A Study on Loose Laboratory Reports in A Hospital (일개(K) 병원의 누락 조직검사결과지에 관한 조사연구)

  • Background : The medical record is a compilation of pertinent facts of a patient's life and health history, including past and present illness and treatment. It is written by the health professionals contributing to that patient's care. And the medical record is the permanent, legal document which must contain sufficient information to identify the patient, justify the diagnosis and treatment, and record the results. As such, it must be accurate and complete. So we try to analyze the medical record especially a kind of incomplete record, loose laboratory reports. Methods: During the one-year period(from January to December 1988), a medical record practitioner examine and analyze the record of laboratory reports at K Hospital in Seoul. A total of 320 loose laboratory reports for 3,818 admitted laboratory reports. And a medical record practitioner and a physician review and analyze the influencing factors for the various reasons of clinical and laboratory aspects. Result: The loose percentage by department is the highest in obstetrics(40.4%) but the highest loose rate is in pediatrics(25.0%). The most of omission is occurred in operation room(80.3%) than OPD(19.7%). The change of diagnosis is according to duration of laboratory and more changable in cancer patient. Conclusion : Regular analysis of the documentation in the medical record so it fulfills its purposes of communicating patient care information. So it serves as evidence of the patient's course of illness and treatment for various legal, reimbursement, and peer evaluation review. And it is very important aspect of quality assurance in medical activities.

  • The remote healthcare monitoring system enables a doctor to diagnose and monitor patient's health problem from a distance. Previous researches have focused on key establishment method between a patient and a particular doctor to solve personal health information disclosure problem in data transmission process. However, when considering a misdiagnosis of doctor, the result of a diagnosis by a many doctors is more reliable. In previous work, in order to select multiple doctors, patient should generate shared key for each chosen doctor and perform many times encryptions. Therefore, in this paper, we propose a secure data transmission protocol for receiving diagnosis from multiple doctors using identity-based proxy re-encryption scheme. In proposed protocol, a patient don't need key management work for session key. Also, monitoring server performs re-encryption process on behalf of patient. So, we can reduce computational burden of patient in previous work.

Quality Evaluation of Medication Counseling in Korean Community Pharmacies (지역약국에서 시행하는 복약지도의 질적 수준 평가)

  • Background: Medication counseling is important to improve qualify of pharmaceutical care, but there are lack of studies to investigate satisfied counseling practice in community pharmacies. Purpose: This study was to investigate current medication counseling provided by community pharmacists. Method: Questionnaires to assess medication counseling practices were mailed to nationwide 1,269 community pharmacists who were Sookmyung Women's University alumni. Result: One hundred sixty five pharmacists were responded to the questionnaires (response rate 13.63%). 16.8% of them all respondents had counseling room in their pharmacies. 75% and 29.9% of respondents provided patient counseling always for new and refilled prescriptions, respectively. Counseling was provided primarily by verbal, but especially for drug name, usage and storage, it was provided in written concurrently. 31.1% of respondents was satisfied with counseling practices by themselves, and a major barrier of unsatisfied counseling was lack of time in 64.6% of respondents. Majorities of respondents (66.3% and 76.3%) spent 1-3 minutes and <1 minute, for new prescription and refilled prescription, respectively. Approximate 99% of respondents presented necessity of standardized patient counseling information for drugs and 97.6% responded that quality of patient counseling would be improved by using software containing counseling information. Old respondent group had higher accumulative patient counseling practice scores and pharmacy owner group took much time for counseling, resulting in higher scores too. Conclusion: This study showed that qualified patient counseling practice has not been reached in community pharmacies. Much more discussion to seek ways to improve the quality would be necessary.

  • In order to apply the patient monitoring system to the hospital field, it is necessary to be able to measure and analysis data the major bio-signals that are basically covered by the existing patient monitoring system. We have implemented a wearable device and the patient monitoring system for measuring ECG and oxygen saturation. The implemented system transmits the measured bio-signal to the server on the nursing station via Bluetooth. It is represented by graph waveforms and numerical values that can be checked by the medical staff in the patient monitoring system. The validity of this system is verified by comparing the data collected through the designed system with the data obtained from the conventional equipment.

A Study on Needs of the Spinal anesthesia Patients (척추마취 수술환자의 간호요구)

  • The purpose of this study was to identify the needs which were perceived by patients who were received spinal anesthesia for surgery. The subjects consisted of 50 adult patients who were admitted to 2 university hospitals and 2 general hospitals in Pusan city and 1 general hospital in Koje City for surgery under spinal anesthesia. Thirty eight percent of subjects received information about anesthesia before the operation. The instrument for this study was developed by the researcher based on literature and a pretest. Data were collected from December 10, 1999 to February 10, 2000 and were analyzed by content analysis. The results were that there were 533 meaningful statements in the needs of spinal anesthesia patients. The needs of spinal anesthesia patients had 51 items (preoperation (6), induction of anesthesia(5), intraoperation (27), postoperation(13)) and 6 categories (information, emotional welfare, physical welfare, post anesthetic management, control of physical environment, humane treatment). From the results, it can be concluded that: 1. In the pre-operation period, we have to explain anesthesia procedures, adequate position of anesthesia, duration before anesthesia wears off and sensation of paralysis. We have to supply emotional support to relieve anxiety because of anesthesia. 2. In induction of anesthesia, we have to support patient's position for anesthesia, and relieve anxiety so that patients participate in induction of anesthesia well. 3. In intra-operative period, we have to check the level of anesthesia, and keep up a comfortable position for operation and care for physical discomfort such as thirst, nausea, vomiting, dyspnea and to maintain body temperature of the patient. Since the patient is conscious, we have to communicate with the patient to relieve anxiety, maintain privacy, inform the patient of the process of the operation and encourage the surgeon to explain the outcome of the operation. The operating team needs the careful about what they say and to place the instrument well. We have to ventilate the room air and reduce noise. 4. In the post-operative period, we have to explain the purpose and duration of bed rest, complications of anesthesia and care for physical discomfort such as pain, dysuria, headache, backache. Also we have to maintain body temperature of the patient and maintain privacy.

Telemedicine robot system for visual inspection and auscultation using WebRTC (WebRTC를 이용한 육안 검사 및 청진용 원격진료 로봇 시스템)

  • When a doctor examines a patient in a hospital, the doctor directly checks the patient's condition and conducts a face-to-face diagnosis through dialogue with the patient. However, it is often difficult for doctors to directly treat patients. Recently, several types of telemedicine systems have been developed. However, the systems have lack of capabilities to observe heart disease, neck condition, skin condition, inside ear condition, etc. To solve this problem, in this paper, an interactive telemedicine robot system with autonomous driving in a room capable of visual examination and auscultation of patients is developed. The developed robot can be controlled remotely through the WebRTC platform to move toward the patient and check a patient's condition under the doctor's observation using the multi-joint robot arm. The video information, audio information, patient's heart sound, and other data obtained remotely from patients can be transmitted to a doctor through the web RTC platform. The developed system can be applied to the various places where doctors are not possible to attend.

A Web-based Platform for Managing Rehabilitation Outcome Measures

  • Background: Effective management of clinical assessment tools is critical in stroke and brain injury rehabilitation research. Managing rehabilitation outcome measures (ROMs) scores and training therapists in multicenter randomized clinical trials (RCTs) is challenging. Objects: The aim of this study was to develop a web-based platform, the Korean Rehabilitation Outcome Measurement (KoROM), to address these limitations and improve both therapist training and patient involvement in the rehabilitation process. Methods: The development of the KoROM spanned from June 2021 to July 2022, and included literature and web-based searches to identify relevant ROMs and design a user-friendly platform. Feedback from six physical therapy and informatics experts during pilot testing refined the platform. Results: Several clinical assessment tools categorized under the International Classification of Functioning, Disability, and Health (ICF) model are categorized in the KoROM. The therapist version includes patient management, assessment tool information, and data downloads, while the patient version provides a simplified interface for viewing scores and printing summaries. The master version provides full access to user information and clinical assessment scores. Therapists enter clinical assessment scores into the KoROM and learn ROMs through instructional videos and self-checklists as part of the therapist standardization process. Conclusion: The KoROM is a specialized online platform that improves the management of ROMs, facilitates therapist education, and promotes patient involvement in the rehabilitation process. The KoROM can be used not only in multi-site RCTs, but also in community rehabilitation exercise centers.

  • RFID(Radio Frequency IDentification) is a method of remotely storing and retrieving data using devices called 'RFID tags'. An RFID tag is a small object, such as an adhesive sticker, that can be attached to or incorporated into a product. RFID tags contain antennas to enable them to receive and respond to radio-frequency queries fro an RFID transceiver. Eventually, RFID SYSTEM is very interesting to watch people live linger than they used to before. Some of reasons for that phenomenon is the improving medical studies. As we live healthier and get to know more about our body, we can extend our lives. This Platform is Physical Health Promotion Service, Home Health Monitoring Service, Mental Health Promotion Service, Home Patient Management Service, Chronic Disease Management Service, Emergency Patient Management Service, Medical Information and Consulting Service of the RFID.

  • The purpose of this article is (1) to measure the attitudes of health care consumers towards medical service, the physicians' duty to provide information and patient self-determination options, (2) to discover the their related variables. The attitude of health care consumers towards medical service reveals statistically significant corelation with age and education. Among the statistically significant independent variables it is significantly related with age in the multiple regression analysis. The attitude of health care consumers towards the physicians' duty to provide information reveals statistically significant corelation with age, education and the attitude of health care consumers towards medical service. Among these independent variables it is significantly related with the attitude of health care consumers towards medical service in the multiple regression analysis. The attitude of health care consumers towards patients' self-determination options reveals statistically significant corelation with age, the attitude of health care consumers towards medical service and the attitude of health care consumers towards the physicians' duty to provide information. Among these independent variables it is significantly related with the attitude of health care consumers towards the physicians' duty to provide information in the multiple regression analysis.

  • 본 논문에서는 센서 네트워크를 이용해서 환자 모니터링 시스템(PVMS : Patient Vital Sign Monitoring System)을 구현하였다. 최근 의료 서비스에 유비쿼터스 컴퓨팅 기술을 적용한 사례들이 늘고 있다. 기존 사례에서는 센서 전지 수명, 이동 통신비, 응급상황 대처 등 개선할 부분이 존재한다. 본 연구에서는 이런 점들을 해결하기 위해 환자의 체온 및 맥박 생체신호를 측정하기 위해 소형센서를 사용하였다. 또한 생체신호 전달을 위해 초저전력 무선통신 노드를 사용하여 언제 어디서나 환자 모니터링이 가능하고 의료진에게 응급상황을 신속하게 전달할 수 있는 웹기반 시스템을 개발하였다. 본 연구의 결과는 병동환자뿐만 아니라 활력증후를 상시로 모니터해야하는 원거리 환자를 위한 의료시스템 구축에도 활용될 수 있을 것이다.
