Acute Systemic Infusion of Bupropion Decrease Formalin Induced Pain Behavior in Rat

  • Background: The chronic pain can disturb physical, psychological, and social performances. Analgesic agents are widely used but some antidepressants (ADs) showed analgesia also. Bupropion is using for smoke cessation but it can change morphine withdrawal signs such as pain. This study tested the acute systemic effect of bupropion on formalin induced pain behavior in rats. Methods: Wistar male healthy rats were divided into 7 groups (control, sham, and 5 treated groups with 10, 30, 90, 120, and 200 mg/kg of bupropion, i.p.). The bupropion injected 3 hours prior to formalin induced pain behavior. Formalin (50 ${\mu}l$, 2.5%) was injected subcutaneously in dorsal region of right hindpaw in all animals. Nociceptive signs were observed continuously on-line and off-line each minute. Common pain scoring was used for pain assessment. Results: The analysis of data by one-way ANOVA showed that bupropion can reduce pain scores in the second phase but not in first phase. Bupropion decreased the licking/biting duration significantly in first and second phase of formalin test. Conclusions: The results showed that bupropion has analgesic effects at systemic application. The change of second phase of the pain behavior was significant and it revealed that central mechanisms involve in bupropion analgesia.

  • The reciprocal interaction between sleep and pain has been reported by numerous studies. Patients with acute or chronic pain often complain of difficulty falling asleep, frequent awakenings, shorter sleep duration, unrefreshing sleep, and poor sleep quality in general. According to the majority of the experimental human studies, sleep deprivation may produce hyperalgesic changes. The selective disruption of slow wave sleep has shown this effect more consistently, while results after selective REM sleep deprivation remain unclear. Patients with chronic pain have a marked alteration of sleep structure and continuity, such as frequent sleep-stage shifts, increased nocturnal awakenings, decreased slow wave sleep (SWS), decreased rapid eye movement (REM) sleep, and alpha-delta sleep. Many analgesic medications can alter sleep architecture in a manner similar to the effects of acute and chronic pain, suppressing SWS and REM sleep.

  • In emergency rooms, patients with acute chest pain should be diagnosed as quickly as possible with higher diagnostic accuracy for an appropriate therapy to the patients with acute coronary syndrome or for avoiding unnecessary hospital admissions. At present, electrocardiography(ECG) and biochemical markers are generally used to detect myocardial infarction and coronary angiography is used as a gold standard to reveal the degree of narrowing of coronary artery. Magnetocardiography(MCG) has been proposed as a novel and non-invasive diagnostic tool fur the detection of cardiac electrical abnormality associated with myocardial ischemia. In this study, we examined whether the MCG can be used fur the detection of coronary artery disease(CAD) in patients, who were admitted to the emergency room with acute chest pain. MCG was recorded from 36 patients admitted to the emergency room with suspected acute coronary syndrome. The MCG recordings were obtained using a 64-channel SQUID MCG system in a magnetically shielded room. In result, presence of CAD could be found with a sensitivity of 88.2 % in patients with acute chest pain without 57 elevation in ECG, demonstrating a possible use in the emergency room to screen CAD patients.

대상포진 환자 치험 1례 (A Case Report on Herpes Zoster)

  • Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.

치료용 레이저를 이용한 통증치료 경험 (The Clinical Experiences with Laser Therapy in Pain Patients)

  • Two hundred patients with acute and chronic pain were treated with a low power laser and 115 patients among them were divided into several groups by their pathology and evaluated their response rate to the laser therapy was evaluated through follow-up study. 1) The ages of patients were between the early twenties and late sixties, and there was no differences between sexes. 2) Degenerative spondylosis and chronic lumbar sprain were the most common diseases among those patients. 3) The average duration of therapy was about 16 days and response to the therapy appeared from the fourth day of laser therapy. 4) Acute lumbar sprain and acute spinal compression fracture showed rapid response to laser therapy. 5) The spinal pathology group was the most common at 37.5% of cases and the response rate to laser therapy was the lowest at 58.7%. 6) The articular pathology group occupied 24.6% and the response rate was the highest at 81.3%. 7). The response rate of the posttraumatic and postsurgical pathology group was 76.5%. 8) The response rate of the tendinous and sports pathology group was 75%. 9) The response rate of the miscellaneous group was 66.7%. 10) The mean response rate of all patients was 71.6%.

Effects of Progressive Scapular Stabilization Exercise on Neck, Muscle Strength, Upper Extremity Function in Patients with Acute Whiplash Injury

  • Objective: The purpose of this study was to investigate the effects of a combination of progressive scapular stabilization exercises, neck and thoracic spine mobilization, and thermoelectric physical therapy on pain, range of motion, muscle strength, and function in patients with acute whiplash injury. Design: A randomized controlled trial design. Methods: A total of 24 subjects were included in this study. They were randomly assigned to an experimental group (n=12) that performed scapular stabilization exercises, neck and thoracic spine mobilization, and physiotherapy, and a control group (n=12) that only performed neck and thoracic spine mobilization and physiotherapy. The pre-test was conducted before the intervention and consisted of a total of 12 treatment sessions of 60 minutes each, three times a week. A posttest was conducted 4 weeks later. Pain, range of motion, muscle strength, and function were assessed before and after intervention. Results: The results of the study showed that there was a significant difference in pain reduction and range of motion and muscle strength improvement in the experimental group, and a significant difference was also found between the experimental group and the control group in terms of functional evaluation. Conclusions: The combination of exercise therapy and joint mobilization technique and physical therapy resulted in greater improvements in pain, range of motion, strength, and function assessment, contributing to improved overall function.

추나요법으로 호전된 급성 경항통을 동반한 경추 만곡이상에 대한 증례보고 5례 (Five Case Reports on Acute Neck Pain Patients with Abnormal Cervical Curvature Using Chuna Manual Therapy)

  • Objectives This case study aimed to investigate efficacy of Chuna manual therapy (CMT) on acute neck pain patients with abnormal cervical curvature. Methods CMT was administered to five patients with abnoraml cervical curvature in the Department of Korean Medicine Rehabilitation, College of Korean Medicine, Semyung University Korean Hospital from June 2018 to May 2023. Cervical angle, Jackson's angle, Jochumsen method, Ishihara index and visual analog scale (VAS) scores were evaluated before and after treatment. Results In cases 1, 2, 3, 4, and 5, the cervical angle, Jackson's angle, Jochumsen method, Ishihara index all increased, while VAS scores decreased after CMT. Conclusions The observed improvements suggest that CMT is effective in restoring the cervical curvature and alleviating neck pain in patients with acute neck pain.

교통사고로 유발된 급성 요통 환자의 후관절 소염약침 치료에 대한 초음파 유도하군과 비유도군의 효과 비교: 후향적 연구 (Comparison the Soyeom Pharmacopuncture Therapy Effects of Ultrasound Guided Group and Unguided Group on the Patients' Facet Joint with Acute Low Back Pain Caused by Traffic Accidents: A Retrospective Study)

  • Objectives This study aims to compare the effect of an ultrasound on a guided group and an unguided group on Soyeom pharmacopuncture therapy on the facet joint in patients with acute low back pain caused by traffic accidents. Methods 21 patients with acute low back pain caused by traffic accidents from March 1, 2021 to May 31, 2021 were included in this study. The study was conducted as a retrospective study which analyzes the patient's medical records. 11 patients (Group A) received ultrasound guided Soyeom pharmacopuncture therapy and 10 patients (Group B) received unguided Soyeom pharmacopuncture therapy on the facet joint. Visual analogue scale (VAS) and Oswestry disability index (ODI) was used to evaluate improvements in functions and pain, and five point Likert scale to evaluate patient's satisfaction. Results Both groups showed a statistically significant decrease in the VAS and ODI on the 5th day of hospitalization. However, there was no statistically significant difference between the groups. Difference in the Likert scale between the groups was not statistically significant either. Conclusions We found that ultrasound guided Soyeom pharmacopuncture therapy on the facet joint showed similar efficacy compared with unguided Soyeom pharmacopuncture therapy at facet joint on acute low back pain patients caused by traffic accidents.
