  • This presentation is to enhance the pharmacist's role in Over-The-Counter(OTC) drug selection and patient counseling for diversification of pharmacy management after the separation of prescribing and dispensing practice in Korea. Self-medication by OTC drugs may be viewed as one element of the broader self-care treatment. The patient may use a OTC drug to manage a minor ailment, a process that may be supported by counseling from a pharmacist. Pharmacists involved in self-medication decisions have a greater involvement with patients and an enhanced professional role. (omitted)

일 호스피스실 이용 환자와 가족의 상담내용 분석 (Content Analysis with Counseling Cancer Patients and Their Relatives in Outpatient Hospice Office)

  • Purpose: The purpose of this study was to increase our understanding of the terminal cancer patients and their families concerns. I analyzed the counseling contents of terminal cancer patients and their relatives who referred to hospice office. Method: Data was collected from January 2004 to November 2004. During the counseling, I took notes the key points and contents. 109 patients and/or their families's counseling records were analyzed with the descriptive statistics and content analysis. Result: 73.4% of patients knew their current terminally ill status. The mobility of 86.2% of the patients was worse than ECOG 3 level. Patients have uncontrolled pain(28.4%), emotional distress(55.0%), and physical distress(49.5%). Caregivers of the patients were spouse(46.8%), sons and daughters(24.8%). Family members had problems to tell the bad news to their family cancer patient. 95.4% of the patient and family members was informed about the hospice and palliative care services, and 35.8% of them was referred to the hospice and palliative organizations. Frequently asked questions were 'what is hospice?', 'how much is the cost of hospice services?' etc. Conclusion: Care planning for terminal cancer patients must include patient. Also, professionals should consider and offer accessible, effective and empathetic counseling services to patients and families.

  • Objectives : The purpose of this study is to suggest that psychotherapy must be considered when treating conversion disorder patient with convulsion. Methods : We treated the patient with counseling and autogenic training in psychotherapies, not only traditional methods. Results : In this study, we found difference between when counselling and autogenic training is combined and when they are not practiced. Conclusions : Counseling and autogenic training is good treatment at patient with convulsion of conversion disorder.

MBTI와 한방 치료로 관찰한 양극성 장애 환자의 증례 (Clinical study on a case of Bipolar disorder with MBTI and OMT)

  • The purpose of this study was to investigate the clinical application of oriental medical therapy(OMT) and Myers-Briggs Type Indicator(MBTI) to Bipolar disorder. We treated the patient with OMT and Counseling. Counseling with the MBTI helped the patient to solve the life stress induced by the spouse conflict. Following the treatment, the patient's symptoms(somatic & mental) were improved. The applicability of Counseling with the MBTI has positive effects on the patient with Bipolar disorder.

철학상담을 적용한 여성화병환자 사례보고 - 자아정체성에 기반한 철학상담을 중심으로 - (A Case Study of Philosophical Counseling of a Woman with Hwabyung - With a Philosophical Counseling Method Based on Self-Identity -)

  • Objectives: The purpose of this paper is to report the results of applying philosophical counseling to Hwabyung. Methods: At the time of the medical examination, 15 minutes or more of philosophical counselling method based on self-identity was conducted if possible. Additionally, We applied Hwabyung treatment guidelines (acupuncture, herbal medicine, etc.) to the patient If diagnosed as necessary. We evaluated visual Analogue Scale (VAS), Hwabyung scale, and Symptom Checklist 90-Revision (SCL 90-R) at baseline, and reassessed VAS and Hwabyung scale after about two weeks of treatments. Results: After about two weeks of treatment and philosophical counselling, VAS was between 10 to 1-2 and Hwabyung scale from 59 to 48. Also, the problem of fragmentation in the patient's life process was insighted, and the viewpoint was changed. Conclusions: It is useful to combine Korean traditional medical treatment with philosophical counselling method based on self identity for patients suffering from the Hwabyung. To Hwabyung patient, the view of philosophical counselling of feminism which understands women in political and cultural contexts is helpful.

  • Objectives: Korean Medicine Doctors (KMDs) have treated patients with obesity using a holistic approach with a multicomponent approach on counseling. However, there is currently no data regarding KMDs' counseling practices for weight loss. We conducted a retrospective chart review to better understand the Korean medicine counseling practice for weight loss. Methods: Twenty-one KMDs were involved in this project as practitioners. The contents of counseling were categorized based on patterns according to the counseling manual for the standardized management of obesity in Korean Medicine. Results: The counseling was conducted based on the theory of Korean medicine and the contents of counseling were different from patterns. However, the quality of the counseling were different from each KMD, and the counseling were focused on evaluating patient conditions and not on providing specific recommendations for lifestyle changes. Conclusions: Therefore, specific guidance of counseling for healthier lifestyle and dietary habits, and the training of the standardized manual training are needed.

영양상담이 유방암 수술환자의 영양상태, 식습관 및 영양지식에 미치는 영향 (The Effect of Individual Nutrition Counseling on Diet and Nutrition Status of Postoperative Breast Cancer Patients)

  • This study was conducted to assess the effect of nutrition counseling for postoperative female breast cancer patients (N = 38). In baseline survey, we investigated the nutrition parameters by measuring anthropometric and biochemical blood index and assessing nutrient intake using recall method. Individual nutrition counseling was performed 3 times with 2 - 3 weeks intervals. Patients were offered nutrition information related to breast cancer and desirable life style after cancer operation. The effects of counseling program were assessed 2 month later. Of biochemical parameters, serum total cholesterol level was significantly reduced after nutrition counseling program. Nutrient intakes of postoperative breast cancer patients were generally below the RDA level at the baseline survey. After nutrition counseling, mean daily intake of total energy, protein, calcium were significantly elevated. Of dietary habit, meal regularity and control of portion size were improved and fruits and vegetables intake were significantly increased after nutrition counseling. Control of fatty food intake and alcohol drinking were significantly improved after nutrition counseling program. Score of general nutrition knowledge and knowledge about breast cancer, were significantly increased after nutrition counseling from 2.1 to 3.2 and from 2.3 to 3.8, respectively. It can be postulated that the dietary habit and nutrients intake of postoperative breast cancer patients can be improved by individual nutrition counseling program. In further study, the systematic group nutrition counseling program is needed.

임상영양서비스에 대한 환자의 인식 조사 (Patient Perceptions of Clinical Nutrition Service)

  • The purpose of this study was to evaluate the patient perception of clinical nutrition service. The research was performed by using questionnaires and conducted from February 14 to March 15 at 42 hospitals (over 400 beds). 41.7% of patients experienced nutritional education and counseling. The mean score of the patients' perception on clinical nutritional service was 4.62/5.00 for "nutrition care is important for treatment of the disease", 4.49/5.00 for "diet therapy is necessary for treatment of the disease", 4.16/5.00 for "nutritional counseling call-centers are necessary", 4.13/5.00 for "nutritional consultation fee is required to apply insurance benefits", 4.12/5.00 for "one-to-one nutrition system is necessary", and 3.56/5.00 for "nutrition services I am willing to pay". The patients who had no past experience in nutritional education and counseling showed significantly higher scores for "nutrition care is important for treatment of the disease", "one-to-one nutritional care system is necessary", and "nutritional counseling call-centers are necessary" (P<0.05). The mean scores for the importance (4.26/5.00) and performance (3.88/5.00) of nutrition counseling service were significantly different (P<0.01). "Nutritional counseling is available whenever I want" had the highest gap score between performance and importance among nutrition counseling service items. The importance and performance grid showed that highly important items had high performance (doing great area) and less important items have low performance (low priority).

8년 동안 진행된 만성치열(Chronic anal fissure) 환자에게 치자시탕과 상담요법을 사용하여 호전된 치험 1례 (A Case Study of Chronic Anal Fissure Treated With Chijasi-tang and Counseling)

  • Objectives : This study is aimed to report the outcomes of the treatment of a patient with chronic anal fissure during a 8-year period. Methods : Basic patient’s data was analyzed as a CARE guideline. And related psychological state was measured with BAI, STAI-X-II test. Results : The treatment is based on Shanghanlun and the patient reported better outcomes. The main symptoms of the patient were chronic constipation, blood in the stool. The main clinical findings were that the patient’s symptoms got worse during the examination period because of the anxiety of the exam. And loss of appetite and digestive disorders are due to Jue-Yin-Bing(厥陰病) and the clue of bleeding can be found in article 375 in Shanghanlun. The main diagnoses was chronic anal fissure and main interventions were Chijasi-tang according to article 375 in Shanghanlun and counseling on anxiety. As a results bleeding has stopped after taking herbal medicine and constipation has been treated at the same time. After 6 months, the patient performed BAI, STAI-X-II test and scores got lowered indicating decreasing anxiety level. Conclusion : Chijasi-tang with counseling achieved valid positive results to the patient with chronic anal fissure.

사상의학진료에서 상담기법의 활용 (Application of Counseling in Sasang Constitutional Medicine)

  • Objectives The purpose of this study was to investigate the relationship between counseling skills and Sasang constitutional medicine(SCM). Methods Reviewing the texts relevant to SCM, comparison between counseling skills and the excerpt from the relating books was conducted in terms of the attitude of the counselor, the observation method, the evaluation of the counselee, the relationship between the counselor and counselee, and the counseling method. Results As the attitude of the counselor, many kinds of attitudes and skills including respecting the counselee and reciprocal belief were needed. The excerpt from "Gyeokchigo" meant that the counselor or a doctor should have the virtue of Truth. To evaluate the patient's problems in SCM and the counseling, diet, digestion, and sleep could be asked, but questions about the emotions such as Joy, Anger, Sorrow and Pleasure should be more carefully modified in a modern way, when applying clinical practices. Empathy as the main principle of the counseling could be attributed to the main idea of Dongmu Lee Jema, in which all the people could be in the same status in a natural way reflecting that they like the virtue and dislike the evil. Recommendations of regimen and directives could be followed according to SCM Conclusions Several items of counseling skills and SCM coincide but some of the modern counseling skills are still needed to apply SCM to make the patients with psychiatric problems treated efficiently.
