아로마 요법이 편도선 절제술 환자의 통증에 미치는 영향 (Effects of Aroma Inhalation Therapy on Pain in Patients Following a Tonsillectomy)

  • Purpose: This study was done to examine the effects of aroma inhalation therapy on pain post tonsillectomy. Method: A non-equivalent control group pre- and post-test design was used. The research instruments used in this study were a pain perception measurement and vital signs (systolic & diastolic blood pressure and pulse rate levels). Twenty-five patients in the experimental group went through aroma inhalation therapy for ten minutes after the tonsillectomy. The aroma inhalation therapy used was a blended oil, a mixture of Lavender and Loman chamomile in the ratio of 2:1. The 25 patients in the control group did not receive the therapy. Before and after the experiment, both groups were tested for pain (pain perception and vital signs). Collected data were processed with the SPSS WIN. Ver. 14.0 program and analyzed using frequencies, percentages, ${\chi}^2$-test, Fisher's exact test, t-test and ANCOVA. Results: Pain perception of patients was not significantly reduced. However systolic and diastolic blood pressure and pulse rate were significantly reduced. Conclusion: The results of the study indicate that aroma inhalation is a partially effective nursing intervention to reduce the post-operative tonsillectomy pain.

The outcome of epiduroscopy treatment in patients with chronic low back pain and radicular pain, operated or non-operated for lumbar disc herniation: a retrospective study in 88 patients

  • Background: Patients with lumbar disc herniation are treated with physiotherapy/medication and some with surgery. However, even after technically successful surgery some develop a failed back syndrome with persistent pain. Our aim was to evaluate the efficacy of epiduroscopy in patients who suffer chronic low back pain and/or radicular pain with or without surgery and the gender difference in outcome. Methods: A total of 88 patients were included with a mean age of 52 years (27-82), 54 women and 34 men. 66 of them were operated previously and 22 were non-operated. They all had persistent chronic back pain and radicular pain despite of medication and physical rehabilitation. Visual Analog Scale (VAS) for pain and Oswestry Disability Index (ODI) were evaluated preoperatively, after one month, six months and one-year after the epiduroscopy. Results: All patients, and also the subgroups (gender and operated/non-operated) improved significantly in pain (VAS) and disability (ODI) at one month. A significant improvement was also seen at one year. No differences were found between men and woman at the different follow-up times. A slight worsening in VAS and ODI was noticed over time except for the non-operated group. Conclusions: Epiduroscopy helps to improve the back and leg pain due to lumbar disc herniation in the early stage. At one year an improvement still exists, and the non-operated group seems to benefit most of the procedure.

Usefulness of four commonly used neuropathic pain screening questionnaires in patients with chronic low back pain: a cross-sectional study

  • Background: Recently symptoms-based screening questionnaires have gained attention for screening for a neuropathic pain component (NePC) in various chronic pain conditions. The present study assessed the usefulness of four commonly used NePC screening questionnaires including the Self-completed douleur neuropathique 4 (S-DN4), the ID Pain, the painDETECT questionnaire (PDQ), and the Self-completed Leeds Assessment of neuropathic Symptoms and Signs (S-LANSS) questionnaire in patients with chronic low back pain (CLBP) to assess the presence of NePC. Methods: This is a single-center cross-sectional study where patients with CLBP, with or without leg pain, were included. Participants were initially screened for NePC presence by a physician according to the regular practice, and later assessed using screening questionnaires. The diagnostic accuracy of these questionnaires was compared assuming the physician-made diagnosis as the gold standard. Results: A total of 215 patients with CLBP of which 164 (76.3%, 95% CI, 70.2-81.5) had a NePC were included. S-DN4, ID Pain, and PDQ have an area under the curve (AUC) > 0.8 indicating excellent discrimination. However, S-LANSS has an AUC of 0.69 (0.62-0.75), indicating low discrimination. S-DN4 has a significantly higher AUC as compared to ID Pain (d(AUC) = 0.063, P < 0.01) and S-LANSS (d(AUC) = 0.197, P < 0.01). But the AUC of S-DN4 does not significantly differ from that of PDQ (d(AUC) = 0.013, P = 0.62). Conclusions: S-DN4, ID Pain, and PDQ, but not S-LANSS, have good discriminant validity to screen for NePCs in patients with CLBP. Despite using all the tests, 20-30% of patients with an NePC were missed. Thus, these questionnaires can only be used as an initial clue in screening for NePCs, but do not replace clinical judgment.

Efficacy of transforaminal laser annuloplasty versus intradiscal radiofrequency annuloplasty for discogenic low back pain

  • Background: Discogenic pain is a common cause of disability and is assumed to be a major cause of non-specific low back pain. Various treatment methods have been used for the treatment of discogenic pain. This study was conducted to compare the therapeutic success of radiofrequency (an intradiscal procedure) and laser annuloplasty (both an intradiscal and extradiscal procedure). Methods: This single-center study included 80 patients and followed them for 6 months. Transforaminal laser annuloplasty (TFLA, 37 patients) or intradiscal radiofrequency annuloplasty (IDRA, 43 patients) was performed. The main outcomes included pain scores, determined by the numeric rating scale (NRS), and Oswestry disability index (ODI), at pre-treatment and at post-treatment months 1 and 6. Results: The patients were grouped according to procedure. In all procedures, NRS and ODI scores were significantly decreased over time. Mean post-treatment pain scores at months 1 and 6 were significantly lower (P < 0.01) in both groups, and between-group differences were not significant. The ODI score was also significantly decreased compared with baseline. Among patients undergoing TFLA, 70.3% (n = 26) reported pain relief (NRS scores < 50% of baseline) at post-treatment 6 months, vs. 58.1% (n = 25) of those undergoing IDRA. There were no statistically significant differences between the groups in ODI reduction of > 40%. Conclusions: Our results indicate that annuloplasty is a reasonable treatment option for carefully selected patients with lower back and radicular pain of discogenic origin, and TFLA might be superior to IDRA in patients with discogenic low back pain.

Neuropathic pain feature in cancer-induced bone pain: does it matter? a prospective observational study

  • Background: Cancer-induced bone pain (CIBP) is considered to have both nociceptive and neuropathic components. However, the prevalence, risk factors, and impact of the neuropathic components are yet poorly understood. Methods: We estimate the prevalence of neuropathic pain (NP) features in patients with CIBP at a tertiary care pain clinic setting using the Douleur Neuropathique 4 questionnaire and evaluate their associated factors and their impact after 4 weeks of treatment using the Brief Pain Inventory questionnaire and the Edmonton Symptom Assessment System. Results: A total of 133 patients were recruited. The estimated prevalence of NP was 30.8% (95% confidence interval: 23.6%-39.1%). Initially, the patients with NP had significantly higher average pain scores (6.00 vs. 5.05, P = 0.006), higher total interference scores (5.84 vs. 4.89, P = 0.033), and symptom distress scores (35.88 vs. 26.52, P = 0.002). After 4 weeks of treatment, patients in both groups reported significantly decreased pain intensity and improved quality of life. However, the patients with NP still reported significantly higher average pain (4.61 vs. 3.58, P = 0.048), trending toward higher total interference scores (3.52 vs. 2.99, P = 0.426), and symptom distress scores (23.30 vs. 20.77, P = 0.524). From multivariate analysis, the independent risk factors for NP were younger age, pain in the extremities, and higher average pain scores. Conclusions: NP are common in patients with CIBP. These conditions negatively affect pain intensity and the patient's quality of life before and after treatment.

견부손상환자의 팔의 자세에 따른 초음파 영상을 이용한 견봉하공간 거리의 비교 (Comparison of Distance of Subacromial Space Using Ultrasonographic Measurement on Arm Positions of Shoulder Injured Patients)

  • Purpose : Recently ultrasound images has been used to measure the subacromial space with a linear measurement of the acromiohumeral distance. The purpose of this study was to comparison of distance of subacromial space using ultrasonographic measurement on arm positions of shoulder injured patients. Methods : The subjects were 30 shoulder injured patients (19 males and 11 females). Ultrasonography was performed to measure subacromial space during each shoulder resting position, $45^{\circ}$ abduction, internal rotation with $45^{\circ}$ abduction and external rotation with $45^{\circ}$ abduction. Results : Subacromial space was a significantly difference between injured arm and normal arm at shoulder resting position. At $45^{\circ}$ abduction, subacromial space was narrowing significantly between injured arm and normal arm. At external rotation with $45^{\circ}$ abduction, subacromial space were wider in normal arm than in injured arm but it was not significantly. Intra-observer reliability for ultrasonography measurement of subacromial distance was excellent (.96~.99). Conclusion : These results identified that positions of injured shoulder was related to subacromion space.

필라테스 운동이 여중생의 요통과 체간 등속성 근력 및 평형성에 미치는 영향 (Effects of Pilates Exercises on Isokinetic Trunk Strength and Balance in Female Middle School Students with Lumbago)

  • Purpose: The purpose of this research was to investigate the effects of performing Pilates exercises for eight weeks on the isokinetic trunk strength and balance of female middle school students with lumbago. Methods: Twenty-four female students who met the test requirements were recruited and randomly divided into a control group (CG, n = 16) and a Pilates exercise group (Pilates group; PG, n = 8). The PG performed a Pilates program, which consisted of lumbar muscle strengthening exercises, for 60 minutes three times a week for eight weeks. Data analysis was conducted by two-way repeated ANOVA, and a Bonferroni test was carried out when significant differences appeared. The alpha level was set at 0.05. Results: Following the experimental treatment, the PG showed an increased trunk flexion peak torque of 17% and 13% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased about 31% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Following the experimental treatment, the PG showed an increased trunk extensor peak torque of approximately 29% and 21% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased approximately 27% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Both the CG and the PG showed a 20% and 50% decrease in pain index after the experimental treatment, respectively. After the experimental treatment, the PG had about 50% lower pain than the CG. The average error rate of static balance in the PG reduced by 19% from 3.28±0.45 to 2.65±0.36, confirming a significant improvement. Conclusion: Regular Pilates exercise for eight weeks improved the isokinetic trunk muscle strength of female middle school students with lumbago and relieved their pain. Pilates was also shown to be effective in improving balance.

일개 병원 간호사의 암환자 통증관리에 대한 지식과 태도 (Nurses' Knowledge about and Attitude toward Cancer Pain Management: A Survey from Korean Cancer Pain Management Project)

  • Purpose: This study was conducted to examine the nurses' knowledge about and attitude toward cancer pain management. Method: Study subjects of this retrospective cross-sectional survey study were 343 nurses sampled conveniently in a tertiary teaching hospital in Seoul. Seven measurement tools were used to collect data on demographic data, knowledge about and attitude toward cancer pain management, barrier to cancer pain management, knowledge about and concerns for the opioid use, and professional education of cancer pain management. Result: Nurses showed knowledge deficit when their knowledge on the cancer pain management and the opiod use was evaluated using two case scenarios and the 15-item questionnaire respectively. Ninety-five percent of the nurses believed that cancer pain management is a major problem. However, inadequate staff knowledge of pain management was rated as the single most important barrier to adequate pain management by 20.4% of nurses. Only 10.9% and 23.2% of the nurses replied that nursing school's education and professional education in cancer pain management respectively, were adequate. Conclusion: This study confirms the existence of knowledge deficits and attitudinal barriers among Korean nurses that can impede cancer pain management. This study suggests a need for professional education for nurses on cancer pain management.

족삼리(足三里), 합곡(合谷) 단일(單一) 및 배혈(配穴) 침자(鍼刺)가 상치통(上齒痛) 유발(誘發) 백서(白鼠)의 dEMG에 미치는 영향(影響) (The Effect of LI4, ST36 & LI4+ST36 acupuncture on Rat Jaw Opening Reflex by Upper Incisor Pulp Stimulation)

  • Objectives : The pain was induced on upper and lower incisor of the rat based on the theory of 'connections of upper incisor pain with stomach meridian and lower incisor pain with large intestine meridian'. Such acupoints as LI4 and ST36 were used for alleviation of upper and lower incisor pain. Methods : The digastric myogram (dEMG) was utilized for the pain measurement. Results : The ST36 acupuncture after induction of upper incisor pain was gradually decreased or increased the dEMG. The LI4 acupuncture after induction of upper incisor pain was gradually decreased the dEMG. The ST36+LI4 acupuncture after induction of upper incisor pain was gradually decreased the dEMG. We knew this thing which the ST36+LI4 acupuncture decreased the dEMG most greatly and kept long compared to ST36 acupuncture, LI4 acupuncture. Conclusions : Acupuncture treatment at the loci of not only ST36 acupuncture but LI4 acupuncture were relieved the upper incisor pain. It was well suitable to the theory 'connections of upper incisor pain with stomach meridian' that ST36 acupuncture decreased the upper incisor pain. But there was not to decrease the dEMG for the ST36 acupuncture. We are considered as tracing study continuously about ST36 acupuncture. It was not suitable to the theory 'connections of upper incisor pain with stomach meridian' that LI4 acupuncture decreased the upper incisor pain. These results was considered as the function by the characteristic of the LI4 acupoint.

  • The pain was induced on upper and lower incisor of the rat based on the theory of 'connections of upper incisor pain with stomach meridian and lower incisor pain with large intestine meridian'. Such acupoints as ST36 and LI4 were used for alleviation of upper and lower incisor pain. The digastric myogram (dEMG) was utilized for the pain measurement. The upper incisor pain was gradually decreased during ST36 stimulation and significant differences were observed at 20 and 25 minute point during 60 minutes study. The upper incisor pain was gradually decreased during LI4 acupuncture and 20 minute was the only point that showed a significant difference. The alleviation of lower incisor pain was not obvious during the ST36 acupuncture. The lower incisor pain was gradually decreased during LI4 stimulation with significant differences at 15, 20 and 40 minute point. In conclusion, the upper incisor pain was relieved with ST36 and LI4 acupuncture while LI4 was effective on the lower incisor pain alleviation based on the theory of 'connections of upper incisor pain with stomach meridian and lower incisor pain with large intestine meridian'.
