The Change of Pain in Tonsillar Disease Estimated by DITI (적외선 촬영으로 측정한 편도질환에서의 통증변화)

  • Backgroud and Objectives: Because the pain is subjective in human, we frequently need the objective tool for estimation of pain. The Visual Analog Scale (VAS) is a method for pain grading, but it is also a subjective method. The purpose of this study is to seek an objective method for measurement of pain. Author suggests that the estimation of local body temperature may be associated with the severity of pain in tonsillar disease and the change of it measured by Digital Infrared Thermographic Imaging (DITI), may also be correlated with the change of pain. Materials and Method : Four groups were selected and measured for VAS and body temperature in DITI. Group A and B were post-tonsillectomy groups divided by age. Group C was acute tonsillitis group. Group D was peritonsillar abscess group. Results : In Group A and B, the subjective VAS was significantly correlated with objective local body temperature (correlation coefficient r=0.673, 0.647) and significant correlation was also present in acute tonsillitis and peritonsillar abscess groups(r=0.596,0.642). Conclusion : The change of pain was strongly associated with that of local body temperature in tonsillar disease. DITI is a useful method for objective grading of pain and can be used for the study of postoperatve pain and effectiveness of pain control.

Relationship between Pain-related Variables and Extent of Heart Disease (심장질환자에서 흉부 통증 특성과 심장질환 정도와의 관계)

  • Purpose: To identify the essential characteristics of pain which nurse have to obtain for patients with chest pain, 92 patients who were admitted in medical units to take intensive tests for heart disease were investigated cross-sectionally. Method: Duration, severity, stress, anxiety, perceived severity, number of painful area, number of accompanying symptoms, triggering activity, and pattern were included as the characteristics of pain. Ejection fraction of left ventricle and number of involved area detected by ultrasonography and number of diseased coronary artery detected by cardiac catheterization were assessed as the variables of heart disease extent. Result: Severity of pain was found to be correlated with all three variables of heart disease extent. Perceived severity and number of accompanying symptoms were correlated with two of them. Anxiety, number of painful area and pattern were related with the number of involved area. Conclusion: Pain severity reported by patients is found to be the most important variable to be obtained from patient. Variables such as perceived severity, number of accompanying symptoms, anxiety, number of painful area and pattern also have to be carefully assessed to anticipate the extent of heart disease.

Text-Mining of Online Discourse to Characterize the Nature of Pain in Low Back Pain

  • PURPOSE: Text-mining has been shown to be useful for understanding the clinical characteristics and patients' concerns regarding a specific disease. Low back pain (LBP) is the most common disease in modern society and has a wide variety of causes and symptoms. On the other hand, it is difficult to understand the clinical characteristics and the needs as well as demands of patients with LBP because of the various clinical characteristics. This study examined online texts on LBP to determine of text-mining can help better understand general characteristics of LBP and its specific elements. METHODS: Online data from www.spine-health.com were used for text-mining. Keyword frequency analysis was performed first on the complete text of postings (full-text analysis). Only the sentences containing the highest frequency word, pain, were selected. Next, texts including the sentences were used to re-analyze the keyword frequency (pain-text analysis). RESULTS: Keyword frequency analysis showed that pain is of utmost concern. Full-text analysis was dominated by structural, pathological, and therapeutic words, whereas pain-text analysis was related mainly to the location and quality of the pain. CONCLUSION: The present study indicated that text-mining for a specific element (keyword) of a particular disease could enhance the understanding of the specific aspect of the disease. This suggests that a consideration of the text source is required when interpreting the results. Clinically, the present results suggest that clinicians pay more attention to the pain a patient is experiencing, and provide information based on medical knowledge.

  • Kikuchi's disease (KD) is an idiopathic and self-limiting necrotizing lymphadenitis that predominantly occurs in young females. It is common in Asia, and the cervical lymph nodes are commonly involved. Generally, KD has symptoms and signs of lymph node tenderness, fever, and leukocytopenia, but there are no reports on treatment for the associated myofacial pain. We herein report a young female patient who visited a pain clinic and received a trigger point injection 2 weeks before the diagnosis of KD. When young female patients with myofascial pain visit a pain clinic, doctors should be concerned about the possibility of KD, which is rare but can cause severe complications.

Trigeminal Neuralgia which Caused by Brain Tumor or Cerebrovascular Disease (뇌 종양 및 뇌 혈관 질환에 의해 유발된 삼차신경통 환자의 임상 고찰)

  • A retrospective analysis of 175 patients who were suffering from trigeminal neuralgia was done. We found 21 cases (12.0%) of abnormal findings including brain tumors and cerebrovascular disease on brain MRI. All patients were transferred to department of neurosurgery for operation. Among them, 7 patients refused or gave up operation and received nerve blocks with pure alcohol. Their MRI findings were meningioma, arachnoid cyst, arteriovenous malformation, venous angioma, and frontal sinus cancer This study demonstrates that peripheral nerve block or trigeminal nerve block with pure alcohol would be possible in case of elderly patients, patients who have poor general condition, patients who refuse operation, and brain tumor or cerebrovascular disease which located in dangerous area to be operated.

A Study on the Chief Complaints among Farmers in the Rural Community (농촌주민의 주소증에 대한 조사연구)

  • Objectives and Methods : This study was conducted to investigate chief complaints among famers. The subjects of this study were 640 famers who live in the rural area in Wanju-gun Jeonbuk province. They were received medical treatment by oriental doctors who visited each village from July 1996 to June 1999. The data of this study were collected from their medical charts . Results and Conclusions : The number of female patients was 449(70.16%) and the number of male patients was 191(29.84%). The group of patients above 70 years was 32.81%, 60-69 years 32.03%, 50-59 years 20.15%, 40-49 years 8.90%, and bellow 39 years 6.09%. The most frequent complaint in farmers was low back pain(27.16%) the second was knee joint pain(16.14%), the third was shoulder Pain(12.06%), and the fourth was headache-dizziness(9.59%). The frequency of musculoskeletal disease(64.48%) was most high, and cardiovascular disease(11.33%) was the second, gstrointestinal disease(10.73%) was the third, respiratory disease(3.42%) was the fourth. It is necessary to pay attention to musculoskeletal disease-especially low back pain and knee joint pain- in study and treatment of farmer's disease.

Ultrasound-guided epidural block in axial spondyloarthritis patients with limited spine mobility: a randomized controlled trial

  • Background: Evaluation of the effectiveness of caudal epidural injection on pain, spine mobility, disease activity, and activity of daily living in axial spondyloarthritis (SpA) patients. Methods: A total sample of 47 patients were registered in this study. They were randomly assigned into 2 groups; Group I received caudal epidural injections, ultrasound-guided, with 1% lidocaine hydrochloride mixed with triamcinolone, whereas Group II did not receive any injections. All participants fulfilled the ASAS criteria for axial SpA. Outcome measures were as follows: visual analogue scale, Oswestry disability index (ODI), modified Schober test, lateral lumbar flexion, and Ankylosing Spondylitis Disease Activity Score (ASDAS) with assessment at baseline, 2 weeks, and 8 weeks post-treatment. This clinical trial was registered on clinicaltrials.gov under the number NCT04143165. Results: There was a significant difference between both groups regarding pain, ODI, spine mobility and ASDAS scores in favor of group I. This effect was at its maximum after 2 weeks. Despite the decline of this effect after 2 months, the difference between the groups remained significant. Higher disease activity, younger age, and shorter disease duration were associated with better outcomes. Conclusions: Epidural injection of lidocaine and triamcinolone is a cost effective and a practical technique for controlling pain, as well as improving the function of the spine and disease activity scores in axial SpA patients with acceptable complications and relatively sustained effect.

Continuous Stellate Ganglion Block for Raynaud'S Disease -A case report- (Catheter를 이용한 지속적 성상신경절 차단 경험 -증례 보고-)

  • Stellate ganglion block has been used to treat diseases such as peripheral vascular disease, sympathetic dystrophy, and various pain syndromes involving the head or arm. Raynaud's disease is a syndrome manifested by attacks of pallor, cyanosis, numbness and pain of the digits in response to cold or emotional change. I report one case who was given Stellate ganglion block using 18G teflon Catheter(4.5 cm in length) for Raynaud's disease. Continuous stellate ganglion block is more convinient to inpatient than repeated needle punctures and may reduce major complications and more useful to patient who needs continuous sympathetic block about one week duration.

A Case Report of Korean Medicine Treatment on Back Pain of a Parkinson's Disease Patient (파킨슨병 환자의 요통에 대한 한의학 치료 증례)

  • The purpose of the study is to explore the possibility of Korean medicine treatment on back pain of Parkinson's disease patient through a case study. A Parkinson's disease patient suffering from back pain and both limbs pain was hospitalized and treated with acupuncture and traditional medicine. We evaluated the treatment effect with Numerial Rate Scale (NRS), Oswestry low back pain disability questionnaire (ODI), EuroQol-Five Dimensions (EQ-5D), and Unified Parkinson's Disease Rating Scale (UPDRS). As a result, back pain NRS, lower limbs pain NRS, ODI, EQ-5D, UPDRS turned out to be greatly improved from 9 to 3, 5 to 2, 80 to 64, -0.014 to 0.513, 122 to 37, respectively. This study suggested that Korean Medicine could be effective treatment for back pain of Parkinson's Disease. Further clinical studies are needed to obtain definite conclusion.

A study on the Theory of 'Ja-Yeol(刺熱)' in 32nd Chapter of 'So Moon(素問) Yellow Emperior's Nei-Ching(黃帝內經)' (황제내경(黃帝內經) 소문(素問) 자열론(刺熱論)에 대한 연구(硏究))

  • In this thesis, I intend to study the translational and clinical interpretation through the theory of Ja-Yeol, and reached the following conclusions. 1. Liver-Heat-Disease due to absess of the function of expelling and lifting off, that Liver-Yang cannot lift up to upper-warmer, and stagnate liver. I think the symptoms of yellowish urine, abdominal pain, somnolence, fever belong to the syndrome of 'Gi-Bun(氣分)', and the symptoms of ravings with surprising, distending pain of hypochondrium, restless involuntary movement of the limbs, unable to lie flat belong to the syndrome of 'Hyeol-Bun(血分)'. 2. Heart-Heat-Disease due that 'Eum-Gi(陰氣)' in heart cannot lay down and reach to stagnate at heart, inner part. I think the symptoms of unjoy, acute cardiac pain, fidgetiness, well-nausea, headeche, reddish face, anhidrosis, etc. reveal with Heart-Heat-Disease. 3. Spleen-Beat-Disease due that 'Eum-Gi' in spleen cannot lay down and Yin of spleen changs heat. I think the symptoms of heaviness of head, cheek pain, fidgetiness, cyanosis, well-nausea, fever, not to let flex and reflex with back pain, diarrhea with abdominal pain, left and right cheek pain reveal with Spleen-Heat-Disease. I think symptoms of fever, diarrhea with abdominal pain belong to the syndrome of Yin-exhausion. 4. Lung-Heat-Disease due to that 'Eum-Gi' in lung cannot lay down. When 'Wi-Gi(衛氣)' stagnates at external part, I think, the symptoms of intolerance to wind and cold, yellowish fur, fever reveal. When Wi-Gi stagnates at lung, inner part, I think, the symptoms of dispnea with cough, pain on chest and back, unable to breath deeply, hydrosis and chilling reveal. 5. Kidney-Heat-Disease, in that the symptoms of back pain, leg aching, extreme thirst and frequently drink, fever, pain and stiffness of nape, cooling and aching leg, heat on plantar pedis, not trying to speak reveal is regarded external heat disease of 'Tai-Yang-Gyeong's(太陽經)' disease that asthenic fever open 'Tai-Yang-Gyeong' and lift by not enough of 'Yang-Gi(陽氣)' lifeing up from Kidney space, the water space of five elements.
