source of infection through appropriate administration of antibiotics. In particular, a broad-spectrum antibiotic with anti-pneumococcal activity is essential. Proper antibiotic administration and wound dressing are essential for infection control. Furthermore, close consultation with a hemato-oncologist is necessary for effective infection management based on the professional evaluation of patients' immune mechanisms.
Purpose: This study was to identify the effects of a nurse-led education program using computerized animation video for post-operative colon cancer patients. Methods: a total of 163 patients and 51 nurses were participated in this study. With a non-equivalent control group post-test design, patients were divided into three groups (77 got traditional education, 46 were applying brochure, 40 were watching video). Twelve-item animation video and brochure about the management after discharge for post-operative colon cancer patients were developed based on patient survey and the items of Korea Healthcare Accreditation. Results: The computerized video watching group had better satisfaction than the others, but there was no significant difference about comprehension. When video was applied, satisfaction, usefulness, application, and perceived patients' comprehension of nurses were all increased. Conclusion: This video education program was developed by nurses and it had a special thing for patient to access the same program even after discharge using the authorization system. It would be helpful for nurses to be more concentrated on the direct care for hospitalized patients as well as for patients to provide self-care at home. This program would be adjusted into more various diseases and settings.
Objectives : Recent educational efforts have concentrated on patient's early hospital arrival after symptom onset. The purpose of this study was to evaluate the time interval between symptom onset and hospital arrival and to investigate its relation with clinical outcomes for patients with acute ischemic stroke. Methods : A prospective registry of patients with signs or symptoms of acute ischemic stroke, admitted to the OO Medical Center through emergency room, was established from September 2003 to December 2004. The interval between symptom onset and hospital arrival was recorded for each eligible patient and analyzed together with clinical characteristics, medication type, severity of neurologic deficits, and functional outcomes. Results : Based on the data of 256 patients, the median interval between symptom onset and hospital arrival was 13 hours, and 22% of patients were admitted to the hospital within 3 hours after symptom onset. Patients of not-mild initial severity and functional status showed significant differences between arrival hours of 0-3 and later than 3 in terms of their functional outcomes on discharge. Logistic regression models also showed that arrival within 3 hours was a significant factor influencing functional outcome (OR=5.6; 95% CI=2.1, 15.0), in addition to patient's initial severity, old age, cardioembolism subtype, and referral to another hospital. Conclusions : The time interval between symptom onset and hospital arrival significantly influenced treatment outcome for patients with acute ischemic stroke, even after controlling for other significant clinical characteristics. The findings provided initiatives for early hospital arrival of patients and improvement of emergency medical system.

Objective : Hemangioblastomas are highly vascular and benign neoplasm of the central nervous system[CNS]. They can often be found as multiple lesions, as is commonly observed in von Hippel-Lindau[VHL] disease. The aim of this study is to determine the proper management for multiple hemangioblastomas. Methods : Since 1990, 78cases of hemangioblastoma have been encountered. Among these, 9cases were multiple hemangioblastomas that were treated with surgical resection with or without radiosurgery. The medical, radiological, surgical and histological records were reviewed retrospectively and analyzed statistically. Results : Nine patients presented with multiple hemangioblastomas and were diagnosed as VHL disease. The mean follow-up duration was 75.7months [$6.6{\sim}159.2months$] after the first surgical treatment. Three patients were treated with surgical resection alone and six patients were treated by both surgical resection and radiosurgery. Twenty-one surgical procedures [13 surgical resections and 8 radiosurgery] were performed. One patient required ventriculoperitoneal shunt and a posterior fossa decompressive craniectomy because of post-radiation brain swelling. Another patient refused additional treatment for the newly developed lesions after the successful treatment of initial lesions. The other patient who presented with numerous lesions in the whole brain and spine underwent cranio-spinal irradiation. Remaining patients showed good results. Conclusion : The surgical outcomes for the patients with a single lesion of the CNS hemangioblastoma are favorable. However. the treatment of multiple hemangioblastoma is more difficult, and should be treated by surgical resection and radiosurgery with careful consideration.
