The Causal Relationship of Hospital Inpatient's Perceived Quality, Satisfaction, Service Value, and Intention to Revisit (병원입원환자가 인지하는 의료서비스 질, 만족도, 서비스가치, 병원 재이용 의사간의 인과관계분석)

  • The objective of this study is to analyze the causal relationship of hospital inpatient's perceived quality, overall satisfaction, service value, and future intention to revisit. To carry out this objective, first we analyzed the dimensions of inpatient care service quality using SERVQUAL scale. The SERVQUAL scale is based on the gap theory, that is, the difference of patients' expectations and the actually received medical care service in hospital. On the basis of this theory, we measured the inpatient's perceived service quality and overall patient satisfaction. Data was gathered from a self-administered questionnaire at a 980 bed university hospital in Inchon City. These questionnaire measuring the service quality were distributed to 250 inpatients. The response rate was 66.4%. A total of 166 questionnaires was finally analyzed. To categorize medical service quality, the factor analysis was performed on 42 items. The reliability and validity of these items was evaluated. Finally to test 6 hypotheses, we analyzed the causal relationship of service quality, overall satisfaction, service value, and intention to revisit through the structural equation modeling(SEM). The major results of this study are as follows. First, the dimension of inpatient service quality was categorized into 7 dimensions, that is, personal caring, communication, access, physical environment, facilities and equipment, cleanliness, appropriateness and health status. Second, the reliability and validity of inpatient service quality items was satisfied. Third, as a result of structural equation modeling, the effect of inpatient's perceived service quality on overall satisfaction, service value, and intention to revisit was statistically significant. And total effect on intention to revisit as the core endogenous variable was perceived service quality(1.100), patient satisfaction(0.006), and service value(0.605).

A Study of Student Nurses' Knowledge and Awareness about Patients' Personal Information Protection (환자 개인정보 보호에 대한 학생간호사의 지식과 인식의 연구)

  • This study examined the nursing student's preliminary knowledge and awareness of patient's personal information protection. 187 respondents, who were trained in 2 different nursing colleges in Kyungpook province, participated in the survey from May 18 to June 12, 2015. Using the data collected, descriptive statistics, T-test and one-way ANOVA were implemented. As a result, the student nurses, who had heard about the Personal Information Protection Act showed a significantly higher awareness score of patient's personal information protection behavior than those who did not. The respondents educated in personal information protection were reported to have a higher score of awareness about the patient's personal information protection behavior. In this regard, it is vital for university organizers to develop education associated with ethics and information security, which could enhance the perceptions of personal information protection for student nurses before their working career begins.

Comparison Study of Textbook and Actual Clinical Incidence Angles in Intra-Oral Radiology Examinations (구내 방사선 검사에서 교과서와 실제 임상의 입사각도 비교연구)

  • Existing Intra-oral radiography examinations have problems with information on the angle of incidence used in textbook and those used in the field, and radiologic examinations result vary depending on the operator's skill. Therefore, this study aimed to suggest an ideal angle of incidence for radiological examination so that the proximal areas between teeth are clearly visible without overlapping. First, patient's dental arch in the exact center, and the angle of incidence of the point where the teeth did not overlap was measured using a protractor tool in program. Second, based on the calculated results, we surveyed patient phantoms for student practice during Intra-oral radiography examination and conducted a survey based on qualitative evaluation using a 5-point Likert scale.When comparing the average value obtained by measuring actual patient data and the angle specified in the existing reference book, there is a statistically significant difference in the average between the two groups at p < 0.05 in the proximal area of all teeth, and even if the gender and age group are different, there is a significant difference. Because it did not deviate from the scope, we were able to obtain results that could be sufficiently presented as guidelines. In addition, in a qualitative evaluation based on the images presented by applying the actual measured data to the phantom, images were obtained in which the adjacent surfaces of teeth do not overlap. In conclusion, this study proposes guidelines for the angle of incidence during Intra-Oral Radiography examination to reduce cone-cut images or images with overlapping proximal area and prevent repeat.

Influencing Factors on Stress in Caregivers of Stroke Patients Being Admitted in Rehabilitation Centers (재활전문병원에 입원중인 뇌졸중 환자 주 돌봄 제공자의 스트레스 영향요인)

  • The aim of this descriptive correlation research was to identify the factors influencing stress in caregivers of stroke patients in rehabilitation centers. The data were collected from 200 caregivers at three rehabilitation centers in B City from September 1 to 30, 2015. The data were also collected utilizing the Connor-Davidson Resilience Scale, the Caregiving Mastery Scale, the Medical Outcomes Study (MOS) Scale, and Burden Interview (BI) Scale. The data were analyzed using the SPSS Win 18.0 program. The factor with the greatest impact on stress was social support, which accounted for 9.3% of stress, followed by the duration of giving care to the patient, religious status, economic conditions, caregiver's age, resilience, caregiver's health status, patient's conscious status, and patient's age, all of which accounted for 30% of the stress for caregivers. Therefore, it is essential to develop social support programs that can reduce the stress for caregivers of patients in rehabilitation centers and to develop stress intervention programs, taking all the factors affecting stress into consideration.

Predictors of Readmission after Inpatient Plastic Surgery

  • Background Understanding risk factors that increase readmission rates may help enhance patient education and set system-wide expectations. We aimed to provide benchmark data on causes and predictors of readmission following inpatient plastic surgery. Methods The 2011 National Surgical Quality Improvement Program dataset was reviewed for patients with both "Plastics" as their recorded surgical specialty and inpatient status. Readmission was tracked through the "Unplanned Readmission" variable. Patient characteristics and outcomes were compared using chi-squared analysis and Student's t-tests for categorical and continuous variables, respectively. Multivariate regression analysis was used for identifying predictors of readmission. Results A total of 3,671 inpatient plastic surgery patients were included. The unplanned readmission rate was 7.11%. Multivariate regression analysis revealed a history of chronic obstructive pulmonary disease (COPD) (odds ratio [OR], 2.01; confidence interval [CI], 1.12- 3.60; P=0.020), previous percutaneous coronary intervention (PCI) (OR, 2.69; CI, 1.21-5.97; P=0.015), hypertension requiring medication (OR, 1.65; CI, 1.22-2.24; P<0.001), bleeding disorders (OR, 1.70; CI, 1.01-2.87; P=0.046), American Society of Anesthesiologists (ASA) class 3 or 4 (OR, 1.57; CI, 1.15-2.15; P=0.004), and obesity (body mass index ${\geq}30$) (OR, 1.43; CI, 1.09-1.88, P=0.011) to be significant predictors of readmission. Conclusions Inpatient plastic surgery has an associated 7.11% unplanned readmission rate. History of COPD, previous PCI, hypertension, ASA class 3 or 4, bleeding disorders, and obesity all proved to be significant risk factors for readmission. These findings will help to benchmark inpatient readmission rates and manage patient and hospital system expectations.

Cost-of-illness Study of Asthma in Korea: Estimated from the Korea National Health Insurance Claims Database (건강보험 청구자료를 이용한 우리나라 천식환자의 질병비용부담 추계)

  • Objectives: We estimated the asthma-related health care utilization and costs in Korea from the insurer's and societal perspective. Methods: We extracted the insurance claims records from the Korea National Health Insurance claims database for determining the health care services provided to patients with asthma in 2003. Patients were defined as having asthma if they had ${\geq}$2 medical claims with diagnosis of asthma and they had been prescribed anti-asthma medicines, Annual claims records were aggeregated for each patient to produce patient-specific information on the total utilization and costs. The total asthma-related cost was the sum of the direct healthcare costs, the transportation costs for visits to health care providers and the patient's or caregivers' costs for the time spent on hospital or outpatient visits. Results: A total of 699,603people were identified as asthma patients, yielding an asthma prevalence of 1.47%. Each asthma patient had 7.56 outpatient visits, 0.01 ED visits and 0.02 admissions per year to treat asthma. The per-capita insurance-covered costs increased with age, from 128,276 Won for children aged 1 to 14 years to 270,729 Won for those aged 75 or older. The total cost in the nation varied from 121,865 million to 174,949 million Won depending on the perspectives. From a societal perspective, direct health care costs accounted for 84.9%, transportation costs for 15.1 % and time costs for 9.2% of the total costs. Conclusions: Hospitalizations and ED visits represented only a small portion of the asthma-related costs. Most of the societal burden was attributed to direct medical expenditures, with outpatient visits and medications emerging as the single largest cost components.

  • Purpose : The purpose of this study was to explore the meaning and essence of experience of family members as care givers of patient with head and neck cancer. Methods : This study was based on the colaizzi's phenomenological method, which describes the experiences about the family care givers. We had in-depth interview with family of five head and neck cancer patients. Results : Data was classified by 24 themes, 9 theme clusters and 4 categories. The categories are as follows: 'Shocking and actuality like a deadly maze', 'Cut off from the world by changes in family's physical image', 'Exhaustion of family', 'Take-off for mature care'. Conclusion : The results of the study provide useful information in understanding care givers' experience of patient with head and neck cancer and establishing effective strategies to support these care givers.

Comparison in nursing needs of heart disease patients depending on whether or not readmitted (재입원 여부에 따른 심질환자의 간호요구 비교)

  • The purpose of this study was investigate nursing needs of heart disease patients depending on whether or not readmitted. Who had diagnosed heart disease and admitted on a general hospital was collected as a self-administered questionnaire method from March to June 2012. Collected data were analyzed through the SPSS 18.0 program. The average of total nursing needs is $4.12{\pm}.49$ point in patient who were not in readmission, and $3.89{\pm}.63$ point in patient joined readmission, there were significant differenced(p=.046). Therapeutic nursing needs are higher than others in nursing needs, in the both groups. Each rank and sub-regions is also different nursing needs according to readmission in two group. Physical, Social-emotional, educational and therapeutic nursing needs was correlated each other in depth. Therefore, focusing on patient's nursing needs according to patient's characteristics are required to perform nursing.

The Effects of Acupuncture Treatment on Hwa-byung patient's Insomnia: Patient-assessor blind, Randomized, Placebo-controlled Clinical trial (화병환자의 불면증상에 대한 침치료 유효성 평가연구: 환자-평가자 눈가림, 무작위배정, 일반대조군 임상시험)

  • Objectives: The purpose of this research is to examine the effects of acupuncture treatment on Hwa-byung patient's insomnia. Methods: The study was performed through a patient-assessor blind, randomized, placebo-controlled trial in which the volunteers, data collectors and analysts were unaware of individuals who were receiving the treatment. A total of thirty-seven volunteers were divided into 2 groups. 18 subjects were placed into a trial group and 19 subjects into a control group by a randomization table. The trial group was treated on bilateral Shigu, Ahnmyun, B62 (Shinmaek), and K6 (Chohae), while the control group was not given any other treatment. The ISI(Insomnia Severity Scale) was measured as the 1st evaluative instrument, and then a comparative analysis was conducted by comparing the results with those measured by a Likert scale for major symptoms of Hwa-byung, PSQI, Hwa-byung scale, BDI, STAI, and STAXI. Results: In the ISI, PSQI, and the Likert scale regarding major symptoms of Hwa-byung, Hwa-byung scale, BDI, and STAI, more significant decreases were found in those concerning the trial group compared to those of the control group from a statistical perspective. However, in the STAXI, regarding the trial group, there was no significant decrease compared to the control group. Conclusions: The results suggest that acupuncture treatment is effective in the treatment regarding Hwa-byung patients who suffer from insomnia.

Effects of Patient Controlled Analgesia Convergence Education on Postoperative Pain Management in Spinal Surgery Patients (통증자가조절기 융합교육이 척추수술환자의 수술 후 통증관리에 미치는 효과)

  • The purpose of this study was to investigate the effects of patient controlled analgesia(PCA) convergence education on postoperative pain management in spinal surgery patients. Sixty spinal surgery patients were included. For the experimental group(n=30), PCA education using video, booklet, and a PCA model practice was provided before surgery. The data were collected using questionnaire and were analyzed using descriptive statistics, ${\chi}^2-test$, Fisher's exact test, t-test and repeated measures ANOVA. PCA knowledge-attitude, PCA satisfaction and pain control satisfaction were higher in the experimental group than the control group(p<.001; p=.001; p<.001). Postoperative pain and frequency of additional analgesia use of the experimental group were significantly lower than those of the control group(p<.001; p=.001). This findings showed that the convergence PCA education of audiovisual aids with PCA practice training could be effective pain management intervention in patients undergoing spinal surgery. These apply to development of convergence nursing interventions in clinical practice.
