Magnetoencephalography and Clinical Application (Magnetoencephalography (MEG)의 임상적 유용성)

  • Magnetoencephalography (MEG), the measurement of magnetic fields produced by neuronal current associated with normal and pathologic brain activities, is a totally noninvasive method for localizing functional regions of the brain. During the past several years, many clinical research centers are working to expand various fundamental functional brain regions, which can be easily localized, as well as to characterize magnetic abnormalities which accompany a wide variety of cerebral disease. At present, MEG is used in a number of clinical centers throughout the world for the presurgical functional localization of eloquent cortex, and for the non-invasive localization of epileptiform activity. And also, non-invasiveness means that it can be used for screening and repetitive follow-up measurement without concern for adverse effects. As procedures for activating various functional brain regions are standardized, and as the effects of specific cerebral diseases on the MEG are carefully documented in controlled studies, the number of routine neurological applications for MEG will increase significantly. In this paper, the basic principles of MEG are reviewed briefly with its clinical application to neurologic disease.

Crosstalk between the Producers and Immune Targets of IL-9

  • IL-9 has been reported to play dual roles in the pathogenesis of autoimmune disorders and cancers. The collaboration of IL-9 with microenvironmental factors including the broader cytokine milieu and other cellular components may provide important keys to explain its conflicting effects in chronic conditions. In this review, we summarize recent findings on the cellular sources of, and immunological responders to IL-9, in order to interpret the role of IL-9 in the regulation of immune responses. This knowledge will provide new perspectives to improve clinical benefits and limit adverse effects of IL-9 when treating pathologic conditions.

Effects of Yukmigeehwang-hwan Extracts on the Elastase Activity and DPPH and NO Scavenging Activities Original Articles (육미지황환(六味地黃丸)의 elastase 활성과 DPPH, NO 소거능에 미치는 영향)

  • Objectives : Elastic fibers are found in the skin, lungs, arteries, veins and other structures. Elastases destroy the elastic fibers and cause the emphysema and pulmonary hypertension. Oxidative stress is needed for these pathologic changes. Accordingly, present study was designed to investigate the effect of Yukmigeehwang-hwan extracts (YHE) on elastase activity and anti-oxidative effects of YHE. Methods : The inhibitory effects on elastase and DPPH and NO free radical scavenging activities of YHE were measured. Results : The elastase activity was significantly inhibited by YHE. YHE significantly scavenged DPPH and NO free radicals as well. Conclusion : YHE showed the elastase-inhibiting effects and anti-oxidative activities in vitro. These results suggest that YHE may have potential roles in the treatment of pulmonary emphysema and pulmonary hypertension.

Down-regulation of T Helper 2-Associated Cytokine Expression by Fisetin (Fisetin에 의한 비만세포 Th2 사이토카인 발현 하향 조절)

  • Mast cells play pivotal pathologic roles in allergic disease involving T helper 2 (Th2) cytokine such as interleukin (IL)-4 and IL-13. Fisetin has been known as an anti-allergic agent having inhibitory effects on the IL-4 and IL-13 gene expressions in inflammatory immune cells. However, its molecular mechanisms for suppressive effects of fisetin on IL-4 and IL-13 in activated mast cells have been incompletely elucidated. In this study we found that fisetin significantly inhibited the phorbol 12-myristate 13-acetate (PMA) and ionomycin (PI)-induced production of IL-4 and IL-13 in mast cells. The levels of mRNA were dramatically decreased by fisetin, indicating the suppression might be regulated at the transcriptional levels. Western blot analysis of the nuclear expression of various transcription factors involved in the promoter activation indicated that suppression of c-Fos was prominent together with significant down-regulation of nuclear factor of activated T-cell (NF-AT) and NF-${\kappa}B$, but not c-Jun. Furthermore, the nuclear expression of GATA binding protein 2 (GATA-2) transcription factor was significantly down-regulated by fisetin. Taken together, our study indicated fisetin has suppressive effects on IL-4 and IL-13 gene expression through the regulation of selective transcription factors.

Clonazepam Treatment of Pathologic Aerophagia in Children with Mental Retardation

  • Purpose: Pathologic aerophagia (PA) may lead to bowel perforation or volvulus in mentally retarded patients. The authors investigated the effects of clonazepam on the management of PA in children with severe to profound mental retardation (MR). Methods: This study was undertaken as a retrospective case analysis of 21 PA patients with MR who were followed for over 12 months and diagnosed as having PA. Patients were assigned to two management groups, that is, to a clonazepam randomized open-labeled, treatment group or a reassurance group. The following were recorded and analyzed; age, response, remission rate to clonazepam treatment, and the side effect of clonazepam. It was defined positive response (response+) as being symptom-free for a whole week within 1 month of commencing treatment and remission(+) as being symptom-free for a whole month within 6 months of treatment. Results: The average age of the 21 PA children with MR was 10 years and 13 patients were female. Symptom duration before diagnosis of PA was 7 months. Clinical features of the clonazepam-trial group (n=11) and the reassurance group (n=10) were non-significantly different. Response(+) was achieved by 2 patients (18.2%) in the clonazepamtrial group and by no patient in the reassurance group. Remission(+) was achieved by 6 patients (54.5%) in the clonazepam-trial group and by one patient (10%) in the reassurance group (p=0.040). Conclusion: When PA children with MR with severe bowel distention are considered for surgical treatment to prevent acute abdomen, a trial of clonazepam could be recommended.

The Effects of the Argon Laser Irradiation on the Root Surface : A Scanning Electronic Microscopic Study (아르곤 레이저 조사시 치근면 변화에 관한 주사 전자 현미경적 연구)

  • Since pathologic changes of exposed root surface inhibit cell attachment and new attachment of connective tissue have been made, many efforts were apply to change the exposed root surface condition. Scaling and root planing can not remove the endotoxin completely and forms the smear layer which prohibits the new attachment of connective tissue. Therefore, many kinds of chemicals were used for controlling the pathologic change of the root surface. The purposes of this study was to compare and observe the changes of the exposed root surface treated by scaling and root planning, Tetracycline HCl and Argon Laser. After the scaling and root planning of ten extracted premolars, the differences & the root surface among groups were observed under SEM. Control group showed smear layer and irregular amorphous surface. The dentinal tubule was not exposed. The debris and scale like texture were also observed. Tetracycline HCl treated group showed relatively smooth surface and the collagen fiber was observed in the dentinal tubule. Argon Laser treated group showed the most effective results under the conditions of 0.8 to 1.0w irradiation for 0.5 to 1.0 sec with pulse wave. The results of this study showed that the root surface change was associated with the intensity and the duration of Argon Laser irradiation. Further investigation for the surface change with the Argon Laser irradiation is recommended for understanding of clinical effect.

Recent Research Trends in Korean Medicine Treatment of Diabetes Mellitus - Focusing on Domestic Articles from 2008 to 2013 - (당뇨병의 한의학적 치료에 대한 최신 연구 동향 - 2008년부터 2013년까지 국내 학술논문을 중심으로 -)

  • Objectives : This study was performed to research trends in Korean medicine treatment of diabetes mellitus (DM). Methods : We reviewed 73 studies about diabetes mellitus, which had been published from 2008 to 2013. The article search was performed on the Korean institute of oriental medicine (KIOM) database. Results : 1. Patterns of study design were as follows : 14 review studies, 44 experimental studies, 7 clinical studies, 8 case reports. 2. From the 14 review studies, we noted that literature studies about definition, pathologic mechanism, treatment, pathologic pattern identification of DM and previous study analyses were done. 3. From the 44 experimental studies that are the most common type of DM studies, various Korean medicine treatments such as single medicinal herbs, herbal complexes, diets and pharmacopuncture were used. Anti-diabetic effects were demonstrated in most of the studies. 4. 7 clinical studies about acupuncture, electroacupuncture, herbal medicine, etc. were reported. 5. 8 cases about DM complications and comorbidities were reported. Clinical symptoms improved in all of studies, blood sugar level decreased in four studies. Conclusions : Further clinical and case study based on experimental researches should be performed.

Prediction of Post-Treatment Outcome of Pathologic Voice Using Voice Synthesis (음성합성을 이용한 병적 음성의 치료 결과에 대한 예측)

  • Background and Objectives : Patients with pathologic voice often concern about recovery of voice after surgery. In our investigation, we give controlled values of three parameters of voice synthesis program of Dr. Speech Science. such as jitter, shimmer, and NNE(normalized noise energy) which characterize someone's voice from others and deviced a method to synthesize the predicted voice after performing operation. Subjects and Method : Values of vocal jitter, vocal shimmer, and glottal noise were measured with voices of 10 vocal cord Paralysis and 10 vocal Polyp Patients 1 week Prior to and 1 month after the surgery. With Dr. Speech science voice synthesis program we synthesized 'ae' vowel which is closely identical to preoperative and post-operative voice of the patients by controlling the values of jitter, shimmer, and glottal noise. then we analyzed the synthesized voices and compared with pre and post-operative voice. Results : 1) After inputting the preoperative and corrected values of jitter, shimmer, and glottal noise into the voice synthesis Program, voices identical to vocal Polyp Patients' Pre- and Postoperative voices withiin statistical significance were synthesized 2) After elimination of synergistic effects between three paramenter, we were able to synthesize voice identical to vocal paralysis patients' preoperative voices. 3) After inputting only slightly increased jitter, shimmer into the synthesis program, we were able to synthesize voice identical to vocal cord paralysis patients' postoperative voices. Conclusion : Voices synthesized with Dr. Speech science program were identical to patients' actual pre and postoperative voice, and clinicians will be able to give the patients more information and thus increased patients cooperability can be expected.

  • Impaction of permanent incisor occurs rare than the canine & third molar. But it's often observed in school age child. The causes of impaction are trauma, space deficiency, mesiodens, infections of root apex, etc. In spite of elimination of cause, normal eruption of impacted tooth is rare. Though eruption is normal, the position of tooth will be incorrect. Because the impacted tooth results in malocclusion, root resorption of adjacent tooth, pathologic cystic change, it should be confirmed the precise position by clinical and radiographic exam and found the correct location by appropriate treatment plan. In case of pathologic change of impacted tooth and injury to adjacent tooth, it will be extracted. But through orthodontic retraction, the function and esthetics of tooth can be restored. It is important that impacted tooth should be detected early and diagnosed correctly, and appropriate treatment plan should be made. Before impacted tooth is retracted, the considerations of space for alignment and anchorage should be preceded and through appropriate force and mechanics, the side effects, for example, a root resorption should be minimized. In this study, we guided impacted tooth to normal position by using a forced eruption.

Suppressive Effects of Gamisojaganggi-tang on Immunopathogenesis in OVA-induced Asthma Model (가미소자기탕(加味蘇子氣湯)이 천식 유발 병태 모델에서 분자 및 조직병리학적 변화에 미치는 영향)

  • This study was done to investigate the effects of Gamisojaganggi-tang(GSGT) on immunopathologic changes in OVA-induced asthma model of mice. Pathologic indicators associated with this immune disease, which include cytokines, the number of immune-cells, immunoglobin E (IgE), were examined, and histological changes of bronchial tissues were also examined. The administration of GSGT significantly reduced the lung weight compared with control mice of OVA-induced asthma model. The administration of GSGT significantly reduced the number of total cells in BALF compared with control mice of OVA-induced asthma model. The administration of GSGT significantly reduced the number of eosinophil in BALF compared with control mice of OVA-induced asthma model. The administration of GSGT insignificantly increased the number of monocyte in BALF compared with control mice of OVA-induced asthma model. The administration of GSGT significantly reduced the number of lymphocyte in BAL compared with control mice of OVA-induced asthma model. The administration of GSGT significantly reduced the gene expression of eotaxin in lung tissue compared with control mice of OVA-induced asthma model. The administration of GSGT insignificantly reduced the IL-4 and IL-5 production in BALF compared with control mice of OVA-induced asthma model. The administration of GSGT insignificantly reduced the levels of total IgE and ovalbumin-specific IgE in BALF. The administration of GSGT significantly reduced the levels of ovalbumin-specific IgE whereas the serum levels of total IgE were insignificantly reduced compared with control mice of OVA-induced asthma model. The administration of GSGT moderately reduced bronchial alveolar narrowing, bronchiovascular edema and increase in the size of alveolar space, which shown in control mice of OVA-induced asthma model, in a dose dependent manner. Furthermore, GSGT reduced invasion of inflammatory cells, and proliferation of smooth muscle cells in bronchial tissue. These results suggested that GSGT has suppressive effects on pathologic changes associated with disease progression in asthma through the modulation of immune system. GSGT has potential to use as an anti-asthmatic agents.
