칠정구기증환자 63례에 대한 임상적 고찰 (63 cases report of neurosis patient)

  • This is 63 cases report of neurotic patient which have gone to Dep. of Neuropsychiatry of Dongshin University sunchun oriental hospital from October, 2002 to October, 2003. Objective : To find out distributional characteristics of neurotic patient Result : 1. The distribution of sex is Female and of age is thirty, fourty is large. 2. The distribution of a history of case is chronic patient is large. 3. The distributional ratio of psychosomatic : musculoskeletal : gastrointestine symptom of chief complain was 48.7: 25.3: 14.9. 4. The distribution of yanghyulansin ondam: soganhaewul gudam : bohyulansin description was 41.3: 15.9: 9.5. Conclusion : This 63 cases report is relatively small cases about analysis and comparison of neurotic patient. So, it's needed to abundant study of this, afterwards.

Geographic information system analysis on the distribution of patients visiting the periodontology department at a dental college hospital

  • Purpose: The aim of this study is to analyze and visualize the distribution of patients visiting the periodontology department at a dental college hospital, using a geographic information system (GIS) to utilize these data in patient care and treatment planning, which may help to assess the risk and prevent periodontal diseases. Methods: Basic patient information data were obtained from Dankook University Dental Hospital, including the unit number, gender, date of birth, and address, down to the dong (neighborhood) administrative district unit, of 306,656 patients who visited the hospital between 2007 and 2014. The data of only 26,457 patients who visited the periodontology department were included in this analysis. The patient distribution was visualized using GIS. Statistical analyses including multiple regression, logistic regression, and geographically weighted regression were performed using SAS 9.3 and ArcGIS 10.1. Five factors, namely proximity, accessibility, age, gender, and socioeconomic status, were investigated as the explanatory variables of the patient distribution. Results: The visualized patient data showed a nationwide scale of the patient distribution. The mean distance from each patient's regional center to the hospital was $30.94{\pm}29.62km$ and was inversely proportional to the number of patients from the respective regions. The distance from a regional center to the adjacent toll gate had various effects depending on the local distance from the hospital. The average age of the patients was $52.41{\pm}12.97years$. Further, a majority of regions showed a male dominance. Personal income had inconsistent results between analyses. Conclusions: The distribution of patients is significantly affected by the proximity, accessibility, age, gender and socioeconomic status of patients, and the patients visiting the periodontology department travelled farther distances than those visiting the other departments. The underlying reason for this needs to be analyzed further.

교정과 환자내원상황에 관한 연구 (STUDY ON PATIENT DISTRIBUTION IN ORTHODONTICS)

  • 1,620 patients who visited our department from 1966 to 1977.7.15, were surveyed on the yearly tendency of patient distribution and the state of Angle's clssification. 1. There was increased visiting rate of patint a year. Patient distribution who visited in our department in the year 1976 was five 샤든 than that in the year 1966. 2. 10-15 age group was 41% in total visiting patient and 20 age over group was 20.4%. 3. Permanent dentition group was 66.2% among three dentitions but it was showedthe tendency that age group is lower year by year. 4. Class I malocclusion was 51.1% in total visiting patient, moreclass III maloulsion was prevalent than class II malocclusion.

  • 2065 patients who visited orthodontic department form 1977. 7. 16. to 1981. 9. 30. were surveyed on the yearly tendency of patient distribution and the state of Angle's Classification. The results were as follows: 1. There was increased visiting rate of patient per year except the year 1980. 2. 8-13 age group was 55% in total visiting patient and 20 age over group was 11.0%. 3. Class I malocclusion was 42.3% in total visiting patient, more Class III malocclusion was prevalent than Class II malocclusion.

  • When X-rays were projected into a patient, there occured the phenomena such as penetration, absorption and scattering etc. The penetrating rays were recorded on films as X-ray image used for diagnosis but scattered rays caused the radiation hazard both to the patient, specialist and technicians. The soft tissue includes many organs which are sensitive to the radiation and in may occupy $40{\sim}50%$ of body weight. Therefore X-rays should be carefully projected to the patient and it is strongly recommended to analyse the distribution of X-rays, when ever the patient is exposed to X-rays. In this study, the distribution of X-ray according to the thickness, the radiation field and the tube voltages (kVp) in soft tissue, the following results were obtained: 1. Total transmitted rays which kept the step with X-ray tube voltage (kVp) increased in proportion to the increasing of X-ray tube voltage. 2. The scattered ray rate in the total transmitted ray was not significantly found with X-ray tube voltage. 3. The affecting factors of the scattered ray rate in total transmitted ray were shown through the radiation field and the thickness. 4. The dose of scattered ray by the angle was observed more in direction of primary ray ($0^{\circ}$) and back scattering ($160^{\circ}$) than in direction of $90^{\circ}$. 5. The more the distance from phantom to the patient should be less distribution of scattered ray.

Informational Justice, Cognitive Trust, and Satisfaction: Purchasers' Perspective of Healthcare Distribution Market

  • Purpose: We examined informational justice, cognitive trust, and satisfaction in healthcare distribution market and their associations within the physician-patient (provider-purchaser) relationship. Methodology: 253 valid survey samples collected from patients and used structural equation modelling for analysis. Findings: We postulated that (1) physicians' informational justice has a positive impact on patients' cognitive trust, (2) patients' cognitive trust has a positive impact on satisfaction, and (3) patients' perceived informational justice has a positive impact on satisfaction. Participants were 253 people who had visited a hospital in South Korea in the past year. Results confirmed that the presence of informational justice has a positive impact on patients' cognitive trust and satisfaction in the physician-patient relationship. Additionally, once cognitive trust was built, it positively influenced patients' satisfaction. We discussed the concept and the impacts of informational justice in light of our analyses regarding patients' perceived cognitive trust and their satisfaction in the physician-patient relationship. Implications: These results emphasize the importance of ethics in healthcare, particularly physicians' frankness and honesty when providing information to patients. Further, these findings present implications for physician education, as part of their training must involve building their patients' cognitive trust as a prerequisite for developing patient satisfaction.

시나리오 기반 환자 분배 및 의료진 할당을 위한 재난 대응 최적화 모형 연구 (Scenario-Based Optimization of Patient Distribution and Medical Resource Allocation in Disaster Response)

  • This study proposes an optimization model to plan the patient distribution and medical resource allocation considering the diverse characteristics of disaster. For reflecting the particularity of disaster response, we configured a few scenarios such as availability of emergency surgery of non-major medical staff and the change in number of patients estimated reflecting the uncertainty, urgency and convergence of disaster. And we finally tested the effects of the scenarios' combination on the objective function defined as maximum number of survival patients. Our experimental results are expected to highlight the significance of the proposed model as well as the applicability of scenarios under disaster response.

A Comparative Analysis of Patient Visibility, Spatial Configuration and Nurse Walking Distance in Korean Intensive Care Units(ICUs) - Focused on single corridor, Pod and Composite type units

  • Purpose: The purpose of this paper is to analyze a sample of single corridor, pod type and composite type of ICUs in terms of patient visibility, spatial configurations and nurse walking distance focused on Korean cases. Methods: The measures of static visibility were used to quantify the patient visibility (upper third part of the patient bed) from the nurse station. The measure of space programme and area distribution (patient zone percentage, staff zone percentage, circulation zone percentage, corridor length per bed, distance from nurse station (NS) to patient bed and departmental gross square meter (DGSM) per patient bed) were calculated by using AutoCAD and MS Excel programs. In the second step of analysis the values of space distribution were compared among the three type of ICUs as well as the nurse walking distance, DGSM per bed and gross factor. in the third step of analysis regression analysis was conducted for the possible correlation between visibility and space programme and area distribution factors as well as nurse walking distance factors. Results: (1) It was found that on average composite type unit offer highest value of patient visibility followed by pod type, while single corridor type unit offers the minimum value of patient visibility among the three plan typologies. (2) Average patient visibility and DGSM per bed shows a strong positive correlation ($r^2=0.75$) and p=0.026. (3) Average patient visibility and average distance from NS has a strong negative correlation ($r^2=0.78$), and P=0.02. (4) On average composite type unit offer the minimum walking distance from NS (7.22 meter) followed by pod type unit (8.35 meter) and single corridor (9.76 meter). (5) Maximum distance from NS was noted in single corridor (18.19 meter) followed by pod type unit (15.14 meter) and Composite type unit (11.1 meter). Implications: This study may contribute to the visibility analysis of existing and future ICU design in Korea to achieve maximum patient visibility and reduced nurse walking distance.

Geant4-DICOM Interface-based Monte Carlo Simulation to Assess Dose Distributions inside the Human Body during X-Ray Irradiation

  • This study uses digital imaging and communications in medicine (DICOM) files acquired after CT scan to obtain the absorbed dose distribution inside the body by using the patient's actual anatomical data; uses geometry and tracking (Geant)4 as a way to obtain the accurate absorbed dose distribution inside the body. This method is easier to establish the radioprotection plan through estimating the absorbed dose distribution inside the body compared to the evaluation of absorbed dose using thermo-luminescence dosimeter (TLD) with inferior reliability and accuracy because many variables act on result values with respect to the evaluation of the patient's absorbed dose distribution in diagnostic imaging and the evaluation of absorbed dose using phantom; can contribute to improving reliability accuracy and reproducibility; it makes significance in that it can implement the actual patient's absorbed dose distribution, not just mere estimation using mathematical phantom or humanoid phantom. When comparing the absorbed dose in polymethly methacrylate (PMMA) phantom measured in metal oxide semiconductor field effect transistor (MOSFET) dosimeter for verification of Geant4 and the result of Geant4 simulation, there was $0.46{\pm}4.69%$ ($15{\times}15cm^2$), and $-0.75{\pm}5.19%$ ($20{\times}20cm^2$) difference according to the depth. This study, through the simulation by means of Geant4, suggests a new way to calculate the actual dose of radiation exposure of patients through DICOM interface.

비만환자와 정상인의 상.하지 수분분포 및 근육둘레 비교연구 (Study on Water Distribution and Muscle Circumference of Arm, Leg and Trunk of Between Obese Patient and Normality)

  • The aim of this study was to investigate the importance of behavior habit and body exercise. We divided 164 volunteers into 2 groups. 82 volunteers was obese group($BMI{\geqq}25$), and the others was normal group(BMI<25). And we investigated the difference of water distribution and muscle circumference of arm, leg and trunk of between obese patient and normality. The results are as follows; Normal group was significantly higher than obese group in leg water distribution and muscle circumference rate as compared with arm(P<0.001). Normal group was significantly higher than obese group in leg water distribution and muscle circumference rate as compared with trunk(P<0.001). Each of the relation of water distribution and muscle circumference was significantly correlated with Pearson correlation analysis(r = 0.96, r = 0.6). In conclusion, there is very important that Low limb exercise and behavior habit in obese.
