Psychological Characteristics of Infant Outpatients and their Parents in Korean Traditional Clinic (한의원 내원 환아 및 보호자의 정신건강 특성 연구)

  • Objectives: Since Korean traditional medicine has an integrative perspective by its nature, the patients' psychological and physical problems in a Korean traditional clinic may be resolved through a more integrative approach. The purpose of this study was to describe the latent characteristics of psychological health of child outpatients and their parents visiting Korean traditional clinic with physical complaints and was to propose an alternative curriculum to meet their needs based on their psychological and physical aspects. Methods: Subjects studied consisted of 100 outpatients (59 boys, 41 girls, mean age 55 months, age range 18 to 83 months) and their parents. Behavior characteristics for children were measured by Korean version of Child Behavior Checklist for Ages 1.5-5 (CBCL1.5-5) while parenting stress, depression, and anxiety for parent were measured by Korean-Parenting Stress Index-Short Form (K-PSI-SF), Center for Epidemiologic Studies Depression Scale (CES-D) and State-Trait Anxiety Inventory (STAI), respectively. The data of subjects were compared to those of standardized groups and then were compared according to child's gender within sample. Results: Child participants were reported to have significantly higher scores of all CBCL 1.5-5 subscales than the standardized group. When compared, no differences of K-PSI-SF, CES-D, and STAI was found between parents of participants and the standardized groups. However, parents of child participants with at-risk psychological problems showed significantly higher scores of all parenting stress subscales than those of all child participants. In addition, the mothers of boy participants demonstrated significantly higher anxiety, depression and parenting stress than those of girl participants irrespective of psychological severity. Conclusions: We discussed the implication of these results in clinical situation and make recommendations for curriculum of psychiatry and pediatrics with the aim of improving proper diagnosis, consulting and treatment.

Factors affecting the health-related quality of life of children with cerebral palsy in Indonesia: a cross-sectional study

  • Purpose: Children with cerebral palsy (CP) and their parents experience various problems that can affect their quality of life. This study examined factors affecting the quality of life of children with CP. Methods: A cross-sectional study was conducted in Yogyakarta, Indonesia, from January to August 2019. The participants were consecutively recruited children with CP aged 2 to 18 years and their parents. Ninety-eight children with CP and their parents, specifically their mothers, were recruited. Children's health-related quality of life (HRQoL) was measured using the Pediatrics Quality of Life Cerebral Palsy. Parental HRQoL and stress were measured using the WHOQOL-BREF and Parenting Stress Index (PSI). Results: Functional level V was the most common category for both Gross Motor Function Classification System (GMFCS) and Bimanual Fine Motor Function (BFMF) (35% and 28%, respectively). Children's mean HRQoL was medium (49.81±20.35). The mean total PSI score was high (94.93±17.02), and 64% of parents experienced severe stress. Bivariate analysis showed that GMFCS, BFMF, number of comorbidities, presence of pain, and parental stress were significantly correlated with the total score for children's HRQoL (p<.05). Multiple linear regression analysis (p<.05) demonstrated that more severe GMFCS and parental stress were associated with lower mean HRQoL scores in children. Conclusion: Factors including the level of GMFCS and parental stress affected the HRQoL of children with CP. Parental stress management should be included in the comprehensive management of these children.

Augmentative Effects of Working Memory Training on Behavioral Problems and Parental Stress in Medicated Children and Adolescents with Attention-Deficit Hyperactivity Disorder (약물치료 중인 주의력결핍 과잉행동장애 아동 청소년에서 작업기억훈련이 행동문제와 부모 스트레스에 미치는 부가적 효과)

  • Objectives: Executive dysfunctions including working memory deficit have been suggested to be one of the major neuropsychological etiologies of attention-deficit hyperactivity disorder (ADHD). The purpose of this study was to investigate the augmentative effects of working memory training on the behavioral problems, quality of life, and parental stress of medicated children with ADHD. Methods: Twenty-five children with ADHD, aged 9 to 19 years, who were being treated with ADHD medication, were included. The participants were trained with a commercially available and computerized working memory program (Cogmed(R)) for 5 weeks without any alteration of their medication. The Child Behavior Checklist (CBCL), KIDSCREEN-52 quality of life measure, and Parenting Stress Index-Short Form (PSI-SF) were administered before training, and 4 weeks and 7 months after training, respectively. Results: After completing the training, the anxiety/depression, social problems, thought problems, attention problems, aggressive behavior, and externalizing problems scores in the CBCL were significantly reduced. The score on the Parent-child dysfunctional interaction in the PSI-SF was also decreased. However, the scores related to the quality of life were not changed. These changes were still observed 7 months after the training. Conclusion: Cogmed working memory training can be a promising training option for the additional improvement of behavioral problems and parental stress in medicated children with ADHD.

Patterns of Infant-Mother Attachment and Related Variables (영아-어머니간의 애착유형과 그 관련변인)

  • The objectives of this study were to investigate (1) the relations between infant-mother attachment and maternal sensitivity, maltreatment, stress, and childhood experience, and (2) relations between infant-mother attachment and infant temperament. The subjects of the study were 55 14 to 20 month-old infants (27 boys and 28 girls) and their mothers in Seoul. In order to assess the patterns of infant-mother attachment, each infant-mother dyad was videotaped in the modified Ainsworth's Strange Situation. Each dyad was filmed for 3 minutes in the Questionnaire Situation of Smith and Pederson(Smith, & Pederson, 1988) to assess maternal sensitivity responding to infant's cues. Each mother also was interviewed by using a semi-structured questionnaire made by author to measure maternal maltreatment. Each mother was asked to complete three Likert-type questionnaires, containing Parenting Stress Index (PSI) (Abidin, 1990) to measure the maternal stress, Mother-Father Peer Scale (MFPS) (Epstein, 1983) to measure childhood experience, and Emotionality, Activity, Sociality (EAS) (Buss, & Plomin, 1984) to measure infant's temperament. The statistical procedures used for data analyses were correlation, one-way ANOVA, multiple regression, and Cronbach's α coefficient. The results showed that (1) mothers of insecure-avoidant infants maltreated their infants more than mothers of secure infants, and (2) in the multiple regression analysis, maternal maltreatment was predicted by maternal education, maternal stress (parent domain), and maternal childhood experience in relation to her own mother (acceptance vs. rejection).

An Analysis of Structural Relationships of the Cognitive and Communication Ability Related Variables on Infants (영아기 인지·의사소통 발달 관련요인 간의 구조적 관계 분석)

  • This study investigated the structural relationships among infant's cognitive and communication ability, temperament, teacher-infants interaction, and mothers' parenting stress. Subjects used in this study were 249 infnats whose mean age of 30.21 months, and who were attended in preschool in S city. Research instruments used in collecting data were 'K-ASQ(Korean Ages and Stages Questionnaires)', 'EAS(Emotionality, Activity, and Sociability-Temperament Survey for Children-Parental Ratings)', 'PSI/SF(Parenting Stress Index/ Short Form)', 'ECOI'. The collected data were analyzed through SPSS 18.0 and AMOS 18.0 program. Pearson's correlation and Structural equation model analysis were used to analyze the data. Findings are as follows. First, there were positive correlations between infant's cognitive and communication ability with social-ability temperament, teacher-infants interaction. There were negative correlations between infant's cognitive and communication ability with infant's emotionality temperament, mothers' parenting stress. Second, social-ability temperament directly affected disposition toward cognitive ability, but in-direct effect was not significant. Emotionality temperament partially mediated the effects of parenting stress toward cognitive ability. Third, social ability directly affected disposition toward language ability, but in-direct effect was not significant. There has no significant path of temperament to teacher-child interaction, but teacher-child interaction has direct effect to infant's cognitive ability. Finally, all path about infant's communication ability were not significant.

The Effects of a Group music psychotherapy program on Parenting Stress, and Self-Efficacy in Mothers of Children with Disabilities (그룹 음악심리치료 프로그램이 장애아동 어머니의 양육 스트레스와 자기효능감에 미치는 영향)

  • Mothers of Children with Disabilities have many difficulties in parenting because of their children's disabilities. The purpose of this study was to examine the effects of a Group music psychotherapy programs on parenting stress and self efficacy, and to explore the experiences of the mothers who participated in the program. Twenty mothers of children with disabilities participated in this study. These twenty mothers were divided into 10 mothers of each group. The Experimental group participated in the Group music psychotherapy programs, while the control group did not. The Group music psychotherapy program was implemented for 8 weeks. To prove the effects of the program, pretest-posttest control group design was applied. The differences between pre-test and post-test of two groups were analyzed using independent t-test. The quantitative results of the study were as follows: First, a significant differences was found between the experimental group and control group in parenting stress level. Second, a significant differences was found between the experimental group and control group in self-efficacy level. From the qualitative analyses, four themes emerged from the decrement of parenting stress; Positive change of recognition about their children, Getting rid of stress, Keeping their composure, and Sharing of their emotions. And three themes emerged from the increment of self-efficacy; Discovery of oneself, New stimulus, and Mental support. All the result described above shows that group music psychotherapy program gives positive effect to decrease in parenting stress and also to increase in self-efficacy. And new possibility and effectiveness can be proposed in the music therapeutic approach to the parents of children with disabilities.

Relationship between Parental Stress and Leisure Constrains of Parents who have Children with Disabilities (장애아동 부모의 양육스트레스와 여가제약 분석)

  • The purpose of this study was to examine how parental stress coming from breeding children with disabilities affected their leisure constraints. The participants of this study were parents with mentally retarded children who were involved in physical activities in 6 places; 4 special schools for the children in Seoul and Kyunggi province, a physical education class in a university, a physical education class with a university and a private physical education class. We surveyed the parents selected through purposive sampling and data from total 355 participants involved in the survey were analysed. For this, frequency analysis, exploratory factor analysis, reliability analysis, one-way ANOVA and multiple regression analysis were used. The outcome of this study showed that there was not meaningful difference in breeding stress depending on the types of the disabilities. However the result presented meaningful difference in stress from children adaptation according to the degree of the disabilities. Moreover, the difference were found in some factors such as children's sex and family income. As a result, the finding indicated that parental stress had an effect on the limitation of parents' leisure ability.
