  • 대부분의 가역적 후두부 뇌병증 증후군 (PRES)는 가역적인 경과를 보이나 출혈 또는 경색을 동반한 비가역적 경과들도 보고되고 있다. 전신성 홍반성 낭창 (SLE)과 관련된 PRES는 고혈압이나 면역억제제와 관련이 있다. 저자들은 최초 PRES발병 시 확산강조영상에서 확산제한을 보인 부위에서, PRESS의 반복 발생 시 출혈성 변환을 보인 증례를 보고한다.

  • Takayasu arteritis (TA) is a chronic inflammatory disease of unknown etiology that affects mainly the aorta, main aortic branches, and pulmonary arteries. Diverse neurological manifestations of TA have rarely been reported in children. Posterior reversible encephalopathy syndrome (PRES) is a neuroradiological condition that presents with headache, seizure, visual disturbances, and characteristic lesions on imaging. Inflammatory condition and severe hypertension in TA can cause PRES. We report of a 5-year-old girl with presumed TA who presented with PRES and chronic total occlusion in the renal artery. The findings on magnetic resonance imaging suggested PRES. Left nephrectomy was performed for total occlusion of the left renal artery, and the confirmatory diagnosis of TA was based on the pathologic findings of the renal artery.

아동 실어증 환아 증례 보고 (A Case Report of Child Aphasia)

  • Objectives : The purpose of this study is to report a case showed some clinical improvement in treating child aphasia with oriental medicine. Methods : The patient had some problems in communication, intelligence and behaviors due to aphasia after brain injury caused by epilepsy. We treated the patient with herbal medicine(Yangsintangkamibang), acupuncture, verbal remediation and intelligence-learning treatment. We observed the clinical progress of aphasia and evaluated the case with PRES. Results : Oriental medical treatment alleviated patient's symptoms of aphasia and PRES was also improved. Conclusions : This study shows oriental medicine can be a meaningful treatment for child aphasia, and more studies for oriental medical treatment for child aphasia is needed.

  • Posterior reversible encephalopathy syndrome (PRES) and reversible cerebral vasoconstriction syndrome (RCVS) are relatively uncommon neurological disorders. These two independent syndromes can be concurrent as a part of a continuum process; however, the specific mechanism is not well known. Although the relationship between RCVS and PRES is currently unclear, they could share a common pathophysiology. This case report aimed to determine the pathophysiology underlying the co-occurrence of PRES and RCVS in a patient with an acute exacerbation of chronic obstructive pulmonary disease.

Intestinal duplication revealed by posterior reversible encephalopathy syndrome

  • We report a unique case of intestinal duplication detected on posterior reversible encephalopathy syndrome (PRES) in a 13-year-old girl. She was admitted to the pediatric Emergency Department because of generalized seizures. Radiological assessment revealed a large, well-defined, thick-walled cystic lesion in the mid abdomen, suggestive of duplication cyst associated to a PRES. Exploration confirmed the diagnosis of ileal duplication cyst, and the mass was resected. The postoperative course was uneventful. Both hypertension and neurological dysfunction resolved after the mass resection. A follow-up brain magnetic resonance imaging was performed 9 months later and showed complete resolution of the cerebellar changes. Although extrinsic compression of the retroperitoneal structures has not been reported in the literature as a complication of duplication cyst, we strongly believe that this is the most logical and plausible hypothesis that would explain the pathogenesis of PRES in our patient.

Towards robust viscoelastic-plastic-damage material model with different hardenings/softenings capable of representing salient phenomena in seismic loading applications

  • This paper presents the physical formulation of a 1D material model suitable for seismic applications. It is written within the framework of thermodynamics with internal variables that is, especially, very efficient for the phenomenological representation of material behaviors at macroscale: those of the representative elementary volume. The model can reproduce the main characteristics observed for concrete, that is nonsymetric loading rate-dependent (viscoelasticity) behavior with appearance of permanent deformations and local hysteresis (continuum plasticity), stiffness degradation (continuum damage), cracking due to displacement localization (discrete plasticity or damage). The parameters have a clear physical meaning and can thus be easily identified. Although this point is not detailed in the paper, this material model is developed to be implemented in a finite element computer program. Therefore, for the benefit of the robustness of the numerical implementation, (i) linear state equations (no local iteration required) are defined whenever possible and (ii) the conditions in which the presented model can enter the generalized standard materials class - whose elements benefit from good global and local stability properties - are clearly established. To illustrate the capabilities of this model - among them for Earthquake Engineering applications - results of some numerical applications are presented.

  • 구조재료용 요업체로 널리 쓰이는 질화규소 요업체를 2단계 가스압 소결 방식에 의해 치밀화시켰을 때의 효과를 상압조절, 단순 가스압소결 또는 HIP 처리시와 비교하였다. 상압 소결시에는 $1800^{\circ}C$ 이상에서 분해가 일어나지만 질소 가스에 의한 가스압 소결시에는 소결 온도를 더 높일 수 있어서 ${\beta}-Si_3N_4$ 침상 조직의 발달을 이룰 수 있었다. 질소 가스압이 2MPa 정도이면 $1890^{\circ}C$까지도 상압소결시에 비해 분해가 현저히 억제될 수 있어서, 폐기공이 형성될 수 있는 단계까지는 분해만 억제시킬 수 있는 정도의 비교적 낮은 압력으로 치밀화시키고, 이어서 10MPa의 높은 압력을 가하는 소위 2단계 가스압 소결시 경도와 인성이 증진되는 것을 확인할 수 있었다.

Antioxidative Constituents from Paeonia lactiflora

  • The ethanol extract of the peony root (Paeonia Lactiflora Pall, Paeoniaceae) as well as its major active components including gallic acid and methyl gallate were evaluated for their protective effects against free radical generation and lipid peroxidation. In addition, the protective effects against hydrogen peroxide-induced oxidative DNA damage in a mammalian cell line were examined. The ethanol extracts of the peony root (PREs) and its active constituents, gallic acid and methyl gallate, exhibited a significant free radical scavenging effect against 1,1-diphenyl-2-picryl hydrazine (DPPH) radical generation and had an inhibitory effect on lipid peroxidation, as measured by the level of malondialdehyde (MDA) formation. The PREs did not have any pro-oxidant effect. They strongly inhibited the hydrogen peroxide-induced DNA damage from NIH/3T3 fibroblasts, as assessed by single cell gel electrophoresis. Furthermore, the oral administration of 50% PRE (50% ethanol extract of peony root), gallic acid and methyl gallate potently inhibited the formation of micronucleated reticulocytes (MNRET) in the mouse peripheral blood induced by a $KBrO_3$ treatment in vivo. Therefore, PREs containing gallic acid and methyl gallate may be a useful antigenotoxic antioxidant by scavenging free radicals, inhibiting lipid peroxidation and protecting against oxidative DNA damage without exhibiting any pro-oxidant effect.

Exercise-induced rhabdomyolysis with acute kidney injury complicated by posterior reversible encephalopathy syndrome: a case report

  • Posterior reversible encephalopathy syndrome (PRES) is a rare condition manifested by inflammation in certain areas of the brain. Rhabdomyolysis with acute kidney injury (AKI) complicated by PRES is rarely reported. A 26-year-old female presented with neurological symptoms, high blood pressure, and AKI. Her symptoms improved with blood pressure control, anticonvulsant drug medications, and renal replacement therapy. This case demonstrates that PRES should be considered in the differential diagnosis of patients who have rhabdomyolysis with AKI accompanied by neurological symptoms, including headaches and convulsions.
