Effects of Periodontal Disease on Cardio-Cerebrovascular Disease: A Focus on Personal Income and Social Deprivation (치주질환이 심뇌혈관질환 발생에 미치는 영향: 지역결핍과 개인소득을 중심으로)

  • The study aimed to investigate the effects of differences between personal income and social deprivation on the association between cardio-cerebrovascular disease (hypertension, stroke) and periodontal disease. This study used 12 years of cohort data from the Korea National Health Insurance Service covering the years 2002 to 2013. Among the patients aged more than 40 years who had received treatment for periodontal disease 224,067 and 284,730 who had not received treatment for hypertension and stroke, respectively, were included in the analysis. The Kaplan-Meier analysis revealed differences in the rate of treatment for cardio-cerebrovascular disease (hypertension, stroke) according to regional differences, the rate of treatment increased as the composite deprivation index value increased. The difference in treatment rates for cardio-cerebrovascular disease (hypertension, stroke) according to income was found to be higher in the treatment group with low income. This study empirically proved that the association between systemic disease and periodontal disease varies depending on personal income and the regional socioeconomic deprivation level. This shows that the clinical influence of periodontal illness on systemic disease differs according to the personal socio-demographic characteristic and residential area and that an individual's characteristic (income and the regional) needs to be considered along with the patient's clinical intervention in the disease treatment process.

The Burden of Atopic Dermatitis on Children and Their Families : Quality of Life and Financial Impact in Seoul Area (서울 지역을 대상으로 모집된 유.소아기 아토피피부염 환아의 삶의 질과 경제적 비용 부담에 관한 조사)

  • Objective : Atopic dermatitis (AD) in children may profoundly affect the quality of life (QOL), and also cause financial burden, to the families of those suffering from this ailment. The aim of our study was to examine the quality of life and the financial burden of atopic dermatitis in children and their families to evaluate this relationship with the degree of AD. Methods : 37 infant and child atopic dermatitis patients were included and evaluated using the SCORing of Atopic Dermatitis (SCORAD) Index and Eczema Area and Severity Index (EASI). Patients and carers were asked to fill in the questionnaires about their quality of life and financial costs during the past year. Data about sleep disturbance and pruritus were also obtained. Pearson's correlation was used for statistical analysis. Results : 1. The mean score of Children's Dermatology Life Quality Index (CDLQI) was $10.52{\pm}4.82$, Infants' Dermatologic Quality of Life (IDQOL) was $8.21{\pm}3.95$. 2. The mean score of Family Dermatology Life Quality Index (FDLQI) was $13.30{\pm}5.72$, Dermatitis Family Impact (DFI) was $12.5{\pm}4.98$. 3. By analyzing the questionnaire, the monthly average cost was determined to be 730,800 won for each patient : the direct cost was 283,500 won, and the indirect cost was 447,300 won. 4. By analyzing the correlation between the severity of AD and QOL, subjective SCORAD were significantly and positively correlated with QOL(IDQOL, FDLQI, DFI, CDLQI). 5. By analyzing the correlation between the severity of AD and any economic impact, EASI were significantly and positively correlated with the direct cost. Conclusion : The above results show that the QOL of the patients and carers is significantly related to their disease severity. Atopic dermatitis patients pay an average of 730,800 won a month, and the economic impact on the patients is significantly related to their disease severity. The CDLQI, IDQOL, FDLQI and DFImay potentially be of value to help in the appropriate management of AD and can be used as an added measurement in clinical trials involving AD management.

  • Despite its rare occurrence, early diagnosis and appropriate treatment for neonatal herpes simplex virus infection are mandatory due to its high morbidity and mortality. In Korea, there has been no epidemiologic data on neonatal herpes simplex virus infection, and even case reports are rare. We observed a 16-day-old neonate who presented with fever and seizures. We diagnosed her with meningoencephalitis caused by herpes simplex virus type 2 based on the polymerase chain reaction test, and treated her with intravenous acyclovir and anticonvulsants. The seroprevalence of herpes simplex virus type 2 sharply increases in women in their 30s, and the average age for childbirth has increased to older than 30 years of age in Korea; we therefore expect that the incidence of neonatal herpes simplex virus type 2 infection will rise in Korea, and more attention should be directed to neonatal herpes simplex virus type 2 infection. We report this newborn patient's case along with a literature review.

Comparison of Moxifloxacin Monotherapy versus Cephalosporin-Azithromycin Combination Therapies for the Treatment of Community Acquired Pneumonia (원외획득폐렴 환자 치료에서 Moxifloxacin 단독요법과 Cephalosporin-Azithromycin 병용요법의 비교)

  • Community acquired pneumonia (CAP) remains a prevalent and potentially life threatening illness. American Thoracic Society and Infectious Disease Society America recommend combination therapies with ${\beta}-lactam$ plus a macrolide or a fluoroquinolone monotherapy for the empirical treatment of CAP. The aim of this study was to compare moxifloxacin monotherapy with cephalosporin plus azithromycin combination therapies. From January 2004 to March 2005, 18 patients in the moxifloxacin group(MG) and 21 patients in the cefuroxime or ceftriaxone plus azithromycin group(CAG) with CAP were retrospectively reviewed with regard to clinical, laboratory and microbiological data. Each patient was stratified into mild (risk class I-II), moderate (risk class III) and severe (risk class VI, V) group according to and PSI (Pneumonia Severity Index) score. Each group was compared for microbiological eradication, clinical assessment, the length of hospital stay. As results, Total 39 patients with CAP were reviewed. The appropriateness of admission was 83.3% in MC vs. 76.2% in CAC. The mean length of the hospital day was for 8.31 days vs. 7.39 days, days switching parenteral to oral antibiotics in 5.19 days vs. 5.28 days, clinical improvement in 2.43 days vs. 2.61 days in MG vs. CAC. Radiological improvement required 3.75 days vs 3.63 days in MG vs. CAG and bacteriological eradication rate at discharge was the same in the both groups. Mortality rate was 11.1% (2 of 18) vs 14.3% (3 of 21) in MG vs. CAG (p=0.77). Drug cost of the mean 5 hospital days requiring parenteral antibiotics was the most inexpensive in moxifloxacin group for the 147,045 won, and ceftriaxone 1g-azithromycin group for the 170,285 won, cefuroxime bid-azithromycin group for the 207,800 won, ceftriaxone 2g-azithromycin group far the 220,570 won, cefuroxime tid-azithromycin group for the 251,700 won. There was no significant statistical difference in clinical, bacterial, radiological cure and hospital days, and switch to oral days. In conclusion, that i.v. moxifloxacin monotherapy was as effective as azithromycin plus cefuroxime or ceftriaxone combination therapies fur the treatment of CAP. In drug cost analysis, moxifloxacin is less expensive than CAG.

  • Electro interstitial scan shows potential as a non-invasive screening method. It can discriminate some diseases based on electric current response to induce low intensity direct current to limbs or local area of body. DDFAO was invented in France and it is claimed that multi-channel EIS(Electro Interstitial Scan) is useful for various diseases, especially, diagnoses of endocrine system such as diabetics are very effective. In this study, we verified the repeatability and sensitivity of DDFAO by using a RC phantom model and its clinical usefulness using data obtained from normal and diabetes subject groups. As a result, it showed the repeatability and the output change according to change of phantom characteristic, but it was hard to distinguish normal and patient groups non-invasively with just six surface electrodes of DDFAO. The repeatability and the clinical accuracy was not sufficient for screening or diagnostic purposes, as well. In spite of the results with low repeatability and accuracy conducted in this study, we still need further investigations to improve the EIS-based measurement method; EIS is very convenient and simple and it shows potential as a screening tool of the whole body health conditions rather than localized disease diagnosis.

Stereotactic Vacuum-Assisted Core Biopsy Results for Non-Palpable Breast Lesions

  • Background: The increase in breast cancer awareness and widespread use of mammographic screening has led to an increased detection of (non-palpable) breast cancers that cannot be discovered through physical examination. One of the methods used in the diagnosis of these cancers is vacuum-assisted core biopsy, which prevents a considerable number of patients from undergoing surgical procedures. The aim of this study was to present the results of stereotactic vacuum-assisted core biopsy for suspicious breast lesions. Materials and Methods: Files were retrospectively scanned and data on demographic, radiological and pathological findings were recorded for patients who underwent stereotactic vacuum-assisted core biopsy due to suspicious mammographic findings at the Interventional Radiology Centre of the Florence Nightingale Hospital between January 2010, and April 2013. Statistical analysis was carried out using Pearson's Chi-square, continuity correction, and Fisher's exact tests. Results: The mean age of the patients was 47 years (range: 36-70). Biopsies were performed due to BIRADS 3 lesions in 8 patients, BIRADS 4 lesions in 77 patients, and BIRADS 5 lesions in 3 patients. Mammography elucidated clusters of microcalcifications in 73 patients (83%) and focal lesions (asymmetrical density, distortion) in 15 patients (17%). In terms of complications, 1 patient had a hematoma, and 2 patients had ecchymoses (3/88; 3.3%). The histopathologic results revealed benign lesions in 63 patients (71.6%) and malignant lesions in 25 patients (28.4%). The mean duration of the procedure was 37 minutes (range: 18-55). Although all of the BIRADS 3 lesions were benign, 22 (28.6%) of the BIRADS 4 lesions and all of the BIRADS 5 lesions were malignant. Among the malignant cases, 80% were in situ, and 20% were invasive carcinomas. These patients underwent surgery. Conclusions: In cases where non-palpable breast lesions are considered to be suspicious in mammography scans, the vacuum-assisted core biopsy method provides an accurate histopathologic diagnosis thus preventing a significant number of patients undergoing unnecessary surgical procedures.

Effects of low-dose topiramate on language function in children with migraine

  • Purpose: This study aimed to verify the safety of low-dose topiramate on language development in pediatric patients with migraine. Methods: Thirty newly diagnosed pediatric patients with migraine who needed topiramate were enrolled and assessed twice with standard language tests, including the Test of Language Problem Solving Abilities (TOPs), Receptive and Expressive Vocabulary Test, Urimal Test of Articulation and Phonology, and computerized speech laboratory analysis. Data were collected before treatment, and topiramate as monotherapy was sustained for at least 3 months. The mean follow-up period was $4.3{\pm}2.7months$. The mean topiramate dosage was 0.9 mg/kg/day. Results: The patient's mean age was $144.1{\pm}42.3months$ (male-to-female ratio, 9:21). The values of all the language parameters of the TOPs were not changed significantly after the topiramate treatment as follows: Determine cause, from $15.0{\pm}4.4$ to $15.4{\pm}4.8$ (P>0.05); making inference, from $17.6{\pm}5.6$ to $17.5{\pm}6.6$ (P>0.05); predicting, from $11.5{\pm}4.5$ to $12.3{\pm}4.0$ (P>0.05); and total TOPs score, from $44.1{\pm}13.4$ to $45.3{\pm}13.6$ (P>0.05). The total mean length of utterance in words during the test decreased from $44.1{\pm}13.4$ to $45.3{\pm}13.6$ (P<0.05). The Receptive and Expressive Vocabulary Test results decreased from $97.7{\pm}22.1$ to $96.3{\pm}19.9months$, and from $81.8{\pm}23.4$ to $82.3{\pm}25.4months$, respectively (P>0.05). In the articulation and phonology validation in both groups, speech pitch and energy were not significant, and all the vowel test results showed no other significant values. Conclusion: No significant difference was found in the language-speaking ability between the patients; however, the number of vocabularies used decreased. Therefore, topiramate should be used cautiously for children with migraine.

Development of Workplace Bullying in Nursing-Consequence Inventory (WPBN-CI) (간호사가 경험하는 직장 내 괴롭힘 결과 측정도구 개발)

  • This study developed an instrument to assess the effects of the bullying of nurses in the workplace and evaluated the developed instrument. A literature review and a field study were conducted to identify the concepts related to the bullying of nurses in the workplace. These attributes served as building blocks to produce a 28-item WPBN-CI. The content validity was used to determine the content and face validities, and the number of instrument items was reduced to 14. The final WPBN-CI instrument was evaluated using the SPSS 18.0 program with the data collected from October 21-28, 2013 from 458 nurses from general hospitals in the metropolitan area. The WPBN-CI consisted of 13 items covering three distinct factors (physical and psychological withdrawal, poor quality of patient care, and increasing distrust), with a total variance of 62.9%. The convergent and determinant validities for the WPBN-CI were 100% and 89.7%, respectively. The known-groups validity was proven through the mean difference in the level of depression. The satisfied criterion validity for the WPBN-CI was more than 0.70. The reliability of the WPBN-CI was demonstrated by its Cronbach's ${\alpha}$ of 0.90. The WPBN-CI will identify in advance the results due to the bullying of nurses in the workplace and will help to find solutions to the problem and ensure program effectiveness.

The Difference of Health Related Quality of Life According to Ankylosing Spondylitis Patient's General Characteristics (강직척추염 환자의 일반적 특성에 따른 건강관련 삶의 질 차이)

  • This study was conducted to identify differences between qualities of health related to quality of life according to the general characteristics of ankylosing spondylitis patients. Overall, 275 patients who were older than 19 years, drug-maintained for over 4 weeks from Jan 13th to Feb 12th and diagnosed as ankylosing spondylitis at C university hospital participated in this study. Collected data were analyzed through descriptive statistics, t-tests, ANOVA, Scheffe's-test and multiple regression. The results of the present study revealed that Age (p=.008), Job (p<0.001), Number of comorbidity (p<0.001), Surgery for Ankylosing Spondylitis (p=0.002), Disease duration after onset symptom (p=0.010), Disease duration after diagnosis (p=0.027), Alcohol drinking (p=0.002) and Erythrocyte sedimentation rate (p=0.049) were meaningful factors to the quality of health-related life for ankylosing spondylitis patients, and multiple regression analysis showed that level of education, income, comorbidity and alcohol consumption was a significant variable. In conclusion, the results of this study indicated that, to improve quality of life, arbitration programs for improving general characteristics and considering demographic social characteristics and clinical features need to be provided. Therefore, it is expected that the results of this study would be used as actual grounds to develop integrated arbitration programs.

Trend of Socioeconomic Inequality in Participation in Cervical Cancer Screening among Korean Women (자궁경부암 검진 수검률의 불평등 추이)

  • Objectives : While cervical cancer is one of the leading cancers among women worldwide, there are a number of effective early detection tests available. However, the participation rates in cervical cancer screening among Korean women remain low. After the nationwide efforts in 1988 and thereafter to encourage participation in cervical cancer screening, few studies have investigated the effects of socioeconomic inequality on participation in cervical cancer screening. The purpose of this study was to investigate 1) the level of socioeconomic disparities in receiving cervical cancer screening by age group and 2) if there was an improvement in reducing these disparities between 1995 and 2001. Methods : Using data from the Korean National Health Status, Health Behavior and Belief Survey in 1995, the Korean National Health and Nutrition Examination Surveys from 1998 and 2001 (sample sizes of 2,297, 3,738, and 3,283), age-standardized participation rates were calculated according to education level, equivalized household income, and job status. Odds ratios and the relative inequality index (RII) were also calculated after controlling for age. Results : Women with lower education levels were less likely to attend the screening test, and the disparities by education level were most pronounced among women aged 60 years and older. The RIIs among women 60 years and older were 3.64, 4.46, and 8.64 in 1995, 1998, and 2001, respectively. Higher rates of participation were reported among those in the highest income category, which was more notable among the middle aged women (40s and 50s). An inconsistent trend in the rate of participation in cervical cancer screening by occupational level was found. Conclusions : Indicators of socioeconomic position seem to have varying impacts on the inequalities in the rates of participation in cervical cancer screening according to age group. These results demonstrate the need for more aggressive and age-based interventions and policy programs to eliminate the remaining inequalities.
