Primary Fibromyalgia Syndrome 환자의 치험례 -증례 보고- (Treatment of Primary Fibromyalgia Syndrome Patient -A case report-)

  • Fibromyalgia syndrome is defined as a diffuse, aching musculoskeletal pain associated with multiple and discrete predictable tender points along with stiffness. A primary form associated without any disease is uncommon compared to the secondary diffuse fibrositis. We witnessed a case of a primary fibromyalgia syndrome. A 28-year-old woman was suffering from pain on posterior neck, right shoulder, upper back, left hand and arm for two years. She also complained of morning stiffness, fatigue, and headache. We treated her with stellate ganglion block with 0.25% bupivacaine, medication that included amitriptyline, TENS and superlizer. We also recommended aerobic exercise.

Fibromyalgia: practical considerations for oral health care providers

  • Fibromyalgia is a syndrome characterized by chronic pain in the skeletal system accompanied by stiffness, sleep disturbance, fatigue, and psychiatric problems, such as anxiety and depression. Fibromyalgia commonly affects orofacial health, presenting with a variety of oral manifestations, including temporomandibular disorder, xerostomia, glossodynia, and dysgeusia. Therefore, oral healthcare providers need to be aware of this clinical entity to effectively manage oral symptoms and provide proper oral self-care modification and education on the nature of fibromyalgia. This review focuses on the epidemiology, pathophysiology, clinical manifestation, diagnosis, orofacial concerns, and treatment of fibromyalgia.

A Case of Postural Orthostatic Tachycardia Syndrome Associated with Migraine and Fibromyalgia

  • Postural orthostatic tachycardia syndrome (POTS) refers to the presence of orthostatic intolerance with a heart rate (HR) increment of 30 beats per minute (bpm) or an absolute HR of 120 bpm or more. There are sporadic reports of the autonomic nervous system dysfunction in migraine and fibromyalgia. We report a case of POTS associated with migraine and fibromyalgia. The patient was managed with multidisciplinary therapies involving medication, education, and exercise which resulted in symptomatic improvement. We also review the literature on the association between POTS, migraine, and fibromyalgia.

섬유조직염 환자의 가족지지, 지식, 자기효능감, 증후 및 신체활동장애 간의 관계분석 (Relational Analysis between Family support, Knowledge, Self-efficacy, Syndrome and Physical disability of Fibromyalgia Patients)

  • This study was conducted to identify the relations between the factors influencing syndrome and physical disability of fibromyalgia patients. The subjects of this study were 125 outpatients selected at H. Rheumatism clinic from February to July in 1997, The Collected data were analyzed by Frequency, Pearson correlation using SPSS Windows and Path analysis using LISREL 7.16. The results were as follows. 1. General characteristics of subjects About 2/3 of subjects were over 40years of age and most of them were graduated from high school. The Christian were over a half (54.0%) of subjects. 2. Correlation between Family support, knowledge, Self-efficacy, Syndrome and physical disability. As knowledge was positively related to self-efficacy(r=.306), the family support was positively related to self-efficacy(r=.217), and family support was negatively to syndrome. Self-efficacy showed negative relation with syndrome and Physical disability. Syndrome as physical disability represent positive relation. Therefore all hypotheses were supported. 3. Causal relations between research variables. Family support influenced on syndrome, as syndrome on physical activity, as well as knowledge on self-efficacy & physical disability. The model also showed a good fit to the data[$X^2$=1.21 df=1(p=.546), GFI=.996, AGFI=.971, RMSR=.023]. 4. Correlation between symptoms of Fibromyalyia. Having analyzed the relations between tender point, pain, stiffness, functional disturbance, sleep disturbance, depression, fatigue, anxiety, the results were as follows ; All relations show statistically positive correlation, which means close relationship between symptoms, except between pain and depression, pain and anxiety. As a result, symptoms of Fibromyalgia had close relation. Therefore, these were common, rather than individual symptoms. This syndrome influenced on physical disability, and self-efficacy on the both, as knowledge on self-efficacy. In conclusion, to improve syndrome relief and physical disability of fibromyalgia, nursing interventions such as education, self-efficacy increasing stratagy, and family counseling are needed.

한의학적 치료로 호전된 불면과 우울을 동반한 섬유근통 증후군 환자 치험 1례 (Fibromyalgia Syndrome Combined with Insomnia and Depression Disorder Managed with Korean Medical Treatment: A Case Report)

  • Objectives The study aimed to evaluate the therapeutic effect of Hyungtonghaeul-tang along with Korean medicine treatment for fibromyalgia syndrome. Methods A patient with fibromyalgia syndrome was treated with Hyungtonghaeul-tang as well as Korean medical treatment including acupuncture, electro-acupuncture, bee venom therapy, chuna, aromatherapy, and physical therapy. The counts of tenderness points, widespread pain index (WPI), symptom severity scale score (SS scale score), digital infrared thermal imaging (DITI), Ryodoraku diagnosis, visual analogue scale (VAS), fibromyalgia impact questionnaire (FIQ), and beck depression inventory (BDI) were measured. Results After treatment, the counts of tenderness point, WPI, SS scale score, VAS, FIQ, and BDI were decreased. The DITI and the Ryodoraku pattern recovered normally. Conclusions Hyungtonghaeul-tang along with Korean medicine treatment can be used in the treatment of fibromyalgia syndrome.

섬유근통 증후군 환자에서 Somatomedin C (Insulin-Like Growth Factor 1)의 농도와 임상증상과의 관계 (A Controlled Study on Serum Insulin-Like Growth Factor 1 (Somatomedin C) Levels in Fibromyalgia)

  • Background: Fibromyalgia is a common syndrome of musculoskeletal pain and fatigue. Lacking distinctive histological or laboratory abnormality in diagnosis, it has often been considered a form of "psychogenic rheumatism". Fibromyalgia causes much distress to the affected patients and often frustrates physicians, who are unable to start rational therapy on any logical disease pathology. Methods: Growth hormone is essential for muscular homeostasis. In the present study, the notion that the stage-4 sleep anomaly typically seen in the fibromyalgia syndrome may disrupt growth hormone secretion was tested. Because growth hormone has a very short half-life, serum levels of somatomedin C were measured; somatomedin C is the major mediator of growth hormone's anabolic actions and is a prerequisite for normal muscle homeostasis. Serum levels of somatomedin C using acid-extraction procedure and two-site immunoradiome-tric assay (IRMA) and number of tender points were measured in 27 female patients with fibromyalgia from 40 to 60 years old and 27 healthy controls. Results: There were no differences in the concentration of somatomedin C between fibromyalgia patients and controls ($mean{\pm}SD$: $178.3{\pm}75.5$ ng/ml versus $166.3{\pm}76.6$ ng/ml; p=0.55). And there were no correlations between number of tender point and serum somatomedin C level by linear regression analysis. Conclusions: These findings did not support that there is a distinctive disruption of the growth hormone-somatomedin C neuroendocrine axis in a fibromyalgia syndrome. But we can not discard the hypothesis that disturbed sleep predispose to muscle pain.

한방 복합치료로 호전된 섬유근통 증후군 치료 1례 (One Case Report of Fibromyalgia Syndrome(FMS) Patient Improved by Korean Medical Combined Treatment)

  • Objectives : The purpose of this study is to evaluate the effect of oriental medical combined treatment on the fibromyalgia syndrome patient. Methods : Oriental medical combined treatment was administered during 3 weeks at 2 times admissions. The improvement of clinical symptom was evaluated by VAS(visual analogue scale), SF-MPQ(short form-McGill pain questionnarie) and PRS(pain rating scale), change of tender points. Results : After treatment, the patient's VAS score was reduced to 3-5, 4-5 point. First admission, there is no observable change in the SF-MPQ. But, Second admission, SF-MPQ score was reduced from 16 to 6. The PRS score was reduced from 72 to 24, from 75 to 48 respectively. Tender points was reduced generally. Conclusions : Oriental medical combined treatment for the fibromyalgia syndrome patient was effective. Although this case presented valuable result, further research is encouraged to confirm the effectiveness of this treatment with large number of patient.

섬유조직염 환자의 우울에 미치는 변인 (The Factors Influencing on Depression of Patients for Fibromyalgia Syndrome)

  • Purpose: The purpose of this study is to understand the depression of patients for Fibromyalgia Syndrome(FMS) and to identify the factors influencing depression. Method: The instruments used here are Beck Depression Inventory in depression, the Korean Rheumatology Health Association' instruments in Self-Efficacy. Also, Pain and Fatigue was measured by Visual Graphic Rating Scale. The subject of study is 76 outpatients diagnosing FMS from rheumatism specialists at C hospital in D city. The data has been collected from Sep. 1st to Sep. 30th in 2001. For the analysis of collected data, frequency analysis, independent t-test, analysis of variance, Pearson's correlation and multiple regression analysis were used for statistical analysis with SAS statistical program. Result: General characteristics showing statistically significant difference in depression were age, education, occupation, gender, exercise and sleep in the patients with FMS. Depression for the patients with FMS has negative correlation coefficients with Self-efficacy and ADL, and positive correlation coefficients with Pain and Fatigue. The suitable regression form resulting from the multiple regression analysis to investigate the influencing factors of depression for the partients with FMS was expressed by y =50.067 - 0.278x$_1$ + 1.320x$_2$ (x$_1$: Self-Efficacy x$_2$: Fatigue) and $R^2$ =0.427. Conclusion: The factors influencing on depression of patients for FMS was Self-Efficacy, ADL, Pain, and Fatigue. Further study needs to be done identify methods of overcoming and presentation of depression in FMS.

  • The majority of patients who present with muscle pain does not have a definable myopathic disorder in the usual sense, despite intensive evaluation. Currently, most of these individuals are diagnosed with the syndrome of fibromyalgia, a condition that has been discussed under a number of different names for approximately a century. Fibromyalgia is a form of nonarticular rheumatism characterized by widespread musculoskeletal aching and stiffness, as well as tenderness on palpation at charateristic sites, called tender points. The American College of Rheumatology in 1990 set criteria for the diagnosis of fibromyalgia as that includes at least 3 months of widespread pain and the presence of at least 11 of 18 specific tender points on examination.

섬유근통 환자의 한의학적 치료 효과 증례보고 (The Clinical Study on a Case of Fibromyalgia Syndrome Patient by Korean Medicine Treatment)

  • The purpose of this study is to report a clinical progress of fibromyalgia syndrom patient treated by Korean Medicine treatment. The patient was treated with acupuncture, acupotomy, pharmacopuncture (Bee venom, Hwangryun, Hominis Placental), herbal medicine, moxibustion, cupping treatment. We measured Visual Analogue Scale (VAS), American College of Rheumatology Preliminary Diagnostic Criteria (ACR, 2010), Fibromyalgia Impact Qustionnaire (FIQ), and observed the change in body temperature using Digital Infrared Thermal Imaging (DITI). After Korean Medicine treatment, pain was controlled and VAS, ACR, FIQ were decreased. The result showed that Korean Medicine treatment is effective in fibromyalgia syndrome.
