Analysis of the Hope and Influencing Factors in Hemodialysis Patients (혈액투석환자의 희망정도와 영향요인)

  • The purpose of this study was to investigate the levels of hope and the influencing factors on the hope in the hemodialysis patients. The subjects of this study consisted 101 patients who had received hemodialysis In two hospitals affiliated with Catholic University from June to August, 2000. The instruments used for the study were the Hope scale by Miller(1998) and Quality of life scale, Self esteem scale and Social support scale. Cronbach's $\alpha$ of measurement tools used in the study were 0.93 for the hope, 0.96 for quality of life, 0.78 for self-esteem and 0.92 for the social support. The data were analyzed by mean, t-test, ANOVA. Scheffe test, Pearson correlation coefficients and Stepwise multiple regression using the SAS program. The results were as follows; 1. Out of a score of 164 the mean score of the hope was $107.3\pm16.7$. The mean quality of life score was 102.5 out of 160. The scores concerning social support turned out to have a mean of 67.8 out of 100. The levels concerning self-esteem and self respect had a mean of 25.1 out of 40. 2. There was a positive relation between the quality of life and social support. Furthermore, the hope level was proportional to factors such as quality of life and social support. 3. Social support accounted for $32\%$ of influential variable on the hope. If transportation way to the hospital and quality of life were added. the total predictors explained $45\%$. The results suggested that the hope levels of hemodialysis patients were influenced by their social support and quality of life. Therefore nurses are encouraged to elevate the levels of patients' hope by using the social support and quality of life in caring the patients receiving hemodyalysis.

A Convergence Study of Nurses' Incident Reporting and Perceived Safety Climate (간호사의 사건보고와 안전분위기 인지에 대한 융합연구)

  • The purpose of this study was to identify the factors affecting perceived safety climate and the level of incident reporting attitude, incident reporting knowledge and safety climate. The data were collected by structured questionnaires from 240 nurses and were analyzed with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and multiple regression. The level of incident reporting attitude and incident reporting knowledge was 3.34 and 3.05. The level of safety climate was 3.25. Incident reporting knowledge and safety climate have a significant positive association with incident reporting attitude(r=.33, p<.001; r=.38, p<.001). Incident reporting knowledge was positively associated with safety climate(r=.32, p<.001). Factors influencing safety climate were incident reporting knowledge, belief in improvement and reporting intention which explained 24.7% of the variance(F=12.22, p<.001). The findings indicate that to improve incident reporting knowledge with positive attitude and safety climate should be considered as patient safety strategy and should endeavour to develop interventions for safety.

Simultaneous Augmentation Rhinoplasty with Bony Reduction in Nasal Bone Fracture (비골골절 시 골절정복과 동시에 시행된 융비술)

  • Purpose: The nasal bones are the most common fracture sites of the facial bones, and a careful reduction may still result in secondary deformities, such as saddle nose, deviated nose, hump nose etc, requiring secondary cosmetic rhinoplasty. Therefore, this study examined the clinical characteristics of nasal bone fractures to propose guidelines for patient selection and surgical procedures to achieve more satisfactory results and to prevent secondary deformities with simultaneous augmentation rhinoplasty and bony reduction. Methods: The study was based on 26 out of 149 nasal bone fracture patients who underwent simultaneous augmentation rhinoplasty with bony reduction between May 2008 and April 2009. Retrospective analysis was performed according to the clinical data, surgical techniques and postoperative results. Results: Of the 26 patients, there were 15 males and 11 females. The incidence according to the Stranc's classification revealed that 62% of patients were injured by a frontal impact and 38% by a lateral impact. Frontal impact plane I (50%) was the most frequent type. At the follow up, 18 (81.2%) out of 22 patients were satisfied with their postoperative outcome, and the remaining 4 patients were fair. No one was dissatisfied. However, 5 cases in 3 patients (23%) had some complications; minimal implant deviation in 2 cases, minor irregularity on the nasal dorsum in 2 cases and palpable implant movement under palpation in 1 case. None of these cases required surgical correction. Conclusion: With the proper guidance, simultaneous augmentation rhinoplasty with bony reduction can prevent secondary deformities and satisfy the cosmetic outcomes.

Pulmonary functions of patients with isolated mandibular fractures: a preliminary report

  • Objectives: The aim of this study was to evaluate pulmonary function in patients with mandibular fractures and to determine the pattern of pulmonary functions in these patients. Materials and Methods: This was a cross-sectional study of pulmonary functions in Nigerian non-smoking patients with isolated mandibular fractures managed at our health institution from December 2015 to June 2017. Forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow rate (PEFR), and ratio of FEV1 to FVC (FEV1/FVC) were measured for all participants using a portable spirometer just before treatment. The pulmonary indices were compared with the predicted reference values for Nigerians to determine the respiratory pattern. Results: Forty participants consisting of six females (15.0%) and thirty-four males (85.0%) with a female to male ratio of 1:5.7 were included in this study. The mean patient age was 34.5±13.1 years (range, 17-63 years). The mean FVC, FEV1, FEV1/FVC, and PEFR were 3.8±1.2 L, 3.0±1.0 L, 74.3%±13.8%, and 5.2±2.2 L/s, respectively. Comparison of data with predicted values revealed that 17 subjects (42.5%) had normal pulmonary function pattern while 23 subjects (57.5%) had features suggestive of obstructive and restrictive pulmonary function patterns. Conclusion: Isolated mandibular fractures presented with abnormal pulmonary function pattern.

The Effects of a Task-Related Circuits Program on Functional Improvements in Stroke Patients (뇌졸중 환자에서 순환식 과제지향 프로그램이 기능 증진에 미치는 효과)

  • The purpose of this study was to propose a task-related circuits program for stroke patients and to test the difference in functional improvements between patients undergoing conventional physical therapy and those participating in a task-related circuits exercise program. The subjects were 10 stroke in-patients of the Korea National Rehabilitation Center in Seoul. We measured the following variables: Motor Assessment Scale (MAS), Berg Balance Scale (BBS), Tone Assessment Scale (TAS), speed of gait, rate of step, physiological costs index, age, weight, height, site of lesion, onset day and whether the subject participated in an exercise program. Collected data were statistically analyzed by SPSS 10.0/PC using descriptive statistics, Mann-Whitney U test, Wilcoxon rank sum test and Spearman's correlation. The results of the experiment were as follows: (1) In the pre-test and post-test for function, there was not a statistical significance between the group partaking in a task-related circuits program and the group of conventional physical therapy (p>.05). (2) In the MAS, BBS and speed of gait test, the group undergoing conventional physical therapy showed a statistical significance (p<.05). (3) In the MAS, BBS, speed of gait, PCI, TAS (passive, associated reaction, TAS total score), the group of task-related circuits program showed a statistical significance (p<.05). As a result, the group participating in a task-related circuits program had a more functional improvement than the group participating in conventional physical therapy. Therefore, an intervention recommended for a stroke patient would be a task-related circuits program consisting of a longer session of each task for a more improved functional recovery.

Statistical Considerations of Oral Health Conditions and Dental Management of Autism Spectrum Disease and Cerebral Palsy Patients (자폐 스펙트럼 질환 환자, 뇌성마비 환자의 구강 상태 및 치과 치료에 관한 통계적 고찰)

  • Background: The aim of this study was to investigate and compare the oral health conditions of Autism Spectrum Disease (ASD) and cerebral palsy(CP) patients visiting the Kyung Hee University Dental Hospital from January 2003 to december 2010. Methods: This study analyzed the data (patient's chart and radiograph) of fifty-eight ASD patients and fifty-seven CP patients. The oral health conditions of patients were surveyed by DMFT (dmft) and periodontal condition at the first visit. The performed dental procedure, behavior management methods and follow-up check were analyzed. Results: The average age of patients having ASD and CP was 12 year 2 month (ASD; 11 year 9 month and CP; 12 year 4 month). Within the limit of this study, the rate of dental caries was higher in CP patients than autism patients. Both of diseases showed the periodontal problem. Autistic patients showed more difficult management problems according to the rate of general anesthesia experiences. Conclusions: This article would contribute for dental practitioners to provide appropriate and specialized treatment to ASD patients and CP patients.

Effects of pain catastrophizing and anxiety on analgesic use after surgical removal of impacted mandibular third molars

  • Background: In dentistry, pain is a factor that negatively affects treatments and drug use. The aim of this study was to evaluate the correlations of the postoperative analgesic use with pain catastrophizing and anxiety in patients who underwent removal of an impacted mandibular third molar. Methods: We recruited 92 patients who underwent the extraction of impacted mandibular third molar. In this study, the Pederson index was used to preoperatively determine the difficulty of surgical extraction. Patients were asked to note the number of analgesics used for 7 postoperative days. Patients were divided into two groups based on the Pain Catastrophizing Scale: low and high score groups. State-Trait Anxiety Inventory-trait and State-Trait Anxiety Inventory-state questionnaires were used to determine the anxiety levels of the patients. The obtained data were examined to evaluate the correlations of pain catastrophizing and anxiety with the postoperative analgesic use. Results: In this study, 92 patients, including 60 women and 32 men, were recruited. The analgesic use was higher in women than in men but with no significant difference (P > 0.05). Pain Catastrophizing Scale scores were higher in women than in men but with no significant difference (P > 0.05). The analgesic use was higher in patients with high pain catastrophizing than in those with low pain catastrophizing but with no significant difference (P > 0.05). State-Trait Anxiety Inventory-trait scores were higher in women than in men but with no significant difference. However, state-Trait Anxiety Inventory-state scores were significantly higher in women than in men (P < 0.05). Conclusion: The postoperative analgesic use may be higher in patients who catastrophize pain than in others. Knowing the patient's catastrophic characteristics preoperatively would contribute to successful pain management and appropriate drug selection.

Healthcare Utilization and Expenditure Depending on the Types of Private Health Insurance in Korea (민간의료보험 가입 및 가입유형별 의료이용 특성 분석)

  • Since the introduction of National Health Insurance(NHI) in 1977, it has grown rapidly and contributed to extend patient's access to the health care services. However, limited coverage for health care services of NHI has been ongoing challenge and private health insurance(PHI) has been rising as an alternative source of enhancing coverage and saving out-of-pocket(OOP) expenditure for patients. In this study, after controlling for socio-demographic, economic, health related variables, we identified the patients' healthcare utilization and subsequent OOP expenditure depending on their PHI enrollment and their enrollment types(fixed benefit, indemnity, fixed benefit plus indemnity). Data were collected from the 2010 Korean Health Panel. The unit of analysis was a member of household(n=13,324). Of the 13,324 cases, 70.7% of patients held PHI, in detail, fixed benefit(47.0%), indemnity(3.6%), fixed benefit plus indemnity(20.1%). Major findings showd that patients who enrolled in PHI used more outpatient services(outpatient visit, number of physician visit, number of examination) and spent more OOP expenditure than non-PHI patients. There were also differences of healthcare utilization and OOP expenditure among the types of PHI. In addition, PHI patients used more inpatient services(inpatient use, number of hospitalization, LOS), but there was no significant difference between PHI and non-PHI patients with regard to the OOP expenditure. Thus, we could not find any distinct relationship between the types of PHI and patients' tertiary hospital use. Policy-makers should need careful political deliberation for monitoring the effect of PHI on health care utilization and subsequent expenditure not only to improve patients' coverage but also to save their OOP expenditures.

Clinical Research of the Efficacy and the Safety of Dioscoreae Rhizoma (Sanyak) Pharmacopuncture Therapy for Peripheral Facial Paralysis Patients

  • Objectives: The aim of this study is to evaluate the efficacy and the safety of Sanyak pharmacopuncture therapy for the treatment of peripheral facial paralysis patients. Methods: This study was a retrospective investigation of a total of 70 patients who were inpatients of the Oriental Hospital of Daejeon University between January 1, 2011, and May 31, 2012, and who were diagnosed as having peripheral facial paralysis by physical examination, the patients received three different interventions. Eleven (11) patients were treated with acupuncture and alcohol Dioscorea rhizoma pharmacopuncture (ADG), 25 patients with acupuncture and distillation Dioscorea rhizoma pharmacopuncture (DDG), and 34 patients with acupuncture and non-Dioscorea rhizoma pharmacopuncture (NDG). The resulting data were analyzed. Results: The changed H-B grades indicated significant improvements in all three groups, and the ADG and the DDG groups showed significant results after two weeks of treatment when compared to the NDG group. The changed y-Scores indicated significant improvements in all three groups, and the ADG group showed significant results after 10 and 15 days of treatment when compared to the NDG group. Dioscorea rhizoma pharmacopuncture may be safe for the human body because in most cases, the only abnormal finding was the pain could by the application of pharmacopuncture. Conclusions: The results of this study suggest that Oriental medical treatment with dioscoreae Rhizoma (Sanyak) pharmacopuncture complex therapy may be effective and safe in patient with peripheral facial paralysis.

Clinical Pharmacology of Mycophenolic Acid as Immunosuppressant in Organ Transplaantation

  • Present article reviews about clinical pharmacology of mycophenolic acid (MPA), the active form of mycophenolate mofetil (MMF), as widely used component of immunosuppressive regimens in the organ transplantation field. MMF, used alone or concomitantly with cyclosporine or tacrolimus, has approved in reducing the incidence of acute rejection and has gained widespread use in solid organ such as kidney, heart and liver transplantation. The application of MPA and development of MMF has shown a considerable impact on immunosuppressive therapy for organ transplantation as a new immunosuppressive agent with different mechanism of action from other drugs after early 1990s. In particular aspect, use of MMF, a morpholinoethyl ester of MPA, represented a significant advance in the prevention of organ allograft rejection as well as allograft and patient survival. In considering MMF clinical data, it is important to note that there is a strong correlation between high MPA area under curve(AUC) values and a low probability of acute allograft rejection. Individual trials have shown that MMF is generally well tolerated and revealed that MMF decreased the relative risk of developing chronic allograft rejection compared with azathioprine. Recent clinical investigations suggested that improved effectiveness and tolerability will results from the incorporation of MPA therapeutic drug monitoring into routine clinical practice, providing effective MMF dose individualization in renal and heart transplant patients. Therefore, MMF has a selective immunosuppressive effect with minimal toxicity and has shown to be more effective that other agents as next step of immunosuppressive agents and regimens that deliver effective graft protection and immunosuppression along with a more favorable side effect.
