치매환자복을 위한 직물 디자인 개발 및 제작 -사회적 보호를 위한 심볼 활용을 중심으로- (A Textile Surface Design for Dementia Patient Hospital Clothing Applying Social Care Symbol)

  • The purpose of this study is for development textile design and making fabric actually for dementia patients hospital clothing using symbol which has social care meaning. For achievement the aim, process and research methods were as follows. First the symbol design was developed. Second the symbol was applied as textile design for dementia patients hospital clothing. Symbol design was under processed with a letter 'Alzheimer' and meanings as like 'love', 'happiness' 'care' 'hope' and 12 design samples were developed by CAD and photoshop. Total 15 evaluation members chose 2 design samples. The 2 design sample were practically arrangement 7 textile design pattern with ground different colors. And finally 3 design pattern and 3 colorway were selected and add white ground. Totally 7 textile design was printed on 100% cotton, 20 yarn count, 1/2 twill. Therefore this new textile design for dementia patient's hospital clothing can be used in medical clinically and the symbol can be use for daily care item for the patients more comfort and high quality in their lives in hospital.

Predicting patient experience of Invisalign treatment: An analysis using artificial neural network

  • Objective: Poor experience with Invisalign treatment affects patient compliance and, thus, treatment outcome. Knowing the potential discomfort level in advance can help orthodontists better prepare the patient to overcome the difficult stage. This study aimed to construct artificial neural networks (ANNs) to predict patient experience in the early stages of Invisalign treatment. Methods: In total, 196 patients were enrolled. Data collection included questionnaires on pain, anxiety, and quality of life (QoL). A four-layer fully connected multilayer perception with three backpropagations was constructed to predict patient experience of the treatment. The input data comprised 17 clinical features. The partial derivative method was used to calculate the relative contributions of each input in the ANNs. Results: The predictive success rates for pain, anxiety, and QoL were 87.7%, 93.4%, and 92.4%, respectively. ANNs for predicting pain, anxiety, and QoL yielded areas under the curve of 0.963, 0.992, and 0.982, respectively. The number of teeth with lingual attachments was the most important factor affecting the outcome of negative experience, followed by the number of lingual buttons and upper incisors with attachments. Conclusions: The constructed ANNs in this preliminary study show good accuracy in predicting patient experience (i.e., pain, anxiety, and QoL) of Invisalign treatment. Artificial intelligence system developed for predicting patient comfort has potential for clinical application to enhance patient compliance.

Maxillary space closure using a digital manufactured Mesialslider in a single appointment workflow

  • New digital technologies, many involving three-dimensional printing, bring benefits for clinical applications. This article reports on the clinical procedure and fabrication of a skeletally anchored mesialization appliance (Mesialslider) using computer-aided design/computer-aided manufacturing (CAD/CAM) for space closure of a congenitally missing lateral incisor in a 12-year-old female patient. The insertion of the mini-implants and appliance was performed in a single appointment. Bodily movement of the molars was achieved using the Mesialslider. Anchorage loss, such as deviation of the anterior midline or palatal tilting of the anterior teeth, was completely avoided. CAD/CAM facilitates safe and precise insertion of mini-implants. Further, mini-implants can improve patient comfort by reducing the number of office visits and eliminating the need for orthodontic bands and physical impressions.

Spontaneous bone regeneration in resected non-continuous mandible due to medication-related osteonecrosis of the jaw

  • Few cases of spontaneous bone regeneration after extensive resection of the jaw bone have been reported, but it is more common in young adults or children. In this case, we report spontaneous bone healing in a 73-year-old female patient. On radiological examination, necrotic regions were seen in the right mandible. She was diagnosed with medication-related osteonecrosis of the jaw due to previous bisphosphonate use. After segmental resection, stabilization achieved using a reconstruction plate. The periosteum was preserved during the procedure. Twelve months later, panoramic radiography was taken and bone formation was seen both horizontally and vertically around the plate. If the periosteum is preserved and stabilization is achieved after resection in benign lesions, the bone may regenerate spontaneously regardless of age. Therefore, instead of simultaneous autogenous bone application, such patients may be followed to determine whether spontaneous bone healing will occur. This should improve patient comfort and reduce surgical cost.

The effect of atropine in preventing catheter-related pain and discomfort in patients undergoing transurethral resection due to bladder tumor; prospective randomized, controlled study

  • Background: Catheter-related bladder discomfort (CRBD) has been observed in many patients undergoing a urethral catheterization. CRBD may be so severe that the patients require additional analgesics. Muscarinic receptors are involved in the mechanism of CRBD. The aim of this study is to determine the effects of the antimuscarinic properties of atropine, which is frequently used in current practice on CRBD, by comparing it with sugammadex which has no antimuscarinic effects. Methods: Sixty patients selected for transurethral resection due to bladder tumors were randomized into 2 groups: an atropine group and a sugammadex group, with no antimuscarinic effect. The patients were given rocuronium (0.6 mg/kg) as a neuromuscular-blocker. In addition to the frequency and severity of CRBD postoperatively at 0, 1, 6, 12, and 24 hours, postoperative numeric rating scale (NRS) scores, and postoperative nausea and vomiting were examined. Results: The incidence of CRBD was significantly lower in the atropine group in all postoperative measurements. The score was found to be significantly lower in the atropine group when NRS measurements were performed at all time periods (P < 0.01). There was no difference between the groups in terms of nausea and vomiting (P > 0.05). Conclusions: Atropine is a cheap, easy-to-access, safe-to-use drug for reducing CRBD symptoms, without any observed adverse effects. Since it not only reduces CRBD symptoms but also has a positive effect on postoperative pain, it can be used safely to increase patient comfort in patients receiving general anesthesia and a urinary catheter.

A Study on Pattern Recognition to Compute Guidelines Based on Evidence for Ecological Healing Environment at Agha Khan Hospital in Karachi - Focused on Human Thermal Comfort Model (HTCM), for Karachi, using Climate Consultant Program

  • Purpose: Healthcare is on the whole a personal and critical service that consumer's use, whereas hospitalization is as a rule painful, because nature nurtures and Sun Light Luminosity for healthcare settings is considered healing. The performance and design of climate responsive buildings such as AKU requires a detailed study of attributes of climate both at micro as well as macro level. The therapeutic value of contact with nature through window view, greenery and landscape is calculated there. Method: A two prong strategy is been devised for this article, at micro level three typical morphologies are analysed by creating same environment of neighboring building on sun shading chart, radiation and temperature range. Since the analysis of local climate helps to determine the design strategies for hospital Healing Environment which is suitable for Karachi climate; in order to track the macro climatic behaviour, a considerable analysis of psychometrics chart for AKU Karachi are designed on Climate Consultant (CC) and analysed by Machine Learning. Climate Consultant proposes different design strategies suitable for Karachi. And on the other hand time wise illumination sources for clinical area which are then measured on psychrometric chart- according to singular space: multi patient admission, secondly: acute ambulatory ward, and tertiary: multi windowed space according to the mushrabiyah and sky light pattern. Result: Our findings support the hypothesis that windowed wall is 75-80% more healing wall; an accelerated evidence was found for healing at macro level if the form of the hospital is designed according to the climatologically preferences, whereas at micro level: the light resource becomes the staff attentiveness determinant. In Conclusion evidence was provided that the actual form of luminosity results consequently in satisfaction while light entering from several set of windows and other sources might be valued if design according to the healing environment. The data added on the sun shading chart to calculate rays entraining into space in patient room equal to 124416.21 Watts/ meter $m^2$ is calculated as precise healing rate-and is confirmed by questionnaire from patients belonging from each clinical stage having different illnesses.

베이스 커브에 의한 콘택트렌즈 피팅 효과 (Effects of Base Curve on Fitting with the current Soft Contact Lenses)

  • 콘택트렌즈 피팅후 기초검사와 베이스커브의 무작위 피팅검사, 그리고 베이스커브의 의도적인 변화에 따른 재피팅검사율 이용하여 중함수의 소프트 콘택트렌즈의 피팅과 베이스커브의 효과에 대하여 조사하였다. 시험렌즈 피팅후 렌즈의 위치, 순목후 렌즈의 움직임, 자각적 착용감을 평가하여 적합한 베이스커브를 평가한 결과 본 연구에 사용한 소재의 소프트 콘택트렌즈에서는 베이스커브가 8.4mm의 경우 최상의 피팅 효과를 보였다. 베이스커브를 8.4mm보다 큰 경우 순목후 렌즈 움직임보다 피팅후 렌즈의 중심이탈정도가 심하고 자각적 검사결과 의도적인 재피팅을 시도하였다. 다른 제조회사렌즈는 정교한 디자인과 제조상의 차이 때문에 언제나 같은 방법으로 피팅 결과가 나오는 것은 아니다. 본 실험에 사용한 중함수율 소프트콘택트렌즈는 대다수의 눈에 최적의 피팅을 제공해 주는 것은 8.4mm 베이스커브렌즈가 효과적이었다.

의사소통 장애요인이 환자만족도와 장기적 관계지향성에 미치는 영향 (The Effects of Communication Barriers on The Patient's Satisfaction and Long Term Sustainable Relationship)

  • 본 연구의 목적은 환자가 경험하는 의사소통 장애가 환자 만족도와 장기적 관계지향성에 미치는 영향을 알아보는데 있다. 치과 병 의원을 방문한 환자 214명을 대상으로 설문조사를 실시하였다. 분석은 PASW Statistics 18.0과 IBM SPSS AMOS 21을 이용하였다. 의사소통 장애 요인분석결과 '일방적소통', '신뢰배려부족', '환자영역', '환경영역'의 요인으로 나뉘었다. 분석 결과, 환자가 느끼는 의사소통 장애요인은 환자 만족도와 장기적 관계지향성에 영향을 미치며, 환자 만족도는 장기적 관계지향성에 영향을 미치는 것으로 나타났다. 이 같은 결과를 바탕으로 치과종사자는 환자와 신뢰를 구축할 수 있는 의사소통 능력을 지니는 동시에 환자 만족도를 높여 계속적인 관계를 유지할 수 있도록 해야 한다.

심한 교모증 환자의 완전 구강 회복 (Full Mouth Rehabilitation in Severely Worn Dentition)

  • The patient had bruxism and epilepsy tendency. Inadequate or unstable posterior support was identified due to severe anterior attrition and decreased occlusal vertical dimension. Prematurities of posterior occlusal and wear facets increased the function of anterior teeth, resulting in severe wear. Wear facets displayed sharply defined peripheries, which are matched on articulated diagnostic casts. Also the patient showed C III malocclusion tendency, and lost some facial contour with drooping corners on the mouth. In this case, the alteration of OVD (Occlusal Vertical Dimention) may provide a biologically compatible adjunct to the treatment such as dentofacial esthetics, improved visual proportion in facial heightand mechanical solutions to the force-management of the masticatory system. The patient requires extensive restorative treatment to regain appropriate function, esthetics and comfort. According to the report by Farhad Fays, the average vertical distance from the maxillary to the mandibular mucolabial reflection in the region of the central incisors is approximately 34mm. However, the vertical distance of this patient was found to be 32mm, which was necessary to add gauge 20-sheets to apply vertical dimension. A removable occlusal overlay splint, which restores OVD to the estimated optimalposition, is the general first trial. The patient was observed periodically for 6 weeks, while appropriate adjustments were made vertical dimension to function. When patient felt comfortable with the splint, the teeth were prepared, and provisional restorations are placed for 3 months. The provisional restoration was fabricated by a diagnostic wax-up. When the patient felt comfortable with the provisional restoration, the final restoration mimics OVD, function, and esthetics that have been developed in the treatment restorations. Restoration of the extremely worn dentition presents a substantial challenge to thedentists. Therefore, careful evaluation of the etiology, history, and factors associated with occlusal vertical dimension should be preceded prior to the appropriate treatment planning.

간호중재분류체계 (NIC)를 이용한 내외과계 중환자실 간호중재 분석 (Analysis of the Nursing Interventions done by MICU and SICU nurses using NIC)

  • The purpose of this research was to identify nursing interventions performed by MICU and SICU nurses. For data collection this study used the taxonomy of the Nursing Interventions Classification (NIC : 433 nursing interventions) which was modified by McCloskey and Bulecheck(1996). Each of the 433 interventions were identified as used by MICU and SICU nurses. More than 50% of the ICU nurses performed 280 nursing interventions at least monthly. Rarely used interventions included 26 nursing interventions in the childbearing care class. Overall, both MICU and SICU nurses used interventions in the Physiological : Complex domain most often on a daily basis and the interventions in the Family domain least often. The most frequently reported interventions as being used daily in the MICU were chest physiotherapy, airway suctioning and coughing enhancement and, in the SICU, documentation and airway suctioning. There were significant differences between MICU and SICU nurses in 17 nursing interventions childbearing care, cognitive therapy, communication enhancement, coping assistance, elimination management, lifespan care, health system mediation, immobility management, medication management, neurologic management, patient education, psychological comfort promotion, physical comfort promotion, respiratory management, risk management and information management. The SICU nurses performed there interventions more frequently than the MICU nurses. These findings will help in building of a standardized language for the MICU and SICU and enhance the quality of nursing care.
