The evolving classifications and epidemiological challenges surrounding chronic migraine and medication overuse headache: a review

  • Changes in diagnostic criteria, for example, the various International Classification of Headache Disorders criteria, would lead to changes in the outcomes of epidemiological studies. International Classification of Headache Disorders-1 was based mainly on expert opinion, yet most of the diagnostic criteria were reliable and valid, but it did not include chronic migraine. In its second version, the classification introduced chronic migraine, but this diagnosis resembled more a high-frequency migraine rather than the actual migraine transformation process. It also introduced medication overuse headache, but it necessitated analgesic withdrawal and subsequent headache improvement to be diagnosed as such. Hence patients having medication overuse headache could only be diagnosed in retrospect, which was an awkward situation. Such restrictive criteria for chronic migraine and medication overuse headache omitted a high proportion of patients. International Classification of Headache Disorders-3 allows a diagnosis of medication overuse headache due to combination analgesics if taken for at least 10 days per month for more than three months. Hence the prevalence rate of medication overuse headache and chronic migraine can increase compared to the previous version of the headache classification. Different criteria have been used across studies to identify chronic migraine and medication overuse headache, and therefore the information acquired from previous studies using earlier criteria becomes uncertain. Hence much epidemiological research would need to be interpreted cautiously or repeated with the most updated criteria, since the subjects in studies that apply the latest criteria may be phenotypically different from those in older studies.

The clinical manifestation of migraine and correlation study with autonomic bioelectric response (편두통 환자의 임상 양상 및 생체전기 자율반응과의 상관성 고찰)

  • Objective : We had a clinical report in headache but didn't in migraine. We have planned this study in order to get the basic data of migraine in oriental medicine. Methods : The patient of 36 in migraine checked sex, age, onset, family history, severity of pain, influences of life, induced cause, clinical pain characteristics, associated symptom, treatment style, and prescription, frequency, using period of analgesics by a questionnaire and differentiated syndromes in migraine and evaluated autonomic bioelectric response recorder(ABR-2000). Results : There are 23.4% in prevalence rate of migraine. The ratio of sex is M:F=1:17. The age of an attack is the highest in thirties. The patient are the most in forties. The mean duration of illness is $12.0{\pm}9.9$ years. 83.4% had a family history. 61.1% had a moderate grade in severity of pain. 77.8% selected fatigue in induced cause of migraine. 69.4% had tingling sense, nausea and vomiting in the associated symptoms. 91.7% used analgesics for treatment and 51.5% of them used analgesics voluntarily. 61.9% of them take analgesics less than once in a week. 33.6% had the phlegm syncope headache in differentiation of syndrome. In ABR-2000 results, item of graph showed low tendency mostly. Conclusions : We expected that this report of clinical progress, differentiation of syndromes and ABR-2000 results in migraine would be used basic data by oriental medicine to treat migraine.

A Study on the Relationship between Subjective symptoms and Psychological Symptoms on some VDT workers (일부 VDT 사용 근로자의 자각증상과 심리증상과의 관련성 연구)

  • The study subjects in this study are 295 workers who work on one electronics industry, and they were composed of 93 VDT and 202 non-VDT workers. This study was carried out to obtain the prevalence of subjective symptoms and to compare psychological symptom scores from SCL-90-R between VDT and non-VDT work groups. And to verify the relationship between VDT work related symptoms and psychological symptoms, the author compared SCL-90-R T scores between symptom complaining and non-complaining groups. The results are summarized as follows : 1. Throughout the prevalence study of VDT work related symptoms according to VDT work state, the author could select the final 10 symptoms by sex;'eye fatigue', 'foggy vision', 'neck discomfort or pain', 'shoulder discomfort and pain', 'light headedness', 'dizziness', 'acne', 'itching sensation on face', 'difficulty in concentration', 'memory disturbance' in males, and 'blurred vision', 'foggy vision', 'neck discomfort or pain', 'shoulder discomfort and pain', 'abdominal distention', 'indigestion', 'acne', 'prickling sensation on face', 'difficulty in concentration', 'memory disturbance' in females. 2. Throughout comparison of psychological symptom scores from SCL-90-R between VDT and non-VDT workers, except 'Interpersonal sensitivity' in female, Mean T score were higher in VDT workers, but not significantly different between the two groups. These results indicate that the relation between VDT work status and psychological symptoms are not strongly correlated. 3. Because of the nonspecific characteristics of VDT work related symptoms, from the 40 subjective symptoms, the author selected the final 10 symptoms by sex, especially correlated to VDT, and divided the study subjects into symptom complaining and non-complaining groups. The mean SCL-90-R T scores between the two groups were compared. Except for the opthalmologic symptoms in females, the scores were significantly different between the two groups in all symptoms. And this indicates that the VDT work related symptoms strongly correlated with the present individual psychological conditions. 4. Due to understand that which of the 9 psychological symptom dimensions are strongly correlated with the subjective symptoms, the author excuted muliple logistic regression analysis, and the result were as follows; 'depression' and 'somatization', in male, and 'obsessive-compulsive' and 'somatization' in female.

An Epidemiologic Study of Symptoms of Temporomandibular Disorders in Korean College Students (경기도 지역 대학생의 측두하악장애증상에 관한 역학적 연구)

  • An epidemiologic investigation was carried out to determine the prevalence of symptoms of temporomandibular disorders in college students that aged 19-31 years. 460 students were investigated with a questionnaire from September to December 2006. The obtained results were as follows : 1. The prevalence of symptoms of temporomandibular disorders was 80.6%. 2. The most frequently complained symptom was headache and joint sound was the next one without distinct difference between men and women. 3. While the rate of occurrence of symptom of acute malocclusion decreased with age in men, that of TMJ pain during chewing or speech increased with age in women. 4. Symptoms including TMJ pain during mouth opening, chewing or speech, TMJ fatigue and acute malocclusion occurred significantly more frequently in women than in men. Contributing factors including resting cheeks on hands, stressful state, gum chewing, insomnia and clenching occurred significantly more frequently in women than in men. 5. There was a highly significant relationship between symptoms and contributing factors including resting cheeks on hands, stressful state, unilateral chewing, insomnia and clenching. 6. There was a highly significant relationship between symptoms and general personality.

The Relationship between Allergic Diseases and Functional Gastrointestinal Disorders in Children and Adolescents (소아·청소년의 알레르기 질환 (천식, 알레르기비염, 아토피피부염)과 기능성 위장관 질환과의 관계 (단면 조사 연구))

  • Objectives The purpose of this study is to establish relationship between allergic diseases and functional gastrointestinal disorders in children and adolescents using objective criteria and questionnaires. Methods This study surveyed 237 children and adolescents who visited the department of Pediatrics of ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from September 23rd, 2013 to December 26th, 2013. The Korean version of International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire and the Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) were used. We analyzed the data by using PASW Statistics 18.0 with Chi-square test, Fisher's exact test and Linear by linear association. Results There was no significant difference between prevalence of allergic diseases and functional gastrointestinal disorders. Also, there was no strong relationship between the number of allergic diseases and the functional gastrointestinal disorders. However, some parts showed significant relationships - such as asthma symptoms ever and belch; asthma symptoms last 12 months and belch; allergic rhinitis diagnosis ever and belly aches and abdominal pain around and below the belly button; allergic rhinitis treatment last 12 months and belly aches and abdominal pain around and below the belly button; and atopic dermatitis diagnosis ever and irritable bowel syndrome (respectively; p=0.046, p=0.008, p=0.004, p=0.029, p=0.035). And as the number of allergic diseases increases, the prevalence of functional gastrointestinal disorders are 19.4%, 31.3%, 41.7%, 31.0% respectively. Conclusions Although there was no significant relationship between allergic diseases and functional gastrointestinal disorders, some gastrointestinal symptoms were related to allergic diseases.

Sleep Disturbances and Personality Type Test

  • Purpose: The purpose of this study was to assess the association between sleep disturbances and personality type. Methods: Five hundred twenty-four college students in Gyeonggi-do completed the Myers-Briggs Type Indicator (MBTI) and a questionnaire and collected data were analyzed by SAS 9.4 program. Results: Extroversion (E) type, sensation (S) type, and perceiving (P) type had significantly higher prevalence of insomnia than introversion (I) type (p<0.05), intuition (N) type (p<0.05), and judging (J) type (p<0.001), respectively. Tooth grinding, snoring and insomnia appeared to occur more frequently in feeling (F) type than in thinking (T) type. Tooth clenching, tooth grinding and snoring seemed to occur more frequently in S type than in N type. Insomnia occurred significantly the most frequently in sensation-feeling (SF) type (p<0.05). Tooth grinding and snoring seemed to occur the most frequently in SF type. A significantly increased percentage of sensation-perceiving (SP) type demonstrated insomnia (p<0.001). Tooth clenching, tooth grinding and snoring seemed to occur the most frequently in sensation-judging (SJ) type. Sensitive or nervous type of personality had significantly higher prevalence of insomnia than relaxed or general type of personality (p<0.01). A significantly increased percentage of subjects with bad general health status showed insomnia (p<0.0001). Tooth clenching and snoring seemed to occur the most frequently in subjects with bad general health status. A significantly decreased percentage of normal weight subjects demonstrated tooth grinding (p<0.05). Snoring occurred significantly the most frequently in overweight subjects (p<0.001). Tooth clenching showed significant correlation with stress (p<0.01) and personality (p<0.05). Snoring showed significant correlation with stress (p<0.05) and body weight (p<0.001). Insomnia showed significant correlation with stress (p<0.0001), personality (p<0.01), and general health status (p<0.0001). Conclusions: Sleep disturbances including tooth clenching and insomnia were associated with personality type and it is desirable to manage them considering personality type.

The Relationship between Temporomandibular Disorders(TMD) and Occlusion (측두하악장애와 교합요인의 관계)

  • Temporomandibular disorders have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory nuscles, and associated structures. There have been many different contributing factors of TMDs which were traumatic, occlusal, pathophysiological and psychosocial. Among there factors, the effect of occlusion on TMDs have been a controversy for a long time. The purpose of this study was to investigate the effect of occlusal factors and oral habits on TMDs. In this study, 140 subjects with signs and symptoms of TMDs and diagnosed of TMD in the Orofacial Pain clinic of Yonsei University Dental Hospital though March to July 2004 were selected for the TMDs group and 50 subjects without any signs and symptoms of TMDs as the control group. The subjects were evaluated clinically in TMDs' Occlusal and Prosthodontic Restoration examinations. TMDs' examination was composed of the TMJ pain, sound, locking, temporal or masseter muscle palpation, mandibular movement, oral habits and headache. Occlusal examination was made of overjet, overbite, lost teeth number, nonfunctional interference, midline shift, then pattern of lateral movement and attrition. prosthodontic restoration examination had the existence of restoration, placement, then number of crown or bridge and Satisfiable index which estimated the quality of occlusal state of prosthodontic restorations. Following results were obtained : 1. The prevalence of TMDs was higher in their 20s & 30s, female of the TMD patients group. 2. The clenching frequency in the TMDs group(40.71%) was higher than those in the control group(18.00%), and there was a significant statistical difference(p<0.05). 3. The frequency of Nonfunctional interference in the TMDs group(10.00%) was higher than those in then Control group(2.00%), and there was a significant statistical difference(p<0.05). The result of this study indicated TMDs prevalence was higher in their 20s, 30s, female group of TMDs patients similar to the previous studies. Clenching and nonfunctional interference were estimated as the contributing factors of TMDs.

Prevalence of Work-related Musculoskeletal Disorders in Auto-mission Assembly Plant Workers

  • Objective: This study was performed to investigate the prevalence of musculoskeletal disorders in auto workers of a mission assembly plants. Background: Most studies of musculoskeletal disorders have used car assembly line workers as their participants. However, little research has been done on musculoskeletal disorders afflictions of mission assembly line workers. Method: Through a focus group interview with an ergonomist and a manager at a mission assembly plant site, a questionnaire was developed for musculoskeletal disorders. The questionnaire consisted of five parts, demographic factors, musculoskeletal disorder symptoms, and musculoskeletal disorder experiences; 137 workers participated in this survey. The surveys were analyzed by correlation and Chi-squared analysis. Results: Musculoskeletal disorder symptoms and serious pain were reported in the neck, shoulder, back, and fingers. These problems were statistically related to various demographic factors, such as age, stature, stretching, and work satisfaction. Conclusion: Treatment of musculoskeletal disorders should consider the workers' traits. If there is no specific cause of the pain, developing stretches and exercises before and after work should be applied to prevent musculoskeletal disorders. Application: The results of this study can be used to develop guidelines to prevent injury in auto workers at mission assembly plants.

The farmers consciousness of Health status and food habits in Korea (농업인의 식습관과 건강상태의 관련성 연구)

  • The purpose of this study was to compare the consciousness health status and food habit for farmers in Korea. Eight hundred persons were surveyed by rural leader of RDA, composed with 35.1% of male and 64.9% of female. Data collection includes height weight and blood pressure by measuring method and personal & family situation by interview for influential factor of health status and food habit. The questionnaire was included dietary habit of salt, sugar, dietary fiber and fat intake and other behavior questions related with diet. The subjects were composed 32.5% of elementary school graduated and 50% of middle size farming. Of the all subjects, 54% of were rice planted farming mainly and 19 were over 65 year-aged group. In the concept of disease prevalence. gender difference was exist, liver and heart complains were more frequency in male and waist pain and urinary complain in female, shoulder pain and nervousness were frequently prevalent to all group of farmers. The hypertension prevalence of farmers were 16% by measuring blood pressure, but consciousness of hypertension was very low (5%). The correlation of health status with personal factors were analysed, body weight, number of family, education level, and working level were significant with CMI score and Farmers' syndrome score. The food habit status was negatively correlated with weight and BMI, age was negatively correlated with CMI score and farmers' syndrome. The consciousness of health status and food habit score were significantly correlated with smoking and alcohol drinking. It concludes that good food habit contribute wellness of health. Nutrition education for better food habit would be helpful on health status.

Survey on Period Prevalence Rate and Therapeutic Practice For Low Back Pain in Adult Population of Rural Area (농촌지역 성인의 요통 유병률과 치료방법 조사)

  • To investigate the period prevalence rate and therapeutic practic for low back pain (LBP) in the adult population of rural area, a personal interview was conducted for 2.024 persons or 20-59 years old in Seohu Myon, Andong County, Kyungpook Province between 1st and 20th of April, 1991. The period prevalence rate (l February 1990-31 January 1991) of LBP for 1,106 adults who were interviewed was $47.9\%$. The age adjusted period prevalence rate for males was $43.7\%$ and that for females was $52.3\%$ and the difference was statistically significant (p<0.005). Clinical course of th LBP was acute in $14.1\%$ of males and $9.0\%$ of females, recurrent in $57.0\%$ and $55.2\%$, and chronic in $28.9\%$ and $35.8\%$, respectively. Common causes of the LBP were insidious on set with aging without known cause$(48.1\%)$, heavy work $(15.1\%)$, and trauma $(11.3\%)$. Due to LBP $12.5\%$ of the patients were not able to stand or walk for more than an hour and $2.5\%$ were bed-ridden or unable to carry out daily routine. To have the LBP diagnosed $10.2\%$ of the patients utilized a oriental medical clinic or hospital, $31.3\%$ visited a clinic or hospital, and $56.6\%$ hat not utilized any medical facility. Main reason for not having the LBP diagnosed was that the LBP was tolerable. The most popular therapeutic method that the LBP patients chose at the first was drug and physical therapy. Herb medicine was most commonly used when the first therapeutic method was not effective and the acupuncture was the most popular choice of therapy when the second therapeutic method failed. Folk medicine was utilized in $15.5\%$ of the LBP patients and it included 36 regimens such as tincture of motherwort (Leonurus sibiricus), boiled chicken with liquor, etc. It was revealed by this survey that the LBP is a serious health problem in the rural area and many of the LBP patients do not utilize a clinic or hospital but take non-scientific folk remedy. To prevent the economic waste and side effects of the folk remedy, public health education is needed for tile rational therapy of LBP.
