Novel Connective tissue graft technique for Ridge Augmentation in case of Conventional fixed partial denture : Case reports (치조제 결손부 증대를 위한 새로운 결합조직 이식술)

  • There have been a great number of developments in clinical techniques and dental materials in dentistry to date. Looking at these developments, while it could be seen that functional elements such as mastication were prioritized rather than aesthetic aspect in the past, aesthetic needs of patients have steadily increased over time and accordingly the aesthetic has become considered a priority in the development of dentistry. Although the first to be considered in discussing the aesthetic in clinical dentistry will be the white aesthetic that is the tooth part of prosthesis, the pink aesthetic that refers to the harmony of such prosthesis with gingiva can be an important consideration not to be ignored aesthetically. However, the harmony with the gingiva often cannot be obtained only by the beautiful prosthesis, and in particular, the pontic and implant areas have poor conditions to achieve the gingival (pink) aesthetic due to the absorption of alveolar ridge compared to natural teeth. Among the most important elements of the gingival aesthetic are the gingival level and the interproximal papilla height. It is very difficult to make the gingival aesthetic in the case of insufficient alveolar ridge, and the recovery of ridge volume and contour is necessary in order to overcome this condition. To this end, the most widely used method is the "connective tissue graft". Many techniques of the connective tissue graft have already been introduced for the ridge augmentation, and each technique has different purposes, and advantages and disadvantages. Rather, due to the excessive amount of techniques, there is confusion about selecting the right technique at a certain time. However, the goal is clear. Ways to increase the success rates must be found, and at the same time, a more favorable way to the gingival aesthetic is to be chosen. Thus, in this study, considerations for the gingival aesthetic that makes harmony and the techniques to achieve it are discussed.

Immediate implant placement into extraction sites with periapical lesions in the esthetic zone: a case report (치근단 병소를 가진 치아의 발치 후 즉시 임플란트 식립 및 보철을 통한 심미성 회복)

  • Esthetics is important in restoring maxillary anterior area. Alveolar bone resorption and loss of interdental papilla may be minimized by immediate implantation. Previous studies showed successful results with the immediate implantation in healthy extraction socket, while many of these studies objected the immediate implantation into extraction sites with periapical lesions. Recent studies, however, reported successful results of the immediate implantation into extraction sites with periapical lesions with careful debridement of extraction sockets and general medication of antibiotics prior to implantation. A 73-year-old female visited the department of Prosthodontics in ${\bigcirc}{\bigcirc}$ University Dental Hospital with the chief complaint of fallen post-core and crown on left maxillary incisor. Although the incisor was with vertical root fracture and periapical lesion, the immediate implantation following the extraction of tooth was planned. Thorough socket debridement, irrigation with chlorhexidine, and tetracycline soaking were followed by immediate implantation. The general medication of antibiotics (Moxicle Tab.$^{(R)}$, 375 mg) was prescribed before and after the surgery. Immediate provisional restoration was delivered two days after the surgery, and the definitive metal-ceramic restoration was placed about six months later after reproducing the emergence profile from the provisional restoration. This case presents satisfying result esthetically and functionally upto two years after the placement of prosthesis with the harmonious gingival line and no loss of marginal bone.

Study on Changes of Hair and Scalp Characteristics by Keratin Peptides (케라틴 펩타이드에 의한 모발 및 두피 특성 변화 연구)

  • Using Fervidobacterium islandicum AW-1, keratin peptides were produced and confirmed factors related to the scalp and hair. The cytotoxicity and proliferation tests as a function of the concentration of the keratin peptide did not show toxicity and effect on the cellular proliferation in the immortalized human hair dermal papilla cell line. Hair shampoos and hair essences containing keratin peptides were produced, and conducted human patch test. Result showed no skin irritation. The shampoo and the essence were apploed to 2 groups of 30 healthy adults for 4 weeks and showed statistically significant positive results for gloss, hair loss, scalp trouble, and hair roughness by visual assessment. The scalp water content was significantly increased after 2 and 4 weeks compared to before using the shampoo or the essence. Trans-epidermal water loss (TEWL) and the sebum secretion amount in the scalp were significantly decreased after 4 weeks compared to before. The frictional force against combing before and after using the hair shampoo and the essence for normal hair tress and damaged hair tress was significantly changed. The combing force was increased for normal hair tress and decreased for damaged hair tress. In conclusion, we suggest that keratin peptides are appropriated as cosmetic ingredients to be used in hair and scalp related products.

Effect of Long Term Reverse Feeding on the Reproductive and Non-reproductive Tissues in Male Mice

  • Previously, we demonstrated that the shift and/or restriction of feeding time during relatively short-term period (4 weeks) could alter the pituitary gonadotropin expression and the weights of seminal vesicle and prostate in rats. We also found that the reverse feeding (RF) schedule (up to 8 weeks) might induce an adaptable metabolic stress and cause impairment of androgen-dependent reproductive tissues. In the present study, we extended the RF time regimen up to 12 weeks, and measured the reproductive tissue weights. After 4 and 8 weeks of RF, the weights of epididymis were not significantly different. After 12 weeks, however, epididymis weights of RF animals were significantly different (CON 12W : RF 12W = $48.26{\pm}0.62mg$ : $44.05{\pm}1.57mg$, p<0.05). After 4 and 12 weeks of feeding, seminal vesicle weights of RF animals were significantly decreased (CON 4W : RF 4W = $79.36{\pm}8.34mg$ : $46.28{\pm}2.43mg$, p<0.001; CON 12W : RF 12W = $72.04{\pm}3.76mg$ : $46.71{\pm}2.27mg$, p<0.001, respectively). Prostate weights were not changed by RF. Kidney and spleen weights of RF animals were significantly different on weeks 4 and 12 (Kidney, CON 4W : RF 4W = $249.72{\pm}4.20mg$ : $228.41{\pm}3.03mg$, p<0.001; CON 12W : RF 12W = $309.15{\pm}7.49mg$ : $250.72{\pm}6.13mg$, p<0.001, respectively, Spleen, CON 4W : RF 4W = $111.26{\pm}3.76mg$ : $96.88{\pm}4.69mg$, p<0.05; CON 12W : RF 12W = $123.93{\pm}10.72mg$ : $94.68{\pm}5.65mg$, p<0.05, respectively). Histology analysis of seminal vesicle revealed that the thinner epithelial cell layers, reduced complexities of swollen papilla folding in the exocrine glands on weeks 4 and 12 of RF. There was no histological difference between control and RF group on week 8. The present study indicates that up to 12 weeks RF induced differential changes in tissue weights of male mice. In particular, seminal vesicle, kidney and spleen seemed to temporarily adapted to the RF-induced metabolic stress on week 8 of feeding schedule. These results confirmed the our previous study that the RF might induce an adaptable metabolic stress and cause impairment of androgen-dependent reproductive tissues such as epididymis and seminal vesicle as well as non-reproductive tissues such as kidney and spleen. Further studies will be needed to achieve a better understanding of the how does mealtime shift affect the reproductive function and exact nature of adaptation.

Full-mouth rehabilitation with pressed ceramic technique using provisional restorations (Pressed ceramic technique을 이용하여 제작되는 완전 도재관 완전 구강 회복 증례)

  • With the introduction of dental implants, restoration of missing teeth with conventional fixed or removable partial dentures is being replaced with implants. Especially, with young patients, not only longevity but also esthetic factors need to be considered. Implant restorations provide long-term success functionally but, esthetic complications such as, marginal exposure due to gingival recession, loss of the papilla and dark color of metal abutments may occur. Recently, zirconia restorations with CAD/CAM technology provide functional, biocompatible and esthetic restorations possible. All-ceramic restorations using the pressed ceramic technique show better fracture toughness values than those of the conventional porcelain veneering technique. Pressed ceramic technique creates the veneer design in wax and the lost wax technique is used to create the restoration. The final contour of the restoration may be controlled during wax-up. A 22-year old female patient was restored with dental implants and zirconia restorations using the pressed ceramic technique presenting short-term but optimistic prognosis.

Analysis of crown size and morphology, and gingival shape in the maxillary anterior dentition in Korean young adults

  • PURPOSE. The aim of this investigation was to analyze the dimensions of clinical crowns and to classify the crown and the gingival type in the anterior teeth in Korean young adults. MATERIALS AND METHODS. Casts were obtained from 50 subjects ranging in age from 24 to 32. Measurements of length and width were made on the casts using a pair of digital calipers on the entire dentition. Crown thickness and papilla height were also measured and MDW/CL (mesiodistal width to clinical length) and CW/CL (cervical width to clinical length) ratios of the maxillary anterior teeth were calculated. The K-clustering method was used for CW/CL to classify the anterior tooth shape into three groups (tapered, ovoid, and square), and one-way analysis of variance and Duncan's post-hoc comparison were used to evaluate statistical significance between the groups. Pearson's correlation analysis was performed between tooth shape and papillary height (PH) to demonstrate the correlation between tooth shape and gingival morphological characteristics. RESULTS. The average length of the maxillary central incisors was 9.89 mm; the mesio-distal width was 8.54 mm; and the ratio of width/length was 0.86 in Korean young adults. The average bucco-palatal thickness of the central incisor was 3.14 mm at the incisal 1/3 aspect. Ovoid type was the most common tooth shape (48%), followed by square type (29%) and taper type (23%) in the central incisors of Korean young adults. Tooth shape and gingival type were correlated with each other. CONCLUSION. New reference data were established for tooth size in Korean young adults and the data show several patterns of tooth shape and gingival type. Clinicians should diagnose and treat based on these characteristics for better results in the Korean population.

Long-term Management of a Gingival Fibromatosis Patient with the Primary Dentition (유치열기에서 나타난 치은섬유종증 환자의 장기간 관리)

  • Gingival fibromatosis is a rare oral condition that is characterized by proliferative fibrous overgrowth of the attached gingiva, the marginal gingiva, and the interdental papilla, typically presenting in the growth period. A case of a 27-month-old girl with a generalized severe gingival overgrowth is described herein. The patient had no known systemic disease, but enlarged gingival tissue had gradually covered her teeth. The excess gingival tissue was removed by conventional gingivectomy, which involved extraction of the retentive primary teeth under general anesthesia when she was 5 years old. Post surgical follow-up at 18 months after the surgery demonstrated no recurrence. Resectional surgery of the enlarged gingival tissue is the treatment choice for gingival fibromatosis, although there is a high risk of recurrence. More frequent professional follow-ups and oral hygiene instruction might be required. A delay in the surgical treatment may have significant consequences for the patient, such as primary dentition retention and consequent delay in the eruption of the permanent teeth, difficulties in mastication and phonation, malpositioning of the teeth, and psychological problems. Early surgical treatment should be performed according to the severity of enlargement.

The first discovery of larval Gnathostoma hispidum (Nematoda: Gnathostomidae) from a snake host, Agkistrodon breuicaudn (한국산 살모사에서 최초로 발견한 돼지악구충의 제3기 유충)

  • The present study was performed to observe the infection status of several kind of animals with indigenous Gnnthostomn in Korea, and morphological characteristics of gnathostome larvae detected from pit-viper, Askistronon breuiccudus, for the species bullfrogs. several kinds of snakes, j.e., 55 Elnphe rufodorsata, 2 Dinodon rufozonatum rufzonatum. 62 Rhnbdophis tisrinus tigrinus and 87 Askistronon app., and 438 cats were examined. A total of 21 larval gnathostomes was detected from 12 pit-vipers, A. breuicnudus. They were 2.233 x 0.343 mm in average size and covered with about 210 transverse rows of minute cuticular spines. Their characteristic head bulbs were provided with 4 rows of hooklets of which average numbers in each row were 36.8, 39.0, 41.7 and 44.3. posteriorly. In the cross sections of midgut level, the intestinal wall consisted of a single layer of 19-25 elongate epithelial cells with a single nucleus. SEM observation of the larvae revealed unique features of head bulb , cuticular spines on transverse striations and a cervical papilla. On the basis of above morphological characteristics, they were identified as the advanced third-stage larvae of Gnothostomo hispinum. It was first confirmed that the pit-viper. ABkistronon breuicaunus is the snake intermediate host of G. hispidun.

Experimental Studies on the Hair Growth Activity of Drynariae Rhizoma Extracts in C57BL/6N Mice (C57BL/6N 생쥐에서 골쇄보(骨碎補) 추출물을 이용한 발모(發毛)효과에 대한 실험적 연구)

  • Drynariae Rhizoma has been used for promotes mending of the sinews and bone, tonifies the kidney for such symptoms as weak low back and knees, and stimulates the growth of hair as a tinctute for alopecia in oriental medicine. This experiment examined the effect of an acetone extracts of Drynariae Rhizomas(GSB-1), its EtoAc fraction(GB-2) and n-buOH fraction(GSB-3), on hair growth activity of the C57BL/6L mice after topical application to skin. First, We examined on hair growth activity of extracts of Drynariae Rhizomas compare to control and 1 % minoxidil groups. Second, We investigated on the number of hair follicle and mast cells after topical application of extracts of the Drynariae Rhizomas to skin for 16 day. Third, We investigated immunoreactive density of vascular endothelial growth factor(VEGF), protein kinase C-${\alpha}$(PKC-${\alpha}$) and stem(mast) cell factor(SCF) in skin of C57BL/6N mice by immunohistochemical methods. The results were as follows : Hair growth effect of acetone extracts of Drynariae Rhizomas, its EtoAc fraction and n-BuOH fraction was observed in 98 %, 96 % and 60 % in hair removed skin area in 16 day respectively, Immunoreactive density of VEGF in skin of GSB-1 group was weakly stained compare to control group in 10 day, But GSB-2 and GSB-3 groups were mildy stained in bulge and root sheath of skin. Immunolocalization of SCF antigens was observed weakly stained density in epidermis, bulge, stem cells and dermal papilla of control gruop. but in experimental group, immunoreactivity of SCF antigens was observed mildly stained density in bulge, epidermis and root sheath of GSB-1 gruop, heavily stained density in epidermis, bulge and root sheath of GSB-2 and GSB-3 groups to the hair removal skin of C57BL/6N mice on day 10. These experiment suggest that acetone extracts of Drynariae Rhizomas and its EtoAc fraction may be used for topical treatment of alopecia areata.

Scanning electron microscopic observations of Thezazia callipaeda from human (인체 기생 Thelatria cazlipaeda의 주사전자현미경적 관찰)

  • Four females and a male nematode isolated from 2 patients who visited eye clinics In Seoul were identified as Thelazia callipaeda and their ultrastructures were observed by scanning electron microscopy(SEM). General features of the worms were slender and attenuated at both ends. Vaginal opening was located at 0.27 mm from the anterior end, and in front of the esophagointestinal junction. In the body cuticle transverse striations varied characteristically through the body. The number of cuticular transverse striations was 400∼650/mm at head portion, 250/mm at middle portion and 300∼350/mm at tail portion. The SEM observation of the mouth part of the females showed 6 cord-like cuticular thickenings in hexagonal arrangement and an amphid was observed. A lateral line, a vaginal opening, a pair of phasmids, and an anus were identified in the body portion. A pair of papillae and 6 cord-like cuticular thickenings were on the mouth part of the male. It was difficult to observe structures at the tail of the male except wrinkle-like structures. Most of the larvae isolated from the uterus of a female worm were sheathed and thus cuticular striations were not Eren. Others were unsheathed and revealed cuticular striations. The oval membrane which encysted sheathed larvae was also observed. These are the 18th and 19th record of human thelaziasis in Korea as the literature are concerned.
