Vibration and Ergonomic Exposures Associated With Musculoskeletal Disorders of the Shoulder and Neck

  • Background: According to the US Bureau of Labor Statistics, musculoskeletal disorders (MSDs) accounted for 32% of all nonfatal injury and illness cases in 2014 among full-time workers. Our objective was to review and summarize the evidence linking occupational exposures to vibration and awkward posture with MSDs of the shoulder and neck. Methods: A literature search was conducted using the terms musculoskeletal disorders, vibration, and awkward posture. All types of observational epidemiologic studies, with the exception of case reports, published during 1998-2015 were included. Databases searched were MEDLINE (Ovid), Embase (Ovid), Scopus, Ergonomic Abstracts, NIOSHTIC-2, and Health and Safety Science Abstracts. Results: Occupational exposures to whole-body or hand-arm vibration were significantly associated with or resulted in MSDs of the shoulder and neck. Awkward postures while working were also associated with MSDs in these locations. These findings were consistent across study designs, populations, and countries. Conclusion: Occupational exposure to vibration and awkward posture are associated with shoulder and neck MSDs. Longitudinal studies are required to elucidate the mechanisms responsible for these associations, and intervention studies are warranted.

Conservative approach of a symptomatic carious immature permanent tooth using a tricalcium silicate cement (Biodentine): a case report

  • The restorative management of deep carious lesions and the preservation of pulp vitality of immature teeth present real challenges for dental practitioners. New tricalcium silicate cements are of interest in the treatment of such cases. This case describes the immediate management and the follow-up of an extensive carious lesion on an immature second right mandibular premolar. Following anesthesia and rubber dam isolation, the carious lesion was removed and a partial pulpotomy was performed. After obtaining hemostasis, the exposed pulp was covered with a tricalcium silicate cement (Biodentine, Septodont) and a glass ionomer cement (Fuji IX extra, GC Corp.) restoration was placed over the tricalcium silicate cement. A review appointment was arranged after seven days, where the tooth was asymptomatic with the patient reporting no pain during the intervening period. At both 3 and 6 mon follow up, it was noted that the tooth was vital, with normal responses to thermal tests. Radiographic examination of the tooth indicated dentin-bridge formation in the pulp chamber and the continuous root formation. This case report demonstrates a fast tissue response both at the pulpal and root dentin level. The use of tricalcium silicate cement should be considered as a conservative intervention in the treatment of symptomatic immature teeth.

An Analytical Study on Fatigue and its Associated Variables In Patients with Systemic Lupus Erythematosus in Korea (루푸스 환자의 피로 관련 요인)

  • This study was to investigate the level of fatigue and its associated variables in patients with systemic lupus erythematosus(SLE) in Korea. From March to June, 2001, 100 patients, registered at one university hospital rheumatology clinic in Seoul, were accepted as subjects for this study. The sampling method was adopted a non-probability, purposive technique. The instruments used for this study were The Multidimensional Assessment Fatigue scale developed by Tack and Beck Depression Instrument develped by Beck. The collected data were analyzed by SAS program using t-test, ANOVA with Scheffe-test, Pearson correlation coefficients and stepwise multiple regression. The results were as follows: 1. Total scores of fatigue of the subjects averaged $24.46(\pm10.85)$, degree of fatigue was $5.08(\pm2.29)$, and influence of fatigue was $3.52(\pm2.12)$. 2. Regarding characteristics, more depressive(p=.0001) and more painfuI(p=.0122) patients revealed more fatigue. Also, the subjects with spouse(p=.0337) and having poor quality of sleep(p=.0445) revealed more fatigue. 3. The subjects' total fatigue score, depression, pain and age was correlated positively(r=.53; r=.48; r=.24), and total fatigue score, and exercise time, quality of sleep was correlated negatively(r=-.45; r=-.21). 4. The main influencing factors on the fatigue were depression$(52.92\%)$ and quality of sleep $(8.10\%)$. These two main variables made it possible to explain $61.02\%$ of the varience in fatigue. In conclusion, this study revealed depression and quality of sleep is an important factor that can improve quality of life in patients with SLE. It is recommended that nursing intervention for SLE patients would be focused to decrease depression and to enhance quality of sleep.

Menstrual Discomforts and Coping In Adult Women (성인여성이 경험하는 월경불편감 및 대처방법)

  • The purpose of this study was to identify menstrual discomforts and coping m adult women. The subjects were 577 adult women. aged 20 years old and over. selected by convenient sampling. Data were collected through a questionnaire survey from December 2. 2004 to December 31. 2004. Two instruments were modified and used in this study. the Menstrual Discomfort Questionnaire by Park(1988) and the Menstrual Coping Questionnaire by Billings & Moos(198l). The data were analyzed by descriptive statistics. t-test. and ANOVA with SPSS 10.0 program. The results of this study were as follows. 1. The prevalence rate of dysmenorrhea was $85.6\%$. 2. The total mean score for menstrual discomforts was 2.82 of a possible total of 5. The mean score for each category was 3.10 for water retention. 2.93 for negative affect. 2.89 for pain. 2.74 for autonomic reactions. 2.73 for behavioral change. and 2.59 for concentration. 3. Statistical differences were found for menarche age(F=9.351. p<.00l), feeling to menstruation(F=12.376. p<.001). dysmenorrhea(t=7.3l7. p<.001). onset of dysmenorrhea (F= 12.766. p<.001). taking medication(t=6.289. p<.001). and degree of taking medication (F=12.924. p<.00l). 4. The coping modes with the highest scores were 'rest and go to the bed '$(83.3\%)$. 'regard menstruation as a physiological and temporary phenomenon' $(68.6\%)$, and 'take a warm shower' $(64.5\%)$. We conclude that there were many women with dysmenorrhea. that the first day was the onset of menstrual discomfort. and that it differed by menarche age. feeling to menstruation. dysmenorrhea. onset of dysmenorrhea. taking medication. and degree of taking medication. Nursing intervention has to be considered in programs to reduce menstrual discomfort.

Hermeneutic Phenomenological Study on the Experiences of Employment of Married North Korean Women Defectors Rearing Children (자녀를 양육하는 북한이탈 기혼여성의 직장생활 체험에 관한 해석학적 현상학적 연구)

  • Purpose: This study aimed to understand the experiences of married North Korean women's child-rearing, working lives, and their home and work environment in depth. Methods: This study adopted van Manen's hermeneutic phenomenological method to qualitatively analyze data. The participants were 8 married North Korean women defectors. Data were collected through in-depth interviews and observations from July 4 to August 20, 2018. Results: Nine essential themes emerged: more personal challenges after overcoming a life-threatening crisis; hopes of firmly settling in this land; the wound from the north, which chased them here; a body that becomes stronger through hardship; being stuck in a past full of anxiety and pain; the present is full of hope; hope for the future; sense of alienation from coworkers that cannot be overcome; and sense of power to endure an exhausting work life. Conclusion: This study provided a broader understanding of the life and experiences of married women from North Korea. It highlights the need for nurses to recognize their importance in nursing care. The study also suggests that academic and practical approaches for nursing, and basic data for a nursing intervention for married women from North Korea be provided. The study findings can be used as a basis for preparing a national policy that will help North Korean defectors to find employment and gain stability.

Intramuscular hemangiomas on the masseter muscle and orbicularis oris muscle: a report of two cases

  • Intramuscular hemangioma (IMH) is a rare vascular disease involving skeletal muscle, comprising only 0.8% of hemangiomas. About 10% to 15% of IMHs occur in the head and neck region, mostly involving the masseter muscle. IMH occurs mostly in childhood, but is often not found until unexpected enlargement, pain, or cosmetic asymmetry occurs in adulthood. Several non-surgical treatments including cryotherapy, sclerosant injection, and arterial ligature have been described, but complete surgical resection is the curative intervention. In this report, we present two rare cases of IMH. One IMH case in a 48-year-old male occurred in the masseter muscle feeding from the transverse facial artery. Embolization of the distal branch of the facial artery was first conducted, and then the buccal mass was removed surgically via the intraoral approach. A second IMH case in a 58-year-old female occurred in the orbicularis oris muscle feeding from the superior labial artery, and the mass was excised surgically without embolization.

Bio-mechanical Analysis of the Grapevine Cluster Thinning Task using Working Chair

  • Objective: This study examined the effects of working chair that was developed for farmers who work in grapevine's cluster thinning. Background: Agricultural work involves some of the nation's highest occupational risk exposures. Fruit cultivation has been recognized as one of the most hazardous crops in which to work. Grapevine cluster thinning task involve activities related to the main risk factors associated with upper limb work-related musculoskeletal disorders. Method: An experiment was designed to test the working chair. Six healthy male($25.8{\pm}4.9years$) were selected as the subjects for this study. Electromyography(EMG) was used to monitor the muscles activity, and Electro-goniometer was used to measure the ranges of motions of the elbow, trunk and knee. Subjective test was also carried out to examine discomfort body parts and their pain intensity. A grapevine's working space was built for the experiment and working chair was installed on it. In order to examine the muscle activity and range of motion, subjects used to the working chair for 30 minutes for each experimental condition. Another test without working chair was also performed for comparison. %MVC was used to quantify the level of muscle activity. Results: Decreases of muscle activity was found in all leg muscles and significant decrease of muscle activity was found in left Gastrocnemius. The range of motion of the trunk and knee also decreased when working chair used. Discomfort in lower back, thigh and shank region were reduced significantly. However, in upper limbs muscle activity tended to increase in working chair compared with conventional task. Conclusion: Improvement for cushion in seat back and pan required to reduce discomfort in buttocks. Application: Overall findings verified that the working chair might help to prevent upper limb and lower back MSDs based on the current study. These results can be practically used for work improvement for the grapevine growers to prevent MSDs.

  • Purpose: The purpose of this study is to supply the basic materials for the development of objective tools to measure 'Hwa-byung' and for the development of the theories and statements associated with 'Hwa-byung' by making clear the meaning and characteristics of 'Hwa-byung' and by analyzing the concept of 'Hwa-byung'. Method: The concept analysis of Hwa-byung was proceeded according to the concept analysis process of Walker and Avant(1995). Result: The conceptual characteristics of Hwa-byung resulted from the research are like these : (1) Injustice (2) Anger (3) Rumination (4) Anxiety. Therefore, we can describe Hwa-byung as the feeling repeatedly accumulated through the heavy mental pain, injustice, and anger and it is appeared with the physical and behavioral symptoms. Conclusion: Through this concept analysis, we propose as followings ; First, there's need of the objective tools which make it possible to assess Hwa-byung through the theory development and the statement associated with Hwa-byung. Second, there's need of interdisciplinary approach in the development of nursing intervention to coordinate the characteristics of Hwa-byung by understanding the total nature of Hwa-byung and finding the variables associated with Hwa-byung.

Wuqinxi Daoin exercise for osteoporosis: A Systematic Review (오금희 도인 운동이 골다공증에 미치는 영향 : 체계적 문헌 고찰)

  • Objectives : This review aims to assess the effect of Wuqinxi exercise for osteoporosis. Methods : We searched 9 electronic databases(Pubmed, EMBASE, Cochrane Library, CAJ, Oasis, KISS, NDSL, KMBASE, KISTI) and related 2 journals until November 2017. We included randomized controlled trials(RCTs) of testing Wuqinxi exercise for osteoporosis. The methodological quality of RCTs related assessed by the Cochrane risk of bias tool. Results : 12 RCTs were eligible in our inclusion criteria. No statistical differences were found between the Wuqinxi versus no intervention, Wuqinxi plus antiosteoporosis medications versus antiosteo-porosis medications on lumbar spine, femora bone mineral density (BMD). However, Wuqinxi significantly improved lumbar spine BMD compared with antiosteoporosis medications (P < 0.00001). Additionally, the results showed a remarkable effect in improving pain score(VAS) when Wuqinxi or Wuqinxi plus antiosteoporosis medications (P < 0.0005) was used. Conclusions : There is evidence of Wuqinxi exercise for osteoporosis with meta-analysis. However, our systematic review has limited evidence to support Wuqinxi exercise for osteoporosis. because of low quality of original articles and further well-designed RCTs should be encouraged.

Effect of Maitland Mobilization and Kaltenborn-Evjenth Mobilization on the SLR Angle

  • The aim of this study was to investigate the effect of Maitland mobilization and Kaltenborn-Evjenth mobilization on the SLR angle. Subjects randomly divided into Kaltenborn-Evjenth group(n=8) and Maitland group(n=7). The mean height, age, body weight was $176.00{\pm}5.10cm$, $22.75{\pm}1.83years$, $72.63{\pm}10.65kg$ respectively in Kaltenborn-Evjenth group. The mean height, age, body weight was $175.00{\pm}5.60cm$, $22.29{\pm}3.68years$, $78.00{\pm}12.36kg$ respectively in Maitland group. Hip joint accessary movements with Grade III or IV were applied depend on the patient's condition to the restricted direction for 1 minute each set, and performed 5 set in a Maitland group. Hip joint anteroposterior gliding with Grade III were applied 60 for 1 minutes each set, and performed 5 set in a Kaltenborn-Evjenth group. The angle of first pain was referred to as P1 and subjects were pointed out that they could not bend the knee anymore, then examiner measure SLR angle. The SLR was significantly increased in the Maitland group compared to the Kaltenborn-Evjenth group after intervention(p<.05). In a within group difference, SLR significantly increased in the both groups(p<.05). These results indicated that Maitland mobilization could be recommended the excellent technique to increase the hip flexion in patient with hip hypo-mobility.
