The Assessment of Foodservice Satisfaction by Orthopedic Patients according to their Involvement (정형외과 환자의 관여도에 따른 병원급식 만족도 분석)

  • The purposes of this study were to evaluate the quality of hospital food services in view of patients in orthopedic wards and accomplish the quality improvement in hospital foodservice operations. Quantitative questionnaires for patients containing foodservice satisfaction and demographic information were developed. A survey of 8 general hospitals was undertaken and detailed information was collected from 290 patients in orthopedic wards. The collected data were processed using the SAS PC 6.12 for descriptive analysis, t-test. In demographic information of patients, 32.6% was over 50 years old and 31% was hospitalized over 30 days. 80% of patients was taking normal diet. 47.7% and 47.9% of patients showed moderate appetite and moderate pain respectively. The overall satisfaction score for patients was 3.24 out of 5, showing slightly higher level than the average score(3.00). According to foodservice involvement scores of patients, they were divided into two groups which were high involved group and low involved group. Two groups showed significant differences in taste of meals, variety of menu, punctuality of meal times, temperature of meals and portion size. The foodservice involvement factor which affected significantly patient foodservice satisfaction was 'kindness of foodservice staff'.

Analysis on Military Hospital Nursing Records by NANDA, NIC, NOC System (간호과정 용어체계를 이용한 간호기록 분석 - 군병원 정형외과 재원환자 기록 대상으로 -)

  • Purpose: This study was to construct a useful nursing language system on military nursing field. Method: Military hospital nursing records were analyzed using NANDA(North American Nursing Diagnosis Association), NIC(Nursing Interventions Classification), and NOC(Nursing Outcomes Classification) systems. All kinds of nursing statements from 80 sets of orthopedics inpatient's records were deduced. All nursing statements were mapped to 167 NANDA diagnoses, 433 NIC interventions, and 260 NOC outcomes. Result: 14,744 nursing statements were extracted. Among the extracted nursing statements, 11.75% were linked with NANDA diagnosis, 83.62% were connected with NIC intervention, and 0.96% was tied to NOC outcome. 3.66% of nursing statements were not linked with NANDA-NIC-NOC system. In the nursing statements, 18 diagnoses of NANDA, 63 interventions of NIC, 8 outcomes of NOC were used. Conclusions: The majority of those nursing statements focused on nursing intervention of the nursing process; few nursing plans or goals were found in nursing records. Therefore, it's difficult to make the nursing process network with the nursing statements. Documenting nursing records using a nursing process will contribute to strengthen nursing practice in patient care and to develop nursing as science. Continuous further researches related to nursing records are needed to provide basic data for developing nursing language system and nursing record system.

Exploring the Feasibility of Differentiating IEEE 802.15.4 Networks to Support Health-Care Systems

  • IEEE 802.15.4 networks are a feasible platform candidate for connecting all health-care-related equipment dispersed across a hospital room to collect critical time-sensitive data about patient health state, such as the heart rate and blood pressure. To meet the quality of service requirements of health-care systems, this paper proposes a multi-priority queue system that differentiates between various types of frames. The effect of the proposed system on the average delay and throughput is explored herein. By employing different contention window parameters, as in IEEE 802.11e, this multi-queue system prioritizes frames on the basis of priority classes. Performance under both saturated and unsaturated traffic conditions was evaluated using a novel analytical model that comprehensively integrates two legacy models for 802.15.4 and 802.11e. To improve the accuracy, our model also accommodates the transmission retries and deferment algorithms that significantly affect the performance of IEEE 802.15.4. The multi-queue scheme is predicted to separate the average delay and throughput of two different classes by up to 48.4% and 46%, respectively, without wasting bandwidth. These outcomes imply that the multi-queue system should be employed in health-care systems for prompt allocation of synchronous channels and faster delivery of urgent information. The simulation results validate these model's predictions with a maximum deviation of 7.6%.

Effects of hippotherapy on children with cerebral palsy: systematic review and meta-analysis

  • Objective: The most important goal of hippotherapy is to improve function, gait, and balance. The purpose of this study was to investigate the effects of hippotherapy on function, gait, and balance of children with cerebral palsy (CP). Design: A systematic review and meta-analysis. Methods: We've searched the PubMed, EMBASE, and Cochrane Library CENTRAL databases for English experimental studies published up until September 2018. To analyze the effects of hippotherapy on the function, gait, and balance of children with cerebral palsy, the systematic review and meta-analysis were performed on Patient/Participants/Population Problem, Intervention, Comparison, Outcome with Timing, Setting Study Design. Papers retrieved from the database were removed from the literature management database. Methodological quality evaluation was evaluated using Cochrane's risk of bias. Data was analyzed using the Revman 5.3 program of the Cochrane library. Results: In this study, we found that the effect size of hippotherapy, denoted as d, was -3.82, and that hippotherapy had the most effect on gait, but no statistical significant difference was observed. We also found no significant difference in function and balance after hippotherapy. It was found from the funnel plot that there was no publication bias as the plot was symmetrically distributed around the dotted line. Conclusions: There are many unclear studies and a lack of randomized controlled trials (RCTs) in this research area. More RCTs on the effects of hippotherapy on children with CP should be conducted in the future.

Color Image Segmentations of a Vitiligo Skin Image with Android Platform Smartphone (안드로이드 기반의 스마트폰을 활용한 백반증 피부 영상 분할)

  • In this study, the new color image processing algorithms with an android-based mobile device are developed to detect the abnormal color densities in a skin image and interpret them as the vitiligo lesions. Our proposed method is firstly based on transforming RGB data into HSI domain and segmenting the imag into the vitiligo-skin candidates by applying Otsu's threshold algorithm. The structure elements for morphological image processing are suggested to delete the spurious regions in vitiligo regions and the image blob labeling algorithm is applied to compare RGB color densities of the abnormal skin region with them of a region of interest. Our suggested color image processing algorithms are implemented with an android-platform smartphone and thus a mobile device can be utilized to diagnose or monitor the patient's skin conditions under the environments of pervasive healthcare services.

Systematic Review of Recent Lipidomics Approaches Toward Inflammatory Bowel Disease

  • Researchers have endeavored to identify the etiology of inflammatory bowel diseases, including Crohn's disease and ulcerative colitis. Though the pathogenesis of inflammatory bowel diseases remains unknown, dysregulation of the immune system in the host gastrointestinal tract is believed to be the major causative factor. Omics is a powerful methodological tool that can reveal biochemical information stored in clinical samples. Lipidomics is a subset of omics that explores the lipid classes associated with inflammation. One objective of the present systematic review was to facilitate the identification of biochemical targets for use in future lipidomic studies on inflammatory bowel diseases. The use of high-resolution mass spectrometry to observe alterations in global lipidomics might help elucidate the immunoregulatory mechanisms involved in inflammatory bowel diseases and discover novel biomarkers for them. Assessment of the characteristics of previous clinical trials on inflammatory bowel diseases could help researchers design and establish patient selection and analytical method criteria for future studies on these conditions. In this study, we curated literature exclusively from four databases and extracted lipidomics-related data from literature, considering criteria. This paper suggests that the lipidomics approach toward research in inflammatory bowel diseases can clarify their pathogenesis and identify clinically valuable biomarkers to predict and monitor their progression.

Comparative Review of Design Guidelines of Hospice Facilities for Establishing Standards (호스피스 시설기준 수립을 위한 디자인 가이드라인 비교연구)

  • Purpose: This study aims to analyze design guidelines for hospice facilities in the US, UK, and Canada focused on design considerations and space requirements, and utilizes them as baseline data for establishing standards for Korean hospice facilities. Methods: Comparative review was carried out to investigate hospice care models, design consideration, and room sizes and requirements for design guideline of hospice facilities in United States, UK and Canada identified on electronic database and review articles, and to examine major characteristics and tendencies of hospice facilities. Results: The hospice care models characteristics in design guidelines is generally largely divided into hospital-based hospice facility, Nursing home-based hospice facility, and daycare hospice. The design considerations in hospice facilities focused on medical efficiency, flexibility, barrier-free environment, person-centered care, and stability. There is also a need for single resident room, rooms for the patient's family, and isolation room for infection control. Implications: it is recommended to establish standards for the installation and operation of required and recommended rooms and considerations when establishing the standards of hospice facilities in Korea. This Study is limited to a simple comparative analysis of the framework of guideline.

Evaluation of the Humpty Dumpty Falls Scale: An Analysis of Electronic Medical Records (소아 낙상위험 측정도구 (Humpty Dumpty Falls Scale) 평가: 전자의무기록을 이용하여)

  • Purpose: The aim of this study was to evaluate the efficiency of the Humpty Dumpty Falls Scale as one of the falls risk assessment tools, and also to evaluate risk factors as predictors of falls in pediatric patient populations. Methods: In a retrospective, case-control design with data from the electronic medical records of 13 pediatric patients who fell and 1,941 who did not fall before matching and 429 who did not fall after matching by gender, age, diagnosis, and length of stay. Results: All the variables showed no significant differences after matching. At the cutoff score of 13, sensitivity, specificity, negative and positive predictive values were 92.3%, 37.1%, 99.9%, and 0.01%, respectively. The area under the Receiver Operating Characteristics was 0.597. The results from the logistic regression showed that the pediatric inpatient population who had higher risk scores was significantly associated with falls. The odds ratios ranged from 1.31 to 4.71 with 90% confidence interval. Conclusion: The saturation impairments criterion as one of the diagnostic parameter was negatively associated with falls, but the relative risk score was higher than the other criteria. Therefore, it seems that the diagnostic parameter seems to be required to verify results through large sample studies.

Effects of a Simulation Practicum using Standardized Patients on Communication Skills, Critical Thinking Disposition and Clinical Competency in Nursing Students: Diabetic Care (표준화 환자를 활용한 당뇨병 간호 시뮬레이션 실습교육이 간호대학생의 의사소통능력, 비판적 사고성향 및 임상수행능력에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of a simulation practicum using standardized patients on nursing student's communication skills, critical thinking disposition and clinical competency. Methods: A one group pretest-posttest design was used. The participants consisted of 45 fourth year nursing students at G university. This study developed 2 scenarios for patients with diabetes mellitus and the simulation was conducted for an average of approximately 14 hours. Data were analyzed with descriptive analysis, t-tests and Cronbach's ${\alpha}$ using SPSS 21.0 program. Results: Communication skills, clinical competency and self-confidence for diabetic care were statistically improved; however, critical thinking disposition was not improved compared to that of pretest. Conclusion: Simulation practicum using standardized patients may be effective in enhancing communication skills, clinical competency and self-confidence for diabetic care among nursing students. In addition, standardized patients simulation program to promote the critical thinking needs to be developed and further research related to standardized patients simulation is in need.

Current status of the anterior middle superior alveolar anesthetic injection for periodontal procedures in the maxilla

  • Periodontal procedures require adequate anesthesia not only to ensure the patient's comfort but also to enhance the operator's performance and minimize chair time. In the maxilla, anesthesia is often achieved using highly traumatic nerve blocks, apart from multiple local infiltrations through the buccal vestibule. In recent years, anterior middle superior alveolar (AMSA) field block has been claimed to be a less traumatic alternative to several of these conventional injections, and it has many other advantages. This critical review of the existing literature aimed to discuss the rationale, mechanism, effectiveness, extent, and duration of AMSA injections for periodontal surgical and non-surgical procedures in the maxilla. It also focused on future prospects, particularly in relation to computer-controlled local anesthetic delivery systems, which aim to achieve the goal of pain-free anesthesia. A literature search of different databases was performed to retrieve relevant articles related to AMSA injections. After analyzing the existing data, it can be concluded that this anesthetic technique may be used as a predictable method of effective palatal anesthesia with adequate duration for different periodontal procedures. It has additional advantages of being less traumatic, requiring lesser amounts of local anesthetics and vasoconstrictors, as well as achieving good hemostasis. However, its effect on the buccal periodontium appears highly unpredictable.
