Evaluation of Coronary Artery Disease with Gated Blood Pool Scan Using Dipyridamole (Dipyridamole 부하 심장 풀 스캔을 이용한 관동맥질환의 평가)

  • Thirty-one patients with coronary artery disease and twenty-sir normal subjects underwent $^{99m}Tc-GBPS$ before and after coronary vasodilatation was induced by dipyridamle 0.54 mg/kg given IV over 4 min. LVEF, ${\Delta}EF$ and regional wall motion by phase analysis were measured during rest and dipyridamole infusion. The results were as follows: 1) Mean LVEF of normal subjects was significantly higher than that of MI group (p=0.001), but similar to that of angina group during rest. Among MI group, mean LVEF of anterior MI group was significantly lower than that of inferior MI group during rest (p=0.024). 2) The normal subjects had a significaat increase in mean LVEF during dipyridamole infusion $(+12{\pm}3.8)$, while the CAD group had no increase $(+2{\pm}5.0)$ (p<0.001). If an increase of LVEF during stress is less than 5%, it suggests an abnormality. The sensitivity and specificity of LVEF changes after dipyridamole infusion were 81%, 96%, respectively. 3) With phase analysis, LV mean phase angle of normal subjects and CAD patients was $143{\pm}20.5^{\circ},\;132{\pm}20.6^{\circ}$ respectively, durign rest (p=0.049). But an ncrease of LV mean phase angle during dipyridamole infusion in these two groups was not significantly different. Dipyridamole infusion did not affect standard deviation and FWHM of phase angle. 4) Regional wall motion was abnormal in 5 patients (16%) during dipyridamole infusion. 5) Side effects with dipyridamole infusion include; headache, angina pain, chest discomfirt, nausea, weakness sense. In conclusion, dipyridamole GBPS might be useful in detection and follow up of CAD.

A Case of a Solitary Fibrous Tumor of the Pleura Presenting as Pneumonia and Acute Respiratory Failure (폐렴과 급성 호흡부전으로 나타난 흉막의 고립성 섬유성 종양 1예)

  • Solitary fibrous tumors of the pleura (SFTPs) are relatively rare tumors that originate from the mesenchymal cells of the submesothelial tissue of the pleura. Patients with SFTPs are often asymptomatic, while some patients experience pleuritic chest pain, cough and/or dyspnea. We report here on a case of SFTP, and the patient presented with septic shock and respiratory failure that required mechanical ventilation. A 68-year-old woman was admitted for the evaluation of her dyspnea and generalized edema. Chest imaging studies showed an 18 cm-sized voluminous mass occupying the right thoracic cavity with anterior displacement of hilar structures and atelectasis of the right lung. Immediately after admission, she developed pneumonia and septic shock that required antibiotics and mechanical ventilation. She displayed a partial response to medical treatment, and then complete excision of the tumor was performed and the pathologic examination revealed benign SFTP. Afterward, she fully improved without evidence of recurrence until now.

A Clinical Evaluation of Tuberculous Pleurisy -Report of 52 Cases (결핵성 흉막염 환자의 임상적 고찰)

  • A clinical analysis was performed on 52 cases of tuberculous pleurisy experienced in the department of thoracic and cardiovascular surgery and department of medicine, Chosun University Hospital during a period from Jan. 1994 to Dec. 1995. Among them, male was 39 cases, female was 13 cases, with age ranged from 7 to 73 years. The common symptoms were chest pain 75%, dyspnea 59.6%, cough 55.8%. The most common diagnostic tool was pleural biopsy. The protein levels in the tuberculous pleural effusion were 0.9∼6.5 gmojo, and ratios of effusion protein to serum protein were 0.48 ∼ 1.06. The glucose levels in the tuberculous pleural effusion were 37∼ 112 mg%. The LDH levels in the tuberculous pleural effusion were 80 ∼ 2440 unitlml, and ratios of tuberculous p eural effusion LDH to serum LDH were 0.48 ∼ 1.03. The ADA levels in the tuberculous pleural effusion were 24-63 lU/L. The common surgical methods of treatment in the tuberculous pleurisy were closed thoracostomy in 18 cases(66.7%), and thoracentesis in 5 cases(18.5%). This study compares the clinical results of group A and group B. There were no significant differences for age and sex, lag period from initial symptoms to admission, diagnostic method, and protein, pH, LDH, glucose, ADA levels in tuberculous pleural effusion. Authors noted that the discharge after admission on the tuberculous pleurisy was more faster in patients with surgical treatment than in patients with only medical treatment.   • Osteoarthritis (OA) is a degenerative disease characterized by the progressive degradation of joint cartilage and is accompanied by secondary inflammation of synovial membranes. The purpose of this study describes a preliminary evaluation of the anti-inflammatory activity on test material of Litsea japonica. fruit (LJTM) Also, this study was to evaluate the effects of LJTM on the joint cartilage of rat with OA induced by monosodium iodoacetate (MIA). To study for anti-inflammatory agents effectively, we first examined the inhibitory effect of the LJTM on the production of pro-inflammatory factors and cytokines stimulated with lipopolysaccharide. We identified anti-nociceptive effects of the LJTM by using in vivo peripheral and central nervous pain models. In addition, the aim of this study was to evaluate the effects on mRNA expression of MMP-2, -3, -7, -9, -13, TIMP-1 and –2 in cartilage of OA. In the LJTM inhibited production of pro-inflammatory mediators (NO and PGE2) and pro-inflammatory cytokines (TNF-α and IL-6). In cartilage, Expression of MMPs and TIMPs mRNA was suppressed in LJTM treatment group than in the control group. This study suggests that LJTM are potential candidates as anti-inflammation and anti-osteoarthritis agents (painkillers) for the treatment of OA.

  • Objectives : The purpose of this clinical study was to investigate the efficacy and safety of Hwangryunhaedok-tang on the gastritis treatment. Methods : A total of 20 subjects who visited Pusan National University Hospital from April 2nd, 2019 to September 2nd, 2019 were included. In this study, we evaluated the individual symptoms through questionnaires, laboratory test, BIA (Bioelectrical Impedance Analysis), and adverse reaction according to medication. Statistical analysis was performed with paired t-test and Wilcoxon Signed Rank test. Statistical significance was achieved if the probability was less than 5% (p < 0.05). Results : After medications, the total score of individual symptoms, primary outcome, was significantly decreased. In particular, individual symptoms such as upper abdominal pain, bloating, heartburn and trim showed significant improvement. However, no statistically significant differences were found in nausea/vomiting and anorexia. In additions, the laboratory test showed a statistically significant decrease in total cholesterol, but no statistically significant differences in results of BIA. In the safety evaluation, adverse reactions were not reported. Conclusions : Considering this results, we have confirmed the possibility of Hwangryunhaedok-tang to the gastritis treatment.

Ultrasonographic Features of Intra-abdominal Abscess (복부내 농양의 초음파 소견)

  • Intraabdommal abscess usually causes distress with fever, leukocytosis, pain and toxicity. Diagnosis of intraabdominal abscess is occasionally difficult and It has high morbidity. However radiologic method, such as ultrasonography, CT scan, or RI scan are helpful to early detection of intraabdominal abscess. Among these methods, ultrasonography is a non-invasive technique and performed without discomfort to patient. And also differential diagnosis between cystic and solid lesion is very easy and sequential ultrasonography in same patient is valuable for the evaluation of treatment effect. We analyzed the ultrasonic features of 48 cases with intra-abdominal abscesses and the results are as follows; 1. In total 48 cases, the intra-abdominal abscesses were 30 cases, the retroperitoneal abscesses, 5 cases, and the visceral abscesses, 13 cases. 2. The causes of the intra-abdominal abscesses were perforating appendicitis (25 cases), postoperative complications (5 cases), pyogenic and amebic hepatic abscesses (13 cases), and the others (5 cases). 3. Round or oval shaped lesions were 26 cases (54%), irregular shape, 18 cases (38%), and multiple abscess formation in 4 cases (8 %). 4. The size of the lesions were between 5 and 10cm in diameter in 54% of total 48 cases, and the most frequent feature of the echo-pattern of the lesions was cystic with or without internal echogenicity (69%).

A Clinical Review of Ectopic Pregnancy (자궁외임신의 임상적 고찰)

  • This study was undertaken for the clinical evaluation and statistical analysis on the 88 women with histopathologically confirmed ectopic pregnancies who were admitted and treated from grand opening on May-28, 1983 to Sept.-30, 1985. Incidence of ectopic pregnancy was 1 in 22.2 deliveries and the most common age group was in 30~34 years of age. There was tendency of decreasing incidencies as the gravidity, panty or artificial abortion were over 3. The most common etiologic factor was pelvic inflammatory disease and procedures for family planning were the next common. Common symptoms were lower abdominal pain (85.2%) and vaginal spotting (56.8%). At the admission, hemoglobin level under 10gm% were 44.3%, 69.5% of urine HCG tests were positive. Culdocentesis was positive only in 75.9%. Termination was frequent at 6 ~ 7 weeks of gestation mostly with rupture or abortion. Free blood in the abdominal cavity was averaged 1,224 ml. 4 cases of clinicobiochemically suspected ectopic pregnancies with spontaneous regression were excluded, and there was no fatal case treated in hospital.

The Clinical Observation of Digital Infrared Thermographic Imaging on Bell`s palsy (DITI 진단을 통한 Bell`s palsy의 임상적 고찰)

  • The clinical data and thermographic imaging were analyzed on the 100 cases of Bell's palsy who were treated in the Kwang-Ju Oriental Medical Hospita! of Wonkwang University from February to October 1997. All the cases were taken Digital Infrared Thermograph Imaging(DITI) before treatment and 30 cases of them were taken follow up imaging again after recovery. And the following results were obtained. 1. Of 100 cases, under teenager occupied $1\%,\;teenager\;8\%,\;2nd\;decade\;14\%,\;3rd\;decade\;11\%,\;4th\;decade\;16\%,\;5th\;decade\;26\%,\;6th\;decade\;16\%,\;7th\;decade\;7\%\;and\;over\;80\;occupied\;1\%$. 2. Male occupied $48\%$ and female occupied $52\%$. 3. $42\%\;of\;male\;and\;23\%$ of female had the affected side at left side. And right facial nerve palsy occured at $29\%\;of\;male\;and\;29\%$ of female. 4. The most common cause of Bell's palsy was cold wind $18\%$, and the next were excessive labor $15\%,\;stress\;12\%,drinking\;2\%\;and\;cold\;food\;1\%$. 5. Thermal pattern were present as hyperthermal pattern in $44\%$ and hypothermal pattern in $22\%$. 6. The DITI showed hyperthermal pattern as close as to the onset day and changes to hypothermal pattern as times passed. 7. The DITI pattern and post-auricular pain, the most common prodomal syndrome, showed no significant relationship. 8. The relationship between the grade of paralysis and thermal patttern of DITI showed no significance. 9. Mean temperature of loci in affected side indicrtted $30.27^{\circ}C\;at\;Yang\;baek,\;30.02^{\circ}C\;at\;Taeyang,\;29.25^{\circ}C\;at\;Geoyo,\;29.62^{\circ}C\;at\;Jichang,\;29.78^{\circ}C\;at\;Hakwan,\;29.61^{\circ}C\;at\;Hyupgeo,\;and\;30.59^{\circ}C$at Yeopoong. 10. Mean temperature of loci in unaffected side showed $30.16^{\circ}C\;at\;Yang\;baek,\;30.02^{\circ}C\;at\;Taeyang,\;29.61^{\circ}C\;at\;Geoyo,\;29.68^{\circ}C\;at\;Jichang,\;29.70^{\circ}C\;at\;Hakwan,\;29.57^{\circ}C\;at\;Hyupgeo,\;and\;29.89^{\circ}C$at Yeopoong. 11. Of 30 cases who were taken follow up imaging again after recovery, the relationship between delta T at loci and symptoms showed no significance. It should be needed further investigation in order to apply them for clinical evaluation.

A Preliminary Study on Method for Evaluation and Diagnosis of Late Luteal Phase Dysphoric Disorder in Women - Focusing on Psychiatric Outpatients - (후기 황체기 불쾌기분 장애의 평가 방법 및 진단에 관한 예비 연구 - 정신과 외래환자를 대상으로 -)

  • Reports about the prevalence of premenstrual symptoms state that occurs in 20 to 100% of most reproductive-age women. There is a close association between premenstrual syndrome and affective disorders as well as same some other psychiatric disorders. Late luteal phase dysphoric disorder (LLPDD) is a premenstrual condition defined in DSM-III-R by severe mood changes and other symptoms that repeatedly occur only in the luteal phase of the menstrual cycle. However, DSM-III-R does not specify how to compute the change from the follicular to the luteal phase or how to determine when the amount of change is great enough to warrant the diagnosis nor how to determine occupational or social functional impairment. This study was conducted to evaluate the nature, severity and magnitude of premenstrual syndrome in women with current psychiatric disorders by using prospective Daily Rating Form(DRF), and to measure symptom changes according to three scoring methods for diagnosing LLPDD. Our study obtains the data about premenstrual changes estimated by DRF from 22 women with psychiatric disorders who had met criteria for major depressive syndrome on the Premenstrual Assessment Form (PAF). The data was scored by each three methods and was determined to meet criteria A for LLPDD. The results are as follows: 1) the subjects, when scored according to the percent change method, effect size method and absolute severity method, met the DSM-III-R criteria A for LLPDD in 36.4% (8 subjects), 14% (3 subjects) and 4.5% (1 subject) of the cases respectively. 2) The items of irritability, anger and impatience were occurred most frequently on the DRF, when it was scored according to the three scoring methods. And the item of breast pain was next frequent according to the effect size method and the percent change method but according to the absolute severity method. 3) The PAF item of impaired social functioning was reported by 16 (73%) of the subjects. 4) 4 (18%) of the subjects met criteria A for LLPDD and reported impaired social functioning. The prevalency of LLPDD according to each method varied. The percent change method yielded the greatest (36.4%), and the absolute seventy method yielded the laest (4.5%), The effect size method yielded an intermediate frequency (14%). Therefore, for maximizing the diagnostic accuracy of LLPDD, a diagnostic procedure including a measure of change (e.q., effect size method, percent change method) as well as confirmed diagnosis by DRF, will be needed. Also, an accurate tool to evaluate impaired social functioning will be required.

The Radiologic and Clinical Changes after Open Complete Repair of Massive Rotator Cuff Tears (개방적 완전 봉합술로 치료한 광범위 회전근 개 파열 환자에서 치료 전후의 방사선학적 및 임상적 소견의 변화)

  • Purpose: This study evaluated the preoperative and postoperative radiologic findings of patients with complete repairs after massive rotator cuff tears along with the incidence of re-tear and the clinical outcomes. Materials and methods: This study evaluated 33 cases who had open complete repairs for massive rotator cuff tears and these patients were followed up for more than 24 months. The clinical evaluation was performed according to the shoulder joint function test of the American Shoulder and Elbow Surgeons (ASES), and the degree of arthritis related to the massive rotator cuff tears was evaluated using the Hamada classification. Results: The ASES scores improved from 37.6 preoperatively to 85.6 postoperatively. The mean preoperative acromio-humeral interval (AHI) score was 6.5 mm, which increased to 9.3 mm immediately after surgery, and there was a decreased to 6.5 mm noted at the last follow up. The lower radiology stages of arthritis according to the classification showed better preoperative and postoperative results. Conclusion: An open complete repair as the surgical treatment for a massive rotator cuff tear showed satisfactory results for pain relief and an improvement in the shoulder joint function though re-tear after surgery.
