Twenty two hypertensive and thirty normotensive in-patients were participated in this study to investigate the relationship between plasma renin activity and metabolism of Ca and Na, Prior to pharmacological treatments, renin activity, aldosterone and parathyroid hormone(PTH) levels were measured from the fasting blood samles. Twenty four hour urine samples were collected to analyze urinary levels of creatinine, Ca, Na and K. Habitual intake of Na and Ca were also measured for hypertensive and normotensive patients. Hypertensive subjects were classified into higher reinin hypertensive (HH), medium renin hypertensive(MH) and low renin hypertensive (LH) group according to their renin activities. PTH level of LH group was the highest among three hypertensive groups. It appeared that aldosterone levels of HH group were significantly higher than LH or MH groups(p<0.05). However there were no significan시 differences in aldosterone level between LH group and normotensive group. Habitual intake of Na and Ca were highest in LH group but lowest in HH group, however, they were not statistically different. Positive correlations of systolic blood pressure with PTH(r=0.2597) and aldosterone(r=0.26480existed(p<0.05). Urinary Ca level was positively correlated with urinary Na(r=0.5619), K(r=0.4533) and habitual Na intake(r=0.3253). Above results suggested the possible relationships among renin activity, habitual Ca intake and Na intake and suggested a further study on the interrelationship between the hormonal control of Ca and Na metabolism and blood pressure in hypertension.
To determine the deleterious effects of chronic alcohol consumption on bone especially in adolescents or young adults, 8 week-old Sprague Dawley male rats were fed with Lieber-Decarli ethanol liquid diet, containing $36\%$ of energy as ethanol, ad libitum (ethanol group) or isocaloric normal liquid diet (control group) for 7 weeks. Body weight was significantly lower in ethanol group than that in control group after 1 week of feeding to the end. liver weight and the ratio of liver or kidney weight to body weight in ethanol group were significantly increased when compared to those in control group. Ethanol group showed significantly lower serum protein and albumin levels (p<0.05), higher total cholesterol and HDL-cholesterol levels (p<0.05), and AST, ALT and BUN activities than control group, but serum triglyceride, Ca and phosphate levels were not different. Ethanol group had significantly lower tibial trabecular bone area and serum osteocalcin level than control group (p<0.05), but urinary Ca and NTx (cross-linked N-telopeptide of type I collagen) concentrations and serum testosterone and parathyroid hormone levels were not different. In conclusion, chronic alcohol consumption in growing young male rats may result in osteopenia through the reduction of bone formation as well as liver malfunction.
The role of estrogen in calcium metabolism has not been well documented. To further investigate the effects of endogenous estrogen on the calcium regulating hormones in women, we correlated the changes of estrogen level with those of calcium regulating hormones during ovulation induction in 12 hypogonadal and 8 normally menstruating women. During ovulation induction, the serum leveles of estradiol rose from 40.0 to 831.0pg/ml. There were no significant changes in the serum leveles of total calcium, inorganic phosphorus, parathyroid hormone, calcitonin. However, 1, $25-(OH)_2D_3$ rose significantly from 31.0 to 47.2pg.ml as the endogenous $E_2$ increaed (p<0.005).
The aim of this study was to investigate the interrelationship among urinary excretion dietary habit of Na, Ca intake and hormonal factors in 22 hypertensive and 30 normotensive hospitalized patients. The results were summarized as follows : 1. Urinary excretion of Na in normotensive patients was not significantly different from that of hypertensive patients, while Ca excretion(as expressed on the basis of mg of creatinine) was significantly higher in hypertensive patients. 2. Habitual dietary intake of Na, Ca in hypertensive patients were not significantly different from those of normotensive patients. 3. There were no significant differences in mean plasma renin activity, aldosterone and parathyroid hormone(PTH) level between two groups. However, systolic pressure significantly correlated with PTH(r=0.2597) and aldosterone level(r=0.24648)(P<0.05). In this study blood pressure did not show any significant relationship between urinary Na excretion and habitual dietary Na intake of Na. It is speculated that individual difference of Na sensitivity might result in heterogenous blood pressure response to dietary Na intake. Higher Ca excretion in hypertensive subjects suggested a future study on the interrelationship between Ca metabolism and aldosterone system in hypertension.
As the population of the elderly grows, the prevalence of osteoporosis and its related fractures will increase in both men and women. The etiology, preventive and curative strategies of male osteoporosis are relatively unknown and understudied in comparison with those of female osteoporosis. Even currently approved therapies, e.g. bisphophonates, parathyroid hormone, and testosterone for male osteoporosis are in need of further investigation to test their safety and efficacy. Isoflavones which are found in soy have been shown to positively affect bone by stimulating bone formation while concurrently slowing down bone resorption. These observations mainly come from studies that have employed women or female animal models of osteoporosis. Therefore, there is a need to explore the role of soy and its isoflavones in preventing bone loss or rebuilding bone utilizing men or animal models of .male osteoporosis. From the review of existing literature it is too early to state the extent to which men with osteoporosis can benefit from consumption of soy or its isoflavones. In this review, the efficacy of soy and its isoflavones as alternative and/or adjunctive treatment for male osteoporosis will be discussed.
The purpose of this study was to investigate correlation with self-care behaviors and physiological index according to cognitive function of hemodialysis patients. The data were collected from 108 patients on hemodialysis in university hospital. The period of data collection was from January 4 to January 31, 2010. The instruments of research were MMSE, Self-care behavior scale for hemodialysis patients. The 17.6% of patients showed the cognitive disorder. Self-care behaviors of patients showed that normal cognitive function group was significantly higher than dysfunction group. Creatinine and parathyroid hormone of patients showed that normal cognitive function group was significantly lower than dysfunction group. The cognitive function of patients showed positive correlations with self-care behavior(r=.283, p=.003) and Kt/V(r=.214. p=.026), showed significant negative correlations with phosphorus(r=-.223, p=.020) and creatinine(r=-.318, p=.001). The results of this study can be used to develop nursing education program to improve self-care behaviors according to cognitive function of hemodialysis patients.
Renal Fanconi syndrome (RFS) is caused by generalized proximal tubular dysfunction and can be divided into hereditary and acquired form. Adult-onset RFS is usually associated with drug toxicity or systemic disorders, and modern molecular genetics may explain the etiology of previous idiopathic cases of RFS. Here, we report the case of a 52-year-old woman with RFS whose etiology could not be identified. She presented with features of phosphaturia, renal glucosuria, aminoaciduria, tubular proteinuria, and proximal renal tubular acidosis. Her family history was unremarkable, and previous medications were nonspecific. Her bone mineral density was compatible with osteoporosis, serum intact parathyroid hormone level was mildly elevated, and 25(OH) vitamin D level was insufficient. Her blood urea nitrogen and serum creatinine levels were 8.4 and 1.19 mg/dL, respectively (estimated glomerular filtration rate, $53mL/min/1.73m^2$). Percutaneous renal biopsy was performed but revealed no specific renal pathology, including mitochondrial morphology. No mutation was detected in EHHADH gene. We propose the possibility of involvement of other genes or molecules in this case of adult RFS.
Periodontal disease is primarily associated with bacterial infection such as dental plaque. Dental plaque, an oral biofilm harboring a complex microbial community, can cause various inflammatory reactions in periodontal tissue. In many cases, the local bacterial invasion and host-mediated immune responses lead to severe alveolar bone destruction. To date, plaque control, non-surgical, and surgical interventions have been the conventional periodontal treatment modalities. Although adjuvant therapies including antibiotics or supplements have accompanied these procedures, their usage has been limited by antibiotic resistance, as well as their partial effectiveness. Therefore, new strategies are needed to control local inflammation in the periodontium and host immune responses. In recent years, target molecules that modulate microbial signaling mechanisms, host inflammatory substances, and bone immune responses have received considerable attention by researchers. In this review, we introduce three approaches that suggest a way forward for the development of new treatments for periodontal disease; (1) quorum quenching using quorum sensing inhibitors, (2) inflammasome targeting, and (3) use of FDA-approved anabolic agents, including Teriparatide and sclerostin antibody.
Nephrocalcinosis often occurs in infants and is caused by excessive calcium or vitamin D supplementation, neonatal primary hyperparathyroidism, and genetic disorders. Idiopathic infantile hypercalcemia (IIH), a rare cause of nephrocalcinosis, results from genetic defects in CYP24A1 or SLC34A1. Mutations in CYP24A1, which encodes 25-hydroxyvitamin D 24-hydroxylase, disrupt active vitamin D degradation. IIH clinically manifests as failure to thrive and hypercalcemia within the first year of life and usually remits spontaneously. Herein, we present a case of IIH wih CYP24A1 mutations. An 11-month-old girl visited our hospital with incidental hypercalcemia. She showed failure to thrive, and her oral intake had decreased over time since the age of 6 months. Her initial serum parathyroid hormone level was low, 25-OH vitamin D and 1,25(OH)2 vitamin D levels were normal, and renal ultrasonography showed bilateral nephrocalcinosis. Whole-exome sequencing revealed compound heterozygous variants in CYP24A1 (NM_000782.4:c.376C>T [p.Pro126Ser] and c.1310C>A [p.Pro437His]). Although her hypercalcemia and poor oral intake spontaneously resolved in approximately 8 months, we suggested that her nephrocalcinosis and renal function be regularly checked in consideration of potential asymptomatic renal damage. Hypercalcemia caused by IIH should be suspected in infants with severe nephrocalcinosis, especially when presenting with failure to thrive.
골다공증이란 골량의 감소에 의해 야기되며. 해면골 미세구조의 골밀도가 감소하는 질병이다. 약물치료(부갑상선 호르몬)법은 골소주의 두께 및 골의 강도를 어느 정도 증가시킬 수 있는 호르몬 치료법이다. 척추성형술은 골다공성 척추 압박 골절의 치료를 위하여 척추 해면골에 주사기를 통하여 골 시멘트를 주입하는 최소 침습적 수술법이다. 임상적으로 골 시멘트의 물성치에 영향을 미치더라도 점성 감소와 주입 시간 확보를 위해 중합비율을 변경하여 사용한다. 본 연구의 목적은 골 시멘트 중합비율 변경에 따른 척추 해면골의 역학적 특성을 알아보고자 한다. 본 논문에서 는 수정된 보로노이 도형을 이용하여 척추 해면골의 유한요소 모델을 생성하고, 골 시멘트의 중합비율(0.40~l.07$m\ell$/g)에 따른 척추 해면골의 강도회복 정도를 비교하였다. 또한 골다공증 치료에서 호르몬 치료와 골 시멘트 치료의 상대적인 영향을 비교 분석하였다. 분석결과 골 시멘트 치료모델의 탄성계수와 강도는 정상 상태의 약 50%로 회복되었으며, 이 값은 호르몬 치료 모델의 약 2배이다. 0.53$m\ell$/g의 중합비율에서 골 시멘트의 탄성계수와 강도가 최고이며 1.07$m\ell$/g의 중합비율에서 최소의 탄성계수와 강도(각각 42%와 49%)이지만 약물 치료보다는 더 효과적임을 알 수 있다. 척추성형술시 제작사에서 추천하는 중합비율과 다른 비율을 사용할 경우 골 시멘트의 물성치 감소뿐만 아니라 환자의 골 밀도에 따라서 해면골의 강도회복에 문제가 발생할 수 있다는 것을 알 수 있었다.
