Fixation of Epidural Catheters using blood Set -Report of 120 cases- (지속성 경막외카테터의 거치 방법 -수혈선을 이용하여-)

  • Secure fixation is essential for continous epidural catheterization on a long-term-basis. Adhesive tape or surgical knots were commonly used for those patients, but the surgical knot method has a tendency to cause strangulation of catheter. Another invasive technique, subcutaneous tissue tunnelling is more safe than other methods but requires sophisticated technique and time. We employed a simple device using a blood transfusion set for patients who have epidural catheters placed safely un their backs. In 120 patients treated for postoperative and chronic pain by means of this technique, the results were as follows: 1) Five of 120 patients (4.2%) developed backache and pruritus, but there were no instances of respiratory depression, local infection and headache. 2) Nine of 120 patients (7.5%) failed booster-injections, but two cases were due to be non-technical errors. 3) The duration of fixation was 1~3 days in most cases (85%), the longest being for 21 days.

Effects of Tai Chi Exercise and Auricular Acupressure in Patients with Rheumatoid Arthritis (류마티스 관절염환자를 위한 타이치운동과 이압요법의 효과 - 영국 옥크셔지역을 중심으로 -)

  • Purpose: The purpose of this study was to evaluate the effects of a program of Tai Chi and auricular acupressure in patients with rheumatoid arthritis. Methods: The study design was a one-group pretest-posttest design. The subjects were 14 outpatients with a diagnosis of rheumatoid arthritis based on the American College of Rheumatology Standards. The setting was a Academic Section of Musculoskeletal Disease in the Leeds Institute of Molecular Medicine in United Kingdom. The program was Tai Chi exercise with auricular acupressure per two times a week, for twelve weeks. Results: A program of Tai Chi with auricular acupressure showed significant improvements in amount of pain of RADAI, 28 Joint count swollen, fatigue, physical, affect and symptom of AIMS2, function and symptom of ASES (p<0.05). However, no differences were found in the score of self-efficacy relate to the pain, and quality of healthy related life to social interaction. Conclusion: In order to improve the quality of healthy related life and self-efficacy relate to the pain, it requires long-term intervention. Also different research designs are needed to confirm the effect of this study.

Acupuncture for Endometriosis; A Systemic Review (자궁내막증의 침치료 효과에 대한 체계적 문헌 고찰)

  • Objectives: This study was aimed to review the effect of acupuncture on endometriosis related pain in patients. Methods: Researchers searched data through 6 online databases up to November 2020. The data was limited to randomized controlled trial studies on endometriosis patients diagnosed with laparoscopy. Results: 7 Randomized controlled trials were included in this study. All of studies were published in Chinese, in China. Intervention of treatment group were composed of simple acupuncture and electroacupuncture. Intervention of control group were made up with mifepristone, herbal medicine and acupuncture. Outcome measurements were effective rate, serum Cancer antigen-125 (CA-125), Visual analogue scale (VAS)/pain score, and recurrence rate in 1 year. In all outcome measurements, treatment group were more effective than control group and it was statistically significant. Conclusions: Acupuncture therapy is effective in decreasing endometriosis related pain and preventing recurrence. However, because endometriosis is easy to recur, additional long-term research is needed.

THE BIPOLAR SMOULDER PROSTHESIS LONGER TERM RESULTS (5-10 YEARS) IN THE MANAGEMENT OF PRIMARY GLENOHUMERAL OSTEOARTHRITIS

  • This is a prospective study in which we evaluate the 5-10 year results of BiPolar shoulder arthroplasty in 64 patients (71 shoulders) with primary glenohumeral osteoarthritis. Fifty two patients (56 shoulders) were followed for greater than 60 months (average 79months)1 and no patients were lost to follow Lip. The average age of the patient at operation was 72.5 years. The UCLA score increased from 10.8 preoperatively to 25,7 postoperatively. The final Constant score in this elderly subset of patients averaged $65\%$ (unadjusted). Eighty seven percent of patients were satisfied with their final result. Excellent pain relief was achieved with a VAS of 2.5 (0=no pain, 15=excruciating pain). Active anterior forward flexion improved from $45^{\circ}$ to $104^{\circ}$. Seventy five percent of patients reveal persisting head-shell motion at an average of 7 years. There were two reoperations because of humeral stem loosening; both stems should have been cemented at the initial arthroplasty. It is demonstrated that BiPolar shoulder arthroplasty is durable over time, with clinical results equivalent to that in the literature when compared with hemiarthroplasty and total shoulder replacement.

Sclerotherapy of Peritoneal Inclusion Cysts: Preliminary Results in Seven Patients

  • Objective: To evaluate the technical feasibility and the clinical effectiveness of sclerotherapy for the treatment of peritoneal inclusion cysts (PICs). Materials and Methods: Between June 1996 and February 2001, eight PICs in seven female patients aged 28-43 (mean, 36) years were instilled with sclerosant (povidone-iodine in three, ethanol in three, both povidone-iodine and ethanol in one). All seven patients subsequently experienced less abdominal pain. After drainage via an 8.5-Fr pigtail catheter inserted in the PICs (transabdominally in six cases, transvaginally in one), sclerosant equivalent in volume to about one-third that of drained fluid was introduced daily until the drained volume was less than 5ml. Follow-up by means of clinical procedures and ultrasound was performed every three months, at which time the success rate, possible complications and recurrence were determined. Results: Sclerotherapy was technically successful in all seven patients, though immediately after the procedure, minor complications were noted in three patients (mild pain in two, mild fever in one). During the follow-up of 4-60 (mean, 24.7) months, sclerotherapy proved successful and without long-term complications in all seven patients: lower abdominal pain disappeared and the diameter of the cysts decreased more than 50%, with complete regression in four cases. During the follow-up period there was no recurrence. Conclusion: Sclerotherapy following catheter insertion is technically feasible and effective for the treatment of PICs.

  • Objectives : This study aims to examine the cause, symptoms, and treatment of Shiji in Korean Medicine, to establish foundation for applied research in the future. Methods : Contents on Shiji in the Donguibogam were selected and categorized according to cause, symptom and treatment. Medical significance and its relationship with contemporary diagnosis were examined. Results : Shiji is caused by weakness in the Spleen and Stomach, overeating, consumption of raw or cold food, external contraction, emotional damage, etc., while gastrointestinal symptoms such as stomach pain and diarrhea and other symptoms such as cold damage, phlegm, cough, back pain, flank pain are triggered. For treatment of gastrointestinal symptoms, Pingweisan is applied as basis, while for other symptoms, Erchentang supplemented with digestion stimulating medicinals such as Shanzhazhi, Shenqu, Maiya, etc., is applied. Conclusions : Shiji is related to conditions such as IBS, gastroesophageal reflux, functional indigestion, food-related skin allergies, polyps, etc. Shiji not only refers to stagnated food within the body, but should be understood as a broad term that includes reactive dysfunction of the nervous system caused by poor food hygiene and gastrointestinal hypofunction.

The Relationship between Temporomandibular Disorders(TMD) and Ear symptoms (측두하악장애와 귀 증상과의 관계)

  • Temporomandibular disorders(TMD) have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory muscles, and associated structures. Since Dr. James Costen has mentioned about the relationship between TMD and posterior bite collapse, there have been many controversies about the etiology and comorbidities of TMD. The purpose of this study was to investigate the relationship between TMD and hearing acuity in 158 volunteers with or without TMD symptoms. The subjects were examined clinically about TMJ sound, missing teeth, tinnitus, TMJ pain and masticatory muscle pain and the hearing acuity were measured by Audiometer(ITO AE-1000, Japan). The result of this study indicated that there was no significant differences between TMD symptoms and hearing acuity relatively.

  • In general, treatment of the patients with closed lock divides into a conservative and a surgical treatment. Surgical treatment has been often applied in case that occlusal splint therapy was not effective on the patient with closed lock. In recent, some clinicians reported good results with mandibular manipulation. Three patients complained limitation of month opening-(mean mouth opening was 22.3mm) and TMJ pain. Articular discs were displaced anteriorly on MRI. Two patients didn't improve the symptoms with long term occlusal splint therapy. We applied mandibular manipulation after injection with 2% lidocaine into the upper joint space of the affected TMJ and directly inserted occlusal splint to all patients. At the follow-up check, mean mouth opening was 41.7mm. TMJ pain decreased, condyle and disc relationship was improved functionally on MRI.

Case Report : Treatment of Disc Displacement without Reduction Patient Using ARS (전방재위치 교합장치를 이용한 비정복성 관절원판변위 환자의 치험례)

  • The treatment of anterior disc displacement without reduction patients needs speicific treatment methods different from other Craniomandibular Disorders. Those are manual manipulation, anterior repositioning splint; ARS and step-back. It is well known that the use of Anterior Repositioning Splint is effective on relief of TMJ symptom. But the side effect of long- term ARS wearing, which is irreversible posterior open bite induction, has made many clinicians avoid ARS treatment. This report introduces a clinical case recently proven part- time wearing ARS method for treatment of Disc Displacement without reduction patient' that can reduce side effects with good efficacy. But now clinical statistical studies and basical histophysiological studies are more needed.
