일 상급종합병원 간호사의 4차 산업혁명에 대한 인식 및 미래핵심간호역량 중요도-실행도 분석 (A Study on the Analysis of Nurses' Perception of the Fourth Industrial Revolution and the Importance and Performance of Future Core Nursing Competencies in a Tertiary Hospital)

  • Purpose: This study is descriptive survey research on the analysis of nurses' perception of the 4th industrial revolution and the importance and performance of future core nursing competencies in a tertiary hospital located in Seoul. Methods: Data were collected from 149 nurses with more than a year of work experience and analyzed using descriptive statistics, t-test, one-way ANOVA, and Importance Performance Analysis(IPA) with the IBM SPSS/WIN 25.0 program. Results: The nurses' perception of the 4th industrial revolution was 3.23±0.71 out of 5 points. The importance of future core nursing competencies was 4.31±0.48, and the performance of it was 3.47±0.54. The analysis results of IPA showed that A (area of continuous maintenance) included critical thinking, problem-solving skills, teamwork and collaboration, evidence-based practice, communication, quality improvement and safety, professionalism, self-regulation and self-management, and personal literacy. The specific competencies were not included in B (area of priority improvement). Creativity, informatics, healthcare policy, leadership, research ability, and continuing education were included in C (area of progressive improvement). Knowledge and patient-centered care, ability to manage resources as well as professional, legal, and ethical responsibility were included in D (area of overinvestment). Conclusion: The nurses seemed not to be fully prepared for the 4th industrial revolution. However, they were well aware of the importance of the future core nursing competencies. Therefore, if nurses increase the performance of core competencies in order of priority according to the IPA results, they will be able to independently lead the changing nursing field.

간호사의 비판적 사고성향, 문제해결능력 및 자기효능감이 환자안전역량에 미치는 영향 (The Survey on the Influence of Clinical Nurse's Critical Thinking Disposition, Problem-solving Skill and Self-efficacy on Patients Safety Competencies)

  • 본 논문은 임상간호사의 환자안전역량 강화를 위한 기초자료로 제공하기 위해 환자안전역량에 영향을 미치는 요인을 파악하고자 비판적 사고성향, 문제해결능력 및 자기효능감을 독립변수로 정하여 서술적 조사연구를 시행하였으며, 중소병원에 근무하는 6개월 이상 경력의 임상간호사 373명을 대상으로 하였다. 자료분석 결과 환자안전역량에 영향을 미치는 요인은 문제해결능력(${\beta}=.403$, p<.001)이 가장 큰 것으로 나타났으며 자기효능감(${\beta}=.156$, p=.005), 환자안전사고 보고 경험(${\beta}=.137$, p=.002), 비판적 사고성향(${\beta}=.130$, p=.018) 순으로 영향을 미치는 것으로 나타났다. 이들 변수는 대상자의 환자안전역량에 대해 42.3%(adj $R^2=.423$) 설명하였다(F=20.305, p<.001). 비판적 사고성향, 문제해결능력, 자기효능감은 학부과정에서부터 강조되는 것으로 실제 의료현장에서 환자의 안전을 최일선에서 다루는 임상간호사에게는 더욱이 필요한 요인이라고 볼 수 있다. 따라서 환자안전역량 강화를 위해서는 의료기관 자체 내에서 임상간호사를 위한 비판적 사고성향, 문제해결능력 및 자기효능감을 높이기 위한 교육프로그램과 같은 다양한 전략과 활동들이 요구된다고 생각된다. 또한 환자안전을 최우선으로 생각하고 환자안전사고 발생 시 적극적인 보고를 독려하는 한편 환자안전사고 보고를 저해하는 요인들을 면밀히 사정하여 보완하여 보고율 향상과 더불어 환자안전역량 강화에도 힘써야 할 것이다.

시뮬레이션 교육의 간호학 적용 (Use of Simulation in Nursing Education)

  • Purpose: Since the 1990s, there has been an increasing interest in patient safety and quality of care. As a result, undergraduate nursing students have difficulties in clinical nursing practice such as health assessment or providing nursing skills. The purpose of this paper was to review current issues related to use of simulations in nursing education. Method: We conducted a thorough literature review including related proceedings to identify present issues in use of simulation education in nursing. Result/Conclusion: Simulation education in nursing differs from that in medical science. In nursing education, we need to focus on developing competencies for nursing students, for example, nursing process, nursing skills, and therapeutic communication skills With an increasing number of human patient simulators, we suggest a more careful approach including faculty development, curriculum development, and cost effective strategic planning. We propose a reliable and valid scenario development among nursing faculty as a consortium in the future.

방사선사 핵심 직무역량 모델의 적합성 및 타당성 검증 (A Goodness of Fit and Validity Study of the Korean Radiological Technologists' Core Job Com petency Model)

  • 방사선사는 사람의 생명을 대상으로 하므로 전문적인 직무역량(job competency)이 요구된다. 그럼에도 현재 한국에서는 방사선사의 직무역량을 밝히고 있는 연구가 이루어진 바 없다. 이에 한국 방사선사의 핵심 직무역량 지표를 정의하고 역량군별 요인모델을 제시하고자 방사선사 직무역량에 대한 설문을 실시하여 최종 147명의 응답내용을 취합하였다. 응답 내용은 PASW Statistics Version 18.0와 AMOS Version 18.0을 이용하여 탐색적 요인분석과 확인적 요인분석을 실시하였다. 요인분석 결과 전문직업성', '환자관리', '건강 및 안전', '장비의 운영', '절차와 관리'의 5개 핵심 직무역량군과 17개의 하위 역량으로 구성된 모델을 구성하였다. 5개 핵심 직무역량군의 측정 모델 RMSEA 값은 0.1, CFI와 TLI 값이 0.9에 근사하게 나타나 양호한 적합도를 보였으며, 타당도 분석 결과 평균분산추출 값 0.5이상, 개념신뢰도 값 0.7이상으로 각 하위 역량에 포함된 세부 항목 간에 상관관계도 높게 나타났다. 이 연구의 결과는 한국의 의료 환경에서 방사선사에게 요구되는 직무역량을 명확히 제시하여 역량 중심의 인적 자원 양성과 관리에 필요한 구체적인 정보를 제공해 줄 수 있을 것으로 기대된다.

의료기관인증제도의 국내외 비교 연구 -한국, 미국 및 호주 중심으로- (A Comparative Study on National and International Hospital Accreditation Systems Focusing on Korea, the U.S., and Australia)

  • 본 연구는 국내 의료기관인증제도와 미국과 호주의 의료기관인증제도 및 국제의료질향상학회의 국제인증프로그램에 대해 비교 고찰함으로써 우리나라 의료기관인증제도의 발전을 위한 시사점을 제시하고자 시도되었다. 본 연구에서 도출된 시사점은 다음과 같다. 첫째, 환자 및 지역사회 중심의 관점 강화, 인력배치 기준 강화 등 의료기관 인증기준의 개선이 필요하다. 둘째, 의료기관인증제도를 통한 환자안전의 강화가 필요하다. 셋째, 의료기관인증제도와 임상질지표 및 환자경험평가 연계를 강화해야 한다. 넷째, 조사위원의 역량을 강화해야 한다. 다섯째, 중소병원의 인증제도 참여 활성화가 필요하다. 여섯째, 의료 질 평가 결과 공개의 강화가 요구된다. 본 연구 결과는 우리나라 의료기관인증제도의 개선을 위한 기초자료로 활용될 수 있을 것이다.

한국 방사선사의 초음파진단검사 직무역량에 관한 고찰 (Job Competency in Ultrasonography of Korean Radiological Technologists)

  • 캐나다 등 많은 국가들이 초음파사(sonographer) 면허제도를 방사선사면허와 별도로 운영하고 있다. 그러나 한국에서는 방사선사가 의사의 지도아래 초음파영상검사를 수행하고 있다. 이에 방사선사의 초음파영상검사에 대한 직무역량을 분석하여 체계적으로 교육할 기회를 갖기 위하여 캐나다 초음파사의 표준직무역량(National Competency Profile)을 토대로 한국에서 방사선사가 초음파영상검사를 수행함에 있어 요구되는 직무역량과 세부역량들에 대한 직무 내용타당도를 측정하였다. 한국 방사선사 직무역량에 포함된 핵심역량의 중요도과 직무수행 정도를 비교분석한 결과 전체 중요도의 평균은 4.087점, 전체 수행도의 평균은 3.640점으로 중요도가 더욱 높았으며, 통계적으로 유의한 차이를 보였다. 그 결과 'A 의사소통', 'B 전문가적 책임', 'D 장비취급', 'G 근무 장소의 건강 및 안전'은 직무 내용타당도가 높았다. 그러나 'C 환자 평가와 돌봄', 'E 비평적 사고와 문제해결', 'H 영상'은 직무 내용타당도가 낮게 나타나 이들 역량을 강화하고 보완할 수 있는 방안이 모색되어야 한다.

한국에서 역량바탕의학교육의 성공적인 실행을 위한 제언 (Recommendations for the Successful Design and Implementation of Competency-Based Medical Education in Korea)

  • Competency-based medical education (CBME) is an outcome-oriented curriculum model for medical education that organizes learning activities and assessment methods according to defined competencies as the learning outcomes of a given curriculum. CBME emerged to address the accountability of medical education in response to growing concerns about the patient safety in North America in the 1970s, and the number of medical schools adopting CBME has dramatically increased since 1990. In Korea, CBME has been under consideration as an alternative curriculum model to reform medical education since 2006. The purpose of this paper is three-fold: (1) to review the literature on CBME to identify the challenges and benefits reported in North America, (2) to summarize the process and experiences of planning and implementing CBME at Inje University College of Medicine, and finally (3) to provide recommendations for Korean medical schools to be better prepared for the successful adoption of CBME. In conclusion, one of the key factors for successful CBME implementation in Korea is how well an individual school can modify the current curriculum and rearrange the existing resources in a way that will enhance students' competencies while maximizing the strengths of the school's existing curriculum.

우리나라 전공의 수련교육 구성 및 운영에 대한 제안 (The Proposal for Residency Educational Programs)

  • In clinical clerkships, residents function as trainees, workers, and teachers for other medical students. Although residents care for patients in harsh environments and encounter precarious patient-safety situations, they are working towards becoming competent specialists. Residency education programs are very important in cultivating specialists able to adapt to the rapidly-changing medical environment, and are also necessary to improve the quality of specialist training. Competent specialists not only need clinical competency, but also a wide range of abilities including professionalism, leadership, effective communication, cooperation, and attention to continuous professional development/continuing medical education activities. Each Korean association of specialties has its own educational goals and standardized education programs to help residents learn specific techniques and competencies related to medical care for patients, though the training environment of each residency is different within each trainee hospital. Although it is also important to evaluate residency education programs, currently there is only an examination of knowledge and assessment of skills based on mini-clinical evaluation exercises or direct observation of procedural skills. In order to develop an objective and estimable evaluation tool that can assess the overall achievement level within each training course, it is necessary to evaluate the knowledge, skills, and attitudes of residents. Residency education programs need further attention and reform.

Effectiveness of Simulation-Based Education for Caring Patients with COVID-19

  • Purpose: The role of medical staff gained immense significance in the context of the prolonged coronavirus disease (COVID-19) pandemic. However, few studies had explored the impact of simulation-based education on the ability of nursing students to care for the patients of COVID-19. This study provided nursing students with simulation-based education in caring for the patients of COVID-19 and confirmed its effectiveness. Methods: This study used a non-equivalent control group pretest-posttest design. The participants were recruited from the nursing departments of two universities in Korea through convenience sampling. A total of 79 participants were included: 37 in the intervention group and 42 in the control group. The intervention group received four sessions of simulation training based on the National League for Nursing Jeffries simulation theory. Results: The intervention group showed an improvement compared to the control group in terms of knowledge related to coronavirus, confidence in performing infection control skills, and perception of preparedness for caring for the patients of COVID-19, with a high-level of satisfaction and self-confidence in learning. There was no significant difference between the two groups in terms of anxiety. Conclusion: This simulation is expected to be a significant strategy for alleviating the global burden in terms of staff safety and patient outcomes by improving the competencies of prospective medical staff in responding to pandemics.

  • The purpose of this study is to investigate various opinions on changes and revisions in the Medical Technician Act, to compare the curriculum of radiological technologist and physicians or dentists, and to compare the definitions and scope of work of radiological technologist in Korea and abroad. From the information, The goal is to review whether the phrase 'guidance of a doctor or dentist' specified in the definition of the 'Act on Medical Technicians, etc.' is realistically appropriate. radiological technologist receive specialized college education on radioligical science & medical imaging. The training hours for radiolgical science student are greater than medical students. In addition, radiological technologists are continuously developing their competencies for new knowledge and skills through continuing education in clinical fields. In particular, radiological technologist are making steady research efforts to reduce patient exposure and improve medical image quality. As a result of this investigation, it is considered that the term "guided by a doctor or dentist" as currently defined in the 'Act on Medical Technicians, etc.' may need to be revised in consideration of the professionalism of the radiological technologist.
