Purpose: This study aimed to estimate the appropriate nurse staffing ratio in intensive care units (ICUs) by measuring nursing workload based on patient's severity and needs, using the Korean Patient Classification System for critical care nurses. Methods: The data were collected from January 18 to February 29, 2016 using a standardized checklist by observation or self-report. During the study period, 723 patients were included to be categorized from I to IV using the patient classification system. Measurement of total nursing workload on a shift was calculated in terms of hours based on the time and motion method by using tools for surveying nursing activities. The nursing activities were categorized as direct nursing care, indirect nursing care, and personal time. Total of 127 cases were included in measuring direct nursing time and 18 nurses participated in measuring indirect and personal time. Data were analyzed using descriptive statistics. Results: Two patients were classified into Class I (11.1%), 5 into Class II (27.8%), 9 into Class III (50%), and two into Class IV (11.1%). The amount of direct nursing care required for Class IV (513.7 min) was significantly more than that required for Class I (135.4 min). Direct and indirect nursing care was provided more often during the day shift as compared to the evening or night shifts. These findings provided the rationale for determining the appropriate ratio for nursing staff per shift based on the nursing workload in each shift. Conclusions: An appropriate ratio of nurse staffing should be ensured in ICUs to re-arrange the workload of nurses to help them provide essential direct care for patients.

PURPOSE. The aim of this systematic review is to address treatment outcomes of Mandibular implant overdentures relative to implant survival rate, maintenance and complications, and patient satisfaction. MATERIALS AND METHODS. A systematic literature search was conducted by a PubMed search strategy and hand-searching of relevant journals from included studies. Randomized Clinical Trials (RCT) and comparative clinical trial studies on mandibular implant overdentures until August, 2010 were selected. Eleven studies from 1098 studies were finally selected and data were analyzed relative to number of implants. RESULTS. Six studies presented the data of the implant survival rate which ranged from 95% to 100% for 2 and 4 implant group and from 81.8% to 96.1% for 1 and 2 implant group. One study, which statistically compared implant survival rate showed no significant differences relative to the number of implants. The most common type of prosthetic maintenance and complications were replacement or reattaching of loose clips for 2 and 4 implant group, and denture repair due to the fracture around an implant for 1 and 2 implant groups. Most studies showed no significant differences in the rate of prosthetic maintenance and complication, and patient satisfaction regardless the number of implants. CONCLUSION. The implant survival rate of mandibular overdentures is high regardless of the number of implants. Denture maintenance is likely not inflenced substantially by the number of implants and patient satisfaction is typically high again regardless os the number of implants.

Purpose: This descriptive study compared the perceptions, determinants, and needs of patients, family members, nurses, and physicians regarding life-sustaining treatment decisions for patients with hematologic malignancies in the hematology-oncology department of a tertiary hospital in Seoul, Korea. Methods: In total, 147 subjects were recruited, gave written consent, and provided data by completing a structured questionnaire. Data were analyzed using analysis of variance, the chi-square test, and the Fisher exact test. Results: Nurses (F=3.35) and physicians (F=3.57) showed significantly greater familiarity with the Act on Decisions on Life-Sustaining Treatment than patients (F=2.69) and family members (F=2.59); (F=19.58, P<0.001). Many respondents, including 19 (51.4%) family members, 16 (43.2%) physicians, and 11 (29.7%) nurses, agreed that the patient's opinion had the greatest effect when making life-sustaining treatment decisions. Twelve (33.3%) patients answered that mental, physical, and financial burdens were the most important factors in life-sustaining treatment decisions, and there was a significant difference among the four groups (P<0.001). Twenty-four patients (66.7%), 27 (73.0%) family members, and 21(56.8%) nurses answered that physicians were the most appropriate people to provide information regarding life-sustaining treatment decisions. Unexpectedly, 19 (51.4%) physicians answered that hospice nurse practitioners were the most appropriate people to talk to about life-sustaining treatment (P<0.001). Conclusion: It is of utmost importance that the patient and physician determine when life-sustaining treatment should be withdrawn, with the patient making the ultimate decision. Doctors and nurses have the responsibility to provide detailed information. The goal of end-of-life planning is to ensure patients' dignity and respect their values.

Background and Objectives : This study is purposed to investigate the statistically significant discrimination model for predicting vocal fold nodule and polyp's lesional grade, with patients' background data and objective voice evaluation parameters. Materials and Method : The retrospective research was carried out at the Ewha Womans University Hospital. 122 patients' voice examination data had been selected, and lesion screening (Grade I, II, and III) was conducted by 2 ENT specialists, with each patient's vocal fold pictures achieved during the laryngoscopy examination. Results : The Lesional Grade Discrimination Model with which the lesional grade of vocal fold nodules and polyps could be predicted was derived by the ordinal logistic regression analysis (using SPSS 10.0). With this model the lesional grades of 73 out of 122 patients(59.8%) were correctly predicted to their formerly screened ones. Conclusion : This model applied the multivariate approach, which statistically combined these currently used parameters, Jitter, Shimmer, MFR, MPT, and patient's background data such as gender and dysphonia period. It might explain the status of benign lesion of vocal folds, and furthermore expect the physiological function of vocal folds.

In this study, we have tried to identify the relationship between the patient's health belief and the implementation of functional movement rehabilitation and ability to perform ADL in stroke patient, and have done this study to use this as evidence in treating stroke patients in clinical trials by providing a basis data of stroke patients' successful rehabilitation program. We targeted stroke patients who were diagnosed with stroke and are hospitalized in D hospital in J City, Jeollabuk-do, South Korea. More over, we distributed a total of 150 questionnaires to patients who understand the purpose of the research and agreed to participate. We were able to collect 132questionnaires at the end. However, 11 had insufficient data so we only analyzed and evaluated 121 of the said questionnaires. We did frequency analysis to understand the general characteristics by using SPSS for Windows(ver.12.0) program and we used Pearson's Correlation Coefficient and regression analysis to know the relationship between the patient's health beliefs and the implementation of rehabilitation and ability to perform ADL in stroke patients. The results of this study shows that patients with higher perceived sensitivity, perceived benefit and perceived severity equates to an implementation of the rehabilitation better, however patients with a lower perceived disability equates an implementation of functional movement rehabilitation better than those with a higher perceived disability.

As medical insurance had been implemented for Magnetic Resonance Imaging (MRI) from January 1, 2005, this study investigated whether there had been any change in the amount of the medical care utilization of patients who undertook MRI before and after the insurance coverage, and was to examine factors affecting the amount of medical care utilization of MRI. Data were collected from patients who undertook MRI before and after the insurance coverage for a year at a general hospital in Kyeanggi-do. $X^2$ and t-test were used for the analysis of their general characteristics, the number of MRI, and its medical costs before and after the insurance coverage, and hierarchical multiple regression analysis for the factors affecting the amount of the medical care utilization of MRI. The results of this study were as follows. First, the number of MRI after the insurance coverage was significantly decreased. Second, there was no significant difference in the total medical costs of MRI after the insurance coverage, but a significant difference was found in patient's share of medical costs. Third, six variables were found to be affecting the amount of the medical care utilization of MRI, and the variables showed to lead the number of MRI decrease after the insurance coverage. These six factors explained 21.4% of the total number of MRI. As MRI had been covered by insurance, the use of MRI and patient's share of the costs were deceased, but the total medical costs were not affected. Reasons for that could be found in that MRI insurance, different from the case of CT insurance coverage, was allowed not to cover some items and the kinds of diseases subjected to the insurance coverage were extremely limited, lowering insurance prescription rate. In addition to that, the average medical cost of MRI was not changed after the insurance coverage. Therefore, as future measures for the MRI insurance, coverage, it should be considered to allow insurance coverage to no coverage items and to expand the scope of benefit coverage, or to lower patient's share of the costs. Furthermore, researches should be done to explore how recipients will act and how suppliers will react if the coverage is expanded, including expanding the scope of coverage and reducing patient's share of the costs, as well as to conduct research on its economic analysis according to case mix.

This study was designed to identify, describe and classify orientations of cancer patient's family members to death and to identify factors related to their attitudes on death. Death to the male is understood as a comprehensive system and believed to be highly subjective experience. Therefore attitude on death is affected by personalities. As an attempt to measure the subjective meaning toward death, the unstructured Q-methodology was used. Korean Death Orientation Questonaire prepared by Kim was used. Item-reliability and Sorting-reliability were tested. Forty five cancer patients' family members hospitalized in one university medical center in Seoul were sampled. Sorting the 65 Q-itmes according to the level of personal agreement ; A forced normal distribution into the 11 levels, were carried out by the 45 P-samples. The demographic data and information related to death orientation of the P-sample was collected through face to face in depth interviews. Data was gathered from August 30 till September 22, 1995. The Z-scores of the Q-items were computed and principal component factor analysis was carried out by PC-QUANL Program. Three unique types of the death orientation were identified and labeled. Type I consists of twenty P-samples. Life and death was accepted as people's destiny, They firmly believed the existence of life after life. They kept aloof from death and their concern was facing the and of the life with dignity, They were in favor of organ donation. Type II consists of Nine P-Samples. They considered that death was the end of everything and did not believed the life after life. They were very concerned about the present life. Type III consists of Sixteen P-samples. They regarded the death as a natural phenomena. And they considered that the man is just a traveller and is bound to head for the next life which is believed to be free of agony, pain or darkness. They neither feared death nor its process. Their conserns were on the activities to prepare themselves for the eternal-life after death. Thus, it was concluded that there were three distinctiven type of attitudes on death among cancer patient family members, and their death attitudes were affected by demographic and socio-cultural factors such as sex, education, and religion.

This study was conducted to identify the levels of patient safety culture, critical thinking disposition, and patient safety nursing activities and determine the factors affected by patient safety nursing activities among nurses in comprehensive nursing care service wards. Subjects consisted of 150 nurses from general hospitals in B city, Korea, and data were analyzed using the SPSS Win 21.0 program. The mean scores of awareness of patient safety culture, critical thinking disposition, and patient safety nursing activities were $3.32{\pm}0.32$, $3.50{\pm}0.31$, and $4.28{\pm}0.46$, respectively. The awareness of patient safety culture was significantly different by clinical career in the present unit (F=4.79, p=0.001). The critical thinking disposition was significantly different by age (F=3.89, p=0.010) and position (F=6.40, p=0.002). There were differences in the scores of patient safety nursing activities according to position (F=3.19, p=0.044). Additionally, hospital environment (${\beta}=0.25$, p=0.014), supervisor attitude (${\beta}=0.20$, p=0.046), and position (${\beta}=0.14$, p=0.040) accounted for a 44.4% variance in patient safety nursing activities. To promote patient safety nursing activities in comprehensive nursing care service wards, positive awareness of the hospital's safety environment and support from the manager are required.

With the development of information and communication technologies, the growing volume of data is increasing exponentially, raising interest in big data. As technologies related to big data have developed, big data is being collected, stored, processed, analyzed, and utilized in many fields. Big data analytics in the health care sector, in particular, is receiving much attention because they can also have a huge social and economic impact. It is predicted that it will be able to use Big Data technology to analyze patients' diagnostic data and reduce the amount of money that is spent on simple hospital care. Therefore, in this thesis, patient data is analyzed to present to patients who are unable to go to the hospital or caregivers who do not have medical expertise with close care guidelines. First, the collected patient data is stored in HDFS and the data is processed and classified using R, a big data processing and analysis tool, in the Hadoop environment. Visualize to a web server using R Shiny, which is used to implement various functions of R on the web.
The purpose of this study was to investigate the student nurse's perception and performance about the protecting behavior of the medical information. The subjects of the study were 365 senior nursing students. The data was analyzed by SPSS PC 19.0 program. 1) The mean scores of perception and performance by student nurse's on protecting behavior of the medical information were $4.39{\pm}0.40$ and $3.71{\pm}0.57$. 2) The perception on protecting behavior of the medical information was heavily dependent on the following age. 3) The performance on protecting behavior of the medical information was heavily dependent on the following career interests and work unit. 4) There was a positive correlation between the perception and performance about the protecting behavior of the medical information. The results showed the statistically significant partial correlation between the perception and performance. Therefore, it is suggested to apply the concrete education program to enhance the perception in order to improve the performance.
