급성 괴사성 췌장염에서 발생한 결장 합병증 -1 예 보고- (Colonic Complication of Acute Necrotizing Pancreatitis - a Case Report -)

  • We present a case of a colonic involvement associated with necrotizing pancreatitis, with a review of the literature. A 10 year old boy had an appendectomy at the local clinic ten days ago. On admission, he complained nausea, vomiting and severe constipation. His abdomen was distended and he had tenderness on the left abdomen. Laboratory and radiologic studies revealed findings consistent with acute pancreatitis with colonic complication. He was treated conservatively for 30 days but did not improve. On hospital 30th day, abdominal pain developed and his vital sign changed. Abdominal CT suggested ischemic change of the transverse colon. At laparotomy, the left colon showed stenosis. The greatly distended transverse colon was resected and a transverse end colostomy was done. He was discharged at postoperative 45th day with improvement and colostomy closure was performed 8 months later.

췌장염에 합병된 위장관 출혈의 혈관내 치료에 대한 증례 보고: Hemosuccus Pancreaticus와 췌장대장루 (Case Reports of Intravascular Treatment for Gastrointestinal Bleeding Associated with Pancreatitis: Hemosuccus Pancreaticus and Pancreaticocolic Fistula)

  • 고령의 만성 음주력이 있는 환자들이 지속되는 흑색변, 복통과 빈혈을 주소로 내원하였다. 입원하여 시행한 복부 전산화단층촬영상 출혈을 동반한 췌장 낭성 병변이 보였다. 저자들은 카테터 경유 혈관조영술로 명확한 활동성 출혈 소견을 확인하였으며, 동맥색전술을 시행하였다. 시술 이후 출혈 증상은 호전되었다. 저자들은 위장관 출혈의 드문 원인인 췌장염에 합병된 hemosuccus pancreaticus와 췌장대장루를 혈관중재시술을 통해 치료한 2예를 보고하고자 한다.

정전가미이진탕(正傳加味二陳湯)이 Dibutyltin Dichloride(DBTC)로 유발된 흰쥐의 췌장염에 미치는 영향 (The effects of Jengjengamiyjin-tang on Pancreatitis induced by DBTC in Mouse)

  • Objectives : We examined the effect of Jengjengamiyjin-tang, a traditional herbal formula, on the experimental pancreatitis induced by Dibutyltin Dichloride(DBTC). Methods : 15 mice were divided into 3 groups; the normal group and the control group which were administered water only after pancreatitis elicitation and the test group which were administered Jengjengamiyjin-tang for 7 days after pancreatitis elicitation. Results : In the activities of amylase, lipase and alkaline phosphatase in serum, the test group showed significant decrease compared with the control group after 21 days. In the common morphology and histochemical study, various injuries by hemorrhagic erosion were observed in the control group, while they were significantly decreased in the test group. In the immunohistochemical study, the manifestation of COX-1, 2, HSP27, HSP70, MAC387, Ki-67 decreased significantly in the 14th day in the test group, however, they increased in the 21st day more than the control group(p<0.05). Conclusion : According to the above results, it is supposed that Jengjengamiyjin-tang has anti-inflammatory effect on the experimentally induced pancreatitis and may be applicable to pancreatitis especially in the acute stage.

소아 가성비장동맥 파열의 동맥색전술 치험 1예 (Arterial Embolization for the Ruptured Splenic Artery Pseudoaneurysm in a Child)

  • Pseudoaneurysm of the splenic artery may arise from a vascular erosion by a surrounding inflammatory processes in acute and chronic pancreatitis. Rupture of the pseudoaneurysm may threaten the patient's life. Conservative management for massive hemorrhage may cause 100 percent mortality and even with prompt therapy there is a high mortality. Preoperative detection of bleeding source is desirable because of the difficult identification of the bleeding site at laparotomy. Angiographic identification and embolization of the hemorrhagic vessels in selected cases may obviate the risk of urgent surgery. The authors have recently managed a case of ruptured splenic artery pseudoaneurysm combined with a pancreatic pseudocyst in a 6 years old boy. A bolus enhanced CT scan and angiography confirmed the diagnosis. We managed this child successfully with the urgent transcatheter arterial embolization followed by elective surgery.

Gastric Pneumatosis and Its Gastrofibroscopic Findings in Life-Threatening Superior Mesenteric Artery Syndrome Complicated by Anorexia Nervosa in a Child

  • A 14-year-old girl was admitted to the emergency department for excessive bile-containing vomiting and severe abdominal pain. She had been healthy until she intentionally lost 25 kg over a 6-month period. Thick, bloody bile-mixed food particles were drained from the stomach through a nasogastric tube. Abdominal computed tomography revealed huge stomach dilatation with extensive gastric pneumatosis, possible near rupture, acute pancreatitis, and a very narrow third of the duodenum, indicating superior mesenteric syndrome. Gastrofibroscopy revealed multiple hemorrhagic ulcers and numerous beadlike cystic lesions in the stomach. Laboratory examination results were notable for severe deficiencies in critical nutrients, including iron, zinc, proteins, and prealbumin, as well as undernutrition-associated endocrine complications such as hypothyroidism and hypogonadotropic hypogonadism. Excessive vomiting ceased after the endoscopic removal of stagnant gastric contents. Gastric pneumatosis improved after 3 days of supportive care.
