일 대학병원 중환자실 간호사의 손 위생 수행 시점에 대한 지식 조사 (Survey of ICU Nurses' Knowledge of the Specific Moments of Hand Hygiene)

  • Purpose: This study was conducted to identify ICU nurses' knowledge of the five moments of hand hygiene and the ambiguity of these moments when demonstrating hand hygiene. Methods: The subjects were 200 intensive care unit nurses at a university hospital. Data was collected using self-report questionnaires, translated according to the instructions of training films developed by WHO, and analyzed using descriptive statistics and ranking tests. Results: The highest number of correct answers was regarding the moment before contact with a patient and the lowest was regarding the moment after contact with a patient. The rate of providing wrong answers regarding required moments of hand hygiene was high. Conclusion: The study identified ICU nurses' knowledge of specific moments of hand hygiene; they had difficulty differentiating between the moments that happened simultaneously, i.e. after touching a patient, and that patient's surroundings, and there was ambiguity concerning patient areas and medical treatment areas. It was concluded that it is necessary to educate nurses regarding both required and unrequired moments of hand hygiene and to ensure that they can distinguish between these moments.

환자 및 시설 특성이 장기요양서비스 이용 노인의 자원소모량에 미치는 영향 (The Effects of Patient and Facility Characteristics On the Resource Use by the Elderly in Long-term Care Services)

  • The purpose of this study is to measure the resource use of the elderly in long-term care services and to examine the effects of patient and facility characteristics on their use of resources. The data were collected from 510 old people over sixty years of age, residing in five long-term care hospitals and two skilled nursing homes during the period between December 1, 2000 and February 28, 2001. For a full sample, when the first level of RUG(Resource Use Group)-III categories were employed as the proxy of patient severity, facility characteristics, such as location, size and ownership, have large effects on the resource use measured by service intensity, whereas patient characteristics such as severity have little or no effect. The resource use is significantly high if the facility: (1) is located in rural areas (gun): (2) has mare than 200 beds; (3) is a long-term care hospital; (4) is private; and (5) has a low percentage of medical aid patients. The analysis of the resource use in each RUG-III categories, for which ADL(Ability of Daily Living) were employed as the prosy of patient severity, shows a similar result. The loose relationship between the needs of residents and the resource use seems to be closely associated with the ineffective reimbursement system for providers. The current reimbursement system has no provision for quality improvement and reimburses facilities simply according to their types: fee-for-service for long-term care hospitals, and monthly-flat-rate or full-coverage-national-aid for skilled nursing facilities. It will be necessary to develop a more reasonable reimbursement system that takes patient's severity into account and gives incentives for long-term care providers to offer cost-effective services.

일개 국립대학병원의 진료.교육.연구 기능에 대한 평가 (Evaluation of Patient Care, Education, and Research as Indicators of University Hospital Performance)

  • The purpose of this study was to evaluate the environments of patient care, education, and research as indicators of university hospital performance. The objective data for this study were collected by secondary data. The subjective data were collected by a total of 523 questionnaire survey interviews conducted in C university hospital, of which 123 were students, interns, or residents, 76 were professors, and the remaining 324 were patients. The major statistical methods used for the analysis were t-test and ANOVA. We found that the environment of patient care is better than the environment of education and research in C university hospital. While the concern for patient care of C university hospital was about 60%, the equivalent figures for education and research were both 20%. And the patients' satisfaction level was higher than the others. The environment of education and research was found to be deficient in some aspects, especially in such areas as facilities, equipment, and investment. In particular for professors the time for research was very limited because of the demands of patient care. These findings clearly demonstrate that university hospitals need to pay greater attention to the areas of education and research, as well as to patient care. This article concludes with a discussion and summary and presents issues in need of further study.

자세안정성 원리에 기반한 환자이동기술 개발 및 효과검정 (Development of Patient Transfer Techniques based on Postural-stability Principles for the Care Helpers in Nursing Homes and Evaluation of Effectiveness)

  • Purpose: This study was done to develop a postural-stability patient transfer technique for care helpers in nursing homes and to evaluate its effectiveness. Methods: Four types of patient transfer techniques (Lifting towards the head board of the bed, turning to the lateral position, sitting upright on the bed, transferring from wheel chair to bed) were practiced in accordance with the following three methods; Care helpers habitually used transfer methods (Method 1), patient transfer methods according to care helper standard textbooks (Method 2), and a method developed by the author ensuring postural-stability (Method 3). The care helpers' muscle activity and four joint angles were measured. The collected data were analyzed using the program SPSS Statistic 21.0. To differentiate the muscle activity and joint angle, the Friedman test was executed and the post-hoc analysis was conducted using the Wilcoxon Signed Rank test. Results: Muscle activity was significantly lower during Method 3 compared to Methods 1 and 2. In addition, the joint angle was significantly lower for the knee and shoulder joint angle while performing Method 3 compared to Methods 1 and 2. Discussion: Findings indicate that using postural-stability patient transfer techniques can contribute to the prevention of musculoskeletal disease which care helpers suffer from due to physically demanding patient care in nursing homes.

환자-가족중심 의료서비스의 융합적 성과: 체계적 문헌고찰 (Effectiveness of Patient and Family-Centered Care interventions: A Systematic Review)

  • 본 연구의 목적은 환자-가족중심 의료서비스의 특성과 성과를 체계적 문헌고찰 방법으로 분석하여 서비스의 융합적 유효성을 평가하는 것이다. 15개 전자 데이터 베이스에서 환자-가족중심 의료서비스의 성과를 연구한 RCT 문헌 검색, 선정, 자료 추출, 질 평가를 시행하였다. 선정된 21편 문헌의 환자-가족중심 의료서비스의 공통적 특성은 정보제공과 교육, 의사소통, 가족 및 친구의 참여였다. 환자-가족중심 의료서비스의 성과 측정 변수는 89개였고, 환자 성과는 사망률, 재원기간 등으로 측정되었으며 18편의 문헌에서 환자 성과가 유의하게 향상되었다. 본 연구는 환자-가족중심 의료서비스의 성과를 통합적으로 제시하여 서비스의 유효성을 증명함으로써 의료기관과 지역사회의 지속적인 의료질 향상 활동을 위한 당위성을 제공하였으며 향후 의료의 질 향상과 환자안전을 위한 환자-가족중심 의료서비스의 융합적 적용과 이에 대한 근거기반 연구가 필요함을 제언한다.

  • 최근 의료계에서 환자가 의료서비스의 소극적인 객체가 아니라 주체로서 인식되면서 환자의 위상을 강화하기 위한 다양한 시도가 이뤄지고 있다. 지금까지 의사가 주도했던 의료커뮤니케이션이 소비자주권의식과 의료정보의 활성화로 점차 환자중심으로 이동하고 있다. 이 글에서는 IT의 발달로 의료정보가 대중화되면서 환자들이 환우회, 소비자운동 등을 통해 의료소비자로서 위상을 강화하는 과정을 살펴본다. 이를 위해 의사와 환자의 커뮤니케이션에 영향을 미치는 의료정보의 역할, 환자 커뮤니티의 소비자운동 등을 다루고자 한다. 또한 정보통신기술을 활용한 의료시스템이 뿌리를 내리기 위해 연결망을 형성하고 실패하는 과정을 ANT(행위자-연결망이론)를 통해 분석한다. 환자중심의 의료서비스는 IT기술을 이용한 다양한 헬스케어 서비스 시장을 창출하며 의료정보의 비대칭성 개선에 기여하고 있다. IT융합 헬스케어 시장의 확대는 의사와 환자의 전통적인 관계에 근본적인 변화를 가져오면서 의료서비스시장을 재편하고 있다. 환자가 주도하는 모바일 헬스케어 모델의 확산 등 환자중심 의료가 가속화되고 있는 것이다.

중화민국(타이완) "안녕완화의료조례(安寧緩和醫療條例)"의 연혁과 내용 (Taiwan's Palliative and Hospice Care Act - Legislative Background and Controversial Issues -)

  • In Republic of Chaina (Taiwan), Natural Death Act named "Anning Huauhe Yiliao Tiaoli" which means palliative and hospice care act was enacted in year of 2000. And enforced in the same year. Many scholars say that Taiwan's Act took Many U.S.A.'s acts such as 'Federal Patient Self-Determination Act 1990', 'California Natural Death Act 1976' and 'Washington Natural Death Act 1979' for a model. Taiwan's Act adopts a few outstanding systems - 'advance declarations' including 'living will' and 'durable power of attorney for health care', 'family-determination system' for a patient who is in a persistent unconscious state. This paper disusses this Act. 'The content is as follow: 1. A background of legislation. 2. The purpose of legislation. 3. The concept of terms. 4. Patient's self-determination. 5. Subrogated determination by family. 6. Keeping documents. 7. Punitive provision. 8. The relationship with euthanasia. 9. Controversial issues.

국가 암 비용 감소를 위한 환자중심 진료의 적정성 확보 전략 (Strategies for Appropriate Patient-centered Care to Decrease the Nationwide Cost of Cancers in Korea)

  • In terms of years of life lost to premature mortality, cancer imposes the highest burden in Korea. In order to reduce the burden of cancer, the Korean government has implemented cancer control programs aiming to reduce cancer incidence, to increase survival rates, and to decrease cancer mortality. However, these programs may paradoxically increase the cost burden. For examples, a cancer screening program for early detection could bring about over-diagnosis and over-treatment, and supplying medical services in a paternalistic manner could lead to defensive medicine or futile care. As a practical measure to reduce the cost burden of cancer, appropriate cancer care should be established. Ensuring appropriateness requires patient-doctor communication to ensure that utility values are shared and that autonomous decisions are made regarding medical services. Thus, strategies for reducing the cost burden of cancer through ensuring appropriate patient-centered care include introducing value-based medicine, conducting cost-utility studies, and developing patient decision aids.

A Concept Analysis of Quality Nursing Care

  • Purpose: This study aimed to perform a concept analysis of quality nursing care. Methods: Walker and Avant's concept analysis method was used to carry out this study. Results: The defining attributes identified were as follows: caring, the nurse-patient relationship, and patient needs. Antecedents included patient characteristics, individual factors (age, education, knowledge, competence, and experience), job position, and environmental factors. The consequences of quality nursing care have significant influence on both patients and nurses. Conclusion: The findings can aid researchers in obtaining a better understanding of quality nursing care, and stakeholders can consider the factors related to quality nursing care and its consequences to improve the nursing process.

Completeness of Patient Care Report (PCR) by Paramedics

  • This study evaluated the completeness of patient care report (PCR). A retrospective quality analysis was conducted using raw data of 122,140 EMS activity reports prepared by paramedics in Gyeonggi-do from April 1 to May 31, 2021. In all, 67,830 cases of normal transfers were statistically analyzed using IBM SPSS Statistics version 22, and statistical significance was set at p<0.5. The 119EMT_2 certificate was omitted in 50,037 (73.8%) cases, followed by time-related items in 1,227 (1.8%) cases. In the primary assessment of vital signs, systolic blood pressure was omitted and erroneous in 1,218 (1.9%) and 1,129 (1.8%) cases, respectively. In the secondary assessment, the completeness of all vital sign items was approximately 70%. Advanced emergency care and online medical control (OLMC) reporting showed discrepancies in all items. As the severity of the patient's condition increased, the errors in the Patient care report (PCR) also increased, at a significant level (p= .00). Paramedics must be aware of the importance of completing the activity report.
