Female worker′s menstrual discomforts and coping -focus on hospital workers- (여성근로자의 월경시 불편감과 대처방법에 따른 완화정도 -의료기관 종사자를 대상으로-)

  • Purpose: This study is to investigate the pattern of discomfort, coping style and relief level of female workers. Methods: The subjects were 394 women who worked at three general hospitals located in S city and agreed to the purpose of this study. Data were collected April 20~30, 2010, and one-way ANOVA and Pearson's correlation were performed using descriptive statistics using SAS 9.2. Results: showed that discomfort during menstruation was significantly different from age, marital status, pregnancy experience, and birth experience. The average degree of discomfort during menstruation was $2.46{\pm}0.68$, which was the average of 5, and the pattern was pain, water accumulation and behavior change. The degree of mitigation according to coping strategies was the most effective at $3.55{\pm}0.58$, and the relationship between coping behavior and discomfort during menstruation was correlated only with avoidant coping. p = .001). Conclusion: In order to mitigate the inconvenience of menstruation, women should seek to cope with their own coping strategies rather than passive coping methods and suggest the development of an educational program that can relieve the discomfort during menstruation.

Quality of life and its related factors in patients with Korean chronic obstructive pulmonary disease (만성폐쇄성폐질환 환자의 삶의 질 관련요인)

  • The purpose of this study was to identify the degree of quality of life (QoL) and its related factors in patients with Korean Chronic Obstructive Pulmonary Disease (COPD). With data collected by Korea National Health and Nutrition Survey in 2013, general and disease-related variables, pulmonary function test, and EuroQol-5Dimension (EQ-5D) were analyzed. The mean of EQ-5D index was 0.916 in patients with COPD and 0.941 in non-COPD. The EQ-5D index and its sub dimensions (mobility, self-care, usual activity, pain/discomfort, anxiety/depression) of COPD patients were significantly lower than that of non-COPD. However, difference in COPD patients' airway limitation was significant only for self-care of EQ-5D (${\chi}^2=9.50$, p=.013). The related factors of QoL in COPD patients were age, gender, level of education, quartile of household income, smoking status, and number of comorbid diseases. Based on the results, it is important to pay close attention to COPD patients' QoL as well as comprehensive interventions which possibly improve their QoL.

The Effect of Korean Elderly's Prosthetic Status on Health Related Quality by Using EuroQol-5 Dimension (한국 노인의 보철상태에 따른 건강관련 삶의 질)

  • The aim of this study was to evaluate the relationship between prosthetic status and health related quality of elderly by using EuroQol-5 dimension (EQ-5D) which is a standardized instrument used as a measure of health outcome. Data from 1,179 elderly subjects (over 65 years old) from the fifth Korea National Health and Nutrition Examination Survey were used in our analysis. Oral examination were conducted to obtain wearing prosthetic and needed prosthetic. EQ-5D is a widely used health condition measuring instrument. It consists of the five dimensions: Mobility, Self-care, Usual activities, Pain/discomfort and Anxiety/depression. EQ-5D index is a score calculated out of a weighting system. In this study, the mean score of the EQ-5D index among the elderly was 0.86. For EQ-5D index gender, female showed statistically significantly lower quality of life than male. The related factors EQ-5D index, when the lower demand for fixed prosthodontics and full denture. Wearers in partial and complete denture than non-wearers showed higher EQ-5D index. In order to improve EQ-5D index among the elderly who demand prosthetic status, which has lost its function through providing proper dental prostheses.

A ground theory approach on fatigue of the female elderly with osteoarthritis (관절염을 앓고 있는 노인 여성 피로 경험의 근거 이론적 접근)

  • The purpose of this study was to identify and construct a substantive theory about the experience of fatigue among the female elderly with osteoarthritis. The interviewees were 6 female elderly with osteoarthritis. Of them 3 were admitted General hospital, the remaining 3 didn't admitted hospital. The data were collected through in-depth interviews by the investigators from May 20th, 1995 to June 19th, 1995. The collected data were coded into concepts and categories according to Strauss & Corbin's grounded method. The data thus collected were analyzed immediatly after interviews and fed into next round of interviews until the data collection reaches the saturation point where no additional concepts emerge. The 35 concepts were from analyzing the grounded data. The results of this study were as follows : 'change of the health condition','change of the life pattern', 'physiologic change', 'change of the coping pattern', 'performance of homework', 'activity daily living', 'tireness', 'ache', 'change of general apperance', 'sleep disturbance', 'powerlessness'. 'Physiologic unbalance', 'change of the family dynamics', 'physical imbalance', 'loss of the control', aviodance', 'conversion', 'leaving', 'indifference', 'hesitation', 'the pursue of information and trial', 'the western therapy', 'heat therapy', 'the oriental therapy','exercise', 'weight control', 'support', 'solution', 'being as before', 'adding more'. Six categories emerged from the analysis of ground data. They were as follows : 'stimuli', 'fatigue', 'passive coping', 'active coping', 'disolution', 'remains'. 13 hypothesis were derived from the integration of categories as follows : 1. The stronger the stimulus as perceived by the subjects, the more the fatigue will exrerience. 2. The more severe the fatigue, the more severe the tireness. 3. The more severe the fatigue, the more the hardness. 4. The more severe the fatigue, the more the variability of appear will experience. 5. The more severe the fatigue, the more the pain will experience. 6. The more severe the fatigue, the more the insomnia will experience. 7. The more severe the fatigue, the more the powerless will experience. 8 The more severe the fatigue, the more the psychological unbalance will experience. 9. The more severe the fatigue, the more variability of the family dynamics will experience. 10. The more severe the fatigue, the more the physical unbalance will experience. 11. The more severe the fatigue, the more loss of control will experience. 12. The subjects who experienced the fatigue will be to show passive coping and active coping mechanism.

Analysis of health-related quality of life factors in south Korean one-person households (한국 1인 가구의 건강관련 삶의 질 영향 요인 분석)

  • The purpose of this study was to investigate one-person household its health-related quality of life(HRQOL) using data from the seventh Korea national health and nutrition examination survey(KNHANES Ⅶ-2) 2017. The subjects were 301. The data were analyzed by frequency, descriptive analysis and linear regression analysis using SPSS complex samples Win 25 program. The HRQOL score was 0.92, and 2.5% of the subjects in the pain and discomfort area had severe problems. HRQOL related to the subject was 0.034, 0.039, and 0.031, respectively, when the household income level was lower, the education level was lower than elementary school graduates, and if they did not drink at present. HRQOL was 0.072, 0.065, 0.103, and 0.040 in perceived health status good or moderate, the absence of activity limitation and arthritis, was explained 43.1%. There is a need to develop interdisciplinary approaches with great interest in improving the one-person households HRQOL.

  • Intravenous infusion therapy is a standard nursing procedure in medical institutions that provides patients with drugs, fluids, blood, and nutrients into the patient's mucus. It is mainly performed and managed by nurses. Additionally, it is an injection method that injects drugs directly into the blood vessels, and is used to achieve rapid results in emergency situations, and quick and accurate effects can be expected. Even experienced nurses through education and training often make mistakes, which can not only cause discomfort to patients but also cause various problems that threaten patient safety. Various studies are being conducted to reduce the pain caused by these mistakes. This paper acquired images of veins on the back of the hands of three subjects through an image detection device and conducted a study to derive an algorithm to provide clear vein images through image processing of the collected images. To sharpen the acquired vein images, existing algorithms Histogram Equalization, CLAHE, and Unsharp Masking were selected and combined. A histogram graph was used to compare images derived by applying individual algorithms and algorithm combinations to images. The histogram graph was checked by calculating the difference between the minimum and maximum values of distributed pixels and averaging them. The algorithm combination presented in this paper was 209.1, which was higher than the average values of individual algorithms of 138.7, 132.3, and 126.2, and it was confirmed that visibility was good even in actual images.

Inappropriate Care of Oncologic Emergency in Korea (암환자 응급진료의 현황 및 문제점)

  • Purpose : Cancer is a common cause for admission to emergency room(ER). Cancer patient present to ER with undiagnosed cancer, acute vague problem of cancer, or treatment related complication. But there is little information on the magnitute of the problems in the Korea. The purposes of our study were to evaluate the appropriateness of care for oncologic emergencies in Korea. Materials and Methods : This study was undertaken of all cancer patients above 15 years old presenting to the Seoul National University Hospital ER, who visited during the period from Oct. 16 to Nov. 15, 1997. ER record was reviewed and ER doctors evaluated patients' reason for visiting ER, appropriateness in patients' utilization of ER, oncology emergency Results : 266 cancer patients(17.4% of total patients) visited ER during this period and 166 cancer patients(62.4%) utilized inappropriately ER. Their average stay is 32.0 hours and 65.8% of them stayed for 6 hours. There were complaints of patients such as pain(44.8%), abdomen distension(9.4%), and dyspnea(7.5%). The most common oncologic emergency in ER during this study were gastrointestinal(34.0%), neurologic(21%), hematologic(8.0%), infectious(7.0%), respiratory(6.0%), and genitourinary(5.0%). Conclusion : For the care of symptoms like pain, most of cancer patients utilize ER inappropriately due to lack of attention from primary health delivery system. For the better care, the palliative medicine should be established in Korea.

A study on knowledge, self-efficacy and compliance in Reumatic arthritis Patients (류마티스 관절염 환자의 지식, 자기효능감 및 치료이행과의 관계연구)

  • Reumatic arthritis is a disease with joint pain being one of the key symptoms. The patient suffers from the pain, stiff sensation and edema due to the inflammation taking Place In one or more joints. Accompanying these problems are fatigue, unusual exhaustion, fever, tachycardia and weakness. Inaddition, joints are often deformed and muscles shrink along with the progress of edema, coupled with depression and psychological instability resulting from the loss of the mobile function and limitations on the daily life. Some patients become fed up with the long and hard flight with the disease and just give up, which aggravates the symptoms. Others come to the hospital only when the conditions have become serious. We need to prevent these and guide the patients in the right direction. Against this backdrop, this study aims to look into the relations between the knowledge on the part of the patients together with their feeling of self-efficacy and the compliance. The results are expected to help the patients improve their life, In addition to providing useful materials for setting up appropriate plan for nursing intervention. The study was conducted by distributing questionnaire to 88 patients selected from the out-patient department of a university hospital in Inchon, from April 6 to 27, 1998. The following tools were used the yardstick of self-efficacy, developed in 1997 by the Society for the Health of Rheumatism Patients, was used for measuring the levels of knowledge and the feeling of self-efficacy. The degree of compliance was measured by the data collected from documents in addition to the results of the analysis of the interviews with the patients. The reliability of the tools was confirmed. In the analysis, the general characteristics were expressed in figures and percentages. The levels of knowledge, feeling of self-efficacy, and compliance were expressed in the average values and standard deviations. The relations among the variables following the general characteristics were analysed by the t-test and one-way ANOVA. The Pearson correction coefficient was used for the analysis of factors. Multiple-loop analysis was used to identify the variables affecting the compliance. The following are the results of this study. 1. Among the 88 patients, 18 were men and the remaining 70 were women, with a ratio 1 : 3.87. Regarding the age groups, 23 were between 50 and 59 years old, with those between 50 and 69 accounting for 51.1% of the total. High school graduates or higher amounted to 58%. Religious patients was 67% or 59 persons. Fifty nine percent were unemployed, and 58.3% (49 persons) had two children or fewer. The period of suffering from rheumatism varied between 2 months and IS years, with 70% less than years. 2. The average figure In relation to the of knowledge was 17.63 points over 30 or 58. 76%, which means a medium level. 3. The average figure of the feeling of self-efficacy was 60.06 points. 4. The level of compliance was 3.26, which was above average. 5. The relation between the feeling of self-efficacy and compliance showed an "r" value of 0.37, which was significant. It means that the higher the feeling, the greater the compliance points. 6. The analysis of the knowledge level revealed that the difference is found only between the college graduates and junior-high graduates or lower. 7. The feeling of self-efficacy varied along with the age and education level. 8. The general characteristics of patients as discussed above did not show significant difference with the compliance. 9. Regarding the elements influencing the compliance, the number of children, period of suffering, income, age, feering of self-efficacy, knowledge, and compliance had 54% of significance. In conclusion, rheumatism victims can lead a better life if they are appropriately educated, based on efficient training program from the early days of the disease ; if they become able to manage themselves thanks to the training ; and if they are helped by a program focusing on the increase of the feeling of self-efficacy aimed at changing patient's behavior.

The Patient Recognition, Acceptability and Evaluation of Feasibility for Day Surgery (종합병원 일부 입원환자의 당일수술에 대한 태도와 당일수술 적용 가능성 평가)

  • Objectives . This study was conducted in order to investigate the degree of recognition, acceptability, and altitude towards day surgery of patients who were hospitalized with diseases that were candidates for day surgery; in order to analyze the average length of stay for treatment of the ailments; and to analyze the percentage of patients who could be discharged on the same day after the surgery ,using the post-anesthesia discharge scoring system. Methods : Data was collected between February 1 and March 31, 1999 from 353 patients who received surgery for cataract, adenoid hypertrophy, inguinal hernia, strabismus, ptosis, cholelithiasis, hemorrhoid, or anal fistula, at a general hospital in Daegu city. The patients were interviewed and surveyed by a post-anesthesia discharge scoring system(PADS) in order to collect data on patient condition such as vital signs, activity and mental status, pain, nausea and vomiting, surgical bleeding, intake and output after the surgery. Results : Among the 353 patients, 52.7% were after of the day surgery and 52.7% were interested in day surgery. Of the respondents, 43.1% said 'my ailment was not serious and the surgery was simple' and 30.4% said 'according to my condition rest at home was desirable' as the reasons for wanting day surgery Alternatively, 56.5% of those declining day surgery said the 'uncertainty of staying home' was the reason. The greatest concern in discharging within 24 hours after surgery was a post-op emergency situation. On the other hand, the shortened hospitalization was the largest advantage of day surgery with 39.1% responding this way, followed by the savings in hospitalization costs (25.8%) and emotional stability (13.7%). The majority of those surveyed (47.6%) believed that discharge should be determined within 1-2 days after the surgery. The average hospital stay was 3.1 days for dischargeable ailments. Pain (45.6%), nausea and vomiting (10.5%), and headache (7.9%) were the common symptoms following surgery. The percentage of patients who were able to be discharged within 24 hours after surgery revealed 95.2% were dischargeable after approximately 3 hours, 99.2% dischargeable after 12 hours, and 100% dischargeable after 24 hours. Conclusions : According to the PADS score, the cataract extract and strabismus correction patients were eligible for day surgery and the further evaluation concerning the reason for delayed recovery of the other diseases is needed.

A Study on Demand and the Supply for Home-based Cancer Patient Management Projects of Public Health Centers (보건소 재기 암환자 관리사업에 대한 환자의 요구도 및 제공정도)

  • Purpose: As a part of the analysis of home-based cancer patients management of public health renters in cities, counties and districts across the nation, this study is to understand the degree of patient demands for that management and the degree and scope of the supply for the patient's demand. Methods: Developed the questionnaire which was constituted of degree of demand and supply for home-based cancer patient management and analyzed data centering on the frequencies and percentages by utilizing SPSS WIN 12.0. Results: The services provided through the home-based cancer patients management project include physical, emotional, spiritual and education/informative services. A survey was conducted for home-based cancer patients about these services, and its result showed that the degree of demand and supply was highest for emotional service, followed by education/informative service, spiritual service and physical service in the order of the demand-supply degree. When main items for each service were examined, it was found that: in the case of physical service, pain control was provided murk lower than its demand, while excretion disorder control and individual hygiene is provided murk more than its demand. In the case of emotional service, the degree of demand was overall higher than that of supply; spiritual service was provided appropriately to the degree of demand. Conclusion: This study examines the home-based canter patients management project of public health centers and compares and analyzes the degree of demand for patient services and the degree of services that are actually provided. The findings could be used as based data for the development of effective programs in future on the basis of actual demands of home-based cancer patients.
