요통과 비만과의 상관성에 관한 연구 (The Study on Relation of Low back pain and Obesity)

  • Objective: The purpose of this study is to examine the relation between the Obesity and the prevalence of LBP. Methods: From February 2001 to April 2001 we studied body composition, Body Mass Index, Basal Metabolic Rate, Obesity Degree, Waist Hip Ratio, percent body fat and muscular form of 40 patients with LBP and 40 patients with Internal disease, who visited Dong-Guk University Bundang Hospital by using Inbody 3.0. Results: 1. Sexual ratio in this study is 1:4(male:female). 2. Fluid, protein, mineral mass and BMR in patients with LBP are lower than those in patients with Internal disease, but percent body, WHR and obesity degree with LBP are higher than those in patients with Internal disease. 3. LBP prevalence is shown to rise with increasing WHR. 4. LBP prevalence is shown to rise with decreasing BMR. Conclusions: Patients with LBP are obeser than patients with Internal disease. LBP prevalence is shown to rise with increasing WHR in statistics. But LBP prevalence is shown to rise with decreasing BMR in statistics.

Modic Degenerative Marrow Changes in the Thoracic Spine : A Single Center Experience

  • Objective : The purposes of this study were to evaluate the prevalence, types, and locations of Modic changes (MCs) in the thoracic spine in a large number of subjects, and to investigate the relation between the distributions of MCs and disc herniations (DHs) in the thoracic spine. Methods : Two experienced musculoskeletal radiologists assessed the presence of MCs and DHs by consensus in the thoracic MRIs of 144 patients with non-specific back pain. Patient ages ranged from 22 to 88 years (mean=$53.3{\pm}14.66$ years), and 72 were female (50%). The prevalence, distribution, relation of MCs and DHs was recorded. Results : MC was observed in 8 of the 144 patients (5.6%) and 10 of 1728 segments (0.58%). The most common MC was type II. Of the 8 patients exhibiting MC, 6 had type II (75.0%), and 2 had mixed MCs (type I/II or type II/III). MCs were distributed mainly at the mid-thoracic level (from T5/6 to T9/10). DH was detected in 18 patients (12.5%), 36 of 1728 segments (2.1%). Of the 10 segments exhibiting MC, 5 had DHs at the same level (50.0%). Accordingly, DH was strongly associated with MC (p=0.000). Conclusion : A low prevalence of MC was observed in the thoracic spine, and type II MC predominated. The low prevalence of MC in the thoracic spine suggests that it was caused by a relative lack of mobility as compared with the cervical and lumbar spines. And DHs were found to be strongly associated with MCs even in the thoracic spine.

감염 근관의 흑색세균의 동정 (PREVALENCE OF BLACK-PIGMENTED BACTERIA IN INFECTED ROOT CANALS IN KOREA)

  • The role of bacteria in root canals and periapical infections is well known and established. In these bacteria, black-pigmented bacteria(BPH) play important role in endodontic infection. BPB are Gram negative anaerobic rods which are closely related 50 clinical symptoms such as pain, percussion, tenderness, foul odor, etc. In America and Europe, many studies on BPB have been done and are continued. But, relatively few studies have been done in Korea, especially its prevalence in Korean population is not yet studied. The purpose of this study is to establish prevalence of BPB in infected root canals and periapical abscesses in Korean people. Microbial samples were collected from the root canals of 34 intact tooth with periapical rarefactions of endodontic origin and 3 periapical abscesses. All samples were incubated in an anaerobic chamber(Coy, Model No. 77. Ann Arbor, Michigan, USA.). Identification of In microorganism was based on its growth in the anaerobic chamber, colonial pigmentation, colonial morphology, Gram stain, and Rapid ID32A(BioMericux SA/69280 Marcy-l'Etoile/France) results. In addition, the polyme ase chain reaction using specific primers for 16S rRNA genes were used differentiate Prevotella nigrescens for Prevotella intermedia. The results were as follows : 1. In this study, thirteen (35%) of thirty seven samples were positive for the growth of BPB. In thirteen samples, sixteen strains of BPR were found. 2. The most frequently identified BPB in root canals and abscesses in Korean were P. nigrescens 5/37(14%) and P. intermedia 5/37(14%). Porphyromonas gingivalis 3/37(8%), Porphyromonas endodontalis 2/37(5%) and Prevotella loecheii 1/37(3%) were also found. 3. In this study, no significant differences were found between the prevalence of BPB in Korean and that of American and European.

노인 치매 환자의 통증 역치 및 통증 경험의 특성 (Characteristics of Pain Threshold and Pain Experience in Elderly Patients with Dementia)

  • 연구목적 본 연구는 치매 군과 비치매 군의 통증 역치 및 통증 경험의 특성을 비교 하고자 한다. 방 법 2010년 강원 조기치매 검진사업의 일환으로 시행된 것으로, 연구 대상은 지역사회 거주중인 65세 이상 노인 8,302명이었다. 이 중, 한국판 간이정신상태검사(MMSE-KC) 점수가 저하 된 1259명을 선별했고, 이중 365명이 한국형 치매 진단평가 도구(CERAD-K)를 이용한 정밀검진을 받았다. 검사 결과 및 전문의의 판단 하에 정상, 경도인지장애, 치매 군으로 진단 후, 최종 비치매 군 90명, 치매 군 57명이(경도-중등도 알츠하이머형 치매) 분석되었다. 압통각계(Pressure algometer)를 이용하여 실험적으로 통증 역치를 측정했고, 자가보고식 검사인 Brief pain inventory(BPI)를 이용하여 통증 경험(통증 심도, 통증 방해도)을 조사했다. 집단 간 차이 분석은 Pearson Chi-Square와, 혼란변인(성별, 나이, 교육수준, GDS, 당뇨)을 보정한 ANCOVA검정을 시행했다. 결 과 인구학적 특성상 치매 환자 군에서 여성이 많고, 연령이 높고, 교육 연수가 낮았다. 통증 역치는 두 군에서 통계적으로 의미 있는 차이가 발견되지 않았다. BPI결과에서 비치매 군이 어깨 통증, 전체 통증 개수, '통상적 일'의 통증으로 인한 방해를 의미 있게 더 많이 호소했다(p<0.05). 그러나 두 군에서 통증 유병률 및 치료는 차이가 없었다. 결 론 치매 군은 비치매 군보다 통증을 더 적게 경험했다. 이는 치매 환자의 통증 역치는 보존되었으나, 내성이 증가한다는 선행 연구를 뒷받침 하는 증거로 여겨진다. 따라서, 치매 환자를 위한 적극적인 통증 사정 및 치료를 제안한다.

Osteoarthritic changes and condylar positioning of the temporomandibular joint in Korean children and adolescents

  • Purpose: To investigate the prevalence of osteoarthritic changes and condylar positioning of the temporomandibular joint (TMJ) in Korean children and adolescents with or without temporomandibular disorders (TMDs). Materials and Methods: A total of 101 asymptomatic and 181 symptomatic children and adolescents aged 10 to 18 years old were included in the study. Osteoarthritic changes such as flattening, sclerosis, osteophytes, or erosion, and the parasagittal positioning of the condyle were assessed using cone-beam computed tomography (CBCT) images. Results: The overall prevalence of osteoarthritic changes was higher in symptomatic (26.8%) than in asymptomatic adolescents (9.9%) (p<0.05). In the symptomatic group, the frequency was higher in males (33.3%) than in females (23.0%) (p<0.05). Erosion was the most common change for the symptomatic group (15.6%), whereas sclerosis was the most common change for the asymptomatic group (5.4%). Posterior condylar position was more frequently observed in the symptomatic group (p<0.05). Erosion was more common in the samples with TMJ pain or mouth opening limitations as compared to those without them (p<0.05). Conclusion: This study showed that osteoarthritic changes in TMJ were common in children and adolescents, with a much higher prevalence in symptomatic patients.

일부 생산직 여성근로자의 월경곤란증에 관한 연구 (A study on dysmenorrhea of women labor workers)

  • This study was conducted to investigate the prevalence of dysmenorrhea in women labor workers The subjects were 327 women labor workers in the area of Kyunggi. Kyungsang and Chunla Provinclils. The data were collected from 20. January to 20. February. 1999 by using self-administered structured questionnaires. The contents of, questionnaire were composed of demographic(3 items). work related(2 items). life style (6 items), obstetric-menstrual(8 items) characteristics. and experiences of dysmenorrhea(6 items). The results were as follows 1. Prevalence rate of dysmenorrhea was $76.8\%$. Among women who had dysmenorrhea. $52.0\%$ of them had family history on dysmenorrhea. $46.0\%$ of them experienced limitation of daily activities. $18.3\%$ of them have started the dysmenorrhea since their employment present company. and $8.8\%$ of them visited hospital due to dysmenorrhea of the subjects who took analgesics for relieving menstrnal pain. $7.2\%$ had no effect. 2. The frequency of dysmenorrhea was significantly different by educational level. working period. and the amount of mentrual flow.

Post-traumatic bifid mandibular condyle: A case report and literature review

  • Bifid mandibular condyle (BMC) is an uncommon morphological variant of the mandibular condyle. Although authors have proposed various etiologies for BMC, no consensus has emerged. In addition, varying findings have been reported regarding the epidemiological parameters of BMC (e.g., prevalence, gender ratio, and age), possibly due to its low incidence. BMC is occasionally associated with symptoms of the temporomandibular joint, such as ankylosis, pain, and trismus; however, it is difficult to detect this condition on conventional radiographs. This study reports a case of BMC with radiographic findings, and reviews the literature on the epidemiology of BMC.

대구지역 물리치료사들의 직업과 관련된 근골격계장애 조사 (A survey on the Work-Related Musculoskeletal Disorders in Physical Therapist in Daegu)

  • Disorders of the musculoskeletal system are common in physical therapist. Physical therapist are exposed to occupational risk factors leading to work-related musculoskeletal disorders(WMD). Back pain is an especially frequent complaint. The purpose of this study was to determine the prevalence and anatomical areas of WMD and the job factors that may be associated with these disorders in physical therapists. An 2-page questionnaire was used. 105 physical therapist responded. Despcriptive statistics, Chi-square analysis and odds ratio were used. The results were as follows: 1. Ninty-nine respondents(94.2%) reported experiencing WMD. 2. The highest prevalences of WMD in physical therapist were in the following areas;lowback(74.7%), shoulder(66.6%), neck and wrist(41.4%). 3. The prevalence of WMD was affected by age, years as physical therapist. and number of patients. 4. The job factor rated most problematic for physical therapists was 'performing th same task over and over' (41.3%), 'treatingan excessive number of patients in 1 day'(39.5%)

Insomnia in Cancer - Associations and Implications

  • Background: Insomnia is a common condition in cancer patients. In spite of the high prevalence its associations have not been well studied. Existing data suggests that insomnia is related to depression and pain. However, the impact of ongoing chemotherapy on sleep is not investigated. Aim: To study the relationship between insomnia and chemotherapy after analysing confounding variables. Materials and Methods: Consecutive patients who visited New England Oncology Clinic in Tamworth were recruited. Insomnia was assessed with the Bergen insomnia scale. The Montgomery Asberg Depression rating scale was used to measure depression. Pain was assessed with the Brief Pain inventory. Chronic medical conditions, type of cancer, side effects to chemotherapy, role of steroids and other drugs were studied as confounders. Results: A total of 56 patients participated in the study. Age ranged from 33 to 83 years (mean: 63.6, SD=10.97). There were 29 men and 27 women. 42 patients received at least one form of chemotherapy and 15 were receiving radiotherapy at the time of assessment. Mean insomnia score was significantly higher in those receiving chemotherapy than in those without chemotherapy (8.92 vs 17.2, two tailed p=0.005, 95% CI=2.63-13.71). There was no significant variation in insomnia scores in terms of chronic medical condition, type of cancer, psychiatric history, use of steroids or adverse effects of chemotherapy. However, total insomnia score was correlated with depression rating score (Pearson correlation, r=0.39, p=0.003) and magnitude of pain (r=0.37, p=0.006). On regression analysis only pain was found to be predictive of insomnia. Conclusions: Insomnia in patients with cancer is found to be associated with concurrent chemotherapy and correlated with degree of depression and pain. Identifying factors related to insomnia in cancer population has implications in its management and patient education.

The current status of fibromyalgia in Korea: an electronic population health data study in Korea

  • Background: Fibromyalgia (FM) is a complex disorder characterized by widespread chronic pain and tenderness in the muscles, ligaments, and soft tissues. It is a chronic pain condition often accompanied by other symptoms and comorbidities. To effectively manage FM, it is crucial to obtain fundamental epidemiological data pertaining to the target population. Therefore, this study was conducted to elucidate the epidemiological characteristics of FM in the Korean population. Methods: Population-based medical data of 51,276,314 subscribers to the National Health Insurance Service of Korea from 2014 to 2018 were used in this study. Results: The overall incidence of FM ranged from 441 (2014) to 541 (2018) cases per 100,000 person-years, with a higher prevalence observed among female patients compared to male patients. The incidence gradually increased until middle age, followed by a decrease. The highest incidence rates were observed in the fifth decade of life for females and the sixth decade of life for males. When categorizing the affected parts of the body, the shoulder region was observed to be the most frequently affected. A comparison of the drug prescriptions based on medical specialty showed that antidepressants were the most commonly prescribed medications. The management of FM leads to consistent increases in medical expenses, regional disparities, and variations in prescription patterns across different medical specialties. Conclusions: The findings of this study will not only contribute to the understanding of FM characteristics but also provide a vital foundation for efficient management of FM in Korea.
