Eyelid Tumors: Clinical Data from an Eye Center in Ankara, Turkey

  • Introduction: The aim of the study was to explore the distribution of eyelid tumors in Ankara, the capital city of Turkey, from a histopathological point of view. Materials and Methods: Medical records of 1,502 patients who had eyelid surgery because of tumoral lesions were retrospectively reviewed after obtaining institutional review board approval. A total of 1,541 lesions with histopathologic diagnosis were included. Inflammatory tumoral lesions were excluded. The lesions were categorized into three groups according to the origin: epidermal, adnexal tumors and 'others', including melanocytic, neural and vascular lesions. Results: Of the total of 1,541, 908 lesions were epidermal in origin. Only 22 (1.5%) were malignant, and 6.0% was premalignant lesions such as actinic keratosis and Bowen's disease. Twenty-one of 22 malignant lesions were basal cell carcinoma. There was only one patient with squamous cell carcinoma and no sebaceous cell carcinoma. Among the benign tumors (92.5%), squamous papilloma was the most frequent (21.8% of all lesions). The other frequent lesions were nevus (17.6%), seborrheic keratosis (17.3%), hydrocystomas (10.6%), xanthelasma (7.6%) and epidermal cysts (7.2%). Conclusions: The results of this study are in accordance with published literature. The absence of sebaceous cell carcinomas needs to be stressed.

Nurses' Experience of Middle East Respiratory Syndrome Patients Care (메르스 환자 간호에 대한 간호사의 경험)

  • This study was conducted to investigate the practical experience of nurses in MERS patients care. Nine nurses in one general hospital in S city underwent in-depth interviews from August 2015 to March 2016. The collected data were analyzed according to the procedure of the phenomenological method of Colaizzi. The following five categories were derived: '1. Anxiety and burden due to the risk of transmission of new infectious diseases', '2. Social isolation because of being the nurse in charge of MERS patients', '3. Faced with problems due to an unprepared treatment environment', '4. Overcome the burdensome MERS patient care process', '5. Reflect on the steps for preparing against new infectious diseases'. These results suggest that it is important to draw positive experiences as well as the negative effects of nurses who are involved in treatment of various infectious diseases, including MERS. Based on this, it can be used as a basis for reducing negative psychological and social impacts and improving positive adaptation. Additionally, it is necessary for healthcare workers including nurses to establish the necessary workforces, departments and guidelines for nursing new infectious disease patients.

One Stage Posterior Minimal Laminectomy and Video-Assisted Thoracoscopic Surgery (VATS) for Removal of Thoracic Dumbbell Tumor

  • Objective : This study was conducted to assess the surgical results of one-stage posterior minimal laminectomy and video-assisted thoracoscopic surgery (VATS) for the treatment of thoracic dumbbell tumor and to describe its precise technique. In addition, we investigated the technique's usefulness and limitations. Methods : Seven cases of thoracic dumbbell tumor (two men and five women, mean age, 43 years) were analyzed retrospectively. Pathological findings included schwannoma in four patients, neurofibroma in two patients, and hemangioma in one patient. The location of tumors varied from T2/3 to T12/L1. Dumbbell tumors were resected by one-stage operation using posterior laminectomy followed by VATS without instrumentation. Clinical data were reviewed. Results : The mean follow-up period was 25 months (range, 3-58 months), and the operative time ranged from 255 to 385 min (mean, 331 min), with estimated blood loss ranging from 110 to 930 mL (mean, 348 mL). The tumor was completely resected without instrumentation and postoperative instability in all cases. Postoperative complications included atelectasis and facial anhydrosis in one case each. Conclusion : One-stage posterior minimal laminectomy and VATS may be a safe and less invasive technique for removal of thoracic dumbbell tumor without instability. This method has the advantage of early ambulation and rapid recovery because it reduces blood loss and postoperative pain.

Knowledge and Learning Needs Related to Coronary Artery Disease in Diabetic Patient by Glycemic Control (당뇨병 환자의 혈당조절 정도에 따른 관상동맥질환 관련 지식과 교육요구도 조사)

  • Purpose: The purpose of this study was to investigate the knowledge and learning needs on coronary artery disease in diabetic patients by glycemic control. Methods: The subjects consisted of 188 patients at the hospital who had diabetes mellitus. Data was obtained using a knowledge and learning needs questionnaire from January to April 2006. Results: Treatment method, the levels of fasting blood glucose(FBG), and 2-hour postprandial blood glucose(PP2hr) showed meaningful differences between normo-glycemic group(HbA1c < 7%) and hyper-glycemic group($HbA1c{\geq}7%$). The levels of knowledge on coronary artery disease by glycemic control tend to show higher in normo-glycemic group. Etiology and prevention of coronary artery disease were significantly higher in normo-glycemic group than in hyper-glycemic group. The levels of learning needs on coronary artery disease by glycemic control tend to show higher in normo-glycemic group. The learning needs on items of diet control were higher in normo-glycemic group than in hyper-glycemic group. Conclusion: On the base of these results, we should focus on the coronary artery disease education for hyper-glycemic group. Also individual coronary artery disease educational program should be developed for the patients with different level of knowledge and learning needs.

Constipation in Patients following Orthopedic Surgery: Incidence and Influencing Factors (정형외과 환자의 수술 후 변비 발생과 영향 요인)

  • Purpose: The purpose of this study was to identify the occurrences and the factors contributing to constipation in the post surgical period following major orthopedic surgery. Methods: The sample included 133 patients who had surgery of the hip, knee, or spine. Patients were excluded from the study if there was a history of bowel surgery or constipation from chart review. Data were collected using questionnaires such as Korean version of Modified Barthel index (K-MBI), Hospital Anxiety and Depression Scale, and Constipation Assessment Scale (CAS) before surgery and on the third postoperative day. Results: Reports of constipation occurred in 77 instances (57.9%). The first reported defecation was within $4.0{\pm}2.2days$ following surgery. Mean days of first defecation of constipation group vs. non-constipation group was $5.5{\pm}1.7days$ vs. $2.1{\pm}1.0days$. There were statistically significant differences between patients who reported than those who did not in terms of age, length of NPO period, postoperative ADL, and the reported incidents of preoperative and postoperative depressive symptoms. Postoperative ADL, age, and NPO period were significant influencing factors of constipation and explained 52.4% of the variance. Conclusion: Constipation is a very common symptom for patients undergoing orthopedic surgery. These results indicate a need for improving patient's ADL after surgery to prevent constipation.

Roles of Primary Health Practitioner and Activities to Increase the Job Performance (보건진료원 업무현황 및 업무향상활동)

  • Purpose: The purpose of the study was to analyze the present status of community health practitioner activities and efforts to improve the job performance. Method: This study employed descriptive exploratory design. The sample consisted of 1,892 community health practitioners which was 90 % of population of community health practitioners. The data was analyzed by using SPSS Windows 10.0. Result: The most popular activities of community health practitioners were women's health, chronic degenerative disease management, elderly health, and outpatient care of primary health care. The activities that community health practitioners want to strengthen were outpatient care, disease prevention, rehabilitation, health promotion, and counseling. The efforts to improve the job performance were consult to other health care professionals, discussion with patient and families to choose effective treatment options. Community health practitioners knew that they were exposed to malpractice and hould try to make many efforts to improve their performance. Conclusion: The roles and activities of community health practitioners should be changed to the shift of health care environment and systems.

Comparison of Job Tasks and Task Elements of Korean Nurse Anesthetists by Type of Medical Institution: Hospital, General Hospital and Higher General Hospital (의료기관 규모에 따른 마취전문간호사의 직무관련 특성, 직무 및 직무요소의 수행빈도 차이 비교)

  • Purpose: This study was done to identify job tasks and task elements of Korean nurse anesthetists according to type of medical institution. Methods: A job task scale which consisted of 9 job tasks and 40 task elements was developed. Data were collected from December, 2009 to February, 2010 from 182 nurse anesthetists who were working in medical institutions (response rate: 75.8%). Results: Forty-eight percent of nurse anesthetists were independent from anesthesiologists in anesthetic practice. Preanesthetic nursing assessment was much more frequent in small hospitals than in general hospitals (p<.05), and anesthetic nursing intervention, administering the anesthetics, monitoring the patient's status during anesthesia, and provision of safety and compliance with anesthetic ethics were much more frequent in general hospitals than medical centers (p<.001). There were no differences among the medical institutions for job tasks in post-anesthetic nursing interventions (p=.229), administering anesthetics (p=.354) and monitoring patients' status during anesthesia (p=.099), providing safe anesthetic environment (p=.896), and management of ancillary personnel/equipment (p=.617). Conclusion: Results indicate that nurse anesthetists contribute significantly to anesthetic practice in small hospitals and general hospitals. Therefore, it recommended that nursing leaders make efforts to enact legal nurse anesthetist-related policies for safe and high quality anesthetic nursing care.

Protocols and Results of Resident Neurosurgeon's Transfemoral Catheter Angiography Training Supervised by Neuroendovascular Specialists

  • Objective : Transfemoral catheter angiography (TFCA) is a basic procedure in neurovascular surgery with increasing importance in surgical and non-invasive treatments. Unfortunately, resident neurosurgeons have relatively few opportunities to perform TFCA in most institutions. We report a method developed in our hospital for training resident neurosurgeons to perform TFCA and evaluate the efficacy of this training. Methods : From May 2011 to September 2011, a total of 112 consecutive patients underwent TFCA by one resident neurosurgeon supervised by two neuroendovascular specialists. Patients who underwent elective diagnostic procedures were included in this study. Patients who underwent endovascular treatment were excluded. Demographic data, indications for TFCA, side of approach, number of selected arteries, and complications were analyzed. Results : This study included 64 males and 48 females with a mean age of 51.6 (12-81) years. All procedures were performed in the angiography suite. Common indications for procedures were as follows : stroke-induced symptoms in 61 patients (54.5%), Moyamoya disease and arteriovenous malformation in 13 patients (11.6%), and unruptured intracranial aneurysm in eight patients (7.1%). Right and left femoral puncture was performed in 98.2% and 1.8% of patients, respectively. A total of 465 selective angiographies were performed without complications. Angiographic examination was performed on 4.15 vessels per patient. Conclusion : TFCA can be performed safely by resident neurosurgeons based on anatomical study and a meticulous protocol under the careful supervision of neuroendovascular specialists.

  • The aesthetics of current dermatology clinics are the main space of dermatology, which is gradually becoming an area of specialty. Paths requiring spatial transition due to the expansion of treatment field in dermatology clinics are currently emerging, meaning that the duty to provide medical and aesthetic environment with high spatial connection must be accompanied. Contrary to past clinics where only functional aspects were emphasized, current clinics require a differentiated environment that considers both aspects of function and aesthetics, centered on the patient. The purpose of this research in this perspective is to study the efficient connection between dermatology and aesthetics, while also analyzing identity factors to indicate design factors differentiated from other functional spaces, to apply them as preliminary data for the planning of dermatology clinics. Based on the above, six dermatology clinics located in Seoul were designated for case studies through field studies. Overall, Aesthetics connected to dermatology clinics legally must have separate business registrations and have alternative entrance ways. Currently however, there were many cases where entrance, waiting, reception and receipt were not separated. there were efforts to partially display identities when analyzing design factors that formed the identity of dermatology clinics. However, there were insufficient cases where visual communication factors such as a clinic's spatial identity, logo, signing system, and applied products were integrated into a coherent theme. At this point when dermatology clinics are becoming brands, all fields must merge for integrated identity effects that go beyond the boundaries of contemporary H.I, to clearly display their identity with the clinics' professional image and consistent concept.

Research About Necessity of Visiting Home Physical Therapy (방문물리치료 필요성에 관한 연구)

  • Background : In-home physical therapy program as a result of a study of the need for in-home physical therapy, most patients were needed. This study of patients who received physical therapy were surveyed, and patients need physical therapy for what I have found that factors. The purpose of this study is the in-home physical therapy program for patients to determine that how much needs were investigated. Methods : In this study, 469 people were involved with the patients who received physiotherapy. Surveys were returned to the questionnaire. The collected data was analyzed by Chi-square and one-way ANOVA. Results : The need for in-home physical therapy to 31 to 60 years suggests that the highest. In this paper, the female respondents was higher, the damaged parts Many times patients with musculoskeletal injuries, duration of treatment was 3-1 months there. 469 people need physical therapy visits of respondents that respondents were 423 people. qualification standards of physical therapy visits and asked questions of the state or local government public agencies or public health wad the highest with 40.3%, eligibility criteria for physical therapy visits a physical therapist trained in the regular 43.3%, per visit, treatment 10,000won 43.5% less than the cost of investigation into the health insurance coverage and should apply to questions about whether the response was 50.1%. Conclusion : Further, the study of in-home physical therapy services were the patient's perspective. This information, visit the home physical therapy program will contribute to the legalization.
