  • Guillain-Barre Syndrome (GBS) is an acute inflammatory demyelinating polyneuropathy. It is disorder in which the body's immune system, attacks parts of the peripheral nervous system. It is characterized by the rapid onset of weakness, paralysis of the legs, arms, breathing muscles and face. But the analysis of CSF and electrical tests on nerve and muscle function can be performed to confirm the diagnosis. Most cases occur shortly after a viral infection. This is a clinical ease report on Cuillain-Barre Syndrome with diplopia. The patient, a 52-year-old man had a weakness in both legs and diplopia. His weakness and diplopia improved after oriental medical treatment, so this is reported as a potential treatment. Objective: This study was designed to evaluate the effects on oriental medicine therapy on Guillain-Barre Syndrome with diplopia. Methods & Result: The Clinical data was analyzed on a patient with Cuillain-Barre Syndrome whose main symptoms were diplopia. The patient was treated by acupuncture and oriental medicine. As a result, symptoms was improved remarkably. Conclusion: The patient showed weakness and diplopia. • We present data from three Caucasian men with Zinner syndrome who attended our center for the treatment of primary couple's infertility. Each patient was scheduled for conventional testicular sperm extraction (cTESE) and cryopreservation. Sperm analysis confirmed absolute azoospermia. Patient 1 had right and left testis volumes of 24 mL and 23 mL, respectively; left seminal vesicle (SV) agenesis, severe right SV hypotrophy with right renal agenesis. Follicle-stimulating hormone (FSH) was 3.2 IU/L. Patient 2 exhibited right and left testis volumes of 18 mL and 16 mL, respectively; a left SV cyst of 32 × 28 mm, ipsilateral kidney absence, and right SV agenesis. FSH was 2.8 IU/L. Patient 3 showed a testicular volume of 10 mL bilaterally, a 65 × 46 mm left SV cyst, right SV enlargement, and left kidney agenesis. FSH was 32.0 IU/L. Sperm retrieval was successful in all patients. Nevertheless, cTESE should be performed on the day of oocyte retrieval.

  • The costliness index (CI) is an index that is used in various ways to improve the quality of medical care and the management of appropriate treatment in medical institutions. However, the current calculation method for CI has a limitation in reflecting the actual medical cost of the patient unit because the outpatient and inpatient costs are evaluated separately. It is desirable to calculate the CI by integrating the medical cost into the episode unit. We developed an episode-based CI method using the episode classification system of the Centers for Medicare and Medicaid Services to the National Inpatient Sample data in Korea, which can integrate the admission and ambulatory care cost to episode unit. Additionally, we compared our new method with the previous method. In some episodes, the correlation between previous and episode-based CI was low, and the proportion of outpatient treatment costs in total cost and readmission rates are high. As a result of regression analysis, it is possible that the level of total medical costs of the patient unit in low volume medical institute and rural area has been underestimated. High proportion of outpatient treatment cost in total medical cost means that some medical institutions may have provided medical services in the ambulatory care that are ancillary to inpatient treatment. In addition, a high readmission rate indicates insufficient treatment service for inpatients, which means that previous CI may not accurately reflect actual patient-based treatment costs. Therefore, an integrated patient-unit classification system which can be used as a more effective CI indicator is needed.

  • Background Massive weight loss (MWL) is a very common presentation that you may face as a plastic surgeon. Each patient has his own individual criteria, so, you should work according to a well-organized plan, especially when such cases have concerns about their gluteal area contour that were neglected before by many surgeons. A decision-making strategy was used to give a personalized treatment for targeting gluteal region reshaping of MWL patients. Methods This study considered all patients with MWL subjected to buttock reshaping. There was no randomization in treatment; there was a case-by-case assessment. We analyzed the features of the buttocks, the type of surgery performed, the outcomes, and the complications. Results Fifty two patients were included (41 females and 11 males), ages ranged between 21 and 66 years. Demographic data, preoperative body mass index (BMI), duration of surgery, type of surgery, and postoperative complications were collected. Statistically significant improvements were observed in gluteal ptosis and patient satisfaction grades. Conclusion Aesthetic improvement of the buttocks involves either augmentation or contouring that may be obtained by liposculpture, surgical lifting, or combination. Patients with MWL have high expectations and are often treated with multiple procedures. Thus, an easy strategic approach personalized on each patient to treat multiple adjacent areas in one operation is necessary. Adipose tissue distribution, gluteal skin status, and BMI were the main factors that can forcefully affect our plan to guarantee reduction of unpleasant results and complications and improve patient satisfaction.

  • This Study will focus the fact that large portion of inpatient treatment cost might incurred in nursing hospital and consider whether policy of allowing inpatient treatment is appropriate or not. Finally This study will suggest alternative way to make improvement based on cases from other countries. This study use data published by Health Insurance Review & Assessment Service. & National Health Insurance Service which is very reliable. This Study found biggest medical spending in allowance of medical care is inpatient treatment cost and large portion of inpatient treatment cost might incurred in nursing hospital. This Study found policy of allowing patient to get inpatient treatment is not clearly determinded. Therefore patient who don't actullay need medical service enter and stay in nursing hospital. Their inpatient treatment cost is paid by allowance of medical care and this cost is unnescessary medical cost. This study suggest policy of allowing patient need to be clear. Government should mandate nursing hospital to check whether patient's condition is appropriate to enter and stay in nursing hospital. This study suggest way to reduce unnecessary inpatient treatment cost incurred in nursing hospital

  • Purpose: The purpose of this study was to examine the difference between direct and indirect nursing activity times according to the grade of nursing management fee. Methods: The subjects of this study were 324 working nurses at the time of 3rd grade and 319 working nurses at the 2nd grade of nursing management fee in one university hospital. Nursing activity time was measured in April and May, and in September and October of 2008 for comparison. The data were analyzed by SPSS win using frequency and t-test. Results: Direct nursing care activity time per one patient was significantly higher at 2nd grade than 3rd grade in day shift (t=-2.55, p=.012). Direct nursing care workload per one nurse was significantly lower at 2nd grade than 3rd grade in evening shift (t=2.21, p=.030), whereas indirect nursing care workload was lower at 2nd level in night shift (t=2.43 p=.016). Conclusion: The findings from this study showed that higher nurse-to-patient ratio can provide greater amount of direct nursing care activity for the patient. Also, it decreased workload of nurses. The higher nurse-to-patient ratio will be beneficial for promoting quality of nursing care as well as decreasing nurse's heavy workload.

  • Tuberculosis causes high morbidity and mortality. However, Korea still has the highest tuberculosis (TB) incidence and mortality among OECD countries despite decreasing incidence and mortality due to the development of modern medicine. Korea has now implemented various policy projects to prevent and control tuberculosis. This study analyzes the effects of public-private mix (PPM) tuberculosis control program on treatment outcomes and identifies the factors that affecting the success of TB treatment. We analyzed 130,000 new tuberculosis patient cohort from 2012 to 2015 using data of tuberculosis patient reports managed by the Disease Control Headquarters. A cumulative incidence function (CIF) compared the cumulative treatment success rates for each factor. We compared the results of the analysis using two popular types of competition risk models (cause-specific Cox's proportional hazards model and subdistribution hazard model) that account for the main event of interest (treatment success) and competing events (death).

  • This study was conducted to explore the relationship between locus of control and health belief. the life style of gastric cancer patients. The subjects of this study were 40 of the early gastric cancer and 90 of the advanced gastric cancer who first visiting patient to general surgery out patient department of Chungnam University Hospital for operation. The data was collected with structured questionnaire from July. 1998 to Feb. 1999 The tool were Moon's Health Belief Scale and Multiple Locus of Control of Wallston. Wallston, DeVellis. The data was analysed by SAS program using frequency, $X^2$-test, Pearson's correlation coefficient, ANOVA. Scheffe-test, t-test. The results were as follows 1. 30.8% of subjects were early gastric cancer anf 69.2% were advanced gastric cancer. The subject knowing about diagnosis was 83.1%. 16.9% did'nt know about diagnosis. 2. The correlationship between Health belief and Locus of control of subjects was not supported. 3. There was statistically no difference of life style between early and advanced gastric cancer patients. 4. There were statistically significant differences in perceived sensitivity according to weight. educational level. and birth order, in perceived barrier according to educational level. and in perceived severity according to occupation among demographic characteristics of study subjects. From above results. health professional has to educate general population to detect gastric cancer early to improve survival rate because early gastric cancer is high in survival rate, and to prevent recurrence and to maintain continuing healthy status. In the future, gastroscopy also has to expand to detect early. For there was no difference in life style between early and advanced gastric cancer. carcinogen related to diet should be emphasized through education. The perceived benefit among health belief model was not supported in this study therefore further study and comparison between gastric cancer and normal population are needed.

  • This paper describes the construction of knowledge data retrieval management system based on medical image CT. The developed system is aimed to improve the efficiency of the hospital by reading the medical images using the intelligent retrieval technology and diagnosing the patient 's disease name. In this study, the medical image DICOM file of PACS is read, the image is processed, and feature values are extracted and stored in the database. We have implemented a system that retrieves similarity by comparing new CT images required for medical treatment with the feature values of other CTs stored in the database. After converting 100 CT dicom provided for academic research into JPEG files, Code Book Library was constructed using SIFT, CS-LBP and K-Mean Clustering algorithms. Through the database optimization, the similarity of the new CT image to the existing data is searched and the result is confirmed, so that it can be utilized for the diagnosis and diagnosis of the patient.
