Objective : Upper lumbar disc herniation is rare disease, compared with lower. The lamina of this high level lumbar vertebra is narrower than that of low level, and this have taken surgeon into important consideration for surgical methods because partial removal of lamina for discectomy weakens the base of the articular process and may result in fracture. The authors an accurate preoperative diagnosis that enables the surgeon to operative approach for preserving the facet joint. Methods : Thirteen patients with upper lumbar disc herniation have underone surgical procedure by midline approach for removal of ruptured disc fragment and paraspinal approach for removal of residual disc materials simultaneously without instrumentation. All patients who underwent surgery were analyzed and long-term follow-up was conducted. Results : At a mean follow-up of 24months, there were complete resolution of presenting radiating leg pain in 85% of the patients, 7.5% were left with minimal residual discomfort, and 7.5% derived little or no benefit from surgery. The follow-up radiologic findings of all patients shows that lamina and facet joint have preserved safely and no instability. Conclusion : Simultaneously, paraspinal with midline approach provides highly satisfactory operating methods by simplifying exposure and greatly limiting the risk of complications. This provides the basis for a planned surgical approach in which destruction of the facet joint can be avoided.
Complementary and alternative medicine has gained popularity and respectability in recent years in the United States. Since aging is often associated with chronic health conditions that commonly lead to physical and psychosocial disabilities (e.g., depression, functional and/or cognitive disabilities, and decreased quality of life), older adults often seek options to maintain health and treat chronic conditions as an adjunct to conventional medical care. Herbal products, the most commonly used among various complementary and alternative medicines (CAM), should be used with caution due to potential herbal-drug interactions (related to polypharmacy) and herbal-disease interactions (related to comorbidities). Five of the most common chronic conditions in older adults are chronic pain, cardiovascular problems, hypertension, diabetes, and chronic lung problems. A high rate of falls or risk of falling is also a problem unique to this older population. For these conditions, only a few types of CAM (e.g., acupuncture, qi gong, tai chi) were tested, with promising results. However, in spite of evidence supporting the use of certain types of CAM to alleviate some common chronic conditions, findings are limited in terms of other types of CAM tested and both short and long-term effects. More rigorous clinical trials of various CAM types are thus warranted to advance scientific knowledge and establish evidence-based practices to care for the growing number of older adults who deserve to have a better quality of life.

Traumatic bone cyst (TBC) occurs preferentially on the mandibular symphysis and body, but rarely on the mandibular condyle. When TBC occurs in the condylar area, it can usually be related with or misdiagnosed as a temporomandibular joint disorder. A 15-year-old female patient visited the Temporomandibular Joint Clinic with a 5-year history of pain and noise localized in the left temporomandibular joint. On imaging, a well demarked oval-shaped radiolucent lesion was observed on the left condyle head. The patient underwent cyst enucleation and repositioning of the bony window on the lateral cortex of the affected condyle head under the impression of subchondral cyst or TBC; however, no cystic membrane was found. The bone defect resolved and showed no recurrence on the serial radiographic postoperative follow-up for 43 months after surgery.

Organizational growth is achieved through the process of finding opportunities in the environment and establishing a business model with internal and external resources. Bestian Hospital, which primarily focuses on saving the lives of patients with severe burns, has enlarged its business domain through deep understanding of burn patients' problems, including pain and complications during treatments, long-term treatments, skin reconstruction, and so on. Now Bestian is accelerating research for development of antipyretics and cosmetics for burn patients. The success of Bestian has been due to utilization of human and material resources that are essential to performance in the medical field. Also, Bestian properly used a management service organization(MSO) model and constructed an information technology(IT) system for supporting its businesses. However, previous successes do not guarantee continued success. Bestian is entering a new domain with different challenges than it has experienced so far, and how it deals with these challenges will decide its future.

Objectives : There have been many reports that show that Korean Medical Treatment of ALS patients can maintain or improve their major and minor symptoms, but this has not been reviewed in literature. The aim of this study is to review studies of ALS patients treated with Korean Medicine. Methods : A search was completed of studies containing word ALS in English and in Korean from KIOM OASIS. Results : There were no cases of ALS patients that were fully cured. However with Korean Medical treatment, there were articles that reported improvement or maintenance of major symptoms, recovery of minor symptoms like pain, chest discomfort, insomnia, cough, sputum, etc, and success in the long term care of ALS patients. Conclusions : Although the current study has limitations, our findings support the potential use and effectiveness of Korean Medical Treatment for ALS patients.
Purpose: We evaluated the complications and failures after total ankle arthroplasty during at least 2 years short term follow up. Materials and Methods: There were 45 cases of 42 patients of HINTEGRA$^{{R}}$ (Newdeal SA, Lyon, France) model from November 2004 to August 2006. Follow up averaged at least 2 year. We evaluated the complications and analyzed the cause of the failures. Results: There were totally 15 cases of complication, 5 cases of medial impingement syndrome, 3 cases of varus malposition, 2 cases of delayed healing of wounds, and each one case of deep peroneal nerve problem, medial malleolus fracture, post-operative deep infection, gouty arthritis pain, and Achilles tendinitis. Conclusion: Total ankle arthroplasty had higher complication rate than any other joint arthroplasty, so we need a more meticulous preoperative and peri-operative care.

Objectives : We present a successful administration of acupuncture in alleviating persistant peripheral neuropathy due to bortezomib, a potent therapeutic agent of mutlple myeloma. Methods : A patient, experiencing numbness, coldness, tingling and weakness in her feet and hands after bortezomib treatment, was administered acupuncture for 32 weeks, 3 times/week for first 4 weeks and 2 times/week the last 28 weeks, in bilateral acupuncture points, ST36, ST40, EX-LE10, LI11, TE5, and EX-UE9. Responses were assessed at the end of every 4 weeks with Eastern Cooperative Oncology Group(ECOG) grading system, National Cancer Institute - Common Toxicity Criteria(NCIC-CTC) v4.0, Numeric Rating Scale(NRS) 0-10, and Neuropathy Pain Scale (NPS). Results : ECOG was improved from 2 to 1, NCIC-CTC from 2 to 1, NRS from 8 to 1, and NPS from 41 to 5 through 32 weeks of acupuncture treatment. Conclusions : It is conceivable that acupuncture can be a help to relieve bortezomib induced peripheral neuropathy.
Objectives : The aim of this study was to report the cases of knee injury that are treated with both CPM and Korean medicine. Methods : We treated 3 patients who had knee joint pain and gait disturbance by knee injuries with Korean medicine and CPM treatment. We evaluated the outcomes by checking NRS, ROM, Lysholm knee score, walking state and other symptoms. Results : The NRS, ROM, Lysholm knee score, and other symptoms of 3 patients were improved before they. These results show possibilities of simultaneous treatment of Korean medicine and CPM. But there is a limit that the number of cases is not sufficient, and there was no long-term follow-up after discharge. Conclusions : Simultaneous treatment of Korean medicine and CPM has an effect on knee joint rehabilitation. Further studies are needed to prove the effectiveness of Korean medicine combined with CPM.
Capsaicin. a vanillyl amide(8-methy1-N-vanilly1-6-nonenamide) with a molecular weight of 305.42, was substance, interrupting the pain conducting pathway Until recently the neurotoxic effects of Capsaicin to adult animals were thought to be limited to the peripheral nervous system. But several reports suggest the possibility of central nervous system changes after Capsaicin administration to the adult rat. Capsaicin desensitization is defined as long lasting, reversible suppression of sensory neuron activity. How fast and for how long the desensitization develops is related to the dose and time of exposure to Capsaicin, and the interval between consecutive dosing. In the long term Capsaicin treatment can lead to morphological degeneration and changes in some small sensory neurons, predominantly unmyelinated C fiber afferent nerve fibers. Clinical interest has recently been roused by evidence that Capsaicin's desensitizing action may be of therapeutic value and that an endogenous Capsaicin-1 ike substance may exist. This study summarizes the fundamental knowledge(mechanism, receptors, et al of Capsaicin) of Capsaicin for physical therapists.
Abdominal cystic lymphangiomas arising from the mesentery, omentum and retroperitoneum are rare and occasionally confused with other cause of acute abdomen. Sixteen children who underwent surgery for abdominal cystic lymphangioma between 1984 and 2005 at the Division of Pediatric Surgery, Keimyung University Dongsan Medical Center were evaluated retrospectively. There were 9 boys and 7 girls. Age ranged from 12 days to 13 years (mean age: 4.7years). The cysts were located in the omentum (4 cases),mesentery (9 cases: jejunum: 3, colon: 6). retroperitoneum (2 cases) and pelvic cavity (one case). The cyst content was hemorrhagic (8 cases), serous (5 cases), and inflammatory (2 cases), and chyle (one). Prenatal diagnosis was made in 3. The clinical features were variable, but abdominal pain, mass, and abdominal distention in order. There were 3 emergency operations in patients with complicated cyst, who were suspected of having panperitonitis and volvulus preoperatively. Complete excision was accomplished in all cases. There was one mortalityin a newborn with E.coli sepsis. Intestinal obstruction developed in 2 cases in long-term follow-up. No recurrence was observed. Although intraabdominal lymphangioma arising from mesentery, omentum and retroperitoneum are rare, it should be considered as a possible cause of acute abdomen.
