분만기관 선택과 이용 후의 평가 (Choice of Medical Care Institution for Delivery and Evaluation of the Institution after Delivery)

  • There exists a general consensus in Korea that patients tend to concentrate in large hospitals and this tendency is partly responsible for inefficiency in health services. The process of choosing a medical care provider for health care services and evaluating the provider after utilization seems to involve many diverse factors to become very complex. Therefore a systemsatic study is needed to achieve sufficient understanding of the proeess. For this point of view, this study investigates patient's selection of medical care institution for delivery care services and their evaluation of the institution after delivery. In more specific, the objectives of the study are twofold: 1) to identify the factors associated with expectant mothers' choice of type of medical care institution for delivery among tertiary hospitals, general hospitals, small hospitals, and clinics: and 2) to understand the factors affecting patient evaluation of the medical care institution after delivery. The data used for the analysis were collected through face-to-face interviews with those women who had childbirth during the period from January 1, 1996 to the date of interview in February 1998. The survey was conducted using preqared structured questionnaire in Seoul. The sample was drawn from each of arbitrarily defined four regions of Seoul, Northeast, Northwest, Southeast and Southwest, in proportion to the number of births reported in 1996 in each of them. The distribution of the interviewed women by educational level was made similar to that of mothers of new babies reported in 1996. The sample size was planned to be about 300, but ended up with analytical sample of 319. Major conclusions emerged from the analysis can be summarized as follows: 1) Large hospitals were evaluated as much better for technical quality than other types of institutions, whereas they were compared similar to or worse for other attributes. And it was found that technical quality of care is considered as the most important condition of medical care institution for delivery, while the amount of direct cost is considered as the least important one. Taken together, the utilization of large hospitals is not likely to decrease even though they cannot give satisfaction to patients in other aspects than technical quality. 2) The activeness in the search for information affected the respondents' evaluation of medical care institutions, which would influence their later decision or recommendation to other persons as to the choice of source of health care services. Therefore, increased efforts should be directed to improving availability of useful and correct information for patients in relation to the utilization of health care services. 3) Since the findings of this study were obtained from the analysis of delivery care services, their applicability to other kinds of services may be limited. Thus it would be useful to conduct a comparative study of several kinds of services explicitly taking into account the characteristics of those services in the analysis.

  • Purpose: The purpose of the study was to develop an educational program reflecting the educational needs of Hospice Smart Patient service providers. Method: The description, goal, curriculum, method, and process evaluation of the educational program were constructed based on Modified Tyler-type Ends-Means Model followed by the analysis of current curriculum and needs of service providers. Results: The curriculum was constructed based on hospice volunteer program currently offered in Korea and the recommendations of hospice service volunteers and experts. A total of 90 hr was required to complete the curriculum that was composed of 'Introduction to cancer', 'Treatment and treatment complications of cancer', 'Post-treatment nutritional care', 'Helpful information', 'Introduction to hospice and palliative care', 'Comprehension of life and death', 'Holistic hospice and palliative care', 'How to communicate as a smart patient', 'Hospice and ethics', 'Pediatric hospice', 'Bereavement management', and 'Clinical practicum'. Conclusion: It is necessary to implement the developed educational program and evaluate its effectiveness, as well as making the service available to a greater number of cancer patients.

Treatment Strategies of Improving Quality of Care in Patients With Heart Failure

  • Heart failure (HF) is a global health problem closely related to morbidity and mortality. As the burden of HF increases, it is necessary to manage and treat this condition well. However, there are differences between real-world practice and guidelines for the optimal treatment for HF. Patient-related, healthcare provider-related, and health system-related factors contribute to poor adherence to optimal care. This review article aims to examine HF treatment patterns and treatment adherence in real-world practice, identify clinical gaps to suggest ways to improve the quality of care for HF and clinical outcomes for patients with HF. Although it is important to optimize treatment based on evidence-based guidelines to the greatest extent, it is known that there is still poor treatment adherence, and many patients do not receive guideline-directed medical therapy, especially at the early stages. To improve medication adherence, qualitative evaluation through performance measurement, as well as education of patients, caregivers and medical staff through a multidisciplinary approach are important.

Outcomes of Closed versus Open Rhinoplasty: A Systematic Review

  • Open and closed rhinoplasty are two main approaches to perform nasal modifications. According to current literature, there is no current consensus among plastic surgeons and otolaryngologists on which technique is preferred in terms of aesthetic result, complications, and patient satisfaction. This study uses published research to determine whether open or closed rhinoplasty leads to superior patient outcomes. Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines for systematic reviews were followed and a literature search was conducted in four databases based on our search strategy. Articles were then imported into COVIDENCE where they underwent primary screening and full-text review. Twenty articles were selected in this study after 243 articles were screened. There were 4 case series, 12 retrospective cohort studies, 1 prospective cohort study, 1 case-control, and 2 outcomes research. There were three cosmetic studies, eight functional studies, and nine studies that included both cosmetic and functional components. Sixteen studies utilized both open and closed rhinoplasty and four utilized open rhinoplasty. Both techniques demonstrated high patient and provider satisfaction and no advantage was found between techniques. Based on available studies, we cannot conclude if there is a preference between open or closed rhinoplasty in terms of which technique leads to better patient outcomes. Several studies determined that open rhinoplasty and closed rhinoplasty leads to comparative patient satisfaction. To make outcome reporting more reliable and uniform among studies, authors should look to utilize the Nasal Obstruction and Septoplasty Effectiveness scale and the Rhinoplasty Outcome Evaluation.

한국형 외래환자분류체계의 개발과 평가 (Development and Evaluation of Korean Ambulatory Patient Groups)

  • With the prospect of rapidly growing health insurance expenditures, particularly spending for ambulatory care, the introduction of a case-based payment method is discussed as an alternative to the current fee-for-service based method. A system to measure case mixes of providers is a core component of such payment systems. The objective of this study were to develop a classification system for ambulatory care, Korean Ambulatory Patient Group (KAPG) based on the U.S. APG version 2.0 and to evaluate the classification accuracy of the system. A database of 64,258,386 records was constructed from insurance claims submitted to the Health Insurance Review Agency (HIRA) during three months from August 2002. A total of 41,347,307 records with a single visit was used for the development and 7% random sample of the database was used for the evaluation. Additional groups were defined to include both physician and hospital fees in the classification, age splits were added to classify the entire population as well as the population older than 65, and the definition of medical groups used by the HIRA was adopted. The variance reduction in charges achieved by KAPGs was computed to evaluate the accuracy of classification. A total of 474 KAPGs was defined compare to 290 groups in the U.S. APG. The variance reduction for charges of all visits ranged from 20% to 37% depending on the type of provider, and ranged from 22% to 42% for non-outliers, that were better than those achieved by the system currently used by the .HIRA for its internal review purpose. Although further study is required to improve the classification for complicated care in larger hospitals, the results indicated that KAPGs could be used for better management of costs for ambulatory care.

간호 개념의 의미에 대한 조사연구(경인지역 성인을 대상으로) (A Study on Adult's Perception of Nursing Concept)

  • 'What is nursing' this question could always be arised and such a question could bring forth the new possibility to definete the nursing concept more clearly. At the middle of 19th Centry, Mrs. Nightingale defined the concept of nursing as follows; 'Nursing is a kind of treatment act to aid the patient so that the health of patients may be recovered naturally, as keeping the most comfortable circumstances.' But after then, the role and function of nurses about purpose and method of nursing has continuously been studied, as the social circumstance has been changed. The fact that care provider and client have the same concept about nursing is very important at the first step of assessment. But at the present time, the care provider and client have not same concept yet, so the difference of unrsing concept between care provider and client is analyzed in this study. This study would be belived to be helpful for the advance of nursing in the future. In this study, 20 questionare from nursing objectives developed by Abdellah(basic care needs, sustenal care needs, remedial care needs and restorative care needs) are used for adult. The data of this study by the 6 point rating scale are analyzed by SAS as follows; 1. Respondent's view is that nursing is necessary in case of group(school or company) rather than private and in case of abnormal conditions rather than normal conditions. 2. Every questionares of nursing objectives are divided into 4 points of view such as basic care needs, sustenal care needs, remedial care needs and restorative care needs are examined. The evaluation by 6 point rating scale revealed that $5.08\pm0.65$ point in basic care needs $4.93\pm0.68$ point in sustenal care needs $4.91\pm0.80$ point in remedial care needs and $4.61\pm0.91$ point in restorative care needs. While basic care needs and substenal care needs that need more physical care show high points, remedial care needs and restorative care needs that need more psychological, social and spiritual care show low points. 3. It was checked whether there is any significant difference between above 4 point of views in nursing objectives and qeneral characteristics or not. As a result, there is significant difference between 4 point of view and ages, educational level, marriage, composition of children. And also there is significant difference between religion and basic care needs, remedial care needs and restorative care needs. But there is no significant difference between any point of view and sex, occupation, experience of admission and experience of family admission. As this study is based on the data gethered from a restricted area, the result can not represent the opinion of all the clients. Therefore the same kind of study should be carried out on many areas repeatedly and also it should be tried to extract objective concept. And also periodical study is needed to observe the changing process of nursing concept.

의료서비스 마케팅을 위한 품질지각과 만족에 관한 연구 (A Study on Qulity Perceptions and Satisfaction for Medical Service Marketing)

  • INSTRODUCTION Service quality is, unlike goods quality, an abstract and elusive constuct. Service quality and its requirements are not easily understood by consumers, and also present some critical research problems. However, quality is very important to marketers and consumers in that it has many strategic benefits in contributing to profitability of marketing activities and consumers' problem-solving activities. Moreover, despite the phenomenal growth of medical service sector, few researchers have attempted to define and model medical service quality. Especially, little research has focused on the evaluation of medical service quality and patient satisfaction from the perspectives of both the provider and the patient. As competition intensifies and patients are demanding higher quality of medical service, medical service quality and patient satisfaction has emerged as a critical research topic. The major purpose of this article is to explore the concept of medical service quality and its evaluation from both nurse and patient perspectives. This article attempts to achieve its purpose by (1)classfying critical service attibutes into threecategories(satisfiers, hygiene factors, and performance factors). (2)measuring the relative importance of need criteria, (3)evaluating SERVPERF model and SERVQUAL model in medical service sector, and (4)identifying the relationship between perceived quality and overall patient satisfaction. METHOD Data were gathered from a sample of 217 patients and 179 nurses in Seoul-area general hospitals. From the review of previous literature, 50 survey items representing various facets of the medical service quality were developed to form a questionnaire. A five-point scale ranging from "Strongly Agree"(5) to "Strongly Disagree"(1) accompanied each statement(expectation statements, perception statements, and importance statements). To measure overall satisfaction, a seven-point scale was used, ranging from "Very Satisfied"(7) to "Very Dissatisfied"(1) with no verbal labels for scale points 2 through 6 RESULTS In explaining the relationship between perceived performance and overall satisfaction, only 31 variables out of original 50 survey items were proven to be statistically significant. Hence, a penalty-reward analysis was performed on theses 31 critical attributes to find out 17 satisfiers, 8 hygiene factors, and 4 performance factors in patient perspective. The role(category) of each service quality attribute in relation to patient satisfaction was com pared across two groups, that is, patients and nurses. They were little overlapped, suggesting that two groups had different sets of 'perceived quality' attributes. Principal components factor analyses of the patients' and nurses' responses were performed to identify the underlying dimensions for the set of performance(experience) statements. 28 variables were analyzed by using a varimax rotation after deleting three obscure variables. The number of factors to be extracted was determined by evaluating the eigenvalue scores. Six factors wereextracted, accounting for 57.1% of the total variance. Reliability analysis was performed to refine the factors further. Using coefficient alpha, scores of .84 to .65 were obtained. Individual-item analysis indicated that all statements in each of the factors should remain. On 26 attributes of 31 critical service quality attributes, there were gaps between actual patient's importance of need criteria and nurse perceptions of them. Those critical attributes could be classified into four categories based on the relative importance of need criteria and perceived performance from the perspective of patient. This analysis is useful in developing strategic plans for performance improvement. (1) top priorities(high importance and low performance) (in this study)- more health-related information -accuracy in billing - quality of food - appointments at my convenience - information about tests and treatments - prompt service of business office -adequacy of accommodations(elevators, etc) (2) current strengths(high importance and high performance) (3)unnecessary strengths(low importance and high performance) (4) low priorities(low importance and low performance) While 26 service quality attributes of SERPERF model were significantly related to patient satisfation, only 13 attributes of SERVQUAL model were significantly related. This result suggested that only experience-based norms(SERVPERF model) were more appropriate than expectations to serve as a benchmark against which service experiences were compared(SERVQUAL model). However, it must be noted that the degree of association to overall satisfaction was not consistent. There were some gaps between nurse percetions and patient perception of medical service performance. From the patient's viewpoint, "personal likability", "technical skill/trust", and "cares about me" were most significant positioning factors that contributed patient satisfaction. DISCUSSION This study shows that there are inconsistencies between nurse perceptions and patient perceptions of medical service attributes. Also, for service quality improvement, it is most important for nurses to understand what satisfiers, hygiene factors, and performance factors are through two-way communications. Patient satisfaction should be measured, and problems identified should be resolved for survival in intense competitive market conditions. Hence, patient satisfaction monitoring is now becoming a standard marketing tool for healthcare providers and its role is expected to increase.

메타분석을 이용한 임상영양서비스의 비용-효과성 평가 (Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. 취약계층 대상 보건의료·복지 네트워크 사업 성과에 대한 질적연구 : 달구벌건강주치의사업을 중심으로 (A Qualitative Research on the Evaluation of Healthcare and Welfare Network for Vulnerable Populations : Focusing on the Dalgubeol Health Doctor Services)

  • 본 연구는 달구벌건강주치의사업에 참여하고 있는 기관의 실무자와 서비스 이용자들의 경험을 토대로 질적연구를 통하여 보건의료·복지 서비스의 통합적인 지원의 성과와 개선방안을 살펴 보았다. 분석 결과 달구벌건강주치의사업은 의료취약계층의 복합적 수요 해결에 효과적이었다. 의료와 복지 사각지대에서 삶의 희망을 잃어버리고 의료와 복지서비스 이용에 대한 두려움을 가진 이용자들에게 신속 간단한 대상자 선정과 자원연계를 통한 통합적 서비스 제공으로 복합적 수요 해결과 삶에 대한 긍정적인 태도 회복, 삶의 질 개선에 기여하였다. 달구벌건강주치의사업은 대상자 의뢰기관부터 서비스 제공기관까지 공공뿐 아니라 민간기관이 참여하는 포괄적 네트워크를 구축하였으며, 대구의료원을 중심으로 5개 상급종합병원의 참여로 경증부터 최중증까지 대상자의 중증도에 적합한 치료를 지원하는 모형을 구축함으로써 의료혜택에서 소외된 대상자를 적극적으로 발굴하고 실제 이들의 건강 문제를 해결함으로써 건강 형평성을 개선한 것으로 파악된다.

고객의 개인적 요소가 대기시간 경험에 미치는 영향에 대한 탐색적 연구 (An Exploratory Study on Customers' Individual Factors on Waiting Experience)

  • 서비스를 구매하기 위해 고객들은 자주 대기를 경험하게 된다. 고객은 대기로 인해 불만족을 느끼고 서비스 장소를 이탈할 수도 있기 때문에 서비스 제공자가 대기시간을 관리하는 것은 중요하다. 이를 위해 대기시간에 관한 연구가 다양하게 진행되어 왔다. 그 결과, 서비스 제공 단계별 대기시간 연구, 실제와 지각된 대기시간에 대한 연구, 대기를 경험하는데 영향을 미치는 대기환경과 대기정보 등 매개변수에 대한 연구, 고객의 특성에 따른 대기경험의 차이 연구 등을 통해 고객의 대기경험을 개선시키고자 하였다. 본 연구에서는 Hui and Tse(1996)가 제시한 고객의 행동적 메커니즘의 평가적 요소, 즉 지각된 대기시간, 대기수용정도, 대기에 대한 감정적 반응을 고객이 대기할 때 경험한다는 것을 바탕으로 서비스 제공자가 대기 시점에서 통제하기 어려운 고객의 개인적 요소들(고객충성도, 거래중요도, 대기기대수준)이 미치는 영향을 알아보기 위해 실제 대기상황(패밀리 레스토랑, 놀이공원)에서 탐색적 연구를 하였다. 두 번의 실험을 진행한 결과는 다음과 같다. 첫째, 레스토랑에서 조사한 결과, 고객의 거래중요도가 대기경험에 긍정적으로 미치는 영향이 큰 것으로 나타났다. 이는 레스토랑의 서비스 제공자가 고객의 거래중요도를 높일 수 있는 전략을 통해 고객의 이탈을 효과적으로 방지할 수 있음을 시사한다. 둘째, 놀이공원에서 조사한 결과는 거래중요도뿐만 아니라 고객충성도가 대기경험에 긍정적인 영향을 미치는 것으로 나타났다. 이를 통해, 놀이공원의 서비스 제공자는 지속적으로 고객충성도를 높일 수 있는 전략을 통해 필연적으로 발생하는 대기에 대한 고객의 불만을 최소화할 수 있음을 시사한다. 위의 연구결과는 서비스 장소의 특성에 따라 고객이 대기를 다르게 경험하기 때문에 서비스 제공자는 해당 서비스 장소에 따라 고객특성을 파악하여 이를 강화, 개선해 대기시간을 관리하는 것이 효과적일 수 있음을 시사한다. 셋째, 지금까지 대부분의 연구에서 조절적인 변수로 취급되었던 거래중요도, 대기기대수준 등의 고객 특성이 대기경험에 영향을 미치는 주요 독립 변수로 연구됨으로써 대기경험에 직접적인 영향을 미칠 수 있다는 것을 밝혔다. 즉, 획일화된 서비스전략이 모든 고객을 만족시키지 못하기 때문에, 고객의 개인적 요소들에 따라 대기경험에도 차이가 생길 수 있음을 감안한 마케팅 전략이 필요하다. 또한, 대기시간 연구에서 다루어지지 않았던 고객충성도가 대기경험에 미친다는 것도 알아내었다. 고객충성도는 중·장기의 긍정적인 서비스 경험에 의해 형성되는 것이기 때문에 누적되는 고객만족의 중요성을 위해 대기시간 관리에서 중요한 개념이라고 할 수 있다. 넷째, 실제 대기시간과 대기경험 간의 관계에서 지금까지 대기시간 연구들이 지각된 대기시간을 위주로 연구해왔으나, 실제 대기시간이 대기경험에 미치는 영향도 주목할 필요가 있다는 것을 밝혔다. 특히, 본 연구에서는 한계 대기시간과 함께 실제 대기시간의 영향력을 알아봄으로써, 동일한 실제 대기시간에도 개인이 느끼는 한계 대기시간에 따라 대기경험에 다양한 반응이 있음을 알 수 있었다. 마지막으로, 대기경험 간의 관계에 있어서도 Hui and Tse(1996)의 연구와 달리 고객은 대기시간을 지각하면, 이에 대한 수용여부를 결정하고, 대기상황에 대한 감정적 반응을 한다는 어느 정도의 방향성을 발견할 수 있었다.
