  • To develop effective training methods for strengthening a weakened quadriceps femoris muscle in hemiplegic patients, we examined the effects of maximal isometric contraction of the nonparalyzed knee joint on the electromyographic activities of the paralytic muscle. An electromyogram (EMG) was used to record the electromyographic activities of the paralytic quadriceps femoris muscle in 27 hemiplegic patients. The maximal isometric contraction was measured for each subject to normalize the electromyographic activities. The maximal isometric extension and flexion exercises were randomly conducted when the knee joint angles of the nonparalyzed knees were $0^{\circ}$, $45^{\circ}$, and $90^{\circ}$. The patients were encouraged to maintain maximal isometric contractions in both knee joints during each measurement, and three measurements were taken. A one-minute rest interval was given between each measurement to minimize the effects of muscle fatigue. An average from the three values was taken as being the root mean square of the EMG and was recorded as being the maximal isometric contraction. The electromyographic activity obtained for each measurement was expressed as a percentage of the reference voluntary contraction, which was determined using the values obtained during the maximal isometric contraction. The results of this study are summarized as follows: First, when the knee joint angle of the nonparalyzed knee was $0^{\circ}$, the electromyographic activities of the paralytic medial aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). Second, when the knee joint angle of the nonparalyzed knee was $90^{\circ}$, the electromyographic activities of the paralytic lateral aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). The results show that myoelectrical activities of paralytic quardriceps were not related to measurement angles and exercise directions of the nonparalized knee joint. Studies on various indirect intervention to improve muscular strength of patients with nervous system disorders of the weakened muscle should be constantly conducted.

  • The literature review about bladder management method given to maintain and improve health of neurogenic bladder patients was done. Because the study of neurogenic bladder patients in nursing field is not enough, I tried to find report the study tendency through literature review, 1. There are five types of neurogenic bladder such as uninhibited neurogenic bladder, reflex neurogenic bladder, autonomous neurogenic bladder, sensory paralytic neurogenic bladder, and motor paralytic neurogenic bladder. 2. The accurate assessment of neurogenic bladder is done mainly through urodynamics and especially cystometrogram and urethrogram are used. 3. As the study of therapeutic management, the effect of Desmopressin, bladder auto-augmentation, incision of external urethral sphincter muscle, subarachnoid block and pudendal never block using phenol was studied. 4. For the study of general management, the effect of bladder training progam, intermittent catheterization and infection control has been studied but there has not been any obvious study in nursing field. Reviewed the study condition, it is necessary to develope bladder training program in order to increase life quality of neurogenic bladder patients.

  • Objectives : Acupuncture therapy is known as a effective method to CVA with paralysis. To make clear about effectiveness of acupuncture affecting to active and static postural adaptation for the patient with hemiparesis, we studied whether acupuncture changed ability of maintenance with one leg standing posture, and character of gait such as gait velocity, cadence, stride length, step length and base of support. Methods : This clinical study has been carried out with 10 cases of CVA patient with hemiparesis. We treated patients with acupuncture for 4 weeks, estimated each paralytic leg and well leg before 1st treatment and after last treatment, and compared the change of one leg with the other. To estimate the change of ability of static postural adaptation, we checked the time of duration with one leg standing posture. And about active postural adaptation, we used temporal distance gait analysis with Ink-Foot-Print method. Results and Conclusions : In static postural adaptation, paralytic leg significantly improved the duration with one leg posture. And In active postural adaptation with gait analysis, paralytic leg showed significant improvement in stride length and step length. Base of support and cadence were also significantly improved.

  • Objectives: Electroacupuncture has the effect of recovering paralytic nerves and muscles. To treat disproportional muscles of expression with electroacupuncture, it is essential that we know the correct point of paralytic muscle. Methods: We investigated 20 cases of patients with facial palsy sequelae. We measured nasolabial angles, checked grade of muscle palsy, and tested ENoG. Results: This study showed significant correlation between nasolabial angles with these muscle groups (zygomatic group I, zygomatic group II, orbicularis oris muscle). Conclusions: Disproportional facesare fixed by muscles of expression observed in facial palsy sequelae. We can treat muscular paralysis of these muscle groups with electroacupuncture for more complete recovery.

  • Objective: This study investigated the effect of pelvic tiltng according to the paralytic side on gait in stroke patients during a 10 m functional movement timed up and go (TUG) test. Method: In this study, gait parameters were measured using a gait analyzer for 20 stroke patients and their gait was analyzed during a 10 m TUG test. For statistical analysis, an independent sample t-test were performed for age, height, and weight among general characteristics of subjects and homogeneity was tested by performing a chi-square test for gender, paralysis side, period of onset, and K-MMSE score. In order to understand the relationship between each variable, Pearson correlation analysis was performed on the variables. Results: First, the right-hand paralyzed group showed correlations in cadence and gait velocity in the up and down tilt of the pelvis, and the left-hand paralyzed group showed correlations in cadence and step length in the anterior and posterior tilt of the pelvis. Second, the tilt of the pelvis was correlated with the Sit to stand, walk forward, walk backwards, turn around at the end point, sit on a chair and the total TUG time in the right hemiplegic group compared to the left hemiplegic group. Conclusion: In this study, a significant correlation was confirmed as a result of gait analysis of right-handed stroke patients divided into a right paraplegic group and a left paraplegic group. In the future, it is suggested that treatment for improving gait of stroke patients should be treated differently for the right and left paralyzed side.

  • The purpose of this study was to investigate the effect of Moxibustion on urinary incontinence, voiding performance, muscle strength of the hip and ADL in stroke patients. The subjects of this study were 20 stroke patients hospitalized at the division of oriental medicine in K university Hospital in the period from December 26, 2001 to March 15, 2002. The research design was one group pretest-posttest design. The pretest and posttest included measuring frequency of incontinence, voiding performance score, healthy and paralytic muscle strength of the hip, and the performance level for ADL after Moxibustion for one week. Data was analyzed using the SPSS package program with Wilcoxon Signed Ranks test. The result of this study were as follows : 1) The frequency of incontinence was decreased from 3.80(SD=3.41) to 2.10(SD=2.10) after Moxibustion, and that was statistically significant(z=-3.803, p=.000). 2) The score of voiding performance was increased from 22.15(SD=12.04) to 30.65(SD=14.73) after Moxibustion, and that was statistically significant(z=-3.828, p=.000). 3) The healthy muscle strength of the hip was increased from 25.97(SD=26.87) to 46.00 (SD=40.75) after Moxibustion, and that was statistically significant(z=-3.183, p=.001). 4) The paralytic muscle strength was increased from 17.42(SD=27.04) to 33.58(SD=37.28) after Moxibustion, and that was statistically significant(z=-2.315, p=.021). 5) The performance level for ADL was increased from 17.15(SD=9.52) to 24.60(SD=13.38) after Moxibustion, and that was statistically significant(z=-3.416, p=.001). According to the results of this study, Moxibustion is effective for decreasing urinary incontinence, and improving voiding performance, muscle strength of the hip and ADL. Consequently, these findings showed that Moxibustion is effective in improvement of urinary incontinence in stroke patients.
