In vitro Anticancer Activity of Paclitaxel Incorporated in Low-melting Solid Lipid Nanoparticles

  • Triglyceride solid lipid with medium chain fatty acid, tricaprin (TC), was used as a core matrix of lipid nanoparticles (LN) to solubilize water-insoluble paclitaxel and enhance the stability of nanoparticles by immobilization of incorporated drug in the solid core during storage at low temperature. In the present study, TC-LN containing paclitaxel was prepared by hot melt homogenization method using TC as a core lipid and phospholipids as stabilizers. The particle size of TC-LN containing paclitaxel was less than 200 nm and its zeta potential was around -40 mV. Calorimetric analysis showed TC core could be solidified by freezing and thawing in the manufacturing process in which the hot dispersion should be prepared at elevated temperature and subsequently cooled to obtain solid lipid nanoparticles. The melting transition of TC core was observed at $27.5^{\circ}C$, which was lower than melting point of TC bulk. The particle size of TC-LN remained unchanged when kept at $4^{\circ}C$. Paclitaxel containing TC-LN showed comparable anticancer activity to the Cremophore ELbased paclitaxel formulation against human ovarian (OVCAR-3) and breast (MCF-7) cancer cell lines. Thus, lipid nanoparticles with medium chain solid lipid may have a potential as alternative delivery system for parenteral administration of paclitaxel.

  • Omega (${\omega}$)-3 polyunsaturated fatty acids appear to be effective in preventing and treating parenteral nutrition-associated liver disease, and several mechanisms were proposed for this observation. An 8-week-old male infant with cholestasis and acholic stool was diagnosed non-syndromic intrahepatic interlobular bile duct paucity by open-wedge liver biopsy. Initially he was treated with usual supportive medical therapy, including ursodeoxycholic acid. However, the clinical status and laboratory tests did not improve. Omega (${\omega}$)-3 polyunsaturated fatty acids (initially intravenous administration and oral administration later), were started and his liver function, including aminotransferase level and bilirubin levels normalized, and the ivory stool color turned green. We report the possible effectiveness of ${\omega}$-3 polyunsaturated fatty acids as a potent choleretic agent for non-syndromic intrahepatic interlobular bile duct paucity, a very rare structural pediatric hepatic disease.

Analysis of the Nursing Interventions Performed by Hospital Nurses Using NIC (간호중재분류(NIC)에 근거한 간호중재수행분석 I -병원 간호사를 중심으로-)

  • The purpose of this research was to identify nursing interventions performed by hospital nurses in Korea. The sample consisted of 311 nurses working in three hospitals. The Nursing Interventions Use Questionnaire developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean using the method of back-translation. Eighteen interventions were performed at least daily. Interventions in the Physiological : Basic domain were most frequently used at least daily. No interventions in the Family and Behavioral domains were used by nurses at least once a day. The most frequently used interventions was Documentation, followed by the interventions Medication : Parenteral, Intravenous(IV) Insertion, Temperature Control, and Shift Report. The intervention performed least often was Reproductive Technology Management. Nurses working in intensive care units on the whole performed interventions most often, while nurses working in obstetric, gynecological, and pediatric units performed them least often. The nurses working in intensive care unit, medical and surgical care units performed the interventions in the Physiological : Basic domain more often than the nurses working in obstetric, gynecological, and pediatric units. The nurses working in obstetric, gynecological, and pediatric units used the interventions in the Family domain more often than the nurses working in the other three units. This study contributes to the documentation of nursrs' work in Korea. Further study will be needed to validate nursing activities of each NIC intervention.

Wernicke's encephalopathy in a patient with masticator and parapharyngeal space abscess: a case report

  • Wernicke's encephalopathy is a fatal neurological disease caused by thiamine deficiency. Many reports indicate that Wernicke's encephalopathy is caused by malnutrition. We report the case of a 79-year-old female patient who had a left masticator space and parapharyngeal space abscess who was diagnosed with Wernicke's encephalopathy. She reported problems while eating due to the presence of the abscess, but the true quantities of food she was ingesting were never assessed. Clinicians have a responsibility to provide adequate nutritional support by ensuring that patients receive adequate nutrition. Clinicians should also keep in mind that Wernicke's encephalopathy may occur in patients who experienced prolonged periods of malnutrition.

  • The present study is concerned with the demonstration of the relationship between the behavior and the brain concentration of noradrenaline resulted from pretreatment of amphetamine in isolated or aggregated rats. The experimental subjects were rats weighing from 120g to 200g housed 1, 2, and 6 in a cage. Analeptic activity of amphetamine was measured by determining the sleeping time induced by pentobarbital sod. The noradrenaline content in brain was determined with Aminco-Bowmann's spectro-photofluorometer by Lee's modification of Shore and Olin method. Results: 1) The analeptic activity of amphetamine on the sleeping time induced by pentobarbital sod. was more increased in the grouped rats than in isolated animal. 2) In being isolated and grouped rats, the sleeping time induced pentobarital sod, was markedly prolonged by pretreatment of amphetamine. 3) Means of housing rats, e.g., isolation or aggregation did not seem to affect the brain noradrenaline depleting action. 4) Repeated daily parenteral administration of amphetamine sulfate for a period 1 to 3 weeks resulted in decrement of brain noradrenaline concentration in being isolated and grouped rats. 5) The prolongation of sleeping time of the isolated or aggregated rats, when pretreated with amphetamine, compared with that of stock rats, seems to be attributable rather to the means of housing than the variation of the noradrenaline caused by amphetamine.

Medicinal Leech Therapy for Salvage of the Failing Flap (피판 부전증의 구제를 위한 약용거머리 치료법)

  • The medicinal leech, Hirudo medicinalis, has been used for salvage of the venous-congested flap following reconstructive surgery, with increasing frequency during the last two decades. Medicinal leech therapy is a safe, efficacious, economical, and well-tolerated intervention. The flap salvage with leeching occurs in approximately 70% of cases, and leeches need to be used as early as possible. At the time of the bite, evacuation of blood and injection of the leech saliva products, including anticoagulants and inhibitors of platelet aggregation, maintain capillary circulation of the flap, and then venous capillary return is established across the wound by angiogenesis. One of the salivary products, hirudin, represents the first parenteral anticoagulant introduced since the discovery of heparin. We analyzed two cases in which flap salvage with leeching was attempted, and reviewed medicinal leech therapy on the basis of our experiences and literature review. This has not been documented in the head and neck surgery literature in our country.

  • Zinc is employed in body cell growth and immune function. Zinc deficiency causes growth retardation, night blindness, delay of wound healing, skin disorders, alopecia, et al. Pharmacokinetic study of zinc in Korean has not yet been proceeded to apply to patient. In this study, we investigated to adjust zinc dosage for maintaining constant zinc optimum serum level. Five NPO inpatients (3 males, 2 females) in Seoul National University of Hospital were evaluated, retrospectively. Zinc was mixed with parenteral nutrition solution and administrated intravenously in 24 hour-continuous infusion, and zinc dosage range was 0.17-0.45 mg/kg/day. AUC to zinc dose (AUC/Dose) was $2.421\pm1.007\;kg{\cdot}min/ml.$ 0.5 mg/kg/day zinc administration may achieve the plasma zinc concentration of $120\;{\mu}g/dl.$(Kor. J. Clin. Particle Size Distribution and Stability of Lipid Emulsion in Total Nutrient Admixtures (Total Nutrient Admixtures(TNAs)에서 지질유제의 입자크기 분포 및 안정성)

  • The particle size distribution and physical stability of commercial lipid emulsion inject, $intralipos^R$ before and after mixture with total parenteral nutrition(TPN) was tested. Three TNAs were prepared by adding intralipos to P-TPN, Neo-TPN and IVH-2 respectively. Particle size of fat emulsion in three TNA preparations were measured by using LPA-3000 photon correlater. Each TNAs was stored for 48 hours at $4^{\circ}C\;and\;25^{\circ}C.$ During storage, three TNAs showed the particle size in the range of 40-1000nm(about $100\%$ of total fat) and in the range of 1000-8000nm(less than $0.005\%$ of total fat). All TNAs were stable in terms of pH and visual appearance. The results showed that added lipid emulsion was stable for 48 hours at $4^{\circ}C\;and\;25^{\circ}C$.

Video-assisted Thoracoscopic Stapled Resection for Spontaneous Pneumothorax (비디오 흉강경을 이용한 자연성 기흉의 치료)

  • Video-assisted thoracic surgery [VATS is emerging as a viable alternatives to thoracotomy when surgical treatment of spontaneous pneumothorax is required.Apical blebs and bullaes of the lung can be resected,and pleural abrasion can be accomplished with minimal postoperative pain and a shorter postoperative stay in hospital. We compared our results with thoracoscopic management of spontaneous pneumothorax in 20 patients [group I with a group of 32 patients previously subjected to lateral limited thoracotomy [group II . Indications for operation, sex distribution, and average age [groupI, 24.7 years ; group II, 34.4 years were comparable. Operation time [112.42 54.7 min versus 124.8 35.3 min ; P 0.03 and chest tube duration [64.4 52.3 hours versus 97.7 45.4 hours ; P 0.01 were less in group I. Postoperative hospital stay was less in group I[3.84 0.99 days;P 0.01 , as was the use of parenteral narcotics after 48 hours. [5/20=25% versus25/32=78% . Pain was quantitated by verbal rating scale in postoperative 1 to 3 days. Patients undergoing VATS experienced significantly less postoperative pain. Postoperative complication was less in group I[1/20=5% versus 3/32=8.3% . In conclusion, Video-assisted thoracoscopic management of spontaneous pneumothrax allows performance of the standard surgical procedure while avoiding the thoracotomy incision.Video-assisted thoracic surgery [VATS is safe and offers the potential benefits of shorter postoperative hospital stays and less pain with cosmetic benefits.

Solid Lipid Nanoparticles as Drug Delivery System for Water-Insoluble Drugs

  • Solid lipid nanoparticles (SLNs) have emerged to combine the advantages of polymeric nanoparticles and lipid emulsions in early 1990s. SLNs can present several desirable properties derived from the solid state core. When formulating SLNs, there should be careful considerations about the physical state of the inner solid lipid core and its polymorphism and supercooling behavior. In this review, SLNs were compared to lipid emulsion and emulsion of supercooled melt to understand the unusual behaviors compared to lipid emulsions and to have insights into stability and release mechanism. SLNs have been regarded as biocompatible system because lipids are usually well-tolerable ingredients than polymers. Several studies showed good tolerability of SLNs in terms of cytotoxicity and hemolysis. Similar to various other nanoparticulate drug delivery systems, SLNs can also change biodistribution of the incorporated drugs in a way to enhance therapeutic effect. Most of all, large scale production of SLNs was extablished wihtout using organic solvents. Although there is no SLN product in the market till date, several advantagious properties of SLNs and the progress we have seen so far would make commercial product of SLNs possible before long and encourage research community to apply SLN-based formulations for water-insoluble drugs.
