A study for innovation of Adult nursing assessment tool (간호정보조사지 개선을 위한 연구)

  • This study tried to suggest the basic materials that can be efficiently applied in clinical cases by understanding problems through a content analysis of an adult nursing assessment tool and opinion agreement about nurse's practical usage presently used in the hospital. The study was carried out in 36 attached hospitals in nationwide universities from May to December, 1999, the two hundred and twenty five reports were for analysis. The contents of the collected nursing assessment tool were analyzed. It was found that the tool had been used with various names and content and there were instances of partial omission of a number of items, such as documentation and time records. Other results revealed that they mostly had a systematic classification of items, formation of details a form of a check list, and the effect of saving time. In spite of the adult nursing assessment tool, it was suggested that its style be subdivided according to the specialized of a department and that standardized style be amended and supplemented. The respondents also answered that there had to be education about continuous and sufficient health assessment skills on the physical examination record. The most frequently suggested items to be added were: past history, marital status, patient and caregiver's address and telephone number. It was found that a patient's education career, economic status, religion, hygienic practice, sexual life and hobby were the most frequently omitted items on record. The reason given wes because the items were associated with his/her privacy. These results highlight the importance of analyzing the content with an complete data collection, supplying basic content for a more accurate nursing record, computerization, sharing information and standardization of the form.

Changes in Facial palsy Patient's Quality of life based upon Oriental-Western Medicine Treatment (한양방 협진치료가 안면마비환자의 삶의 질 변화에 미친 영향)

  • Objective : In period of convalescence and aftereffect, facial palsy patients suffer from social and psychological problems, besides experiencing physical inconvenience. So Quality of life is important Evaluation in treatment or facial palsy. Nevertheless the aims of recent study were only trying to explain about objective symptoms. Therefore, Oriental-Western Medicine was performed, effectiveness of treatment were measured in Quality of life. Methods : Acute facial palsy patients who visiting whin 5days completed questionnaire about Quality of life, if he(or she) participated voluntarily. Questionnaire are comprised of general characteristics, Facial Disability Index(FDI), WHOQOL-BREF, VAS and House-Brackmann grade. Questionnaire used two times, the first medical examination and 4weeks later after starting Oriental-Western Medicine. The statistical analysis was performed by GraphPad Prism 4.0. T-test was used to verify effectiveness between the two groups. Results : 1. When we compared the first medical examination with 4weeks later, score of FDI-Physical function and FDI-Social/Well-bieng function increased but they were not valid statistically. 2. When we compared the first medical examination with 4weeks later, in WHOQOL-Brefoverall domain and physical domain, score increased. In WHOQOL-Bref-psychological, Social, Environment domain, score decreased. but, they were not valid statistically. 3. VAS, House-Brackmann grade decreased, but, they were not valid statistically. Conclusion : The number of subjects with facial palsy in our study(N=5) was too small, and the period of study(4 weeks) was short, too. For this reason, our data were not valid statistically. But Facial palsy Patient's Quality of life has risen.

  • Iris diagnosis is an alternative medicine to diagnose the disease of the patient by using different of the iris pattern, color and other characteristics. This paper proposed a disease prediction algorithm that using the iris regions that analyze iris change to using differential image of iris image. this method utilize as patient's health examination according to iris change. Because most of previous studies only find a sign pattern in a iris image, it's not enough to be used for a iris diagnosis system. We're developed an iris diagnosis system based on a iris images processing approach, It's presents the extraction algorithms of 8 major iris signs and correction manually for improving the accuracy of analysis. As a result, PNSR of applied edge detection image is about 132, and pattern matching area recognition presented practical use possibility by automatic diagnostic that presume situation of human body by iris about 91%.

Two-short implant supported single molar restoration in atrophic posterior maxilla : a clinical study (위축된 상악구치부에서 두 개의 짧은 임플란트 지지형 단일치관의 임상연구)

  • Purpose: The aim of this retrospective study was to compare marginal bone loss and survival rates of double short implants(multiple implant) which had been installed and restored in severely atrophic maxillary molar site without a grafting procedure. Material and Method: The subjects were patients (90 patients, 180 implants) who had been installed double short implants in severely atrophic maxillary single molar site without bone augmentation procedure from 2006 to 2014 in dental clinic in Chuncheon city. Following data were collected from dental records and radiographic panoramic views: patient's age, gender, smoking status, implant site, timing of implant installation, residual ridge height. The correlation between those factors and survival rate and marginal bone loss were analyzed. Statistical analysis was performed using Chi-square test, Student's t- test and ANOVA. Result: Eleven implants in 6 patients failed and the cumulative survival rate was 93.9%. No significant differences were found in relation to the following factors: patient's age, gender, implant site, timing of implant installation (P> .05). There were significant differences in smoking status and residual ridge height(P< .05). The average follow-up time was $45{\pm}14.7months$. The mean marginal bone loss of survived 169 implants was $0.08{\pm}0.59mm$. Conclusion: Despite the short term outcomes, the survival rate of double short implants was comparable to normal length implants. This study demonstrated that placement of double short implants without the use of bone grafting procedure for severely atrophic posterior maxilla is a simple and predictable treatment procedure.

  • Recently, the occurrence and death rates of cancer have increased rapidly. In oriental medicine, the tongue gives some kind of special physiological information on human body. Oriental medical doctors have used information about the color, degree of wetness and shape of the patient's tongue to determine patient's disease and body condition. This fact leads us that the tongue image is one of the most important clinical data for helping doctor's decision making. It also has significant meaning to cancer patients related with traditional theory of oriental medicine. In diagnosis and treatment of cancer, tongue diagnosis can give some prognosis and change o cancer. It also detect the sign of early stage cancer, but it cannot diagnose the kind of cancer and/or benign or malignant. Nowadays BioTechnology(BT) has developed rapidly, but there are a lot of limits(economy, accuracy, clinical significance, etc). Tongue diagnosis is very economic and practical way of diagnose and has a lot of possibility of development. The need for doctors and it also helps the development of tongue diagnosis related with cancer.

Trend and Implication of OECD Hospital Performance Project (OECD 병원 성과 프로젝트의 동향과 국내 시사점)

  • The Organization for Economic Cooperation and Development, which has continuously evaluated the performance of healthcare systems, has recently invested much effort into hospital performance measurement. The purpose of this paper is to introduce the hospital performance measurement programs operated by international organizations or at the national level based on the OECD's hospital performance project. Health Insurance Review & Assessment service (HIRA)'s quality assessment was analyzed based on the analytical framework of the OECD's hospital performance project. The hospital performance measurement programs of WHO, Canada, Australia, United States and United Kingdom are briefly explored, in view of the conceptual framework, key performance dimensions and indicators that are currently in use. The OECD suggested seven key dimensions of hospital performance: timeliness, efficiency, continuity, effectiveness and appropriateness, staff orientation, patient orientation and safety. The analysis of the quality assessment program of HIRA, which operates 36 diseases and procedures and 347 indicators, shows that the numbers of indicators are relatively small in the areas of safety, patient centeredness and efficiency. Continuity of care and staff orientation are not fully developed also, but the situations are similar in other countries. In conclusion, hospital performance measurement using stable and comprehensive data should be developed to improve overall system performance, and discussions on a conceptual framework that can lay out directions and key performance domains need to take into place.

The Effects of Nursing Information at Discharge on Level of Knowledge and Daily Activities of Open Heart Surgery Patient (퇴원시 간호정보제공이 개심수술 환자의 지식과 일상활동에 미치는 효과)

  • This study examined the evaluation of the information effects of the teaching on knowledge and daily activities of open heart surgery patients between 2 and 6 weeks after discharge. The subject was 29 patients being taught with teaching materials at discharge as experimental group, 20 patients who received no education as control group among the patients who had undergone open heart surgery in S.N.U.H. And research method was non-equivalent control group non-synchronized quasi-experimental design. As the tool of this study, 30 items of knowledge measurement scale which was extracted among the content of teaching materials to evaluate the effect of education and 28 items of which were designed to measure the daily activities of patients with myocardial infarction for the estimation of the degree of observance in daily activities were used. For data analysis, frequency, t-test, Pearson's correlation coefficient and Cronbach's $\alpha$ were used. The result were as follows; 1. Informations given through teaching materials were effective for increasing the knowledge of the patient with open heart surgery. The knowledge of patients increased to the top level (p<0.05) in 2 weeks after discharge. In control group, the knowledge level of patients did not increase after discharge. 2. The knowledge level daily activity of the experimental group was somewat higher than that of the control group, but there was no significant difference. The score .of the experimental group was 69.66 in 6 weeks after discharge much less than the top level score 112. 3. The correlation between knowledge and daily activities was not significant, suggesting the fact that the increase of knowledge did not influence the daily activities significantly. Recommendation was suggested that; 1) Further studies might be .needed with the increasing numbers of the subjects. 2) Daily activities of the patients with open heart surgery should be investigated for long term period until they recovered normal activities.

Factors associated with unexpected revisit to an emergency medical center (예고되지 않은 응급의료센터 재방문에 영향을 미치는 요인 분석)

  • The objectives of this study were to identify factors associated with unexpected revisit to an emergency medical center (EMC) located in Seoul and to examine reasons for revisit. During March, June, September and December, 2002, a total of 168 patients had unexpected revisits to the EMC within 48 hours of a previous discharge. As a 1:1 matched control, we included 136 patients who: discharged from the EMC during the same time period: did not return to the EMC; had the same diagnosis and age(${\pm}5$) with the case. In this study, factors associated with unexpected revisits were defined as characteristics of a previous discharge, which were classified into three: sociodemographic, EMC visit-related, and discharge management factors. Reasons for revisit were categorized into disease, physician, patients, and system-related factors. Data were collected by medical chart review with assistance from clinicians of the EMC. Logistic regression results showed that patients who headed home after discharge without follow-up schedule had a 27.6 times higher risk of revisiting EMC than those who were hospitalized following EMC visit. Patients discharged on his own will had a 5.9 times higher risk of revisiting than those discharged following physician's advice. Patients requiring continual observation at the time of discharge were more likely to revisit by 8.7 times than those discharged with improved condition. About 69.13% of the revisits were due to disease-related factors, followed by 13.90% due to patient-related factors, 8.64% due to system-related factors, and 8.34% due to physician-related factors. It appears that the most significant factors influencing revisits are discharge management factors such as patient's condition at discharge, whether the discharge was accorded with physician's advice, and whether returning home without follow-up schedule. Therefore, appropriate discharge management is necessary to prevent EMC revisit.

The Effect of Hand Reflexology on Pain, Skin Temperature and Nursing Practice (입원 환자에서 손 반사요법의 통증, 피부체온 및 간호실무 효과)

  • Purpose: This study is aimed to examine the effectiveness on pain, feeling and nursing practice of hand reflexology applied to in-patient in the clinical setting. Method: The pre/post test methods using VAS for pain and feeling, physiologic measuring, and questionnaire of nursing care were conducted. Subjects were selected 45 in-patients from a surgical ward in hospitals during Nov.-Dec. 2005. 5 minute hand reflexology therapy was applied to both hands of participant by two research assistants at the same time. Data were analyzed based on SAS program using frequency, $x^2$-test, paired t-test, ANOVA and Pearson's correlation coefficients. Result: 1. After receiving hand reflexology therapy, the subjects showed significant pain relief(t=-4.94, p=.0001), improvement in feeling(t=19.44, p=0001) and an increase in skin temperature(t=3.54, p=.001). 2. The applied skills that the participants preferred were press-rotate(80.0%), press-walk(35.56%) and press-rolling(31.11%). The effectiveness of nursing practice scored $3.99{\pm}1.97$ out of 5. Nurse-Patient Relationship and effective response of nursing intervention ranked the highest 4.31. Conclusion: Hand reflexology was considered as an effective nursing intervention in the clinical settings, but in order to evaluate re-verifies and reliances for this effects, it is necessary that comparative studies should be conducted.

Effect of Informational Support by Hospice Team on Family Caregivers of Terminally III Cancer Patient (말기암 환자 가족에 대한 호스피스 팀의 정보적 지지 제공 효과)

  • To evaluate the effect of informational support by hospice team on family caregivers of terminally ill cancer patients. 22 family caregivers of D University Hospital in Daegu city were participated. The research was conducted from Aug. 16th to Oct. 28th 2000 by using self-reported questionnaires. The instruments used in this study were the Weinert's scale of perceived social support. Spielberger's state anxiety inventory. CES-D. and Ellison and Paloutzian's spiritual well-being scale. The intervention was designed to give educational and counselling program up to 7 times within 4 weeks. Educational and counselling booklets which made by the researcher were used step by step by hospice team, he data were analysed frequency. percentage. Wilcoxon Singed Ranks Test with SPSS Win l0.0/PC. The results obtained from this study were as follows; 1. The perceived social support of family caregivers was significantly increased after ready planned informational support was applied by hospice team(z=-3.045. p=0.002). 2. The anxiety of family caregivers was significantly reduced after ready planned informational support was applied by hospice team(z =-3:348. p=0.001). 3. The depression of family caregivers was significantly reduced after ready planned informational support was applied by hospice team(z=-3.641. p=0.000). 4. The spiritual well-being score of family caregivers was not significantly improved after ready planned informational support was applied by hospice team(z=-0.422. p=0.673). In conclusion. the results of this study clearly suggests that the informational support provided by hospice team not only increased the family caregivers' who are caring for terminally ill cancer patients. Therefor the informational support program designed by researcher for family caregivers who are caring for terminally ill cancer patients should be utilized and expended.
