The Comparative Study of Effectiveness of Cotreatment with $Samgieum-gagam$, Acupuncture and Sweet Bee Venom Acupuncture on Post-stroke Shoulder Pain (삼기음가감방과 봉약침, 체침의 병행치료가 뇌졸중 후 견관절 통증에 미치는 효과 비교연구)

  • Objectives : The purpose of this study is to evaluate the effect of cotreatment with $Samgieum-gagam$, acupuncture and sweet bee venom acupuncture on post-stroke shoulder pain. Methods : The subjects in this study were 43 patients with post-stroke hemiplegic shoulder pain. These patients were randomly divided into four groups : the group treated with acupuncture, the group cotreated with acupuncture and sweet bee venom acupuncture, the group cotreated with acupuncture and $Samgieum-gagam$, the group cotreated with acupuncture, sweet bee venom acupuncture and Samgieumgagam, They were treated for 4 weeks, and the effectiveness was assessed by visual analogue scale(VAS), Fugl-Meyer motor assessment(FMMA), painless passive ROM of shoulder external rotation(PROM), Modified Ashworth sacle(MAS) and Satisfaction. Results : All groups showed significant change in VAS, FMMA and PROM. The group cotreated with acupuncture, sweet bee venom acupuncture and $Samgieum-gagam$ showed more effectiveness in VAS and FMMA than the group treated with acupuncture. There was no significant difference in MAS among groups. Conclusions : This study suggests that cotreatment with $Samgieum-gagam$, acupuncture and sweet bee venom acupuncture is more effective than acupuncture treatment only on post-stroke shoulder pain. Further study based on many other combination methods, larger population, and long term follow-up is needed to confirm this suggestion.

Effect of Cupping Therapy on Range of Motion, Pain Threshold, and Muscle Activity of the Hamstring Muscle Compared to Passive Stretching

  • PURPOSE: Flexibility and range of motion are very important factors in sports performance, rehabilitation, and musculoskeletal pain. The purpose of this study was to measure the effects of cupping therapy on flexibility, muscle activity, and pain threshold of hamstring muscle compared to passive stretching in healthy subjects. METHODS: Thirty healthy subjects were randomly assigned in a crossover design to cupping therapy and passive stretching. Subjects were tested to compare their effects according to the intervention such as Passive range of motion (PROM) (straight leg raising) and active range of motion (AROM). And algometer (pain) testing and MVC assessment using EMG were performed as dependent variables. RESULTS: The cupping therapy group and passive stretching group showed significant differences in all variables including PROM (p=.00, p=.00), AROM (p=.00, p=.03), Pain Threshold (p=.03, p=.08), Semitendinosus MVC (p=.01, p=.00), and Biceps femoris MVC (p=.01, p=.16). There were no significant differences between the two groups in all variables. CONCLUSION: These findings of this study suggested that cupping therapy has as much positive effect on flexibility, pain threshold, and muscle contraction as passive stretching. Also, it is more convenient and easier to work on patients than passive stretching. Therefore, cupping therapy should be considered as another option to treat range of motion, pain, and muscle activity in the clinical field.

Effects of Lower Extremity Strengthening Exercise Combined with Balance Exercise on Lower Extremity Function, Range of Motion, Muscle Strength, and Balance in Patients with Knee Osteoarthritis (균형운동을 동반한 하지 근력강화운동이 슬관절골관절염 환자의 하지 기능수준과 관절가동범위, 근력, 균형 수준에 미치는 영향)

  • PURPOSE: Balance exercise as well as lower extremity strengthening exercise (LESE) is known to be effective in patients with knee osteoarthritis (KOA). The purpose of this study was to investigate the effectiveness of performing LESE in conjunction with balance exercise on lower extremity function, range of motion, muscle strength, and balance in patients with KOA. METHODS: The subjects of this study were 25 patients with KOA who were recruited and randomly divided into two groups: 1) those who performed LESE with balance exercise; and 2) those who performed only LESE. Both the groups also received general physical therapy and performed aerobic exercise. The interventions were performed 3 times a week for 4 weeks. To determine the effectiveness of the interventions, we measured Western Ontario and MacMaster Universities Arthritis Index (WOMAC) score, numerical rating scale (NRS) score, passive range of motion (PROM), chair stand test (CST), and Berg Balance Scale (BBS) score at the initiation of the interventions and again after 4 weeks, at the time of completion of the interventions. RESULTS: After 4 weeks of the interventions, both the groups showed significantly improved WOMAC (p<.01), NRS (p<.01), PROM (p<.05), CST (p<.05), and BBS (p<.01) scores. However, there was no significant difference between the groups in terms of the clinical outcomes observed. CONCLUSION: These results suggest that the addition of balance exercise to a LESE regimen in patients with KOA did not provide any additional benefit.

Intelligent Lighting Development having Auto-Address in DMX512 Protocol (DMX512 프로토콜에서 자동주소를 갖는 지능형 조명 단말기 개발)

  • The lighting industry has become the bigger and more systemized using the semiconductor LED with each control R(Red), G(Green), B(Blue). The communication standard DMX512(ANSI E1.11) protocol of it basically needs the address which can be distinguished between the lighting terminals. The conventional method has to give the address of the lighting any methods like dip switch, EEROM and PROM. Then the lighting terminal can receive RGB data from DMX512 communication. According to electrical characteristics DMX512 protocol has to the splitter every 32 lighting terminals. If 512 lightings is all connected, maximum 16 splitters are needed. This paper is solved above weakness in DMX512 protocol using serial connection all lighting terminals. Also I developed the intelligent lighting terminal with auto-addressing. This was solved the inconvenience of the address assignment and the usage of the splitter. The developed products is applied and selled the semiconductor lighting terminals using ON-OFF control, dimming control, sequential control and rainbow control.

Clinical Observation on Delivery of Low Birth Weight Unfant (저출생 체중아 분마에 대한 임상적 고찰)

  • A clinical study was made on 365 low birth weight infant and 406 normal birth weight infant who had been born at Kangnam St. mary's Hospital during past 3 years from Jan. 1, 1995 to Dec. 31, 1997. the data of this study were gathered through reviewing of medical records. 1. Comparison of general characteristic with of obstetric characteristic 1) Old maternal age, previous abortion and previous LBWI delivery in the group of low birth weight infant(LBWI) mother were more prevalent than those in the group of normal birth weight infant(NBWI)mother 2) Cesarean section, abnormal presentation and multiple pregnancy in the group of LBWI mother were prevalent than those in the group of NBWI mother. 3) regular antenartal care and visiting rate of tertiary hospital in the group of LBWI mother were more prevalent than those in the group of NBWI mother. 2. Frequency of low birth weight infant 1) Anmual average frequency of LBWI was 6.5% and monthly frequency was the highest in January and december. 2) The frequency of LBWI was the highest in 37-40wks of gestational age and was the highest in 2251-2500 gm of birth weight. 3) The frequency of congenital anomaly in the group of LBWI was more prevalent than that of NBWI. 3. Mortality rate of LBWI The mortality rate of LBWI was 9.2%. The highest mortality rate was noted before 27wks of gestational age, less than 1000gm of birth weight and within 12hrs of delivery. 4. The most common complication of pregnant women was pre-term labor, the most complication relating to placenta was premature rupture of membrane(PROM) and the most fetal complication was fetal distress in delivered LBWI. 5. Significant relating factors of low birth weight infant delivery were associated with maternal age, previous delivery, previous low birth weight delivery, pre-eclampsia, anemia, oligohydramnios, PROM, placenta previa, abruptio placenta, fetal sex, fetal distress and congenital anomaly.

  • In this paper, we design a entorpy encoder of HDTV/DTV encoder blocks on the basis of MPEG-II. The designed entropy encoder outputs its bit stream at 9Mbps bit rate inserting zero-stepping block to protect the depletion of buffer in case that the generated bit stream is stored in buffer and uses not only PROM bit combinational circuit to solve the problem of critical path, and packer block, one of submerge, is designed to packing into 24 bit unit using barrel shifter, and it is constructed to blocks of header information encoder, input information delay, submerge, and buffer control. Designed circuits is verified by VHDL function simulation, as a result of performing P&R with Gate compiler that apply $0.8{\mu}m$ Gate Array specification, pin and gate number of total circuits has been tested to each 235 and about 120,000.

Effects of Modified Mulligan Technique accompanied by Taping on the Scapular Posture and Shoulder Range of Motion of Stroke Patients (테이핑을 동반한 수정된 멀리건 기법 적용이 뇌졸중 환자의 견갑골 자세와 견관절 가동범위에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of between modified mulligan technique and modified mulligan technique with taping on the active range of motion & passive range of motion, scapula index into the stroke patients. Methods: The subjects with stroke were randomly divided into two groups. Group 1 (n=9) was conducted modified mulligan technique and Group 2 (n=9) was conducted modified mulligan technique with taping week three times for 4weeks. Active range of motion (AROM), passive range of motion (PROM) and scapula index (SI) were measured by goniometer and tape measure. Wilcoxon signed-rank tests were used to compare differences before and after intervention. Mann-Whitney U-test were conducted to compare before to after intervention in the two groups. Results: AROM was significantly different both groups (p<.05) and between groups were not significantly different into pre and post intervention (p>.05). PROM was significantly different both groups (p<.05) however, between groups were not significantly different into pre and post intervention (p>.05). SI was significantly different only group 2 and between groups were not significantly different (p>.05). Conclusions: This study demonstrated effective bo-th modified mulligan technique and modified mulligan technique with taping on the active range of motion and passive range of motion. Because only modified mulligan technique with taping are effective on the scapula index we recommend modified mulligan technique with taping than modified mulligan technique.

A Case Report of Oriental Medical Treatments Combined with Exercise Therapy on the Frail Elderly Patient with Gait Disturbance and Cognitive Decline (노쇠 환자의 보행장애 및 인지 기능 저하에 대한 운동요법을 병행한 한방치료 증례보고)

  • The purpose of this study is to report the effect of oriental medical treatments with exercise therapy on the frail elderly patient with gait disturbance and cognitive decline. The frail elderly patient with gait disturbance and cognitive decline was treated by oriental medical treatments combined with exercise therapy during 3 months. The improvement of clinical symptom was evaluated by Manual Muscle Test (MMT), Passive Range Of Motion (PROM), Mini Mental State Examination-Korea (MMSE-K). After 3 months, all of both lower extremity Manual Muscle Test (MMT) improved to Grade 4. Both knee extension Passive Range Of Motion (PROM) and Mini Mental State Examination-Korea (MMSE-K) also increased from -30/-30 to 0/-5, from 7 to 25 respectively. It could be suggested that oriental medical treatments with exercise therapy were effective on the frail elderly patient with gait disturbance and cognitive decline. And further research is encouraged to confirm the effectiveness of this treatment with a larger number of patients.

Telecommand Decryption Verification for Engineering Qualification Model of Command Telemetry Unit in Communications Satellite (통신위성 원격측정명령처리기 성능검증모델 원격명령 암호복호 검증)

  • In this paper, the decryption function of CCSDS telecommand of CTU EQM for the security of communications satellite was verified. In order to intensify the security level of DES CFB decryption algorithm applied to CTU EM, 3DES CFB decryption algorithm using three keys is implemented in the CTU EQM. As the decryption keys increased due to the 3DES algorithm, the keys and IV are stored in PROM memory, and used for the telecommand decryption by taking the keys and IVs corresponding to the selected key and IV indexes from the memory. The operation of the 3DES CFB is validated through the timing simulation of 3DES CFB algorithm, and then the 3DES CFB core implemented on the A54SX32 FPGA. The test environment for the telecommand decryption verification of the CTU EQM was built up. Through sending and decrypting the encrypted command, monitoring the opcodes, and confirming LED on/off by executing the opcodes, the 3DES CFB telecommand decryption function of the CTU EQM is verified.

Immediate effect of self-myofascial release on hamstring flexibility

  • Objective: This study aimed to identify the area with greatest effect using self-myofascial release technique (self-MFR) in the hamstring, suboccipital, and plantar regions. Design: Cross-sectional study. Methods: Twenty-two adult subjects were evaluated for flexibility and hamstring pain threshold after self-MFR. Based on the superficial back line, the self-MFR application areas were the suboccipital region, hamstring, and plantar regions. Self-MFR was applied to each area using a wooden pole for a total of 4 minutes. Self-MFR was applied for 3 days at the same time of day, which was randomly assigned for each subject. Treatment was applied to one area each day. The sit and reach test (SRT), active range of motion (AROM), and passive ROM (PROM) were used to determine changes in flexibility, and an algometer was used to determine pain threshold. Pre/post-self-MFR effectiveness was tested using a paired t-test. Repeated measurement was used to compare self-MFR effects in the suboccipital, hamstring, and plantar regions. Results: When the self-MFR technique was applied to the 3 areas, the SRT showed significant improvement over baseline (p<0.05). Bilateral AROM and PROM showed significant improvements (p<0.05). When the self-MFR technique was applied to the hamstring, the semimembranosus showed a significant change in pain threshold (p<0.05). Conclusions: Our findings suggest that indirect application based on the Anatomy Trains could be effective for those who need to improve muscle flexibility. Moreover, self-MFR easily alleviates myofascial pain while maintaining flexibility, and can be performed at any time and place.
