A Study on Fall Prevention System in Patient Bed

  • In this paper, we investigate the patient fall prevention system to prevent the patient from falling out of the bed unintentionally on the bed of the bed. Patients stay in bed for many hours of hospitalization. During the hospitalization period, patients have low controllability of the body, as compared with normal persons, and fall due to intentional movements, resulting in a fall of the patient, can be a fatal threat to the patient. Therefore, an efficient fall prevention system is required. In this paper, the distance map to the patient is generated by the distance measuring sensor on the bed of the patient, and the risk is determined by estimating the position of the patient based on the distance map. As a result, when the distance map of the dangerous area is 150 mm or more, it is determined to be dangerous, and good results are obtained.

환자안전법상 ADR제도 적용을 위한 제언 (Suggestion for the Application of the ADR system under the Patient Safety Act)

  • In the past, there has not been a law with the main purpose of preventing or preventing a risk in advance in order to protect the safety of patients in relation to medical services. It is evaluated that the enactment of the Patient Safety Act has a very important meaning in protecting patient safety as the top priority and further improving the quality of medical care. However, looking at the status of patient safety accidents reported to the Patient Safety Reporting System after the Patient Safety Act was enacted and implemented, various types of risk factors for patient safety still exist in the medical field. Meanwhile, Korea Consumer Agency and Korea Medical Dispute Mediation and Arbitration Agency, the existing domestic ADR specialized agencies, have been operating reasonable damage relief procedures such as recommendation of settlement, mediation, and arbitration according to the purpose of their establishment. Therefore, with the aimof broadening the choice of compensation system for patients, we propose the establishment and revision of ADR-related laws to apply the damage relief procedures of both institutions.

환자만족이 의료이용에 미치는 영향 - 환자 - 의사 커뮤니케이션 만족을 중심으로 (The Effect of Patient-Physician Communication Satisfaction on Healthcare Utilization)

  • Purpose: Importance of patient satisfaction related to patient-centeredness has been emphasized, and it is known to have effect on various health outcomes including health resource utilization. However, the effect of patient satisfaction has been discussed mostly in terms of hospital marketing in Korea. This study aims to examine the effect of patient satisfaction in patient-physician communication on healthcare utilization in a nationally representative adult population of South Korea. Method: Patient satisfaction with physician communication is assessed using 4 items in the 2011 Korea Health Panel Survey. Generalized linear regression analysis is conducted using 9,325 adults' healthcare utilization in 2012. Findings: Adjusting for the socio-demographic, economic factors, individual health status, health behaviors and healthcare utilization in 2011, more satisfied individuals, more likely to utilize the outpatient service, especially in clinical setting. Practical Implications: The study findings suggests that in context of South Korea healthcare system such as insufficient medical consultation time and the absence of health delivery system, patient satisfaction as a subjective healthcare quality indicator would have effect on the individual's outpatient visit. This study contributes to stimulate patient satisfaction research and discussion in South Korea to further explore its relationship with potential and various health related outcomes. Further implications of the study are discussed.

물리치료환자 만족도 도구의 타당도 및 신뢰도 분석 (Validity and Reliability of a Measure of Patient Satisfaction with Physical Therapy)

  • Patient satisfaction is an important factor in evaluating the quality of care. Patient satisfaction may be used to evaluate provider services and facilities, and used to predict the patient returns to a facility. The patients decision whether the patient returns to a facility or whether the patient recommends the facility to other people may be affected by a variety of factors of patient satisfaction. This study is to develop and test of a translated and modified Goldstein's instrument that measures patient satisfaction among physical therapists patients and clients. A self-administered questionnaire survey was conducted in Seoul, Chung-Joo and Bu-Cheon cities. Survey data was obtained from 743 patients who visited the physical therapy practice at university hospitals, general hospitals and clinics. The instrument developed by Goldstein was used and translated into Korean. Several items were added to the instrument. Patient's opinions of service in each domain measured using 5-point Likert-type scales that ranged from strongly disagree to strongly agree. Principal components analysis with varimax rotation was used to indicate which questions examined similar components of patient satisfaction. Component analysis indicated eight scales (kindness, scheduling, recommendation, convenience of parking, privacy, and waiting time). Different types of validity were established well. The coefficient of reliability (Cronbach alpha =.97) obtained for the instrument was clearly within a desired range.

일 지역 종합병원 간호사의 환자안전문화 인식이 안전간호활동에 미치는 영향 (Effects of Perceived Patient Safety Culture on Safety Nursing Activities in the General Hospital Nurse's)

  • Purpose: This descriptive correlation study was done to identify how perception of patient safety culture of general hospital nurses affects safety during nursing activities. Data from this study should provide information on management of patient safety as well as improvement in patient safety. Method: Participants in this study were 357 clinical nurses working in a general hospital in M city which had two medical evaluations. A survey was conducted to gather the data. Results: The score for perception of patient safety culture of the general hospital nurses was 3.42, out of a possible 5 points, and the score for safety care activities was 3.90. There was a statistically significant positive relationship between the nurses' perception of patient safety culture and their safety care activities, Perception of patient safety culture, Supervisor/manager, communication and procedures, and frequency of accident reporting were factors that impacted significantly on safety nursing activity. Conclusion: The results of the study indicate that patient safety cultural perception significantly affects the safety of nursing activities and thus systematic educational strategies to increase perception should be provided to increase the level of patient safety culture. Also, other specific methods that increase the level of patient safety culture should be considered.

A Structure Model of Clinical Nurses' Silence on Patient Safety

  • Purpose: The aim of this study was to establish a hypothetical model on silence regarding patient safety and to verify the model's goodness of fit and hypotheses. Methods: The participants in this study were 330 registered nurses working in tertiary hospitals with over 300 beds. Data were collected between July 1, and August 30, 2017, from nurses who agreed to participate. A covariance structure analysis was performed. Results: The model of fit index was $x^2=59.54$, normed $x^2=2.29$, GFI=.97, AGFI=.93, SRMR=.05, NFI=.99, CFI=.95 and RMSEA=.05. The organizational culture had an influence on patient safety motivation (${\beta}=.26$, p=.003) and attitude (${\beta}=.43$, p<.001). RN-MD collaboration had an influence on patient safety motivation (${\beta}=.33$, p<.001), attitude (${\beta}=.35$, p<.001), and patient safety silence (${\beta}=-.17$, p=.026). Supervisory trust had an influence on patient safety motivation (${\beta}=.26$, p<.001), attitude (${\beta}=.12$, p=.036), and patient safety silence (${\beta}=-.23$, p=.002). Patient safety motivation had an influence on patient safety silence (${\beta}=-.33$, p=.006). The model of patient safety silence explained 36.0% of the variances. Conclusion: This study is meaningful in that it provides basic data for nursing education and program development for rejecting patient safety silence.

환자 존중 개념 개발 (Concept Development Using Hybrid Model for the Concept, Patient Respect)

  • Purpose: This research is a descriptive study to explore "patient respect" in nursing. Respect was analyzed as a concept in the domain of the patient. The Hybrid Model suggested by Schwartz-Barcott and Kim was used in this study. Method: For the theoretical phase, nursing and other literature were reviewed to analyze attributes and develop a working definition of the concept, respect. For the fieldwork phase, four subjects in two general hospitals in Seoul participated. With the participants' permission, the data was collected between January and April, 2002, through in-depth interview and participant observation. The data analysis progressed at the same time as the fieldwork. Data analysis proceeded according to the analysis method of Strauss and Corbin. Result: The final attributes of patient respect are suggested by consideration, recognition, cordial treatment, concern, honesty, acceptance. The final definition of patient respect as a concept in the domain the of patient is suggested by "Patient respect is that the patient is recognized as an individual with worth and is accepted, and considered to be that kind of an individual person. In addition, the patient is recognized to be an independent person and is treated with concern and honesty." Conclusion: The results of the analyses is helpful in integrating into a comprehensive description of the concept, Patient Respect.

정신과환자의 입원진료서비스에 대한 보호자의 만족도가 병원재이용에 미치는 영향 (Influences of Satisfaction of Mental Disease Patient's Protector about Medical Service on Utilizing Hospital)

  • This study was conducted to understand the influences or satisfaction of mental disease patient's carer about medical service on utilizing hospital, for 202 mental disease patients who are in 19 hospitals from March 2, to 31, 2000 with questionnaire. Factor for understanding satisfaction of patient's carer includes ability of medical professional, view of facility, kindness of steps, convenience of medical procedure, cleanness of environment and convenience of traffic. The data collected were analyzed by SPSS program and the results of this study are summarized as follows; 1. There are not significantly difference bet wren general character and whole satisfaction of patient's carer. 2. Each factor has the positive relation with whole satisfaction of patient's carer about medical service. 3. Whole satisfaction of patient's carer have the significant positive relation with utilizing hospital($\beta$=.527, p=.000), this means that the higher whole satisfaction of patient's carer, the higher their tendance of utilizing hospital. 4. Knowledge for mental disease of patient's carer appear the positive results in view of medical care. In conclusion, the policy has to be developed in order to prevent the stop of utilizing hospital of patient and their carer. But there is a limit to except mental disease patients in this study.

Objectives We analyze Doctor-Patient Relationship in anthropological medicine, medical sociology Oriental Medicine and find out solutions about problems of Modern Medicine which are dehumanized through the meaning of Doctor-Patient Relationship in Sasang Constitutional Medicine. 2. Methods We consider various studies which are related with doctor-patient relationship from models of the Doctor-Patient Relationship, anthropological medicine, medical sociology and Oriental Medicine. And we compare and analyze Sasang Constitutional Medicine's meaning of doctor and patient through Sasang Constitutional Medicine's view of treatment and philosophy. 3. Results and Conclusions 1) In various studies, Doctor-Patient Relationship have been dualistic perspective which is divided into doctor and patient, and lay emphasis on relationship between them. 2) In Sasang Constitutional Medicine, anyone might be a patient because they are human being, anyone might be a doctor as cure their own disease. In other words, doctor and patient are not separated but united. A point of regarding human is the core of illness and cure can present new direction that solves problems of dualistic perspective Modern Medicine.

중소병원 간호사의 전문직 자율성, 조직몰입 및 환자안전문화인식이 환자안전관리활동에 미치는 영향 (Effects of Professional Autonomy, Organizational Commitment, and Perceived Patient Safety Culture on Patient Safety Management Activities of Nurses in Medium and Small-Sized Hospitals)

  • Purpose: The purpose of this study was to examine the effect of professional autonomy, organizational commitment, and perceived patient safety culture on patient safety management activities of nurses in medium and small-sized hospitals. Methods: A cross-sectional design was employed. Self-reported questionnaires were completed by 121 nurses with at least 3 months of working experience in medium and small-sized hospitals located in B city. Data were analyzed using descriptive statistics, a t-test, a one-way ANOVA, Pearson correlation coefficients, and a multiple regression analysis. Results: Professional autonomy (r=.22, p=.016), organizational commitment (r=.34, p<.001), and perceived patient safety culture (r=.55, p<.001) had a statistically significant positive correlation with patient safety management activities. The factors that might affect patient safety management activities were professional autonomy (${\beta}=.23$, p=.003) and perceived patient safety culture (${\beta}=.55$, p<.001). The explanatory power of these factors for patient safety management activities was 33.5% (F=21.19, p<.001). Conclusions: The development of repetitive and continuous education programs is needed to improve a nurse's professional autonomy and perceived patient safety culture.
