A case of chronic cerebral paragonimiasis westermani

  • We report a chronic cerebral paragonimiasis from a 41-year-old Korean man who complains a headache and weakness of left motor neuron components. Magnetic resonance images of the brain revealed conglomerates of multiple ring-like enhancements in tempore-occipital and frontal lobes of the right hemisphere. An intradermal test for paragonimiasis westermani was positive. The patient was born near an endemic area of paragonimiasis and used to eat boiled or grilled freshwater crayfish in his childhood. Nodules in the brain were resected through craniotomies. The eggs of P. westemani were identified pathologically and parasitologically in the calcified necrotic lesions. Examinations on sputum and fecal specimens for the eggs of P. westemani were shown to be negative and a chest radiograph was normal. It is presumed that the brain lesions were formed by P. westemani approximately 30 years ago.

Updates on parasite infection prevalence in the Joseon period based on parasitological studies of human coprolites isolated from archaeological sites in the cities of Euijeongbu, Gumi, and Wonju

  • Parasite infection rates estimated by examining ancient coprolites can provide insights into parasitism in Joseon society. Using newly discovered Joseon period cases is essential to regularly update the parasite infection rates and reinforce the reliability of our previous estimations. In the present study, we investigated parasite infections in Joseon coprolites newly isolated from the cities of Euijeongbu, Gumi, and Wonju. We then updated the overall parasite infection rates of Joseon period samples (n= 30) as follows: 86.7% (26/30) for Trichuris trichiura, 56.7% (17/30) for Ascaris lumbricoides, 30.0% (9/30) for Clonorchis sinensis, and 30.0% (9/30) for Paragonimus westermani. The parasite infection rates in the Joseon society, estimated through coprolite examination, were very similar to those determined previously despite the addition of new cases to the existing data pool.

Molecular Description of Macroorchis spinulosus (Digenea: Nanophyetidae) Based on ITS1 Sequences

  • We performed a molecular genetic study on the sequences of 18S ribosomal RNA (ITS1 region) gene in 4-day-old adult worms of Macroorchis spinulosus recovered in mice experimentally infected with metacercariae from crayfish in Jeollanam-do Province, Korea. The metacercariae were round, $180{\mu}m$ in average diameter, encysted with 2 layers of thick walls, but the stylet on the oral sucker was not clearly seen. The adult flukes were oval shape, and $760-820{\mu}m$ long and $320-450{\mu}m$ wide, with anterolateral location of 2 large testes. The phylogenetic tree based on ITS1 sequences of 6 M. spinulosus samples showed their distinguished position from other trematode species in GenBank. The most closely resembled group was Paragonimus spp. which also take crayfish or crabs as the second intermediate host. The present study is the first molecular characterization of M. spinulosus and provided a basis for further phylogenetic studies to compare with other trematode fauna in Korea.

  • 폐흡충증은 폐흡충(Paragonimus westermani)의 중간 숙주인 민물 게, 민물 가재 등의 섭취를 통해 인체에 감염되는 기생충 질환으로, 기흉, 흉수, 낭성 종괴 등 다양한 임상 양상을 보인다. 고립 폐 결절로 형태로 발현하는 경우에는, 폐결핵 및 폐암 등과의 구분이 중요하다. 양전자 방출 단층촬영 검사가 감별 진단에 도움이 되지만, 폐결핵 및 폐 히스토플라스마종 등의 진균 감염에서의 위양성이 보고된 바 있다. 저자들은 48세 남자로 단순흉부촬영상 우연히 발견된 고립 폐 결절에 대한 평가 위해 내원한 환자에게서, 흉부 CT 및 FDG-PET에서 좌폐상엽의 폐문주위에 FDG 섭취가 증가된 고립 폐 결절 양상을 보여 폐암으로 의심하였으나, 폐엽 절제술을 시행 후 폐흡충증으로 진단하여 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

Identification of Functionally Different Rat IgE in RBL-2H3 Exocytosis

  • Background: IgE is closely related to the development of allergies. However, the poor relationship between the specific IgE level and the severity of allergic diseases suggests the possibility of functionally different IgE isoforms. With this in mind, rat basophilic leukemia (RBL)-2H3 activation was analyzed with each type of rat IgE for two parameters, exocytosis and IL-4 mRNA production. RBL-2H3 has been well documented in the rat mucosal mast cell line. Methods: RBL-2H3 cells sensitized with each kind of rat IgE was activated by cross-linking FcRI with B5 (monoclonal anti-rat IgE mouse IgG antibodies). The RBL-2H3 exocytosis was measured by analyzing the ${\beta}$-hexosaminidase level, and the level of IL-4 mRNA synthesis was analyzed using semiquantitative RT-PCR. Rat IgE, which was produced by a parasite infection (REP), was prepared using either Paragonimus westermani metacercariae (REP-PW) or Anisakis simplex third stage larvae (REP-AS). A rat IgE prototype of IR162 was prepared by a peritoneal injection of immunocytoma. Results: The level of exocytosis showed a linear relationship with the rat IgE concentration when REP-PW or REP-AS was applied. However, it exhibited a biphasic response with IR162. In addition, the time course of heating at $56^{\circ}C$ illustrated the similarity between REP-PW and REP-AS, which differed from that of IR162. In contrast, the level of IL-4 mRNA synthesis in the RBL-2H3 cells with IR162 was comparable to that of either REP-PW or REP-AS. Conclusion: These results suggest that functionally different rat IgE isoforms exists in RBL-2H3 exocytosis.

종숙주에서 발육중인 폐흡충의 종숙주에 대함 감염력 (Infectivity of Parngonimus westermani developing in a final host to another final host)

  • 폐흡충의 피낭유충이 종숙주에 도입되면 십이지장에서 탈낭하여 장벽을 통과하고. 복강과 흉강을 돌아다닌 후 8주일이면 성충으로 발육한다. 이 연구에서는 종숙주에서 발육중인 폐흡충 발육단계의 어느 시기까지 종숙주에 대한 감염력을 유지하는지를 관찰하였다. 가재에서 분리한 폐흡충 피낭유충을 고양이에 경구감염(경구감염) 시키고 3일 7일, 10일, 14일, 21일 및 28일 후 각 발육단계의 충페론 얻었다. 그리고 각 단계를 고양이에 다시 경구감염시킨 다음 1-12주일 후 고양이를 해부하여 감염 여부를 조사하였다. 종숙주에서 3일간 발육한 충체와 7일간 발육한 충체를 경구감염시킨 고양이에서 각각 투여 충체수의 31.4% 및 22.6%를 검출하였다. 종숙주에서 10일, 14일, 21일, 28일간 성장 발육한 충체를 경구감염시킨 고양이에서는 감염충체를 발견할 수 없었다. 이상의 결과 종숙주에서 최소한 7일간 발육한 폐흡충의 유충은 다시 종숙주를 감염시킬 수 있는 능력을 지니고 있었다.

  • 폐흡충 피낭유충 4주, 7주 및 16주 충체에서 분자량 15, 17, 27, 28 및 53 kDa인 시스테인 단백질 분해효소를 정제하고 각 효소가 IgG를 분해하는 양상을 비교하였고. 탈낭시킨 폐흡충 피낭 유충을 IgG 용액에 배양하여 유충이 분비하는 27 및 28 kDa 단백질분해효소가 IgG을 분해하는 양상을 관찰하였다 1시간 배양하였을 때 IgG heavy chain은 Fab로 분해되었으며 10시간 반응시킨 결과 전체 IgG 분자가 분해되었다. 피낭유충에서 정제한 27 및 28 kDa 효소, 4주, 7주(성장충) 충체 및 16주 성충에서 정제한 15, 17, 27, 28, 53 kDa 효소와 IgG를 반응시킨 결과 모 든 효소가 IgG를 hinge region에서 분해하였다. IgG 분해 양상은 피낭유충에서 가장 강하고, 성충으로 성장하면서 각 효소의 IgG 분해능이 점차 감소하는 양상을 보였다.

Intestinal Parasite Infections among Inhabitants in Yanbian Prefecture, Jilin Province, China

  • To investigate the prevalence of intestinal parasite infections in Yanbian Prefecture, Jilin Province, China, epidemiological surveys were conducted on a collaboration basis between the Korean Centers for Disease Control and Prevention and the Yanbian Center for Disease Control and Prevention. A total of 8,396 (males 3,737 and females 4,659) stool samples were collected from 8 localities and examined with the formalin-ether sedimentation technique, and additionally examined with the cellotape anal swab to detect Enterobius vermicularis eggs. The overall rate of intestinal parasites was 1.57%. The prevalence of Ascaris lumbricoides was the highest (0.80%), followed by Entamoeba spp. (0.23%), heterophyid flukes (0.15%), Clonorchis sinensis (0.08%), Enterobius vermicularis (0.07%), hookworms (0.06%), Trichostrongylus spp. (0.06%), Giardia lamblia (0.04%), Paragonimus spp. (0.02%), Diphyllobothrium spp. (0.02%), Trichuris trichiura (0.02%). The prevalence by sex was similar, 1.58% (n=59) in males and 1.57% (n=73) in females. By the present study, it is partly revealed that the prevalences of intestinal parasite infections are relatively low among the inhabitants of Yanbian Prefecture, Jilin Province, China.

동시 다발성 폐흡충증의 임상적 치험 (Paragonimiasis with Simultaneous Multifocal Lesions)

  • 42세 남자가 객혈을 주소로 본원으로 전원되었다. 흉부 전산 촬영상에서 우상엽 폐첨부에 공동성 병변과 종격동의 종괴가 보였다. 폐첨부의 병변과 종격동 종괴에 대해 쐐기 절제술과 완전 절제술을 시행하였다. 병리학적 판독상 폐흡충증에 의한 염증성 병변이었다. 술 후 복부 전산 촬영상 폐흡충증으로 의심되는 병변이 가로 결장과 하행 결장에 나타났다. 환자는 술 후 2일간 praziquantel을 복용 후 합병증 없이 퇴원하였다. 현재까지 2년 동안 환자는 재발 없이 양호한 상태이다.

  • Parasites of wild animals are closely related with parasites of domestic animals. Wild animals take charge of an important role at parasitic infestation of domestic animals because of unrestrained movement. The authors carried out the work of actual condition of parasitic infestation on wild animals, total 1,014 cases, in the Korean Zoo. The results are summarized as follows: 1. Total rate of parasitic infestation was 36.1% with infestation of 366 among 1,014 cases. The rate of single infestation was 32.6% with infestation of 331 cases, double infestation 3.1% with 31 cases, triple infestation 0.2% with 2 cases and quadrople infestation 0.2% with 2 cases. 2. The parasites on the zoo animals were identified as follows: Lion: Sarcoptiform, Toxocara sp., Toxascaris leonina, Ancylostoma sp. and Isospora spp. Puma: Toxocara sp., Ancylostoma sp. and Isospora sp. Leopard: Toxocara spp., Ancylostoma sp., Trichuris sp., Dibothriocephalus sp. and Physaloptera sp. Wolf: Sarcoptiform and Dibothriocephalus spp. Fox: Trichuris sp., Capillaria aerophila, Spirocerca sp., Paragonimas kellicotti. Jackal: Sarcoptiform, Ascaris sp. and Echinococcus granulosus. Wild Cat: Dibothriocephalus sp. Tiger: Toxascaris leonina. Bear: Sarcoptiform, Metastrongylus apri, Ancylostoma sp. and Ascaris sp. Raccoon and Raccoon dog: Sarcoptiform, Paragonimus kelliotti, and Isospora sp. Boar: Oesophagostomum spp. and Eimeria spp. Mortkey: Sarcoptiform, Trichuris sp., Physaloptera spp.. Among the zoo animals, wild carnivora were infestated with the parasites which are common parasites of dogs and cats, wild herbivora were infestated with the parasites of herbivora domestic animals. and wild fowls were infestated with the parasites of domestic fowls.
