A systematic review of the keystone design perforator island flap in the reconstruction of trunk defects

  • The keystone design perforator island flap can be utilized in the repair of trunk defects. A systematic review was carried out to identify the complication rates of the use of this flap to treat such defects. The MEDLINE, Embase, Cochrane Library, and PubMed Central databases were searched for articles published between January 2003 and December 2018 that reported the use of keystone design perforator island flaps in the repair of trunk defects. Study selection was conducted in alignment with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Eight articles involving a total of 54 flaps satisfied the inclusion criteria. The most frequently reported cause of trunk defects was oncologic resection (64.4%). The overall complication rate was 35.2%, and complications included infection (11.1%), wound dehiscence (7.4%), delayed healing (7.4%), and partial flap loss (1.9%). The keystone design perforator island flap is associated with a high success rate and low technical complexity. Despite minor complications, keystone design flaps could be a preferred choice for trunk reconstruction.

보형물을 이용한 즉시 유방재건술에서 노출된 보형물의 보존 (Preservation of Exposed Breast Implant for Immediate Breast Reconstruction)

  • Purpose: Common complications of immediate breast reconstruction with implant are capsular contracture, malposition of implant, hematoma and seroma. Especially, the most severe complication is implant exposure caused by inflammation or necrosis of skin flap margin of mastectomy site. This article reports the experience of cases of salvage in such an exposure of implant. Methods: From July, 2002 to Feb., 2009, sixty-five patients who underwent immediate breast reconstruction with implant were retrospectively analyzed. Exposure of implant was happened in 5 of 65 patients and they were treated at out patient district. Two of five patients were reconstructed with saline implnt and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$ and Serratus muscle flap. Remaning three patients were reconstructed with silicone implant and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$. Results: In the group of patients who underwent reconstruction with saline implant, implant exposure was found in one patient due to partial necrosis of the margin of skin flap and debridement and primary repair were done. In the other one patient, dressing with antibiotic ointment were done. And debridement and primary repair were proceeded. In the group of patients who underwent reconstruction with silicone implant, implant exposure was found in one patient. After removal of the implant, tissue expansion was done and a new silicone implant was inserted. Implant exposure were found in the other two patients, antibiotics ointment application and primarily repaired. Conclusion: It was the common knowledge that the exposed implant should be removed. But salvage of the exposed implants may be possible with proper treatment. Four of five patients (80%) with exposed breast implant were salvaged with conservative management.

The Effects of Polydeoxyribonucleotide on the Survival of Random Pattern Skin Flaps in Rats

  • Background Partial or complete necrosis of a skin flap is a common problem. Polydeoxyribonucleotide (PDRN) can be extracted from trout sperm and used as a tissue repair agent. The aim of this study was to investigate whether PDRN could improve the survival of random pattern skin flaps in rats. Methods Twenty-two male Sprague-Dawley rats were randomly divided into two groups: the PDRN treatment group (n=11) and the control group (n=11). Caudally pedicled random pattern skin flaps were elevated on their dorsal skin and resutured. The treatment group received daily intraperitoneal administration of PDRN (8 mg/kg/day), and the control group received fluid vehicle (NaCl 0.9%, 8 mg/kg/day) from day 0 to day 6. On day 7, the flap survival was evaluated and the harvested tissue surrounding the demarcation line of the necrotic area was stained with H&E, anti-rat vascular endothelial cell growth factor (VEGF) antibody, and PECAM-1/CD31 antibody. Results The average necrotic area of the flap in the PDRN group was significantly smaller when compared with that of the control group. Histologic and immunohistochemical evaluation showed that granulation thickness score and VEGF-positive staining cells were marked higher in the PDRN group than in the control group. PECAM-1/CD31-positive microvascular densities were significantly higher in the PDRN group when compared with the control group. Conclusions This study confirms that PDRN improves the survival of random pattern skin flaps in rats. These results may represent a new therapeutic approach to enhancing flap viability and achieving faster wound repair.

의인성 혈관 손상의 임상적 고찰 (Clinical Feature of Iatrogenic Vascular Injury)

  • Purpose: As the care of surgical patients becomes increasingly complex and catheter-based techniques are more frequently applied, the pattern of iatrogenic vascular injuries may be increasing. Major vascular injuries can jeopardize a patient's life or limb survival. The purpose of this study was to examine the current etiology and prognosis for iatrogenic vascular injuries. Methods: We reviewed medical records of 29 cases of iatrogenic vascular injury that were treated Seoul National University Bundang Hospital between October 2003 and October 2008. We studied clinical variables including demographics, cause of injury, clinical presentations, management and prognosis. Results: The mean age was 60.8 years (range: 25-86), and the male to female ratio was 1.9 : 1. The causes of injuries were operation related complication in 18 cases (62.1%), endovascular intervention and diagnostic angiography in 11 cases (37.9%). The types of vascular injury were partial severance in 14 cases, pseudoaneurysm in 8, arteriovenous fistula (AVF) in 3, thrombosis in 2, complete severance in 2. Especially, device related complication including percutaneous closing device were occurred in 9 and the others came from inadvertent physician's procedure. Primary repair were done in 12 cases, end-to-end anastomosis in 5, interposition graft in 4, ligation in 2, patch angioplasty in 1, peudoaneurysm excision and arteriorrhaphy in 1, hematoma evacuation in 1, and endovascular repair in 3. There were 2 cases of mortality, one of them due to hemorrhagic shock and the other due to septic shock. Conclusion: Proper selection of treatment modalities should be important to have better outcome according to the type of injury as well as anatomical location. Each physician should be familiar to new device as well as patient's topographical feature. Immediate referral to vascular specialist is also essential to reduce morbidity.

Postoperative clinical outcomes and radiological healing according to deep and superficial layer detachment in first facet involving subscapularis tendon tear

  • Background: Subscapularis tendon insertion at the first facet has separate layers (deep and superficial). The purpose of this study is to evaluate postoperative clinical outcomes and radiological healing according to each layer of detachment in the first facet involving subscapularis tendon tear. Methods: Eighty-three patients who underwent arthroscopic repair due to First facet involving the scapularis tendon tear accompanying small to medium sized posterosuperior cuff tear were classified into three groups (group A: deep layer partial detachment, group B: deep layer complete detachment, but no superficial layer detachment, and group C: deep layer and superficial layer complete detachment). Subscapularis tendon healing was evaluated using computed tomography arthrogram and clinical result was evaluated using American Shoulder and Elbow Surgeons (ASES) shoulder score, Constant score and University of California Los Angeles (UCLA) shoulder score. Results: Retear rate of the subscapularis tendon was 2.2%, 18.2%, and 33.3% in group A, group B, and group C, respectively. These rates showed statistically significant difference among the three groups, which were classified by deep and superficial layer detachment in the first facet (p=0.003). Group A showed significant difference in subscapularis tendon healing compared with group B and group C (p=0.018 and p<0.001, respectively), but there was no statistical difference between group B and group C (p=0.292). Regarding clinical outcomes, there was no significant difference among three groups in ASES and UCLA score at final follow-up (p=0.070 and p=0.106, respectively). Conclusions: Complete detachment of deep layer may be related with retear occurrence regardless with detachment of superficial layer, but clinical outcome may not be related with each layer detachment in the first facet involving subscapularis tendon tear.

Efficient repair of damaged FRP-reinforced geopolymeric columns using carbon fiber reinforced polymers

  • Geopolymer concrete (GC) can be competently utilized as a practical replacement for cement to prevent a high carbon footprint and to give a direction toward sustainable concrete construction. Moreover, previous studies mostly focused on the axial response of glass fiber reinforced polymer (glass-FRP) concrete compressive elements without determining the effectiveness of repairing them after their partial damage. The goal of this study is to assess the structural effectiveness of partially damaged GC columns that have been restored using carbon fiber reinforced polymer (carbon-FRP). Bars made of glass-FRP and helix made of glass-FRP are used to reinforce these columns. For comparative study, six of the twelve circular specimens-each measuring 300 mm×1200 mm-are reinforced with steel bars, while the other four are axially strengthened using glass-FRP bars (referred to as GSG columns). The broken columns are repaired and strengthened using carbon-FRP sheets after the specimens have been subjected to concentric and eccentric compression until a 30% loss in axial strength is attained in the post-peak phase. The study investigates the effects of various variables on important response metrics like axial strength, axial deflection, load-deflection response, stiffness index, strength index, ductility index, and damage response. These variables include concentric and eccentric compression, helix pitch, steel bars, carbon-FRP wrapping, and glass-FRP bars. Both before and after the quick repair process, these metrics are evaluated. The results of the investigation show that the axial strengths of the reconstructed SSG and GSG columns are, respectively, 15.3% and 20.9% higher than those of their original counterparts. In addition, compared to their SSG counterparts, the repaired GSG samples exhibit an improvement in average ductility indices of 2.92% and a drop in average stiffness indices of 3.2%.

관절경하 회전근 개 봉합술 후 파열 정도에 따른 통증 회복 양상 (Pain Recovery Pattern According to the Integrity after an Arthroscopic Rotator Cuff Repair)

  • 목적: 관절경하 회전근 개 봉합술 후 파열 정도에 따른 통증 회복의 양상과 경과에 영향을 미치는 인자에 대해 조사하였다. 대상 및 방법: 2006년 1월부터 2008년 4월까지 관절경하 회전근 개 봉합술을 시행받은 153명의 환자를 대상으로 하였으며, 평균 연령은 56.4(39~70)세 였다. 수술 후 추시 기간은 평균 20개월(12~30개월)이었으며, 견관절 운동 범위와 임상적 기능적 평가로써 ASES(American Shoulder and Elbow Surgeons) 점수, UCLA(University of California at Los Angeles) 견관절 평가 지수를 이용하였다. 통증에 대한 평가는 주관적 시각 척도를 이용하였다. 파열 정도에 따른 통증 회복 양상은 전층 파열군과 부분 파열군으로 분류하여 Student T-test를 이용하였고, 통증에 미치는 인자는 다중 회귀 분석을 이용하여 통계분석을 시행하였다. 결과: 술 후 주관적 시각 척도(Visual Analogue Scale, VAS)는 파열 정도에 따라 의미있는 차이는 보이지 않았으며, 두 군 모두 지속적 감소 추세를 보였으나, 술 후 3주째에 평균값이 상승한 후 하강하였고, 7주째에 평균값이 다시 상승한 후 하강하였다. 주관적 시각 척도가 2이하가 될 때는 술 후 평균 3개월이었다. 술 후 3개월에 통증에 영향을 주는 요소는 술 전 전방 굴곡과 의미있는 관계가 있었다($r^2=0.377$, p=0.021). 결론: 파열 정도에 따른 통증 회복 경과의 차이는 보이지 않으며, 술 전 견관절 전방 굴곡의 운동범위가 술 후 통증의 정도에 영향을 미치는 것으로 사료된다, 따라서 술 전 견관절 운동 범위를 향상시킬 수 있는 적절한 재활 지침이 술 후 빠른 통증 감소와 기능 회복에 도움을 줄 수 있을 것으로 사료된다.

경증의 폐동맥 고혈압을 동반한 고령에서의 심내막상 결손 환자 치험 1예 (Case Report of Partial Endocardial Cushion Defect with Mild Pulmonary Hypertension in Old Age)

  • 부분 심내막상 결손증은 일차공 결손과 승모판 전엽에 열공을 동반하는 질환으로 완전형 심내막상 결손증에 비해 임상 증상이 심하지 않으나, 승모판 부전이 진행할수록 심비대, 심부전, 폐동맥 고혈압 등이 더욱 쉽게 발현되어 그 예후가 좋지 않고, 따라서 고령에서는 보기 힘든 질환이다. 본원으로 30년 전부터 증상이 있던 67세 부분 심내막상 결손증 환자가 내원하였다. 심도자 검사상 폐동맥압이 45/22 mmHg, 평균 압력은 32 mmHg로 약간의 폐동맥 고혈압이 있었고, 수술 소견상 $3.5{\times}2.5\;cm$ 크기의 일차공 결손과 심한 석회화를 동반한 승모판 전엽의 열공이 보여 일차공 결손 첨포 봉합과 승모판막 치환술을 시행하였다. 수술 후 11일째부터 심한 방설 차단이 발생하여, 영구 심박동기를 삽입하였고, 수술 27일 후 퇴원하였다. 저자들은 임상적으로 보기 드문 고령에서의 부분 심내막상 결손증을 수술적으로 교정하였기에 문헌 고찰과 함께 보고하는 바이다.

확장성 심근증 환아에서의 부분 심실 절제술의 적용 -1례 보고- (Partial Left Ventriculectomy in the Pediatric Patient with Dilated Cardiomyopathy)

  • 심한 울혈성 심부전으로 Dopamine을 1개월간 사용해온 10세의 확장성 심근증 환아에서 심장 이식을 시행하려 하였으나, 적절한 크기의 공여 심장이 없었고, 환아의 증상이 악화되어 부분 심실 절제술과 승모판 성형술을 시행하였다. 수술후 환아의 임상 증상은 현저히 호전되었고 수술전과 수술후 3개월, 6개월 1년째의 심장 초음파 검사 소견을 비교하여 추적 관찰한 결과, 좌심실의 심박출 계수는 수술전 17 %에서 수술후 각각 29%, 35%, 36%로 개선되었고 좌심실 확장기말 직경은 수술전 72 mm에서 수술후 각각 59 mm, 61 mm, 61 mm로 변화를 보였다. 부분 좌심실 절제술과 승모판 성형술의 동반 시행은 심장 이식의 기회가 상대적으로 적은 소아의 확장성 심근증에 있어 심장의 부하를 감소시켜 증상 완화를 시킬 수 있는 외과적 방법으로 심장 이식을 시행할 때까지 유지시켜주는 고식적 가치가 크며 아울러 장기적 추적 관찰이 필요하다고 사료 된다.

Experimental assessment of post-earthquake retrofitted reinforced concrete frame partially infilled with fly-ash brick

  • Many public buildings such as schools, hospitals, etc., where partial infill walls are present in reinforced concrete (RC) structures, have undergone undesirable damage/failure attributed to captive column effect during a moderate to severe earthquake shaking. Often, the situation gets worsened when these RC frames are non-ductile in nature, thus reducing the deformable capability of the frame. Also, in many parts of the Indian subcontinent, it is mandatory to use fly-ash bricks for construction so as to reduce the burden on the disposal of fly-ash produced at thermal power plants. In some scenario, when the non-ductile RC frame, partially infilled by fly-ash bricks, suffers major structural damage, the challenge remains on how to retrofit and restore it. Thus, in this study, two full-scale one-bay, one-story non-ductile RC frame models, namely, bare frame and RC partially infilled frame with fly-ash bricks in 50% of its opening area are considered. In the previous experiments, these models were subjected to slow-cyclic displacement-controlled loading to replicate damage due to a moderate earthquake. Now, in this study these damaged frames were retrofitted and an experimental investigation was performed on the retrofitted specimens to examine the effectiveness of the proposed retrofitting scheme. A hybrid retrofitting technique combining epoxy injection grouting with an innovative and easy-to-implement steel jacketing technique was proposed. This proposed retrofitting method has ensured proper confinement of damaged concrete. The retrofitted models were subjected to the same slow cyclic displacement-controlled loading which was used to damage the frames. The experimental study concluded that the hybrid retrofitting technique was quite effective in enhancing and regaining various seismic performance parameters such as, lateral strength and lateral stiffness of partially fly-ash brick infilled RC frame. Thus, the steel jacketing retrofitting scheme along with the epoxy injection grouting can be relied on for possible repair of the structural members which are damaged due to the captive column effect during the seismic shaking.
